Penanganan Persalinan Bayi Kembar Lima

Pada dekade 1990-an, di Kota Bandung belum semua rumah sakit
menyediakan fasilitas perawatan intensif kasus-kasus bayi
prematur atau belum cukup waktu saat dilahirkan. Pada masa
itu, di Rumah Sakit Santo Borromeus penanganan pasien bayi dan anak
yang memerlukan perawatan intensif masih menjadi satu unit dengan
perawatan intensif dewasa (Intensive Care Unit/ICU).

Pada tahun 1994, Rumah Sakit Santo Borromeus mulai
mengembangkan unit khusus untuk perawatan bayi baru lahir dan anak-
anak yang memerlukan perawatan intensif dengan kondisi kritis serta
berisiko tinggi yang disebut dengan Neonatal Intensive Care Unit (NICU)
dan Pediatric Intensive Care Unit (PICU).

Handling of Quintuplets

In the 1990s, not all hospitals in Bandung provided intensive care
for premature babies or those who were born with insufficient
time. At the time, the handling of infant and child patients who
required intensive care was still done in the same section as adult
intensive care (Intensive Care Unit / ICU) at the Santo Borromeus
Hospital.

In 1994, Santo Borromeus Hospital began to develop special units
for the care of newborns and children who require intensive care with
critical conditions and high risk called Neonatal Intensive Care Unit
(NICU) and Pediatric Intensive Care Unit (PICU).

Ketersediaan unit baru NICU dan PICU di Rumah Sakit Santo 401
Borromeus menjadi harapan bagi masyarakat Bandung dan Jawa Barat.
Rumah Sakit Santo Borromeus menjadi rujukan rumah sakit lain dari
sejumlah daerah yang membutuhkan perawatan intensif untuk bayi dan
anak-anak tersebut. Sejak awal dioperasikan unit dilengkapi dengan
berbagai fasilitas penunjang kehidupan, seperti alat bantu napas
(ventilator) dan peralatan lainnya.

Pada Oktober 1996, “tamu Illahi” hadir di Rumah Sakit Santo
Borromeus ketika Tuhan memercayakan penanganan seorang ibu atas
nama Ellydawaty yang melahirkan bayi kembar lima. Pasien tersebut
datang dari luar Kota Bandung. Banyak orang akan merasa senang
memiliki anak kembar. Walaupun kelahiran kembar lima yang relatif
jarang pada saat itu memiliki risiko yang besar bagi keselamatan ibu dan
bayi. Oleh karena itu, persalinan bayi kembar harus dipersiapkan secara
matang.

The availability of new NICU and PICU units at the Santo
Borromeus Hospital is a hope for the people of Bandung and West
Java. The Santo Borromeus Hospital has become a reference for other
hospitals from a number of areas that require intensive care for these
infants and children. Since its inception, the unit has been equipped
with various life support facilities, such as a breathing apparatus
(ventilator) and other equipment.

A “divine guest” was present at Santo Borromeus Hospital in
October 1996 when God entrusted the care of a mother on behalf of
Ellydawaty, who gave birth to quintuplets. The patient had traveled from
outside of Bandung. Many people would rejoice at the prospect of
having twins. Although the birth of quintuplets was relatively
uncommon at the time, there was a significant risk to both mother and
baby’s safety. As a result, the birth of twins must be meticulously
planned.

Berkat kolaborasi tim, mulai dari dokter kandungan, dokter anak,
perawat, dan bagian terkait lainnya untuk mempersiapkan fasilitas serta
layanan yang prima, persalinan pasien dibantu secara optimal lewat
operasi caesar. Peristiwa kelahiran bayi kembar lima pada masa itu masih
merupakan kasus yang jarang terjadi. Kasus yang ditangani di Rumah
Sakit Santo Borromeus tersebut mungkin termasuk satu di antara
beberapa yang pertama terjadi di Indonesia. Bayi kembar tersebut
berjenis kelamin laki-laki dan perempuan dengan bobot beragam di
bawah normal antara 1,2 – 1,5 kilogram.

Thanks to team collaboration, from obstetricians, pediatricians,
nurses, and other related departments to prepare excellent facilities and
services, the patient’s delivery is optimally assisted by caesarean section.
The birth of quintuplets at that time was still a rare case. The case being
treated at the Santo Borromeus Hospital was most likely one of the
first to occur in Indonesia. The twins were male and female, and their
weights ranged from 1.2 to 1.5 kilograms below normal.

Coronary Artery Bypass Graft (CABG)

C oronary Artery Bypass Graft (CABG) atau operasi bedah jantung
merupakan prosedur yang dilakukan untuk menangani
penyakit jantung koroner dalam kondisi tertentu. Penyakit
jantung koroner terjadi ketika arteri koroner (pembuluh darah yang
memasok oksigen dan nutrien ke otot jantung) menyempit atau
tersumbat. Penyebab sumbatan itu adalah tumpukan plak yang berasal
dari lemak atau kolesterol dan zat lain.

Coronary Artery Bypass Graft (CABG)

Under certain conditions, a coronary artery bypass graft
(CABG) or heart surgery is performed to treat coronary
heart disease. Coronary heart disease occurs when the
coronary arteries (blood vessels that supply oxygen and nutrients to
the heart muscle) become narrowed or blocked. A buildup of plaque
caused by fat, cholesterol, and other substances is the source of the
blockage.

Akibat tumpukan itu, bagian dalam pembuluh darah menjadi kasar
dan sempit. Dampaknya, suplai darah dan oksigen ke otot jantung
terbatas. Operasi jantung CABG bisa mengatasi masalah itu dengan
membuat semacam jalan pintas alias bypass di antara dua titik arteri yang
tersumbat. Jalan pintas tersebut berupa pembuluh darah cangkokan
yang diambil dari bagian tubuh lain.

Pada tahun 1995, Rumah Sakit Santo Borromeus menangani kasus
penyakit jantung koroner yang dialami oleh seorang pasien dewasa laki-
laki. Karena kondisinya yang tidak memungkinkan dirujuk ke rumah
sakit lain, maka tim medislah yang didatangkan. Prosedur CABG
dilakukan oleh tim dokter Rumah Sakit Santo Borromeus bekerja sama
dengan tim dari salah satu rumah sakit di Perth, Australia. Operasi
berjalan lancar dan pasien selamat.

Keberhasilan operasi CABG tersebut semakin menempatkan Rumah
Sakit Santo Borromeus sebagai penyedia layanan kesehatan yang
terpercaya dan dapat diandalkan, juga dalam penanganan penyakit
jantung dan pembuluh darah (kardiovaskular) di mata masyarakat.

The insides of the blood vessels become rough and narrow as a 407
result of the pile. As a consequence, the heart muscle receives a limited
supply of blood and oxygen. CABG heart surgery can solve this
problem by creating a kind of shortcut alias bypass between the two
points where the artery is blocked. A blood vessel graft from another
body part is used as a short-cut.

An adult male patient with coronary heart disease was treated at
Santo Borromeus Hospital in 1995. Because his condition prevented
him from being referred to another hospital, the medical team was
summoned. A team of doctors from Santo Borromeus Hospital
collaborated with a team from a hospital in Perth, Australia, to perform
the CABG procedure. The procedure went well, and the patient
survived.

In the eyes of the public, the success of the CABG operation has
further established Santo Borromeus Hospital as a trustworthy and
dependable health-care provider, particularly in the treatment of heart
and blood vessel (cardiovascular) diseases.

Pemasangan Spinal Cord Stimulation

Keselamatan sesama merupakan salah satu kata kunci dalam
rumusan semangat CB yang terus diupayakan untuk dihidupi
dalam pelayanan. Empati dan bela rasa mewujud sebagai
komunikasi batin yang kuat antara perawat, dokter, pasien, dan keluarga
pasien. Keterbukaan hati untuk ikut merasakan apa yang dirasakan oleh
sesama melahirkan hasrat yang kuat untuk meringankan penderitaan mereka.

Komunikasi batin yang terajut halus berkat adanya empati dan bela
rasa ini sungguh hadir pada saat Rumah Sakit Santo Borromeus
dipercaya Tuhan menangani pasien dengan diagnosis Chronic Regional
Pain and Syndrome (CRPS) tipe 2. Kasus ini merupakan kondisi nyeri
kronis berkepanjangan, biasanya timbul setelah terjadi cedera yang
mengakibatkan kerusakan sistem saraf dan sumsum tulang belakang.

Pasien yang mengalami CRPS ini adalah perempuan dewasa berusia
23 tahun. Pasca-kecelakaan lalu lintas yang dialaminya, ia dirawat dan
ditangani oleh dokter ortopedi dan dikonsultasikan kepada dokter
anestesi untuk manajemen nyeri. Berbagai tindakan dan bermacam obat
penghilang nyeri diberikan kepada pasien, tetapi pasien masih mengeluh
kesakitan yang luar biasa.

Installation of Spinal Cord Stimulation

The safety of others is one of the key words in the formulation
of the CB spirit, which is constantly strived to be lived in
service. Empathy and compassion manifest as strong inner
communication between nurses, doctors, patients, and patients’ families.
The willingness to share what others are feeling creates a strong desire
to alleviate their suffering.

This subtle inner communication was present, thanks to the presence
of empathy and compassion, when the Saint Borromeus Hospital was
trusted by God to treat patients with Chronic Regional Pain Syndrome
(CRPS) type 2. This is a prolonged chronic pain condition that usually
occurs as a result of an injury to the nervous system or spinal cord.

Untuk memperkuat tim penanganan, dokter bedah saraf turut 411
membantu dalam tata laksana nyeri. Tim dokter mencari solusi terbaik
demi kesembuhan pasien. Komunikasi yang intensif terus dijalin antara
tim dokter, pasien dan keluarga pasien hingga solusi yang dipilih adalah
rencana tindakan pemasangan alat bernama Spinal Cord Stimulation (SCS).

Tindakan pemasangan SCS belum pernah dilakukan di Indonesia
sehingga berpengaruh terhadap proses penyediaan alat ini ke Kota
Bandung. Kendala lain adalah SCS belum pernah diimpor ke Indonesia
dan harga alat produksi Medtronic, yaitu perusahaan yang bergerak di
bidang farmasi dan alat kesehatan di Amerika Serikat tersebut cukup tinggi.

Komunikasi yang terajut halus berkat adanya empati dan bela rasa demi
keselamatan sesama rupanya tidak hanya bergulir di lingkungan internal
pelayanan. Segala prosesnya mendapatkan dukungan dari sebuah Badan
Usaha Milik Negara di Kota Bandung dan Pemerintah Kota Bandung,
yaitu Wali Kota Bandung dan jajaran Dinas Kesehatan Kota Bandung.
Perizinan diajukan ke Kementerian Kesehatan sehingga memperoleh
Special Access Scheme (SAS), yaitu izin memasukkan obat/alat dari luar negeri
tanpa izin edar di Indonesia untuk kepentingan pengobatan perseorangan.

The patient with CRPS was a 23-year-old adult female. Following her car
accident, she was treated by an orthopedic doctor and consulted with an
anesthesiologist for pain management. The patient was given various
measures and painkillers, but she still complained of excruciating pain.

Neurosurgeons also assisted in pain management to strengthen the
treatment team. The medical team was attempting to find the best
solution for the patient’s recovery. Intensive communication continued
to be established between the team of doctors, patients, and patients’
families until the solution chosen was an action plan for installing a
device known as Spinal Cord Stimulation (SCS).

Because the act of installing SCS had never been done in Indonesia, it
hampered the process of providing this tool to the city of Bandung. Another
obstacle was that SCS had never been imported into Indonesia, and the price
of equipment manufactured by Medtronic, a company in the pharmaceutical
and medical device sector based in the United States, was quite high.

Setelah melalui proses yang cukup panjang dan penantian lebih dari
satu tahun, akhirnya alat tersebut berhasil didatangkan. Mei 2017 tim
dokter Rumah Sakit Santo Borromeus melakukan trial dan dua pekan
kemudian SCS dipasang pada bagian tulang belakang pasien. Alat
tersebut berfungsi untuk mengontrol nyeri.

Puji syukur kepada Tuhan, tim dokter tidak menemukan persoalan
berarti pada saat pemasangan alat tersebut. Kualitas hidup pasien
pascatindakan meningkat, rasa nyeri pun berkurang sehingga berdampak
pula pada penurunan tingkat ketergantungan pasien terhadap obat
penghilang rasa nyeri. Perkembangan lain yang selalu disyukuri adalah
pasien yang semula tidak dapat berjalan sama sekali, sejak 16 Februari
2021 sudah bisa berjalan kembali tanpa bantuan alat. Walaupun pasien
sudah dapat beraktivitas kembali seperti biasa, tetapi untuk proses
pemulihan yang optimal pasien tetap disarankan menjalani rawat jalan dan
terapi secara rutin.

The presence of empathy and compassion for the safety of others,
as evidenced by successful and smooth communication, did not appear
to be limited to the internal service environment. All processes were
backed up by a Bandung City State-Owned Enterprise and the Bandung
City Government, specifically the Mayor of Bandung and the ranks of
the Bandung City Health Office. The permit was submitted to the
Ministry of Health in order to obtain a Special Access Scheme (SAS),
which was a permit to import drugs/devices from other countries
without a distribution permit in Indonesia for the benefit of individual
treatment. 

After a lengthy process and more than a year of waiting, the tool
was finally brought in. In May 2017, a team of doctors from Santo
Borromeus Hospital conducted a trial, and two weeks later, a SCS
was implanted on the patient’s spine. This device was used to
manage pain.

Praise be to God; the doctors’ team discovered no major issues
during the device’s installation. The patient’s quality of life improved
after the procedure, and the pain level decreased, resulting in a
reduction in the patient’s reliance on painkillers. Another advancement
that was always appreciated, was that patients who were previously
unable to walk at all, since February 16, 2021, have been able to walk
again without the use of tools. Although the patient was able to resume
normal activities, outpatient treatment and regular therapy were still
recommended for an optimal recovery process.

v Keberhasilan menolong bayi dengan berat badan lahir 1.100
gram pada 1997 dan berat badan lahir rendah (BBLR) 700 gram
yang lahir prematur pada Maret 1999.

v Pada 2010 melakukan operasi VATS (Video Assisted Thoracoscopic
Surgery) dan CABG (Coronary Artery Bypass Grafting Surgery).

v Sejak tahun 2009 telah melaksanakan operasi Total Knee
Replacement, Total Hip Replacement, rekonstruksi ACL (Anterior
Cruciate Ligament) dan PCL (Posterior Cruciate Ligament) dan
rekonstruksi sendi lainnya.

v Sejak tahun 2010 sudah melakukan radiologi intervensi, antara
lain TACE (Transcatheter Arterial Chemo Embolization), TACI
(Transcatheter Arterial Chemo Infusion), Coiling pada Aneurisma,

T idak ada yang ingin sakit, apalagi dirawat di rumah sakit.
Perjalanan pasien sampai di rumah sakit bukanlah sebuah
perjalanan yang menyenangkan. Pasien datang dengan
kekhawatiran dan kecemasan, rasa sakit, ketidaktahuan, bahkan
kesedihan. Kebutuhan pasien untuk mendapatkan pelayanan kesehatan
sesuai dengan kondisi dan latar belakang masing-masing menjadi sangat
penting. Pemahaman, simpati, empati serta sikap hormat tenaga
kesehatan dalam melayani pasien menjadi ujung tombak keberhasilan
pelayanan dan penentu pengalaman pasien yang bermakna.

Rumah Sakit Santo Borromeus dengan komitmen kesetiaan pada
misi awal serta penghayatan ICARE dalam setiap gerak pelayanannya
terus berlayar di tengah arus zaman. Setiap bentuk ambil bagian secara
aktif demi keselamatan sesama yang tak jarang penuh perjuangan,

Nobody wants to get sick, let alone be admitted to the hospital.
The patient’s hospital visit is not enjoyable. Patients arrive
with concerns and anxiety, as well as pain, ignorance, and
even sadness. It is critical for patients to receive health services that are
appropriate for their conditions and backgrounds. Understanding,
sympathy, empathy, and respect for health workers are the cornerstones
of service success and determine a meaningful patient experience.

Santo Borromeus Hospital continues to sail in the midst of the
times, with a commitment to its original mission and ICARE’s
appreciation of every movement of its service. Every form of actively
participating in the safety of others, which is often full of struggles,
tough challenges, tirelessly and without giving up by the devotees until
they reach the pier of 100 years of service, has produced results that
can be enjoyed by many people, especially the body.

Pelayanan kesehatan berfokus pada pasien sebagai subjek layanannya 419
merupakan paradigma baru yang diharapkan dapat meringankan “beban”
pasien saat harus menggunakan fasilitas kesehatan. Pasien menjadi pusat
semua upaya kesehatan sehingga pasien harus beroleh informasi dengan
cukup dan benar. Pasien bersama dengan dokter serta pemberi asuhan
lainnya membuat keputusan-keputusan penting dalam proses
penyembuhan. Berbagai pengembangan yang meliputi peralatan dan
perlengkapan medis, manajemen hingga peningkatan sumber daya manusia
tak pernah berhenti. Bahkan, terus dilakukan guna memenuhi kebutuhan
masyarakat akan pelayanan kesehatan yang bermutu dan paripurna.

Eksistensi yang diakui serta dapat dipercaya ini tidak datang secara
tiba-tiba, melainkan hasil dari sebuah perjalanan seabad lamanya.
Perjalanan panjang ini diwarnai dengan berbagai suka dan duka yang
terus-menerus dijalani dalam sukacita serta perjuangan bersama untuk
menjaga pelita tetap bernyala:

Health services that focus on patients as the subject of their services
represent a new paradigm that is expected to reduce the patient’s
“burden” when they must use health facilities. Because the patient is
the focal point of all health-care efforts, he or she must be provided
with adequate and accurate information. Patients, in collaboration with
doctors and other caregivers, make critical decisions in the healing
process. Various developments, such as medical equipment and supplies,
management, and human resource improvement, never cease. In fact,
it is still being carried out in order to meet the community’s demand
for high-quality, all-inclusive health care.

This well-known and trustworthy existence did not emerge
overnight, but rather was the result of a century-long journey. This long
journey is colored by a variety of emotional ups and downs that are
constantly lived in sheer delight and together in the struggle to keep
the lamp illuminated:

Breaking the Limits of Hope

”A ge is a matter of feeling, not of years”, kutip George W. Curtis.
Hidup bukanlah semata ditentukan oleh hitungan tahun,
melainkan rasa hati. Beribu-ribu hati selama bertahun-tahun
telah ikut ambil bagian menanggung karya pelayanan kesehatan yang
sangat berharga ini. Selama 100 tahun, dengan berkobarnya banyak hati
di dalam karya pelayanan ini, Tuhan memberi anugerah denyut
kehidupan-Nya setiap hari hingga saat ini.

Memperingati usia 100 tahun bukanlah merayakan waktu, melainkan
sebuah kematangan serta kedewasaan untuk semakin menyadari bahwa
segala yang ada di dunia ini adalah sementara saja. Zaman berubah dari
masa ke masa, maka dengan sabar dan rendah hati manusia terus
berziarah menyongsong hari depan. Kerendahan hati membawa serta

of years.” Life is decided not only by the number of years,
but also by the state of one’s heart. Thousands of hearts
have contributed to the vital work of health care over the years. For 100
years, with the burning of many hearts in this ministry, God has given
the pulse of His life every day until now.

Celebrating 100 years does not mark the passage of time, but rather
the maturation and knowledge that comes with the realization that
everything in this life is momentary. People continue to crusade to meet
the future in a patient and humble manner as times change. Throughout
the journey, humbleness inspires a deep sense of gratitude. The efforts,
accomplishments, and achievements of the past years are only
meaningful if they are gratefully remembered and lived modestly in the
future.

Allah”, tulis Bunda Elisabeth Gruyters dalam alinea ke-79 dari manuskripnya.
Mengamini ungkapan hati itu, sungguh bahwa tak ada kata lain yang mampu
mewakili untuk mengucap syukur kepada Allah saat mencecap dan menatap
tapak-tapak kasih Allah yang terus-menerus berkarya dari waktu ke waktu
bagi Rumah Sakit Santo Borromeus hingga seabad usianya.

Sejak 1921, Rumah Sakit Santo Borromeus pun telah melibatkan
dan mengobarkan banyak hati untuk terus berkembang menanggapi
tuntutan zaman. Perjalanan suka dan duka memberi hikmah yang luar
biasa serta menghantarkannya menjadi rumah sakit andalan masyarakat.

Menjadi andalan berarti pula kesanggupan untuk mewujudkan
harapan bagi yang mengandalkannya. Harapan demi harapan yang
diletakkan tak jarang mampu menumbuhkan kekuatan besar yang
menggerakkan serta melibatkan komitmen bersama seluruh entitas
untuk bangkit mewujudkannya, menembus batas harapan itu sendiri:

wrote Mother Elisabeth Gruyters in the 79th paragraph of her
manuscript. In agreement with the heart’s expression, there are no other
words that may communicate thanks to God when tasting and staring
at the footprints of God’s love that have been continuously flowing for
the Santo Borromeus Hospital for a century.

Since1921, Santo Borromeus Hospital has also involved and ignited
many hearts to continue to develop in response to the demands of the
times. The journey of joy and sadness provided amazing wisdom, which
led to it being the mainstay of the community’s hospital.

Being a mainstay also means the ability to realize hopes for those
who rely on Santo Borromeus Hospital. Each and every hope that is
laid upon is frequently capable of growing into a huge power that drives
and creates an unified commitment from all entities to rise to make it
happen, breaking through the limits of the hope itself:
