EMS Spinal Precautions and the Use of the Long Backboard
National Association of EMS Physicians and American College of Surgeons Committee on Trauma
The National Association of EMS Physicians and the American College of Surgeons Committee on Trauma believe that:
• Long backboards are commonly used to attempt to provide rigid spinal immobilization among emergency medical services (EMS) trauma patients. However, the benefit of long backboards is largely unproven.
• The long backboard can induce pain, patient agitation, and respiratory compromise. Further, the backboard can decrease tissue perfusion at pressure points, leading to the development of pressure ulcers.
• Utilization of backboards for spinal immobilization during transport should be judicious, so that the potential benefits outweigh the risks.
• Appropriate patients to be immobilized with a backboard may include those with:
• Blunt trauma and altered level of consciousness
• Spinal pain or tenderness
• Neurologic complaint (e.g.,
numbness or motor weakness)
• Anatomic deformity of the spine
• High-energy mechanism of injury
and any of thefollowing: • Drug or alcohol
intoxication
• Inability to communicate • Distracting injury
• Patients for whom immobilization on a backboard is not necessary include those with all of the following:
• Normal level of consciousness (Glasgow Coma Score [GCS] 15) • No spine tenderness or anatomic
abnormality
• No neurologic findings or
complaints
• No distracting injury • No intoxication
• Patients with penetrating trauma to the head, neck, or torso and no evidence of spinal injury should not be immobilized on a backboard.
• Spinal precautions can be maintained by application of a rigid cervical collar and securing the patient firmly to the EMS stretcher, and may be most appropriate for:
• Patients who are found to be ambulatory at the scene
• Patients who must be transported for a protracted time, particularly prior to interfacility transfer
• Patients for whom a backboard is not otherwise indicated
• Whether or not a backboard is used, attention to spinal precautions among at-risk patients
is paramount. These include application
of a cervical collar, adequate security to a stretcher, minimal movement/transfers, and maintenance of inline stabilization during any necessary movement/ transfers.
• Education of field EMS personnel should include evaluation of the risk of spinal injury in the context of options to provide spinal precautions.
• Protocols or plans to promote judicious use of long backboards during prehospital care should engage as many stakeholders in the trauma/EMS system as possible.
• Patients should be removed from backboards as soon as practical in an emergency department.

The KEMP Head Immobilizer is easy to use and is compatible with all spineboards on the market, except for EG and CJ boards. Our immobilizers feature side blocks with wide fast-stick closures for a snug fit and up to 50%
more support surface. The KEMP Head Immobilizer has large circular ear holes to monitor bleeding and fluid drainage. It also comes with 2 head/chin straps and a base plate that adheres easily to the board. The

Backboards available as rigid and folding types. Wooden backboards
are 3/4” weatherproof Marine Plywood finished with two coats of clear varnish. Incorporate large hand cut-outs for easy handling. Backboards,
as indicated, have 3/4” runners on bottom for ease in lifting. Four nylon patient restrain-

Bariatric Sked® Stretcher

While endotracheal intubation is considered the most definitive tool for airway management, complications are often documented with it. And with other airway devices available, some experts question whether paramed-
ics should even continue to intubate. This article explains the challenges paramedics face while performing intubation, problems found in studies of paramedic intubation and 10 steps to improve intubation performance.
Intubation Challenges
The goal of endotracheal intubation (ETI) is to place a tube in the patient’s trachea that provides an unobstructed pathway for ventilation. While ventilation and airway protection are life-saving, achieving this through ETI comes with a number of risks and challenges. It can be difficult in some patients to locate the correct landmarks and place the tube in the trachea.
If the tube is mistakenly placed in the esophagus and the error is not recog- nized, serious brain damage or death is likely.
Only the sickest and most injured patients should be intubated by para- medics. These patients often have anatomy that makes finding the airway landmarks difficult, have blood or secretions in their airway, or are com- bative from a head injury. Paramedics also must intubate in uncontrolled environments, such as a small bathroom or dark roadway. Not only are these patients the most difficult to intubate, but it’s attempted under some of the worst conditions.
Problems With Intubation
While it’s intuitive that early intubation of patients with a compromised airway would be helpful, this has not been demonstrated in literature. Studies have found unacceptably high rates of unrecognized tube misplace- ment, as high as 14% in one system.1 Periods of hypoxia were noted during prolonged intubation attempts in head-injured patients,2 and head-injured patients intubated in the field did worse than similar patients who were not.3–5
After the importance of continuous chest compressions was published in the American Heart Association’s 2005 guideline revision, a study showed chest compressions were often interrupted for several minutes during intubation.6 The procedure harmed several patients in these studies, and it did not help patients in cardiac arrest if compressions were stopped.
Education and Experience
Questions have been raised about initial airway education for paramedic students. More live clinical experience is associated with higher success rates,7 but paramedic students get much less operating room clinical intubation time than other students. Reasons for this include alternate airway devices being used in the OR, competition with students from other disciplines and physicians’ fear of litigation.8 While the National Standard Paramedic Curriculum recommends paramedic students perform five in- tubations,itisrecommendedEDresidentsperform35,anesthesiaresidents perform 20–57, and nurse anesthetists perform 200 during training.7
Because the procedure is seldom encountered in many EMS systems, skill maintenance is a concern. Over one year in Pennsylvania, 67% of paramed- ics performed two or fewer intubations and 39% performed none.9 With less initial airway training, intubating in uncontrolled situations and few opportunities to practice, it is not surprising so many problems have been documented.
ETI Alternatives
Other airway devices are now available that can be placed in less time than an ET tube, require less skill to use and carry little risk of misplacement. Instead of a tube in the trachea, these devices isolate the trachea and venti- late through a port above the glottic opening. Known as supraglottic airway
devices, they include the Combitube, laryngeal mask airway, SALT and King Airway. One drawback to the supraglottic devices is they may provide less protection from aspiration than an ET tube. Another is they will not ventilate effectively if the upper airway is obstructed. In cases of airway burns, anaphylaxis or severe neck trauma, only a tracheal tube can prevent an airway obstruction from edema. Since these situations are extremely rare, some experts believe supraglottic devices should replace the ET tube for EMS.

If intubation continues to be performed by paramedics, a smarter approach must be taken. Professional associations must advocate for more live
clinical experience during school. EMS systems must monitor intubation performance and ensure each paramedic’s proficiency. Equipment must be available to help intubate and monitor tube placement afterward. Individual paramedics must recognize when intubation is appropriate, when it is not and when to stop an intubation attempt.
ETI may not be best for some patients. For cardiac arrest, chest compres- sions and defibrillation should never be compromised to place any airway device. For patients with a pulse, other airway management options and transport time must be considered to determine if intubation should be done in the field.
Once the decision to proceed with intubation is made, steps should be taken to maximize the chance of first-pass success. The following steps provide
a methodical approach to manage the airway of any intubation candidate. They emphasize preparation, use of techniques that help intubation and when to use a supraglottic airway device instead. The problems that intuba- tion is intended to help should never be made worse during the procedure. Remember, no intubation is always better than bad intubation, even if an ET tube is eventually placed.

The versatile LSP Minilator features 50 psig outlets, which can be used to operate a demand resuscita-
tor valve, suction device, CPAP mask, or ventilator. All five oxygen outlets can function simultaneously, supplying 40 to 60 psig or low flow oxygen. Threaded barbed fittings with metered orifices can be fitted to the DISS check valves to provide consistent accurate flow rates of 4, 6, 10 or 15 liters per minute. Using standard DISS outlets, the Minilator is easily attached via standard oxygen hoses and/or connectors.
The zippered nylon case keeps the components clean and stores them fully assembled and ready
to operate. Several Minilators can be connected in tandem for emergency or mass casualty
situations where more than five oxygen outlets are required.
The Medstorm series of minilators allows you choose which product works best for your specific situation. They are a compact, lightweight and easy to operate constant flow device which can deliver up to 15 LPM from one to nine patients simultaneously. The Manifold is manufactured from anodized aluminum with check valves and threaded barbed fittings. The Minilator is attached to a high pressure oxygen source via high pressure oxygen supply hose (sold separately).

Magellan-2200 Model 1 Anesthesia Machine
The Magellan-2200 Model 1 is compact, robust and flexible, easy to move and was designed for military field hospitals, general civilian hospitals, office-based anesthesia and surgery centers of every description.
The Magellan-2200 Model 1 is pneumatically powered and may be mounted on a table top or mobile cart.
The Magellan-2200 Model 1 is designed to be a simple to operate unit containing all of the modes and necessary components to deliver a basic anesthetic, including a mechanical ventilator and a single agent vaporizer. All of the safety systems required are employed onboard and easy to utilize.
Physiologic monitoring of the operators choice may be employed as a separate, but necessary device. In standard use, the Magellan-2200 Model 1 may be used with a variety of face masks, endotracheal/tracheostomy tubes and other approved artificial airways that are a standard in any operating theater.

Convenience and Independence
The Model CFP-15M combines the high pressure and variable flow rate available from an oxygen cylinder with the oxygen generation and replenishment of an oxygen concentrator. Empty oxygen cylinders become a thing of the past. By simply connecting an empty oxygen cylinder(s) (up to two H-size at once) to its discharge line and turning it on, the CFP-15M will automatically re-fill the cylinder(s) to 2200 psig and turn itself off once the desired level of pressure is reached. It will top off most cylinders in a few hours, providing indepen- dence from traditional oxygen supply channels.
These portable oxygen generation systems (POGS) have the added benefit of being easily transported to battlefield hospitals. Unlike other POGS, the Model CFP-15M can be assem-
bled onto its own cart for mobility inside the hospital tent. This feature saves space, and eliminates the tripping hazard of other POGS on the market. Additionally, the Model CFP-15M cart has provisions for (2) H-size O2 cylinders, and a station for transfilling to smaller O2 cylinders. Field assembly and set-up of the Model CFP-15M POGS can be done in minutes.

MOGS-100 Cylinder Filling Plant
The OGSITM MOGS-100 Medical Oxygen Generating System has been designed to produce oxygen onsite and fill high pressure cylinders (up to 2200 psig) or to supply oxygen directly at up to 80 psig. This system uses oil-free air compres- sors and modular oxygen generators for redundancy and reliability. The system produces Medical Grade Oxygen in accordance with the U.S. Pharmacoepia specifications. There is a continuous gas quality monitoring system included with automatic shutdown if a fault occurs.
Cost Reduction
This system will reduce oxygen supply costs by 80% or more in most cases. Less than 1 Kilowatt-Hour (KWH) of energy is required to fill one ‘M’-size oxygen cylinder and all delivery and handling charges from your current supplier are eliminated.
Disaster Preparedness
By making and filling cylinders at your site the uncertainty and logistics of deliv- ered oxygen supplies are eliminated. With this system in place care providers are also assured that in the event of a disaster their oxygen supply chain will not be in jeopardy. The system itself can also be transported for use at a disaster site. It can be moved by truck and fits easily through a standard 36” wide doorway.
