The principle of the venturi mask is to deliver a predetermined concentration of oxygen regardless of the patient’s respiratory pattern. Oxygen flows through the diluter dragging in air through the air-entrainment ports.
Venturi masks create a constant proportion of air/oxygen mixture in excess of patients inspiratory flow rate. With gas flow constantly in excess of patient demands and with enhanced CO2 washout, rebreathing is nearly eliminated.
The fixed delivery of oxygen is achieved through the use of color-coded diluters. Each color coincides with a percentage of oxygen concentration and a specific flow rate of oxygen. The kits are conveniently pre-packaged with all diluters or with in- dividual diluters. Bolin Chest SealTM
Sucking chest wounds require an emergency first aid solution that is fast, direct, and effective: the H&H Bolin Chest SealTM (BCS) is specifically engineered to meet these critical needs.
The BCS is a sterile occlusive chest wound dressing for treating open pneumothorax and preventing tension pneumothorax that result from gunshots, stab wounds, or other penetrating chest trauma. The patented (US Patent No. 7,834,231) failure-proof triple-valve design of the BCS allows air and blood to escape while preventing the re-entry of either, thereby eliminating any unwanted gas or liquid exchange at the trauma site.
The large (6” diameter), rugged polyurethane disc structure of the BCS can cover practically any size chest trauma site. The temper of the disc prevents any disc wrinkling from occurring during application. In addition, the wound side of the disc is covered with a thick layer of jell-based adhesive, strong enough to not only seal over hair and blood but also flexible enough to be removed and reapplied to the trauma site if required.
In a recent evaluation, the Bolin Chest SealTM performed superior to the Asherman Chest Seal in overall performance.
Also, a 2013 study demonstrated that the Bolin Chest Seal was superior to an unvented/unvalved chest seal in managing a tension pneumothorax.

USE OF IMPROVED ENGINEERING CONTROLS IN A PREVENTION STRATEGY
Comprehensive Programs to Prevent Needlestick Injuries
Safety and health issues can best be addressed in the setting of a comprehensive prevention program that considers all aspects of the work environment and that has employee involvement as well as management commitment. Implementing the use of improved engineering controls is one component of such a comprehensive program. Since many devices with needlestick prevention features are new, this section primarily
addresses their use, including desirable characteristics, examples,
and data supporting their effectiveness. However, other prevention strategy factors that must be addressed include modification
of hazardous work practices, administrative changes to address needle hazards in the environment (e.g., prompt removal of filled sharps
disposal boxes), safety education and awareness, feedback on safety improvements, and action taken on continuing problems. Several authors have noted the importance of a comprehensive approach [Krasinski et
al. 1987; Hanrahan and Reutter 1997; DeJoy et al. 1995; Ramos-Gomez et al. 1997; Gershon et al. 1995]. The critical role of appropriate training has been emphasized by several recent reports of increased patient bloodstream infections associated with improper care of needleless IV systems, primarily in the home health care setting [Cookson et al. 1998; Danzig et al. 1995; Do et al. 1999; Kellerman et al. 1996]. These data emphasize the need for patient safety surveillance and thorough training as well as occupational injury surveillance when implementing the use of a new medical device.
Case Study of a Successful Comprehensive Prevention Program
The value of a comprehensive approach is illustrated by its success
in a recent report by Dale et al. [1998]. Between 1993 and 1996, the phlebotomy service at a major institution decreased the needlestick injury rate among its 200 full-time phlebotomists from 1.5 to 0.2 per 10,000 venipunctures performed. In comparison, a national survey from 1990 to 1992 found a median needlestick injury rate of about 0.94 per 10,000 venipunctures [Howanitz and Schifman 1994]. A retrospective review of the events contributing to the success of the phlebotomy service included changes in worker education and work practices, the implementation of devices with safety features, and encouragement of injury reporting.
These interventions as well as the implementation of CDC published guidelines and the OSHA bloodborne pathogens standard were associated with the observed steady decline in the injury rate. The authors noted
that an important factor contributing to this success was a thorough
understanding of the injuries that occurred among their staff.
Desirable Characteristics of Devices with Safety Features
Improved engineering controls are often among the most effective approaches to reducing occupational hazards and therefore are an important element of a needlestick prevention program. Such controls include eliminating the unnecessary use of needles and implementing devices with safety features. A number of sources have identified the desirable characteristics of safety devices [OSHA 1999c; FDA 1992; Jagger et al. 1988; Chiarello 1995; Quebbeman and Short 1995;
Pugliese 1998; Fisher 1999; ECRI 1999]. These characteristics include the following:
• The device is needleless.
• The safety feature is an integral part of the device.
• The device preferably works passively (i.e., it requires no activation
by the user). If user activation is necessary, the safety feature can be engaged with a single-handed technique and allows the worker’s hands to remain behind the exposed sharp.
• The user can easily tell whether the safety feature is activated.
• The safety feature cannot be deactivated and remains protective through disposal.
• The device performs reliably.
• The device is easy to use and practical.
• The device is safe and effective for patient care.
Although each of these characteristics is desirable, some are not feasible, applicable or available for certain health care situations. For example, needles will always be necessary where alternatives for skin penetration are not available. Also, a safety feature that requires activation by the user might be preferable to one that is passive in some cases. Each device must be considered on its own merit and ultimately on its ability to reduce workplace injuries. The desirable characteristics listed here should thus serve only as a guideline for device design and selection.
Examples of Safety Device Designs
Figure 3 shows examples of syringes with safety features. These and other examples of safety device designs are listed as follows:
• Needleless connectors for IV delivery systems (e.g., blunt cannula for use with prepierced ports and valved connectors that accept tapered or luer ends of IV tubing)
• Protected needle IV connectors (e.g., the IV connector needle is perma- nently recessed in a rigid plastic housing that fits over IV ports)
• Needles that retract into a syringe or vacuum tube holder
• Hinged or sliding shields attached to phlebotomy needles, winged-steel needles, and blood gas needles
• Protective encasements to receive an IV stylet as it is withdrawn from the catheter
• Sliding needle shields attached to disposable syringes and vacuum tube holders
• Self-blunting phlebotomy and wingedsteel needles (a blunt cannula seated inside the phlebotomy needle is advanced beyond the needle tip before the needle is withdrawn from the vein—see Figure 3)
• Retractable finger/heel-stick lancets

Step 1

Tetanus is a serious, often fatal, toxic condition, but is virtually 100% preventable with vaccination. Any wound or rash has the potential for becoming infected and should be assessed by a health-care provider as soon as possible.
These principles can assist with wound management and aid in the prevention of amputations. In the wake of a flood disaster resources are limited. Following these basic wound management steps can help prevent further medical problems.
Evaluation
• Ensure that the scene is safe for you to approach the patient, and that if necessary; it is secured by the proper authorities (police, fire, civil defense) prior to patient evaluation.
• Observe universal precautions, when possible, while participating in all aspects of wound care.
• Obtain a focused history from the patient, and perform an appropriate examination to exclude additional injuries.
Treatment
• Apply direct pressure to any bleeding wound, to control hemorrhage. Tourniquets are rarely indicated since they may reduce tissue viability.
• Examine wounds for gross contamination, devitalized tissue, and foreign bodies.
• Remove constricting rings or other jewelry from injured body part.
• Cleanse the wound periphery with soap and sterile water or available solutions, and provide anesthetics and analgesia whenever possible.
• Irrigate wounds with saline solution using a large bore needle and syringe. If unavailable, bottled water is acceptable.
• Leave contaminated wounds, bites,
and punctures open. Wounds that are sutured in an unsterile environment, or are not cleansed, irrigated, and debrided appropriately, are at high risk for infection due to contamination. Wounds that are
not closed primarily because of high risk of infection should be considered for delayed primary closure by experienced medical staff using sterile technique.
• Remove devitalized tissue and foreign bodies prior to repair as they may increase the incidence of infection.
• Clip hair close to the wound, if necessary. Shaving of hair is not necessary, and may increase the chance of wound infection.
• Cover wounds with dry dressing; deeper wounds may require packing with saline soaked gauze and subsequent coverage with a dry bulky dressing.
• If wound infections develop, see “Guidance for Management of Wound Infections” (see below).
• Follow tetanus prophylaxis guidelines for all wounded patients.
• Follow tetanus prevention guidelines.
Other Considerations
• Be vigilant for the presence of other injuries in patients with any wounds.
• Ensure adequate referral, follow-ups, and reevaluations whenever possible.
• Dirty water and soil and sand can
cause infection. Wounds can become contaminated by even very tiny amounts of dirt.
• Puncture wounds can carry bits of clothing and debris into wound resulting in infection.
• Crush injuries are more susceptible to infection than wounds from shearing forces.
Guidance for Management of Wound Infections
Most wound infections are due to staphylococci and streptococci. This would likely hold true even in the post-hurricane setting.
• For initial antimicrobial treatment of infected wounds, beta-lactam antibiotics with anti-staphylococcal activity (cephalexin, dicloxacillin, ampicillin/ sulbactam etc.) and clindamycin are recommended options.
• Of note, recently an increasing number
of community associated skin and soft tissue infections appear to be caused
by methicillin-resistant Staphylococcus aureus (MRSA). Infections caused by this organism will not respond to treatment with beta-lactam antibiotics and should be
considered in patients who fail to respond to this therapy. Treatment options for these community MRSA infections include trimethoprim-sulfamethoxazole (oral) or vancomycin (intravenous). Clindamycin is also a potential option, but not all isolates are susceptible.
• Incision and drainage of any subcutaneous collections of pus (abscesses) is also an important component of treating wound infections.
Special Considerations Related to Contamination of Wounds by Water
Contamination of wounds with water (fresh
or sea water) can lead to infections caused by waterborne organisms. Though infections with these organisms are uncommon, even after floods, this possibility should be considered in patients who fail to respond to initial therapies described above. Water-borne organisms
often implicated in these infections include: Aeromonas spp., non-cholera Vibrio spp. and sometimes Pseudomonas or other Gram- negative rods.
Trimethoprim/sulfamethoxazole, amoxicillin/ clavulanate and newer fluoroquinolones (levofloxacin, moxifloxacin, gatifloxacin) will treat Aeromonas and the fluoroquinolones will also treat Pseudomonas and many other Gram- negative pathogens.
Clinicians should consider Vibrio as a possible causative organism of wound infections incurred in coastal waters or from contact with shellfish or marine wildlife. Vibrio vulnificus wound infections may require extensive debridement and mortality can be high.
These infections often manifest with bullous lesions that may be hemorrhagic. Persons with underlying hepatic disease or other immunocompromising illness are at highest risk of Vibrio vulnificus infection. When this infection is suspected, the recommendation is that patients be treated with a combination of ceftazidime and doxycycline.

The patented C-A-T® is a true one-handed tourniquet proven to be 100% effective by the U.S. Army’s Institute of Surgical Research. Tests proved that the C-A-T® completely occluded blood flow of an extremity in the event of a traumatic wound with significant hemorrhage.
The C-A-T® utilizes a durable windlass system with a patented free-moving internal band providing true circumferential pressure to the extremity. Once adequately tightened, bleeding will cease and the windlass is locked into place. A hook and loop windlass retention strap is then applied, securing the windlass to maintain pressure during casualty evacuation. The C-A-T®’s unique dual securing system avoids the use of screws and clips which can become difficult to operate under survival stress or where fine motor skills are compromised.

Sucking chest wounds require an emergency first aid solution that is fast, direct, and effective: the H&H Bolin Chest SealTM (BCS) is specifically engineered to meet these critical needs.
The BCS is a sterile occlusive chest wound dressing for treating open pneumothorax
and preventing tension pneumothorax that result from gunshots, stab wounds, or other penetrating chest trauma. The patented (US Patent No. 7,834,231) failure-proof triple-valve design of the BCS allows air and blood to escape while preventing the re-entry of either, thereby eliminating any unwanted gas or liquid exchange at the trauma site.
The large (6” diameter), rugged polyurethane disc structure of the BCS can cover practically any size chest trauma site. The temper of the disc prevents any disc wrinkling from occurring during application. In addition, the wound side of the disc is covered with a thick layer of jell-based adhesive, strong enough to not only seal over hair and blood but also flexible enough to be removed and reapplied to the trauma site if required.
In a recent evaluation, the Bolin Chest SealTM performed superior to the Asherman Chest Seal in overall performance.
Also, a 2013 study demonstrated that the Bolin Chest Seal was superior to an unvented/unvalved chest seal in managing a tension pneumothorax.
