Long Island, NY
Superstorm Sandy made land fall and with it came a devastating array of issues. One of the areas in which EMS Innovations was able to lend our expertise was in establishing base camp medical facilitites for the many power and utility companies which were responding to the major loss of power for an estimated 8,100,000 people in 17 states as far west as Michigan. The aid EMS Innovations was able to supply helped to ease the burden of resources that these workers had on an already over-taxed local supply of medical items.
Tuscaloosa, AL
A series of devastating storms produced tornadoes which cut a swath of destruction through Alabama and several other states. The hardest hit was the town of Tuscaloosa, Alabama. Left without power, shelter, and basic necessities, the residents were in dire need of assistance. EMS Innovations was on scene the next day to lend support through products and systems from our warehouse in Alabama to alleviate some of the needs of
the local first responders and citizenry. By utilizing our rapidly deployable structures, first responders were able to set up temporary communication and distribution points as well as a camp at a local fire academy.
Little League World Series - Williamsport, PA
EMS Innovations, Inc. was requested to deploy
a 16-bed Triage Unit as an adjunct to the existing systems to handle minor injuries and cases of dehydration at the Little League World Series. However, a potential threat of Measles prompted Pennsylvania’s Department of Health to convert this minor treatment center to handle vaccinations and treatment for potential cases of the disease.
Americus, GA
When Sumter Regional Hospital was destroyed by an F4 tornado, EMS Innovations, Inc. answered the call for help. Requested by the state of Georgia to aid in the response to this incident just hours after the tornado devastation, EMS Innovations, Inc. erected and established a
50-Bed Hospital in a matter of hours.

New Orleans, LA
In the wake of Hurricane Katrina, Joseph G. Ferko, D.O., M.S., President of EMS Innovations, Inc., served as the Medical Logistics Officer for the State of Louisiana during the first seven days after the incident. According to Evon Smith, EMS Director for Louisiana, “EMS Innovations provided 95% of all medical equipment for the State of Louisiana during the first seven days
of the incident.” Working with an available state budget of only 6 million
dollars, EMS Innovations provided 16,000 medical beds with all required
patient care equipment in a 72 hour time frame. This included (3) 1,000 bed
portable hospitals; (9) 750 bed special needs shelters, and (3) search and rescue
field facilities that processed over 20,000 people per day per facility. In addition, a housing tent facility for the incoming volunteers and a centralized warehouse were established in Baton Rouge, LA.

Natural or Man-Made Disasters have their own unique sets of challenges. EMS Innovations offers a myriad of products and solutions to address the specific challenges faced by officials during preparation for and response to a disaster. Part of a comprehensive response should include ADA compliance. The products throughout this section serve simply as suggestions. Please consult you State and Local guidelines before formulating a plan.
Title III of the ADA prohibits discrimination against individuals with disabilities in public accommodations. This prohibition in part requires that physical facilities be accessible if they are newly constructed or altered in a manner that affects the usability of the facility. 1 Any public accommodations that are rebuilt or significantly altered as a result of damage during a disaster must comply with the ADA’s requirements for accessibility. Similarly, facilities that are rebuilt by states and localities (covered by Title II of the ADA) must also comply with the ADA’s requirements for accessibility. 2
Involving people with disabilities in identifying needs and evaluating effective emergency management practices in the planning phase of the response is one of the best ways to assure that access and care is being provided to these sectors of your local population.
“Issues that have the greatest impact on people with disabilities include:
• notification;
• evacuation;
• emergency transportation;
• sheltering;
• access to medications, refrigeration, and back-up power;
• access to their mobility devices or service animals while in transit or at shelters; and • access to information.
In planning for emergency services, you should consider the needs of people who use mobility aids such as
wheelchairs, scooters, walkers, canes or crutches, or people who have limited stamina. Plans also need to include
people who use oxygen or respirators, people who are blind or who have low vision, people who are deaf or hard of
hearing, people who have a cognitive disability, people with mental illness, and those with other types of disabilities.” 3

Preparations and Needs for Service Animals
Amendments to the Stafford Act in 2006, known as the PETS Act (see Chapter 2), require that jurisdictions integrate animal planning into local emergency response plans. [75] Congress intended this law to enhance animal safety in disasters, minimize public health concerns due to mass fatalities of animals, and ultimately encourage pet owners to evacuate with their animals rather than remain in danger with them.

Equipment
To move animals carefully, the US Department of Agriculture Animal and Plant Health Inspection Service (APHIS) recommends using travel containers with:[85]
* Locking bolts to secure the container
* Metal doors since animals can chew through plastic
* Four metal rods that fasten and secure the door into the container * No wheels since airlines will not accept such crates
* Sturdy construction with no weak points
* Adequate ventilation
* Enough space for the animal to turn around
* Access and room for food and water
* Access and room for cleaning.
APHIS suggests including an owner’s t-shirt or article of clothing in the crate that may help keep the animal calmer.
Leashes and muzzles of varying sizes can be useful as animals under stressful conditions or with injuries may bite defensively. Strong, sturdy gloves can prevent scratches to humans. Masks worn by humans can hinder the transmission of germs and diseases from humans to animals.
Emergency managers and the local humane society should encourage pet owners and those with service animals to develop a ready kit or go-bag in order to evacuate their animals. In addition to keeping copies of records and a photo of their animal(s) with them, another copy should be placed in the go-kit that, as appropriate, will contain: [86], [87]
* Proof of vaccination and veterinary records * Licenses, rabies, and ID tags
* Two weeks of food, water, and medications * Bedding and toys
* Litter box, litter, and a scoop
* Food and water bowls
* Information on medication and feeding schedules * Newspapers, pee pads, cleaning supplies
* Collars, leashes, muzzles, harnesses
* First-aid kit
* A manual can opener and spoons
* Stakes and a break-proof rope or tie down.
Food
Animal rescue teams and key animal organizations, such as HSUS and ASPCA, will coordinate with the local EMA and transportation agencies to manage the movement of donated pet food and other pet items for storage and dissemination to where animals are being held for reunification. Transportation agencies may be called upon to assist with donated goods, including pet supplies, and they should be prepared with staff and vehicles.

Bariatric Sked® Stretcher
The Bariatric Sked Stretcher is designed to move obese patients through difficult areas with less risk of injuring your back. It is 4 feet wide, a full 12 inches wider than the standard Sked. It is a full 8 feet
long. It is made of a very tough non absorbing polyethylene plastic. This will allow for con- tainment of the very large patients inside the stretcher thus preventing possible injuries
to them from being moved. It functions just as the standard Sked. It is dragged over whatever terrain the patient is in. It can be dragged up a ramp to the
floor of the ambulance or hoisted onto a bariatric gurney.
It has a pair of lift slings that are long enough to accommodate nearly any size patient. They are used to hoist the patient onto a bariatric gurney for transport in the ambulance. Along with the slings there is also included a large steel carabiner that has a minimum break strength of 9,000 pounds that is used to join the sling ends together and clipped into the hoist mecha- nism. There is also a tow strap included to allow two more people to assist in dragging.

This specially designed NEW Bariatric cuff provides a cost effective solution to the chal- lenges of measuring blood pressure on severely obese or bariatric patients. In many cases large adult cuffs are too small, while thigh cuffs are too wide to fit on the upper arm on all but the tallest patients. Measurement is further complicated by the conical shape of a bariatric patient’s upper arm, making it virtually impossible to obtain a proper fit. One solution - measurement on the forearm is tricky at best and many practitioners don’t have the train- ing or experience with this technique.
The Bariatric AdcuffTM solves all of these prob- lems by allowing manual measurement on the upper arm in patients with a limb circumfer- ence from 44-66cm. SURGE CAPACITY

The Health Resources and Services Administration has identified “surge capacity” as one of its priority areas when evaluating funding of projects related to terrorism response. Within this priority area they have identified benchmarks for applicants that show where focus should be placed. These are applicable to Acute Care Center (ACC) Planning and are as follows:
• Hospital bed capacity (treatment and initial stabilization of 500 adult patients per 1 million population) for acute illnesses or trauma requiring hospitalization from chemical, biological, radiological, nuclear or explosive incidents.
• Isolation capacity
• Deployment of additional healthcare personnel in support of surge capacity (identifying how personnel are recruited, received, processed and managed through the incident)
• Credentialing of personnel
• Establishment of regional pharmaceutical caches
• Personnel protection and decontamination, systems and equipment
• Provision of mental health services and trauma/burn care services
• Provision of communication capability
Surge Response Levels
The methods used to handle patient surge generated by an emergency or disaster will be dependent upon the type of scenario present- ed. For organizational purposes, these methods can be divided into six surge response levels:
• Level 1 - Facility “In-place” surge coverage by hospitals
• Level 2 - Multiple Facilities Coordinated surge coverage by hospitals • Level 3 - Community LEOC coordinated – ACC involvement
• Level 4 - Regional LEOC coordinated – ACC involvement
• Level 5 - State SEOC coordinated – Multiple ACCs
• Level 6 - Federal Support to State response – Multiple ACCs
Level 2 will require greater “mutual aid” type agreements between hospitals to coordinate coverage. Levels 3 through 6 necessitate the establishment of an Acute Care Center (ACC), the involvement of Emergency Management and the implementation of an incident management system to coordinate activities.
