358 My first personal encounter with Suthorn, was 2004 during the1st MIPO coursein Singapore. Even though I saw and listen to him several times before during the Davos courses, when hetalked inhisenthusiasticand decisivemode toremind theaudiencetostrictlyadheretothe AO principles. The mission to conform to the AOprinciples wasalways present when wehad the faculty pre course of the above mentioned first MIPO course. We had a lot of discussions among thefacultyabout bridge plating ofsimple fracturesusing indirect reductionand applying not absolute stability to preserve the biology. The discussionabout thehierarchyofabsolute stability vs biology was then in respect to the MIPO approach not clear yet. But fortunately, theliteratureresulting from several researchin the last 20 years proved with a good level of evidencethat the AO principles werealsotrue in MIPO, meaning that simple fracture may be treated witha bridge plating concept,however weight bearing,and healingmay be delayed or evenresult innonunion. Suthornstrongly believe that for MIPO approach in simple fractures, if applied need absolute stability, even if we must make a mini open reduction to obtain an anatomicalignment toobtainabsolutestability. Suthorn was one of the key promotors of the minimally invasive plate osteosynthesis in the AO,especiallyintheregionof AOAP, wherehe teaches the AO principles to improve fracture care, first among the Thai orthopedic commu- nity and there after in the whole AOAP region, after being taught and inspired by Prof. Hans Willenegger and Prof Siegfried Weller in 1977 as a resident in Bangkok. His innovative spirit felt thenecessity toenlargethetreatment port- folio for fracture care especially in countries, whereintramedullarynailing wasnotacommon procedure, especially in the AOAP region. His teamcould show that thistechniqueissafeafter studying itsapplicationonanatomicalspecimen and onfirstclinicalcases. Suthorn wasnotonly animportant promotorof thistechnique, buthe was devoted toteachit inasafeand reproduc- ible way.Therefore, theMIPO courses werethe firstcoursesinthe AO whereasafeapplication ofminimallyinvasiveinserted platesweretaught using anatomicalspecimens,after learning the processof reductionandmaintaining reduction on foam models. But the worldwide spread of this new method based on courses needed faster promotion, that’s why Suthornand a peer group of AOAP surgeons from Thailand, South Korea,Japan, Singaporeand Hongkong wrote the first Edition of the Minimally Invasive Plate Osteosynthesis(MIPO) bookin2006 which was 6yearslater followed bythesecond expanded edition, whereI wasinvited together with Rodri- go Persantes from Colombia to work intensely together with Suthorntorealizethis book. Thevastexpansionof indications duetothefact ofnew anatomical plate designsand improved instruments, developed by the MIPO working group of the AO, madethis2nd Editionneces- sary. It was due to this work I had the chance to get toknow Suthornnotonlyasa dedicated teacher but also as a motivator and demand- ing project manager. 359 This experience is from my perspective typi- cal for the teacher, the mentor and the human Suthornwho’sMissionwasand still istoimprove fracturecare byteaching and bringing inallhis personaleffort toachievethese goals.Firststart- ing to apply the AO principles inside the Thai communitythenexpand it into AOAP regionwith allhisenergyand becoming thefirst president representing AOAP of the AOfoundation. His mentorship brought up new dedicated AO surgeons like Theerachai Apivatthakakul and Vajara Phiphobmongkoland others, with whom I worked together in different committees and in the MIPO working group, where we had the opportunity to develop MIPO instruments and the MIPO courses whichtook placeallover the world. Evenweneveroperated together, Iexperienced Suthornasfairand supportiveteacher,whowas dedicated toimproveandmaintainthe qualityof operativefracturecare. Ihad thechancetovisit severalhospitalsin Bangkoktogether withhim. In grand rounds we discussed the x-rays from operated patients withthehospital teamalways in a supportive way but with a clear statement whentheapplicationof the AO principles were notas well takenintoconsideration. A typical story which characterizes Suthorn’s dedicationforhismissiontoimprovethe quality of careinthis domainand hisnatureasareli- ablefriend.Theuncleof theresident inmyhos- pital, had a subtrochanteric fracture treated in ahospital inThailand. Hisunclesent thex-rays to my resident toask whyhehad still painand wasnotableto weight bear6 monthsafter the operation. Hesuffered from a nonunion due to animperfectosteosynthesis. Isent thex-raysto Suthorn,and 3 dayslaterhe went tothehospital wherethis patientwasoperated and reoperated on this patient together with the surgeon who did thefirstoperation.Thecase went well,and the patientcould walkagainafter12 weeks. All personal encounters with Suthorn were en- riched bythe warmand heartful presenceofhis lovely wife Sinaporn. She was always present support during his time abroad. I gladly like to recall her open empathic character and her warm heart wherever I meet her, experiencing herhospitalityatherhomeinThailand orabroad onnumerous meeting and courses. . One of thestrongestcharactersand “drivers”of this movementcertainly was Suthorn! I wasalways impressed withtheincrediblecasesas wellas thecreativityand passionhe demonstrated in solving complex situations - despitefrequently only limited availability of resources - I had really never seen being trained in a very wellequipped German Trauma center. With both of us moving up the AO ladder – him always being someyearsahead- wetookon moreand moreresponsibility, Icontinued and until today remain to be impressed by Suthorn’s passion for thesubject matter, particularlyhis wiseand integrating attitudefor the benefitofeverybody involved. In Hungary during the MIO course and the Hun- garianTrauma Congressin Budapest with Reto Babst,Tim Pohlemann, Endre Varga and Zolst Balogh. But probablytheonething that I remember the most was whenyoucalled meand asked meif I was willing to join you as part of the editorial teamfor theMIPOBook2nd Editionwithyouand Reto Babst, it wasoneof the bestexperiences in my life, you were a true friend and a mentor to me, leading me to understand how to workasaneditorand authorona book. 364 Our first meeting in Davos 1978 In 1978, as a young Orthopedic surgeon, you attended your first AO Course in Davos and I had the pleasure to meet you and discuss the importance of teaching also ORP this new AO technique. You were genuinely interested and wanted to know how this could be best per- formed. As the first Director of ORP Education of AO International (1977-1993) I had started a pioneering work to build up a curriculum for teaching ORP.Thisincluded course programs, teaching material,hands-on work materialand fellowships. Alreadyseminarsand courseswere organized onaregular basisinmanycountries, sometimes in conjunction with AO courses for surgeons. It was obvious that you understood thenecessityofcontinuoustraining notonlyfor surgeons, but for the whole surgical team to ensurethe bestcarefor the patient. Introducing ORP in Thailand into the AO World 1978 – 1994 In1979, following aninvitation by Mr. Dusitand Mrs. Varanya Watchatonat Bangkok Unitrade, I visited Thailand for the first time and spoke about AO at a seminar for OR nurses. Despite somelanguage problems, theinterest inlearn- ing this new technique was obvious. Thanks to your enthusiasm, encouraged by AO Inter- national and supported by your superiors you started a training program for a group of ORP nursesat Maha Vajiralongkorn Building at Pra- mongkutklao Hospital. Under your supervision many hours were spent on learning the use of instruments, AO technique and how to speak infrontofanaudience. Finally, in1985thefirst AO coursefor ORP was organized and nursesfromotherhospitals were invited toattend. Next year thesecond course took placeand I was invited asafaculty. The Thai nurse faculty, trained by you, made their presentationsand led hands-onskillslabs with great confidence and professionalism. I was truly impressed and pleased to see their eagerness to improving their teaching skills. Asia-Pacific AO Courses for ORP 1988 - 1994 After the success with ORP courses in Thai- land, the interest increased for similar training also in other countries in the Asia region. My responsibility wastoinitiatethetraining forORP together with the local faculty in conjunction with surgeon courses. Since you were part of theinternational AO faculty,youalsohelped at the ORP training whenever possible. This was muchappreciated bythe participantsnotused to get muchsupport from theirownsurgeons. The combined surgeons and ORP courses meant a lot of work for the faculty, but the response from the participants was always
365 rewarding. It also gave us opportunities to get toknow colleaguesfromthe differentcountries, such as Malaysia, India, Taiwan, Korea, Hong Kong, Japan, Singaporeand Indonesia. Youhavecontinued your workasasuccessful orthopedic surgeon,a constructive member of the AO Foundation Board in numerous po- sitions and also as its creative AO President 2014–2016. The Thai – Sweden Link 2000 - 2021 Tomethe“Land of the Rising Sun” became afavouritealreadyonthefirstvisit in1979. I was met by outstanding kindness and generosity by you and Mr.Dusit and Mrs.Varanya. I was introduced to exquisite Thai food and taken to fascinating placesinand around Bangkok.This unexpected hospitalityand having the privilege to get toknow youand your family,madereturn- ing toThailand feel likecoming home. Alsoafter retirementmyhusband and I returned to Thailand several times as tourists and golf players. Whenever possible we visited you and Sinaporn at your home in Bangkok or we met in Hua Hin, Chiang Mai or Rayong. Many unforgettablehours werespent inyour inspiring company, sharing old and new events in life. These werespecialoccasions when wevisited Thailand. Being an AOBoardmemberyouhad totravel to Europe on several occasions. A few times you were able to make a stop over in Sweden and meet with Bo and me. In June 2001, Sinaporn and youcametovisitusforacoupleof daysat Brokind Estatesouthof Stockholm.Our land lord invited youona guided touraround theestate, followed bya Swedishstyle garden party. Next day we went tosee Göta Canal withits58 locks, built inthefirsthalfof1800 BC. It is part ofa390km long waterwayacross thecountry, from the west coast to the Baltic Sea on the eastcoast.
366 In 2006, on June 7, we met in Kiruna, a city north of the Polar Circle in Lapland, known for mining industry,spacecentresand theIcehotel. You landed unexpectantly at 11 pm in broad, becauseof the midnight sun. Afterspending the“bright”night withlittlesleep inatypicalsapmihut, we wentona guided tour intheLKABmine, whichhasthe world´slargest underground iron mineral mine to a depth of 1045m. Becauseof thecontinuousundermining of thecity, ithad been decided to movemostof thehouses inthecity toanearbyarea.Today, this is about to be finished and mining contin- ues.Thefollowing dayyou weretakenona dog sledgeridefora“Lapland style” picnic byTorne River, insummer temperaturesof +15 degrees! Next place to visit was the site of the unique Icehotel. This hotel is rebuilt every winter, with icefrom theriverand decorated by several in- ternationalartists. Insummer,someof theigloo bedroomsand sculpturescan beseeninaspe- cialhall, whichholdstemperatures below zero. Our Nordicexcursion continued to Stockholm, flying 950kmsouth. Aftersightseeing inthecity to give you a glimps of the capital of Sweden, we visited The City Hall, famous for its grand ceremonialhalls.Thisisthevenueof the Nobel Price banquetheld on Dec.10every year. In the evening, we embarked the Ferry to Hel- sinki, passing throughthearchipelagoin broad daylight, arriving next morning in the capital of Finland.The cityisknownfor itsarchitectureand hasoneof the world’shighesturbanstandards of living. Insummerly weather we madeasight seeing tour of the city, before embarking the Ferry back to Stockholm in the evening. After arrival next morning it was time to say farewell and youreturned to Bangkok. Our contacts continued and I have been able to follow your career at a distance in Sweden. In2008, thankstoyour initiative, I wasinvited to the AO 50thyearcelebrationin Davos,asalate recognitionofmy pioneeringworkforORP. Itwas sucha great pleasureand reward to meetalot ofold AO friendsand recall the good old times. In2014,you weretaking over the positionasthe AO Foundation President at a Trustee meeting in ChiangMai inthenorthofThailand. Youkindly invitedmeand BototheTrusteemeeting, which was a much appreciated event. Since you and Sinaporn were hosts, it meant unforgettable days in a heart warming and generous Thai atmosphere and another great opportunity to meetallThaiand AO friends. Given the Asian background and fa- miliarity with people and problems Asia- already an emerging part of the world-received an additional impetus and importance through Suthorn‘s presidency. A highlight and historic achievement was the signature with the industrial partner inJune2015at theoccasionat the Annual Trustee Meeting in Chang Mai. In honor and gratitude to Suthorn the Agreement wasinofficiallyreferred toasthe Chiangmai Accord. 368 70th year Birthday of Suthorn Bavonratanavech My dear friend Suthorn, It is in the late 80, when Prof. Hans Willenegger,oneof thekeyfoundersof the AO mentioned tomethat therewasayoung dynam- icsurgeon,alsoanOfficer intheThaiarmy, who wasveryactiveinthe Asiancountriesandwould most probably have a bright future. His only handicap washislastnamethatnobodycould spell rightor remember. Sothe AO decided to call him by his first name „Suthorn“ which was easier for everybody. When we first met at an AO Coursein Davosyou musthave beeninthe late thirties. We immediately understood each other welland wereonthesame wavelength. In ZellamSeeat theTrusteesMeeting, Sinaporn and Susy met the first time and immediately became friend, a very close friendship which lastsuntil today. In1987the AOFoundation decided toestablish an AOAlumni Associationfor former AOFellows and participants of AO Advanced Courses. Suthornimmediately grasped the big potential and was very instrumental in establishing the Thai Alumni Chapter which wasoneof themost active group bothintermsof membership and activities. Since weunderstood eachothervery well, itmadetheimplementationofnew projects mucheasier. Onthe basisof thevarious AOAlumni Chapters which had been established in the meantime, a larger regional group, namely AO East Asia could be established thanks to the support of Suthornin1994. Onthe wayto SICOT, Seoul in1993, wherethe AOFoundationalwayswas presentwitha prom- inent scientific booth, we stopped in Bangkok and enjoyed the wonderful hospitality of the Suthornfamilyat theirhome. We willalwaysre- member thetaxi driver fromthe Synthesagency to Suthorn‘shomein Bangkok withtraffic during 3 hours where we almost suffocated. Suthorn was guiding the driver through phone „krub, krup, etc.“ in order to finally find his home to everybody relieve. When the AO Foundation wanted to organize it’s firstTrustee Meeting in Asia, it wasobvious that Thailand was one of the first option. So in 1996 we travelled together with Suthorn and Sinapornto Phuket toinspect thesite, thehotel and establishthe program.Thefollowing Trust- ees Meeting in 1997 was a memorable event inthe beautifulsetting of theLe Meridianhotel. Fortunately we changed the ship that we first wanted tocharter,sinceitsankinthemeantime. After thateverything worked out fine. We have enjoyed numerous AO meetings, AO Courses and Trustees meetings all over the world and were always impressed by your originalappearanceswith Sinaporn, liketheone below in Venice. In2000after theTrusteesMeeting in Vancouver we had organized an optional Alaska Cruise for the Inside Passage. Out of the 96 AO pas-

Youlearned to Choca-la, Choca-la…..! In2004Thailand washeavilyhit bytheTsunami whichcaused 230‘000 deaths inthe Andaman Sea. The Le Meridien Hotel was heavily dam- aged, all the lower facilities were destroyed, but fortunatelythere werenocasualtiesneither guests nor Employees. The Rescue Team of the AO got in contact with Suthorn and other surgeons of the area to find out how we could help. External Fixators were required. The problem insuchasituationisto makethelocal logistics work. Thanks to Suthorn’s excellent connectionstothe AirForce,he wasableto get the Ext.Fixator throughcustomsand undertook personallyto distributethemtoseveralhospitals in Phuket. A great thank you!!! Afterhaving worked throughall thestepsof the AO Foundation’s hierarchy, you became Pres- ident of the AO Foundation during 2014-2016. He planned to walk the short distance to the AO Center with his staff thenext morning. He was surprised when he was picked up by a shuttle bus and asked why. He was told that there wasno pedestrian pathonthis stretchof road and thathe would rather be picked up by car soasnot toendangerhim.
370 Dr. Suthorn wasabletoseeemployees going to work on this main road to the AO Center. Most of the people affected were AO Fellows from abroad, including students from Thailand. On thisnarrow sectionof road of440m,cars, buses and trucks drive pass pedestriansat thespeed limitof80km/h. Especiallyin winter wedged by snow wallsand early darknessonthisunlit road a great danger for the AO staff. Dr. Suthorn was shocked and set himself the goal tochangethisimmediatelyand tocreatea safe wayto workforevery AO Centeremployee who wanted to walk to work by the end of his presidency in 2016. He asked Past President Prof. Dr. Med. Peter Matter for support in this project.Together theyintervened thiscondition and could convince the AOEM (AO executive Management) of the necessity of a footpath. Thisidea wasalsosupported bythenew COO/ CFO Irene Eigenmann and Executive Director Geoff Richards. In December 2016 it finally happened, on the occasion of the AO Davos courses AOF Pres- ident Dr. Suthorn and Past President Prof. Dr. Mattercould enjoythenew safe pedestrian path tothe AOCenter whichwasrecentlycompleted. My relationship with Suthorn beganin1999 whenI waselected as a very young trustee for the AO Foundation as it was known at the time,he wasafriend and supporterof the Oceaniaregionand saw its close links with Asia, long before the regional development of AOT Asia Pacific. He has always been selfless with his time and support, coming to New Zealand as a faculty member and always looking out for meat the many AO and AOT meetingsand courses weattended. When myownfamily visited Thailand with meas part of one of the Chiang Mai courses he went out of his way, through his family travel agent connections to make sure we had the most amazing trip through the “real” Thailand which will be in the mem- oriesof meand mychildrenforever. He is willing to con- sider all options in the name of progress and these pictures probably demonstratetothat. I’m not sure getting in a hot air balloon was what he wanted to do but he was willing to join his colleaguesfor the greater good and enjoyment of all. As many people know,heisa good traumasurgeonaswellasan excellent teacher.Fromthehistoryalbumsofmy AO life, some pictures are picked out to share with you my memories of our relationship and hiscontributionto AO development inJapan. The AO activity (thefirst AO Course) started in 1987and hisfacewasrecognized asearlyas on a group photoof the1988 Course(bycourtesy of Secretariatof AO TraumaJapan).This means the progressof AO inJapan was supported by Suthornfrom its veryearly stage. Theyear1998isthe memorableyear forJapa- neseAOsurgeons,since AOAlumni Association JapanChapterwasinaugurated inthatyear.The group photoshowed the presenceof Dr.White, the President of AOAA, with many godfathers of AO including Suthorn,celebrating theevent. I became AO Trustee in 1999 and made my debutatTrusteeMeeting in Vancouver in2000, supported byother AsianTrustees. Asian group meeting at AO Trustee Meeting in BadRagaz, Switzerland (June2001).Japanwas not a member of AO East Asia (AOEA) at that time, and I was eagerly asked to join AOEA at every opportunity. It took a while to persuade Japanese AO members, but thefollowing year, Japanfinally becamea memberof AOEA. Professors Sawaguchi, Shindo and I were in- vited to the General Assembly of AOEA held in Seoul, Koreain October, 2002 where Japan formally declared theaccessionto AOEA.
375 The pictureshowed arelaxing, good timespent at the dinnerafter thetense meeting. After joining AOEA, wehad manyopportunities to attend AO activities in Asia. (AOEA Meeting in MacauinFebruary2003). Wealwaysenjoyed pleasant time whenever the opportunity arises. This shows a gathering of Asian surgeons at lunch time with Prof. Perren (AO Davos Courses, December2008). Trustee Meeting in 2012 was my memorable event. It was the first and the last Trustee Meeting accompanied by my wife. It was nice trip spent with my good friend, Suthorn and Sinaporn. (Davos,2012). Suthorn is a person of varied attainments. He wasa great mentor toJapanese AO surgeons not only in AO Courses but in other activities suchas Educator’s Seminar. My journey with Prof. Suthorn — who I always have looked up to as my “KUYA” (Filipino for “Big Brother”) started 31years ago as a fresh graduateofOrthopedicresidencysent toattend a basic course in Davos, December 1989. It was my first travel abroad and I was still naive about what AOmeantorstood for.There wasno “ProfessorGoogle”at that timesoithad seemed morelikeatravelopportunitythananything else. Compounding my situation was the political instability my country was experiencing at that time whichcausedmeto beseparated fromthe group which was to initially meet in Tubingen with Prof. Weller. Surviving anexciting first trip abroadwithcancelled hotel reservations, Inone- theless managed to make it to Davos. There, I experiencedmyfirst tasteofsnowwhile dressed in a three-piece suit for the first day—which I quickly recognized was not the appropriate dresscodefor thecourse. Feeling likea provincial from thecountryside, I wasimpressed bythe power toolsand precise methodologyof internal fixation. Iknew forafact that in my entire residency, I had always used ahand drill withlimited choicesforscrewsand implants. However, as I was feeling a certain sense of inferiority compared to most of the participants present, a fellow Asian in a color- ful sweater approached me and tried to make thingseasier for me by patientlyexplaining the tableexercises. In fact he was the only Asian table instructor around—that was my “Kuya” Suthorn. I soon learned thathe wasaThainationaland I read- ilyunderstood why inall theexercises theThai delegationwasnotonlyoneof the biggest Asian delegationsat that time butalsooneof thefast- estand mostaccuratein performing exercises. After four daysofmyfirstconnection with AO—I promised myself that this would not bethelast.
380 Destiny again crossed our paths, as I was on the way home, taking the train from Davos to Zurich. There, I sat beside Suthorn and keenly absorbed hisownlovestory with AO. Hetalked about how he was tasked to make use of the new AO power instrumentation Thailand had acquired and teach the residents to use it. As Suthorn discussed the manner and program- ming ofhis“local’ AO courseand matterssuch aslimited instrumentation,sequential grouping and alternate didactics—it wasno wonder that this wasallsecond naturetohimasheimparted thisknowledgetotheearlycoursesintheregion. Withahunger toknow moreabout AO—I land- ed an AO fellowship program at the Singapore General Hospital under the guidance of Prof. Tong G On and his team. And as I wasabout to wind up my fellowship program, I was invit- ed to dinner by Prof. Tong and guess who the third guest was…it wasnoneother than“Kuya” Suthorn! I marveled as the pair shared their twoloves… AO and Good Food. I had no idea why I was invited tothe bigwigs’meeting and soIsat there quietlyenjoying their discussionsand chitchat. When we finished the main course though, Suthornthen popped themarching order forme toorganizea basiccourse withhim inthenext two years. I was startled and explained to him that Iwasstillajuniorconsultantwithmanymore seniors ahead of me. He then reminded me of how hestarted:start withsmallsteps,organize properlyand thenlet the boss know the plan. Thus started our frequent emailsand planning from short Manilavisits for AO seminars tolec- tures during annual conventions. This laid the ground workfor thefirst AOBasic CourseinMa- nila.“Kuya”wasright that itcould be done, Isaid tomyself.However, thismajor breakthroughwas unfortunatelymetwithaten-yearhiatus because the Philippine had no local Synthes partner at that timeas wasthecaseinThailand and Singa- pore. Prof. SuthornknewIwas disappointedwith this development. Nevertheless, his pep talks neverended;hecontinued toencourage meto lookforalocal industry partnerand continuously get involved intheregional AO activities. All the while, milestones were being achieved withhis leadership —from setting up AO East Asia to AO Trauma Asia Pacific. Therefore, it isnotsurprising atall that with Prof. Suthorn’sinvaluableassistance, the Philippines wasabletosetup itsfirst AOPrinciplecoursein 2006,and lateranaggressivetraining program for local facultywhichlaid the groundworkforan advanced coursein2008—allunder thesupport and planning of Professor Suthorn. Prof. Steven Schelkun was the international chairman of that course, with the local chair- man being ourveryown Prof. Suthorn who was indirectly telling the faculty with pride, “This is myteam.”The greatestacknowledgement that thiscoursereceived wastheassessmentof the international chairman with the following state- ment: “I have recommended to Piet De Boer, Head of Education that the Philippine Ortho- pedic Surgeons are well positioned for further educational opportunities for future courses both Principlesand Advances.”Fromthat point onwards, AO Trauma Philippines realized that the AO Spirit was with us and that we were on the way to progress tothe pointof realizing an AO Cadavericcoursein2018.
381 His impact on orthopedic training in the Phil- ippines can be clearly seen in the different training institutions where many of the leaders and faculty are graduates of the mentorship program of AO. One example is Dr. William Lavadia, Co-chairof thefirst Advanced Course. Hisinnovationsinthe Philippine Board ofOrtho- pedicsas Chairman—suchas theintroduction of the OSKE—wastheresultofourexposureto Prof. Gautiers’ AO Playground. Prof. Suthorn was not only a great mentor, but a true friend willing to tell you outright what is right and wrong. There are times where one would find himatoddswithsome people, butas Iused tojoke withhim—that is whyhis favorite karaokesong was “My Way.” We have both matured in regional circles with notonly bothofustaking the presidenciesofour respective countries’ orthopedic associations but also both of us managing to assume the presidency of the ASEAN Orthopedic Associ- ation. It was indeed a great honor that it was “Kuya” Suthorn who put the medallion of the ASEAN Presidency on me during the turnover rites.Thisclose bonding wasnot limited tothe two of us, as our wives became the best of friends who would get together while Suthorn and Iattended the AO Meetings. There are many lessons that I have learned from Prof. Suthorn and these have guided me up tothis dayinmentoring and planning educa- tional activities. Please allow me to enumerate them as Iend my story: PEARLS OF WISDOM FROM PROF. “KUYA” SUTHORN 1. Never Give Up. There is always a way. If you don’t try you will never find it. 2. It’s not the luxurious accommodations—it’s the faculty and participants which are im- portant. 3. Cost is a matter of programing properly ahead of time. 4. You need a three-tier level to assure conti- nuity of your programs. 5. No matter how high you rise you must keep your feet on the ground. 6. Value true friends and family—most espe- cially your wife, who make sure you run well so the program will run well. 7. Not all things may be the way you want it but at least you tried your best in doing it. 383 I and Suthorn were invited to the HK Orthopedic Association Meeting and COA via AO in 2006. After the HKOA meeting I wastraveling with Suthorn and Frankie to Beijing to joinCOA. Asyoucaneasilyrecognizein these photosI wasayoung and novice facultywhowasnervousonthe podium. When I look back, I was so~lucky and fortunate to have that opportunity to spend fewdayswith greatvisionaryand enthusiasticteacher / leader. It’s not that Suthorn taught me how to be a good faculty in details but it was ratherhisvisionand enthusiasmfor AO impressed methe most. This was a very special travel for me because this was first time I get to know Professor Wang Manyi and Wei Jiethrough AO. This wasoneof myturning pointsin my AO career ! From 1996 to the 2020’s,hehas beenafixtureinourannual AO Trauma Courses, tirelessly giving of his time to be course faculty, giving lectures, leading group discussions, and guiding us through the practical exercises. Professor Suthorn has particularly left his imprint on the “Violations of AO Principles” lecture, whichonly he can give withthe moralauthorityneeded. “Income must not be moreimportant thanoutcome!”
384 Asayoung local facultyinourcourses, Ialways looked forward to Prof Suthorn’s participation inourcourses,soaking up the pearlshe would sharenot just tothe participants, but tothefac- ultyas well. Hehasalways beenapproachable when asking for help in preparing a lecture or getting feedbackon how a particular practical exerciseor group discussion was run. In2010Ihad myfirstopportunityto beinterna- tionalcoursefaculty, being invited to beatable instructor in Davos. Although I had travelled therewith SirMiles,whostayed brieflyforaCDC Meeting, the rest of the time I was on my own, had not Prof Suthorntaken meunderhis wing. He would take me out to Chinese food dinner, introduce me to the other faculty, and was a well-spring ofadviseforanovicefacultyalbeit a lowly table instructor. I was not surprised at all thatevenin Davos,everyone wasfascinated byhis “Violations” lecture. Later, even when he had ascended tothe pin- nacleof the AOTraumaleadership, Prof Suthorn still remembered us, asking for updates on how thesituation wasinourcountryand giving cleveradvise. In 2019, having the opportunity to be invited as regional faculty at the Bangkok AO Trauma BasicCourse, Iwas pleasantlysurprised toonce againmeetup withthe professor,whoremained very generous withhis timeand thankfulofour participationinthecourse. Healsotookthetime totourusaround hishospital facilityand joinus for thefaculty dinners. The AO Trauma Philippines will forever be thankful to Prof Suthorn for his support, lead- ership, mentoring, and friendship to us. 387 ตลอดระยะเวลาของการเรียีนและเป็็นแพทย์์ในสาขา ออร์์โธปิิดิิกส์์ สิ่่�งที่่�นายแพทย์์สุุทรตระหนัักถึึงอยู่่เยู่่สมอคืือปััญหา ของเคสผู้้ป่่วยกระดููกหัักที่่�มีีมากมายหลายแบบ แต่่การรัักษา ยัังอยู่่ในรููปที่่�จำกัำดัและโดยเฉพาะเมื่่�อหวัังผลที่่�จะเกิิดขึ้้�นกับัผู้้ ป่่วยหลัังการรัักษาแล้ว ้อาจยัังไม่่ได้ผล้เต็็ม100%อย่่างที่่�ควรจะ เป็็น ให้้เกิิดปััญหา กระดููกหัักในผู้้สููงอายุุจากโรคกระดููกพรุุนเพิ่่�มขึ้้�น แต่่เมื่่�อลงมืือ รัักษาแล้ว้ในหลายๆ ครั้้�งที่่�ผลของการรัักษาไม่่เป็็นที่่�พึึงพอใจ กับทั้้�งแพทย์์และผู้้ป่่วย จึึงทำำ ให้คิ้ิดอยู่่เสมอว่่าจะมีีการรัักษา แบบใดหรืือเทคโนโลยีีอะไรบ้้างที่่�ให้ผล้ทางการรัักษาที่่�ดีีขึ้้�น และเรียีนรู้้ในการแก้้ไขปััญหาให้้การผ่่าตัดัให้ดี้ีขึ้้�นในรายต่่อไป แต่่แล้วสิ่่ ้ �งที่่�อาจจะเป็็นคำำตอบก็็ปรากฏขึ้้�นตรงหน้้า ใน พ.ศ.2520ขณะที่่�นายแพทย์์สุุทรเป็็นแพทย์์ประจำบ้ำ ้านปีีที่่� 2อาจารย์์ธำำรงรััตน์์แก้ว้กาญจน์์ได้สั่่้�งซื้้�อเครื่่�องมืือผ่่าตัดัเข้้า มาใหม่่เมื่่�อเปิิดออกดููจึ ึงพบว่่าเป็็นเครื่่�องมืือของ AO ที่่�ยัังไม่่ เคยเห็็นกัันมาก่่อนและไม่มี่ ีใครรู้้ว่่าอุปุกรณ์์แต่ล่ะชิ้้�นใช้ทำ้ ำงาน อะไรบ้้าง อาจารย์์ธำำรงรััตน์จึ์ึงมอบหมายให้นำ้คู่่มื ำคู่่มืือ Manual of Internal fixationไปศึึกษา ซึ่่�งเมื่่�อได้ทำ้ ำการศึึกษาแล้ว ้ความ รู้้สึึกเชื่่�อครึ่่�งไม่่เชื่่�อครึ่่�งก็็เกิิดขึ้้�น และเกิิดเป็็นคำถำ ามในใจว่่าผล ของมัันจะดีีจริิงอย่่างที่่�ในหนัังสืือบอกไว้หรื้ ือเปล่่าแต่่จะหาคำำ ตอบจากใครคงไม่่ได้้นอกเสียีจากหาคำำตอบจากการอ่่านคู่่มื ือ ที่่�เสมืือนคััมภีีร์์เล่่มนี้้�ไปพร้้อมๆ กับัทดลองผ่่าตัดัใช้้เครื่่�องมืือ ด้วยตั้วัเอง ในปลายปีีพ.ศ.2520 โอกาสแห่่งการเรียีนรู้้และหาคำำ ตอบก็็มาถึึง เมื่่�อมีีการจัดปัระชุุมและบรรยายจาก Professor Willenegger ซึ่่�งเป็็นประธานของ AOInternational ที่่�ก ที่่รุุงเทพฯ นายแพทย์์สุุทรซึ่่�งขณะนั้้�นเป็็นแพทย์ป์ระจำบ้ำ ้านอายุุเพีียง 25 ปีี ก็็ได้มี้ีโอกาสเข้้าไปร่ว่มฟัังอยู่่ด้ วย ซึ่่ ้ �งหลัังจากที่่�ฟัังแล้วก็้ ็เกิิด เป็็นคำถำามว่่าAOTechniqueจะเป็็นวิธีิีใหม่ที่่�ช่วย่แก้ปั้ัญหาใน การรัักษาผู้้ป่่วยกระดููกหัักให้้ได้ผล้การรัักษาที่่�ดีีขึ้้�นกว่่าที่่�ใช้้อยู่่ ในขณะนั้้�นจริิงอย่่างที่่�บรรยายหรืือไม่่ ซึ่่�งถ้้าเป็็นจริิงนี่่�ก็็จะเป็็น ทางเลืือกสำคัำ ญที่่ ั �จะต่่อยอดให้ทำ้ ำการรัักษาผู้้ป่่วยโรคกระดููก หัักให้้ได้ผลดี้ ีอย่่างมากมาย ด้วยนิ้ ิสัยที่่ ั�ถููกหล่่อหลอมมาตั้้�งแต่่สมัยที่่ ั �เรีียนแพทย์์ ที่่�ว่่า “หากจะรัักษาคนไข้ต้อ้งเข้้าใจอาการอย่่างถููกต้อ้ง และ หากจะทำกำารรัักษาต้อ้งรู้้ว่่ าจะรู้้ว่่ รัักษาอย่างไ ่รและไม่่ลืมมอื งไป ถึึงผลที่่ไ�ด้รัั้บ” ดัังนั้้�นแม้้ในการบรรยาย AO Technique จะดูู เหมืือนว่่าเป็็นเทคโนโลยีีที่่�ดีีแต่่นายแพทย์์สุุทรต้้องการมั่่�นใจ มากขึ้้�นอีีกด้วย้การพิิสููจน์์ความจริิงให้รู้้แน่ชั่ดด้ัวยตั้วัเอง ซึ่่�งคง ซึ่่ ไม่มี่ ีใครให้คำ้ ำตอบได้ ห้ ากไม่่ได้้เดิินทางไปเข้้าคอร์สกั์บ ั AO ที่่� เมืือง Davos ประเทศสวิิสเซอร์์แลนด์ อั์ ันเป็็นที่่�ตั้้�งของ AO ที่่� ใฝ่่รู้้เพื่่อ�ต่่อยอด เป็็นกลุ่่มองค์์กรทางการแพทย์์ที่่�ทำำการศึึกษาวิิจัยัเกี่่�ยวกับัการ ผ่่าตัดยึัดึตรึึงกระดููกที่่�ก่่อตั้้�งขึ้้�นตั้้�งแต่่คศ.1958 ซึ่่�งเป็็นผู้้คิิดค้้น AO Technique ขึ้้�นมาและเริ่่�มทำำการสอนตั้้�งแต่่คศ.1960 ที่่�เมืือง Davos หลัังจากสอบได้วุ้ฒิุิบััตรออร์์โธปิิดิิกส์์ในปีีพ.ศ.2521 นายแพทย์์สุุทรได้มี้ีโอกาสไปเข้้ารับัการอบรม AO Course ที่่�เมืือง Davos ประเทศสวิิสเซอร์์แลนด์ ร่์ว่มกับัอาจารย์์ผู้้ใหญ่่ จากโรงเรีียนแพทย์์อีีกหลายท่่าน โดยอาจารย์์ธำำรงรััตน์์ได้้กรุุณาหาค่่าใช้จ่้่ายในการเดิินทางให้ ส่้ ่วนค่่าลงทะเบีียนกัับ ค่่าที่่�พัักทางบริิษััทบางกอกยููนิ ิเทรดและบริิษััทMathys ประเทศ สวิิสเซอร์์แลนด์์ได้ช่้วย่กรุุณารับผิั ิดชอบให้้ การได้้ไปประชุุมในครั้้�งนั้้�นถืือเป็็นการเปลี่่�ยนแปลง ชีีวิิตการทำำ งานในสาขาออร์์โธปิิดิิกส์์อย่่างสิ้้�นเชิิง เพราะ นอกจากหน้้าที่่�ของความเป็็นแพทย์์ผู้้ทำำการรัักษาผู้้ป่่วย กระดููกหัักแล้ว ต่้ ่อมาในอนาคตยัังต้้องเดิินทางไปประเทศ สวิิสเซอร์์แลนด์อี์ีกหลายครั้้�งเพื่่�อเข้้าคอร์ส ป ์ระชุุม ฟัังการ บรรยายและทำำความเข้้าใจเรื่่�องของ AO Technique อย่่าง ลึึกซึ้้�ง โดยการประชุุมที่่�ได้้ไปเรียีนรู้้ในครั้้�งแรกนั้้�นจะแบ่่งออก เป็็นภาคทฤษฏีีซึ่่�งเป็็นหััวข้้อที่่�ค่่อนข้้างใหม่่และยัังถกเถีียง กัันในตอนนั้้�น และภาคปฎิิบัติัิโดยการใช้้เครื่่�องมืือจริิงในห้้อง ผ่่าตัดัมาหัดปฎิ ับัิติักัิบักระดููกเทีียม ทำำ ให้้เข้้าใจและเห็็นผลของ การกระทำำ ได้ทั้ ันทีีเนื่่�องจากในการผ่่าตัดัจริิงไม่ส่ามารถเห็็นได้้ ทั้้�งหมด สิ่่�งสำคัำญคืั ือการเรียีนรู้้ว่่าทำำอย่่างไรให้ถูู้กวิิธีีจริิงๆและ ถ้้าหากไม่ถูู่กต้้องก็็ควรจะรู้้ถึึงสาเหตุุและแก้้ไขเพื่่�อไม่่ให้้เกิิดขึ้้�น กัับผู้้ป่่วย นั่่�นทำำ ให้ต้้องตั้้�งคำถำามตัวัเองอยู่่เสมอว่่า WHAT, WHEN, WHY และ HOW ในการผ่่าตัดั หลัังจากเข้้าอบรมที่่�เมืือง Davos นายแพทย์์สุุทร ได้้เดิินทางไปดููงานต่่ออีีก 2 สัปดัาห์ที่่์�AOClinic ที่่�UlmUniversity ในเมืือง Ulm ประเทศเยอรมัันนีี ซึ่่�งผลจากการได้้ไปสััมผัส ั AO Technique ด้วยตั้วัเองยิ่่�งทำำ ให้รู้้สึึกมั่่�นใจว่่า AO Technique คืือทางเลืือกที่่�ดีีที่่�สมควรจะนำำ ไปใช้้ในการผ่่าตัดรัักษาผู้้ป่่วย กระดููกหัักต่่อไป และเป็็นสิ่่�งที่่�ควรได้รั้บัการเรียีนรู้้อย่่างจริิงจััง เพราะนอกจากผลที่่�ดีีแล้วยั้ ังช่่วยให้้คนไข้ส้ามารถกลัับไปใช้้งานได้้อย่่างใกล้้เคีียงปกติิและเป็็นการเพิ่่�มทางเลืือกในการ รัักษา ที่่�สามารถพััฒนาไปไกลในอนาคต ซึ่่�งนั่่�นหมายความ ว่่าวงการแพทย์์ไทยจะก้้าวหน้้าขึ้้�นและเป็็นที่่�ยอมรับัในระดับัสากลได้้ หลัังจากกลัับมาจากการประชุุมที่่� Davos นายแพทย์์ สุุทรได้้นำำความรู้้ที่่�ได้้กลัับมาใช้้ฝึึกฝนจนชำำนาญ แต่่เส้้น ทางการริิเริ่่�มสิ่่�งใหม่่ย่่อมไม่่ราบเรียบีเป็็นธรรมดา การต่่อต้้าน และคัดค้ั ้านไม่่เห็็นด้วยกั้บัเทคนิิควิิธีีแบบ AO จากศัลยั แพทย์์ บางท่่านที่่�ยัังไม่่เข้้าใจอย่่างถ่่องแท้ซึ่่้�งถืือได้ว่้่าเป็็นเรื่่�องธรรมดา ของวงการแพทย์์แต่ด้่วย้ความมุ่่งมั่่�นและเชื่่�อเสมอว่่า“หากยััง คงยึึดมั่�น่ในสิ่่�งที่่คิ�ิดว่่าถููกต้อ้ง เพียีงแค่่รอ เวลาและให้้ผลการ รัักษาเป็็นข้อพิ้ สูิูจน์์” นายแพทย์์สุุทรจึึงพยายามฝึึกฝนตัวัเอง
388 และเตรียีมตัวัอย่่างดีีในการผ่่าตัดัแต่ล่ะครั้้�งทั้้�งที่่�เคยทำำมาแล้ว้และยัังไม่่เคยทำำมาก่่อนในประเทศไทย พร้้อมทั้้�งเก็็บข้้อมููลทุุก ขั้้�นตอนในการทำำงาน การใช้้เครื่่�องมืือ AO ปััญหาที่่�เจอและ วิิธีีแก้้ไขเพื่่�อนำำ ไปใช้้เป็็นข้้อมููล ในการส่่งต่่อองค์์ความรู้้ให้กั้บัแพทย์์รุ่่นหลัังๆ แม้บ้างครั้้�งอาจเกิดิความท้้อใจจากคำพููดหรืำ ือความไม่่เชื่่�อถืือของแพทย์์บางคน แต่ทุุ่กครั้้�งนายแพทย์์สุุทรจะบอกตัวัเองอยู่่เสมอว่่า“เมื่่อศัั�ลยแพทย์์ต่างป ่ระเทศสามารถผ่่าตััดและ บรรยายถึึงเทคนิคิต่่างๆ จนผลออกมาเป็็นที่่ยอมรัั�บได้้ ทำำ ไม แพทย์์ไทยจะทำำ ไม่ไ่ด้้ ขอแค่่ตัั วเราเองศึกึษาให้ดี้ี ทำคำวามเข้้าใจ ทุกขัุ้้นตอน�และลงมือืทำตำามได้อ้ย่าง่ถููกต้อ้ง จากนั้้นฝึ�ึกฝนจน ชำนำาญ การยอมรัับก็็จะเกิดขึ้้ ิ นสัักวััน� ” หลัังจากนั้้�น นายแพทย์์สุุทรได้้ใช้้AO Technique ทำำการรัักษาผู้้ป่่วยโรคกระดููกหัักมากมายเป็็นเวลา 3 ปีี จึึง ได้้ขอทุุนและมีีโอกาสเรีียนต่่อเป็็น AO Fellow อีีก 2 เดืือนที่่�เมืือง Basel ประเทศสวิิสเซอร์์แลนด์ ก่์ ่อนที่่�จะไปเข้้าอบรม AO AdvanceCourseแล้วจึ้ ึงเดิินทางกลัับ ในระหว่่างนั้้�นเองที่่�นาย แพทย์์สุุทรตระหนัักว่่า“จะดีีแค่่ไหนหากแพทย์์ไทยคนอื่่น�ๆจะได้้ เข้้าอบรมหรือืเรียนรู้้ียนรู้้ AO ยนรู้้ Technique อย่าง่ ที่่ตน�เองได้มี้ีโอกาส เรีียนแบบนี้้บ้�้าง” จึึงเริ่่�มคิิดที่่�จะเปิิดคอร์ส์อบรมให้กั้บัแพทย์์ สาขานี้้�ในประเทศไทย เพราะประโยชน์ที่่์ �จะเกิิดขึ้้�นจากความรู้้ ใน AOTechnique ซึ่่�งไม่่ได้้เกิิดเพีียงแค่กั่บตัวัผู้้ป่่วยเท่่านั้้�น แต่่ หากศึึกษา AOTechniqueให้ดี้ีนี่่�คืือการต่่อยอดองค์์ความรู้้ให้้ กับัแพทย์์ในสาขาออร์์โธปิิดิิคส์์ได้พั้ ัฒนาตัวัเองและรููป แบบการ รัักษาต่่อไปได้้อย่่างยั่่�งยืืน นายแพทย์์สุุทรได้ทำ้ ำเรื่่�องขอความสนับสนุัุนเครื่่�องมืือ ของ AO ที่่�ใช้้ในห้้องผ่่าตัดัจริิงมาสอนให้กั้บัแพทย์์ไทย โดย อยู่่ภายใต้้เงื่่�อนไขว่่า เครื่่�องมืือที่่�ส่่งมานั้้�นจะใช้้เพื่่�อการเรีียน การสอนเท่่านั้้�นและเครื่่�องมืือเหล่่านั้้�นอยู่่ในความดููแลของ สมาคมออร์์โธปิิดิิกส์์แห่่งประเทศไทย ซึ่่�งจะนำำ มาใช้้เป็็นของ ส่ว่นรวมสำหรัำบทุัุกสถาบัันฝึึกอบรม ซึ่่�งคอร์ส์แรกที่่�จัดขึ้้ ั �นเมื่่�อ ปีีพศ.2528 ที่่ห้�้องประชุุมประจวบ กองออร์์โธปิิดิิคส์์โรงพยาบาล พระมงกุุฎเกล้้าเริ่่�มต้้นที่่�จำำนวนแพทย์์ประมาณ40-50 คน โดย จัดต่ั ่อเนื่่�องติิดต่่อกัันมา 37 ปีีโดยไม่่เคยเว้้น และในปััจจุุบััน สามารถสอนแพทย์์ประจำบ้ำ ้านได้้เกืือบ140 คนต่่อปีี ด้วย้ความมุ่่งมั่่�นตั้้�งใจและเล็็งเห็็นถึึงประโยชน์์ของAO Technique ที่่�จะเข้้ามาพััฒนารููป แบบการรัักษาคนไข้้กระดููก หัักและวงการแพทย์์ไทยได้้เป็็นอย่่างดีีรวมถึึงผลงานมากมาย ที่่�นายแพทย์์สุุทรตั้้�งใจสร้้างขึ้้�นมาเพื่่�อประโยชน์์โดยรวม ทำำ ให้้นายแพทย์์สุุทรเริ่่�มเป็็นที่่�รู้้จัักและยอมรับัจากวงการแพทย์์ ออร์์โธปิิดิิกส์์ทั่่�วโลกและยัังได้รั้บัเชิิญไปบรรยายในประเทศ ส่่งต่่อเพื่่อ�ยั่่� งยืืน ต่่างๆ ในทุุกทวีีปโดยเป็็นวิิทยากรใน AO เกืือบ 300 คอรส์์ใน ช่ว่งเวลากว่่า40 ปีี ขณะเดีียวกัันบทบาทและขนาดของ AOFoundation ได้้ขยายเครืือข่่ายออกไปมากกว่่า 150 ประเทศ โดยแต่่ละ ประเทศจะมีีแพทย์์ตัวัแทนเข้้าร่ว่มบริหิารดำำเนิินนโยบายต่่างๆ ของ AO Foundation และจะมีีการเลืือกตั้้�งตำำแหน่่งประธาน. ในทุุกๆ2 ปีี ซึ่่�งนายแพทย์์สุุทรได้สร้้างผลงานจนเป็็นที่่�ยอมรับัทั้้�งในและนอกประเทศมากมาย จนได้รั้บัความไว้ว้างใจจาก การเลืือกตั้้�งให้ดำ้ ำรงตำำแหน่่งเป็็นคณะกรรมการ(Trustees)อยู่่ หลายสมัย ก่ั ่อนที่่�จะถููกเสนอชื่่�อในการเลืือกตั้้�งให้ดำ้ ำรงตำำแหน่่ง ประธานของ AOFoundationระหว่่างปีี2014-2016โดยมีีการ เลืือกตั้้�งในปีี2011 ซึ่่�งมีีผู้้ถููกเสนอชื่่�ออีีก 2 ท่่าน โดยท่่านหนึ่่�ง มาจากประเทศสหรััฐอเมริิกาและอีีกท่่านหนึ่่�งมาจากประเทศ สวิิสเซอร์์แลนด์ ซึ่่์ �งสุดทุ้้ายแล้ว ้นายแพทย์์สุุทรก็็ได้รั้บัการเลืือก ตั้้�งให้้เป็็น Presidentof AOFoundation คนที่่�13 ซึ่่�ง ซึ่่นับว่ั ่าเป็็น แพทย์์จากเอเชีียคนแรกที่่�ได้รั้บัเกีียรติิให้ดำ้ ำรงตำำแหน่่งนี้้� บนเส้้นทางของ Presidentof AO Foundation ที่่�เ ที่่ป็็น ชาวเอเชียีคนแรก อาจไม่่ได้รั้บัการยอมรับัในความคิดิเห็็น. หรื ือ ศัักยภาพมากนััก แต่่ไม่มี่ ีครั้้�งไหนที่่�จะทำำ ให้ท้้อใจเพราะเชื่่�อมั่่�น เสมอในสิ่่�งที่่�ทำนั้้ ำ �นทำบำนพื้้�นฐานของความถููกต้้อง ความตรง ไปตรงมาและมุ่่งมั่่�นที่่�จะพััฒนา AO ให้้ได้รั้บัการยอมรับทั่่�ว โลกและขยายวงออกไปให้้กว้้างที่่�สุด ุ เพราะผู้้ที่่�จะได้รั้บปัระ โยชน์สูู ์งสุดุจาก AO ไม่่เพีียงแค่่ผู้้ป่่วยในประเทศใดประเทศ หนึ่่�งเท่่านั้้�นแต่คื่ ือผู้้ป่่วยกระดููกหัักจากทั่่�วโลก President of AO Foundation (2014 - 2016)

390 สมควรทำำอะไรเช่่นผู้้ป่่วยระยะสุดทุ้้ายของโรคมะเร็็ง การผ่่าตัดัฉายแสงหรืือการให้้เคมีีบำบัำดัอาจจะไม่่เกิิดประโยชน์์ในการ รัักษาแถมยัังทำำ ให้้ผู้้ป่่วยต้้องทนทุุกข์์ทรมานมากขึ้้�น เพราะใน โรคเดีียวกัันมีีวิิธีีการรัักษาได้หล้ากหลายที่่�เยที่่ป็็นปััจจัยัมากมาย ซึ่่�งแพทย์์ที่่�ให้้การรัักษาจะต้้องนำำมาพิิจารณา เพราะถึึงแม้ว่้่า แพทย์์จะได้้อธิิบายข้้อดีีข้้อเสีียของการรัักษาทุุกอย่่างให้กั้บัผู้้ ป่่วยหรืือญาติิเพื่่�อร่ว่ มในการตัดสิั ินใจเลืือกวิิธีีการต่่างๆ ก็็ยััง ยากในการตัดสิั ินใจว่่าจะเลืือกวิธีิีใด แพทย์จึ์ึงต้้องถามใจตนเอง ว่่า ถ้้าเป็็นตัวัเองหรืือคนที่่�เรารััก เช่่นพ่่อแม่หรื่ ือลููกหลานของเรา สมควรจะเลืือกการรัักษาวิิธีีใดที่่�เหมาะสมที่่�สุด ุเพราะเป็็นการ ไม่ส่มควรอย่่างยิ่่�งที่่�แพทย์์มีีความโน้้มเอีียงในการเลืือกวิิธีีการ รัักษาโดยมีีผลประโยชน์์แอบแฝง ขอยกตัวัอย่่างเช่่นกระดููกหัักแบบที่่�สามารถรัักษาได้้ด้วย้ การใส่่เฝืือกและไม่่มีีข้้อบ่่งชี้้�ใดๆ ว่่าจำำเป็็นต้้องผ่่าตัด ัแต่่แพทย์์กลัับแนะนำำเพีียงข้้อดีีของการผ่่าตัดัเพื่่�อโน้้มน้้าวให้้ผู้้ ป่่วยยิินยอมโดยมีีเหตุผลซุ่่อนเร้้นคืือ การผ่่าตัดที่่ ั �เกิิดขึ้้�น แพทย์์ จะได้้เงิินมากกว่่าหลายเท่่าเมื่่�อเทีียบกับัการรัักษาโดยการใส่่ เฝืือก ซึ่่�งแน่่นอนว่่าหากแพทย์์คนนี้้�เกิิดกระดููกหัักเช่่นเดีียวกับั คนไข้ก็้็จะไม่ย่อมเลืือกการผ่่าตัดัเช่่นที่่�ตัวัเองแนะนำำเพราะรู้้อยู่่ แก่่ใจว่่าไม่มี่ ีความจำำเป็็นต้้องผ่่าตัดั ในช่่วงที่่�เป็็นนัักศึึกษาแพทย์์ระหว่่างที่่�เรีียนในวิิชา ศัลยัศาสตร์ ผ์ มได้้พบเห็็นอาจารย์์ในแผนกศัลัย์์ที่่�มานอนรอเพื่่�อ ที่่�จะทำำการผ่่าตัดัให้กั้บั คนไข้ที่่้ �โรงพยาบาลบ่่อยๆ ตอนนั้้�นผม คิดตื้้ ิ �นๆว่่า ทำำ ไมมาเลืือกเป็็นแพทย์์สาขานี้้�แทนที่่�จะเป็็นแพทย์์ ในสาขาที่่�ไม่ต้่ ้องรัักษาผู้้ป่่วยฉุุกเฉิิน จะได้้นอนพัักที่่�บ้้านซึ่่�งน่่า จะสบายกว่่า แต่่พอได้้เรีียนรู้้งานไปเรื่่�อยๆ ก็็ทำำ ให้้เข้้าใจมาก ขึ้้�นว่่า อาจารย์์ในแผนกศัลยัศาสตร์ทำ์ ำงานค่่อนข้้างหนัักและ ต้้องมีีความรับผิั ิดชอบสููงมาก เนื่่�องจากมัักมีีคนไข้ฉุุ้กเฉิินเข้้า มาบ่่อยๆ คนที่่�จะเป็็นแพทย์์สาขานี้้จึ�ึงต้้องพร้้อมเสียสลีะไม่ว่่า จะเป็็นเวลากี่่�โมงจะตีี2- ตีี3 หากมีีโทรศััพท์์เข้้ามาว่่า มีีคนไข้้ ด่ว่นก็็จะต้้องไปดููแลรัักษาโดยทัันทีี สิ่่�งที่่�เห็็นทำำ ให้คิ้ดิได้ว่้่าไม่ว่่าจะเลืือกเรียีนแพทย์์สาขา ใดก็็ตาม สิ่่�งที่่�ต้้องรู้้ไม่่ใช่่แค่ว่่าจะรัักษาคนไข้้อย่่างไรแต่่จะต้้อง รู้้ว่่าการเป็็นแพทย์์ในสาขานั้้�นๆ ต้้องเจอกับัอะไร ต้้องมีีความ รับผิั ิดชอบต่่อคนไข้้ในสาขานั้้�นแค่่ไหน ถ้้าต่่อไปในอนาคตจะ เลืือกเรียีนอะไร หรืือจะเป็็นอาชีีพอะไรก็็ตาม ต้้องมองให้้ออก ว่่า ในปลายทางอนาคตจะต้้องเจอกัับอะไรแล้ว รั้บผิั ิดชอบ กับสิ่่ ั �งที่่�เจอนั้้�นได้หรื้ ือไม่่จะทำำ ให้ดี้ีได้หรื้ ือเปล่่า ไม่คิ่ ิดเพีียงว่่า อยากได้้อยากทำำอย่่างนี้้�แต่่ไม่รู้่้ว่่าต้้องมีีความรับผิั ิดชอบอะไร เพราะเมื่่�อถึึงเวลาที่่�ต้้องทำหน้ำ ้าที่่�นั้้�นขึ้้�นมาจริิงๆกลัับทำำ ไม่่ได้ก็้็ ไม่ถูู่กต้้องโดยเฉพาะเมื่่�อเป็็นแพทย์์ ต้้องทุ่่มเทใส่่ใจและจริิงจััง กับัการดููแลรัักษาคนไข้้อย่่างเต็็มที่่� นอกจากนี้้ ยั�ังมีีอีีกสิ่่�งหนึ่่�งที่่�ผมได้้เรีียนรู้้จากการเป็็น แพทย์์คืือ แพทย์์ไม่่เพีียงทำหน้ำ ้าที่่�รัักษาคนไข้้แต่่แพทย์์ยััง สามารถเป็็นครููที่่�ถ่่ายทอดความรู้้และประสบการณ์ต่์ ่างๆให้้ กับัแพทย์์รุ่่นน้้อง ได้นำ้ ำ ไปต่่อยอดและพััฒนาการรัักษาต่่างๆ ที่่�ให้ผลดี้ ีกับั คนไข้ นั่่ ้ �นทำำ ให้ผ้มคิิดว่่าหากเราทำหน้ำ ้าที่่�ของการ เป็็นแพทย์์และครููที่่�ดีีได้้เราก็็จะกลายเป็็นส่่วนเล็็กๆในสัังคม ที่่�ช่่วยพััฒนาวงการแพทย์์ของประเทศให้้เติิบโตได้้ไม่่แพ้ป้ระ เทศไหนๆ อีีกด้วย้ กว่่า 40 ปีีของการเป็็นแพทย์์เริ่่�มตั้้�งแต่่เป็็นแพทย์์ ฝึึกหััดจนได้คำ้นำหน้ำ ้าว่่า นายแพทย์์ ผมผ่่านการทำหน้ำ ้าที่่� รัักษาคนไข้ทั้้�งในโรงพยาบาลรััฐและเอกชน ผ มได้้เป็็นครููแพทย์์ ที่่�ถ่่ายทอดความรู้้ต่่างๆ ให้กั้ บศัลยั แพทย์์รุ่่นหลัังๆ ซึ่่�งในสอง บทบาทนี้้�เป็็นสิ่่�งที่่�ผมรัักและมีีความปรารถนาอย่่างยิ่่�งที่่�จะทำำ ทั้้�งสองบทบาทให้ดี้ีที่่�สุด ุเพื่่�อให้้คนไข้้ได้รั้บัการรัักษาที่่�ถููกต้้อง จากแพทย์์ผู้้มีีองค์์ความรู้้ที่่�มาพร้้อมจริยิธรรมที่่�ดีีแต่ก็่ ็ยัังมีีหน้้า ที่่�อื่่�นๆ ที่่�ผมสามารถทำำ ได้้เพื่่�อให้ว้งการแพทย์์ได้พั้ ัฒนายิ่่�งๆ ขึ้้�น ไป นั่่�นคืือการทำำงานให้กั้บัราชวิิทยาลััยแพทย์์ออร์์โธปิิดิิกส์์แห่่ง ประเทศไทยและ AO Foundation ผมเข้้ามาร่ว่มทำำงานให้กั้บัราชวิิทยาลััยออร์์โธปิิดิิกส์์ ตั้้�งแต่่ครั้้�งที่่�ยัังเป็็นเพีียงสมาคมออร์์โธปิิดิิกส์์แห่่งประเทศไทย โดยทำหน้ำ ้าที่่�เป็็นนายทะเบีียนในปีีพ.ศ.2531-2533 และ ทำำงานต่่อเนื่่�องมาในตำำแหน่่งต่่างๆ จนได้้รัับเลืือกตั้้�งเป็็น ประธานรัับเลืือกของราชวิิทยาลััยแพทย์์ออร์์โธปิิดิิกส์์แห่่ง ประเทศไทยในช่่วงปีีพ.ศ. 2545-2547 และเมื่่�อครบวาระ ก็็ขึ้้�นดำำรงตำำแหน่่งเป็็นประธานราชวิิทยาลััยต่่อมาในช่่วงปีี พ.ศ.2547-2549 สำหรัำบ ัการร่ว่มงานกับ ั AO foundation นั้้�นเริ่่�มต้้น จากการเข้้ารับัการอบรมหลัักสููตร AO Basic Course Davos, Switzerland ในปีีคศ.1978เป็็นหลัักสููตรแรก จากนั้้�นผ่่านการ อมรมหลัักสููตรต่่างๆเรื่่�อยมา กระทั่่�งกลายเป็็นวิิทยากรที่่�ได้้เดิิน ทางไปสอนในประเทศต่่างๆจนเข้้ามาทำหน้ำ ้าที่่�บริหิารและได้้รับัความไว้ว้างใจในการเลืือกตั้้�งให้ดำ้ ำรงตำำแหน่่ง President AO Foundationในปีีคศ.2014-2016 หลายคนอาจมีข้ี้อสงสัยที่่ ั �ไม่่กล้้าเอ่ยป่ากถามผมตรงๆ ว่่าเป็็นแพทย์์และครููแพทย์์ก็็มีีงานมากมายอยู่่แยู่่ล้ว ้ เพราะอะไร ผมถึึงยัังคงเข้้ามาทำำงานตรงนี้้� คำำตอบคืือออร์์โธปิิดิิกส์์คืือสาขาวิิชาที่่�เรียีนจบมาแม้้จะเป็็นแพทย์์ที่่�ทำำการรัักษาคนไข้้ได้้แล้วก็้ ็ไม่่สมควรที่่�จะหยุดุ พััฒนาตนเอง เพราะวิิวััฒนาการต่่างๆ ไม่่เคยหยุดนิุ่่�ง จึึงควร 391 ใฝ่่หาความรู้้อยู่่เสมอ และแพทย์์ทุุกคนที่่�เรีียนจบมาแล้ว้ควร มีีส่่วนให้้วิิชาชีีพนี้้�ได้รั้บัการยอมรับนับหน้ั ้าถืือตา สร้้างความ เจริิญก้้าวหน้้าให้กั้ับวิิชาชีีพของตัวัเอง เพราะถ้้าคนที่่�จบใน สาขาวิิชานี้้�แล้วยั้ ังไม่่มีีความคิิดที่่�จะสนใจเข้้ามาช่่วยพััฒนา หรืือต่่อยอดแล้วส้าขานี้้�จะพััฒนาต่่อไปได้้อย่่างไร แพทย์์ทุุก ท่่านต้้องตระหนัักถึึงหน้้าที่่�และสิิทธิิควบคู่่กั ันไป ในแต่ล่ะสาขา วิิชาชีีพของตนเอง หากแต่่ในทุุกบทบาทและทุุกๆ การทำหน้ำ ้าที่่�ไม่่ได้้โรย ด้วย้กลีีบกุหลุาบ กว่่าที่่�จะก้้าวผ่่านไปในแต่่ละเส้้นทาง เลี่่�ยง ไม่่ได้ที่่้�ต้้องมีีอุุปสรรค แม้้แต่่ในวัันนี้้ ทุ�ุกก้้าวเดิินก็็ยัังคงต้้อง เผชิิญกัับอุปสุรรคอยู่่เนืืองๆ หากแต่่ประสบการณ์์องค์์ความ รู้้ต่่างๆ ช่่วยให้ส้ามารถจััดการสิ่่�งต่่างๆ ได้ดี้ีขึ้้�นซึ่่�งผมยึึดมั่่�น เสมอมาได้้แก่่ ความถููกต้อ้งโดยเอาประโยชน์์ของส่่วนรวมเป็็นที่่ตั้้�ง� ความซื่่อตร�งและความจริิงใจในการทำำงานกับัผู้้ร่่ วมงาน การน้อมรัั ้บความผิดิและพร้อม้หาทางแก้้ไขในการทำำงาน การเรียนรู้้ียนรู้้และแยนรู้้ก้้ไขเทคนิคิในการผ่่าตััดเพื่่อพัั�ฒนาให้ดี้ขึ้้ีน � และสิ่่�งที่่�สำคัำ ญที่่ ั�สุด คืุือ“การไม่่ลืมคืวามเป็็นมนุุษย์์” เพราะไม่ว่่าจะเป็็นคนไข้ หรื้ ือผู้้ร่ว่มงาน ทุุกคนมีีความต้้องการ ที่่�ต่่างกััน การเป็็นคนทำำงานหนััก ทำหลำายหน้้าที่่�มีีข้้อดีีที่่�ทำำ ให้้ได้้เห็็นความเป็็นมนุษุย์์ในแง่มุุ่มต่่างๆ มากมาย เสมืือนเป็็นสิ่่�งที่่�คอยย้ำ ำ� เตืือนกับตัวัเองว่่า ทุุกๆอย่่างที่่�ทำำอยู่่ ต้ ้องทำด้ำวย้ความ เข้้าใจในจิิตใจของมนุษุย์์ด้วยกั้ ัน ไม่่ใช่่เพราะมีหน้ี ้าที่่�เป็็นแพทย์์ แต่่ทุุกหน้้าที่่�ในสัังคมที่่�ไม่่ควรหลงลืืมว่่า มนุษุย์์ทุุกคนมีีความ รู้้สึึก มีีชีีวิิตจิิตใจ มีีความคิิดที่่�ต่่างกััน ยิ่่�งเป็็นคนไข้ด้วย้แล้ว ้ภาวะที่่�เจ็็บป่่วยคืือภาวะที่่�ไม่่เพีียงร่่างกายอ่่อนแอ แต่่จิิตใจก็็พลอยอ่่อนแอลงไปพร้้อมกััน แพทย์์จึึงไม่่ควรลืืมในความรู้้สึึก ของคนป่่วย ความเหมาะสมในการรัักษาควรมาพร้้อมความ เข้้าใจในโรคที่่�เผชิิญ เพื่่�อให้้เกิิดความสบายใจและยิินยอมที่่�จะ รับัการรัักษาจากความรู้้สึึกของคนไข้้จริิงๆ แต่่แน่่นอนว่่าในทุุกวิิชาชีีพย่่อมมีีทั้้�งคนดีีและไม่ดี่ ีขอ เพีียงแพทย์์ทุุกท่่านยัังคงระลึึกอยู่่เยู่่สมอว่่า ท่่านเป็็นส่ว่นสำคัำญัในการเป็็นแบบอย่่างให้กั้ ับแพทย์์รุ่่นหลััง ไม่่ว่่าจะเป็็นการ ทำำงานหรืือการดำำเนิินชีีวิิต จึึงควรธำำ รงไว้ด้วย้จริิยธรรมและ ศีีลธรรมอัันดีีอยู่่เสมอ ซึ่่�งตัวผั มเองได้ยึ้ึดมั่่�นในพระราชดำรัำสัของสมเด็็จพระราชบิิดาในการเริ่่�มชีีวิิตของการเป็็นแพทย์์มา โดยตลอด ในชีีวิิตของการทำำงานที่่�เป็็นศััลยแพทย์์ต้้องให้้การ รัักษาผู้้ป่่วยอย่่างเต็็มความรู้้ความสามารถเปรียบีเสมืือนการ รัักษาตนเองหรืือญาติิพี่่�น้้อง ทุุกคนต้้องต้้องเรียีนรู้้ที่่�จะทำำงาน ร่่วมกัับผู้้อื่่�น ให้้เกีียรติิซึ่่�งกัันและกััน สุดทุ้้ายก็็คืือการทำำงาน ร่ว่มกับัองค์์กรวิิชาชีีพหรืือสถาบัันต่่างๆ ซึ่่�งเป็็นงานที่่�เสีียสละ อาจจะไม่่ได้รั้บผลัตอบแทนที่่�มากมายเมื่่�อเทีียบกับัเวลาที่่�เสีีย ไป จนเกิิดเป็็นความรู้้สึึกที่่�ว่่า “ทำำแล้้วได้อ้ะไร” สำหรัำบผัมคิิด ว่่า ความเสีียสละเป็็นการแสดงออกถึึงคุุณค่่าของความเป็็น มนุุษย์์ที่่�ควรจะนึึกอยู่่เสมอว่่ามีีอะไรที่่�จะทำำเพื่่�อตอบแทนให้้กัับผู้้อื่่�นและสัังคม ในชีีวิิตของทุุกคนเมื่่�อมาถึึงบั้้�นปลายของ ชีีวิิตก็็ต้้องนึึกอยู่่เสมอว่่า เราไม่่สามารถที่่�จะนำำอะไรติิดตัวัไป ได้้เมื่่�อสิ้้�นชีีวิิตสิ่่�งที่่�เหลืืออยู่่ก็ ็มีีเพีียงแต่่จะให้้ผู้้คนรำลึึกถึึงและ พููดถึึงตัวัเราอย่่างไร สุดทุ้้ายนี้้ ผ�มอยากจะฝากคำำขวัญัของพลอากาศโท นายแพทย์์ กิิตติิเย็็นสุดุใจ อดีีตเจ้้ากรมแพทย์์ทหารอากาศ ซึ่่�งเป็็นอาจารย์์ศัลยักรรมที่่�ผมเคารพนับถืั ือ นอกจากนี้้�อาจารย์์ ยัังได้ผ่้่าตัดัให้กั้บคุัุณแม่่ของผมเมื่่�อท่่านเป็็นมะเร็็งลำำ ใส้้ใหญ่่ อาจารย์์ กิิตติิเป็็นครููศั ลยัแพทย์์ที่่�เป็็นตัวัอย่่างที่่�ดีีในการดููแล เอาใจใส่่กับัผู้้ป่่วย มีีลููกศิิษย์์และผู้้ร่ว่มงานมากมายที่่�รัักและ เคารพอาจารย์์ ท่่านได้้กล่่าวไว้ว่้่า “ผมเป็็ นคนเต็็มที่กั่�ับงานและทำำงานทุุกอย่่างให้้ดีีที่สุ่�ุด จะปฏิบัิติัต่ิ่อทุุกคนอย่่างเท่่าเทีียมทั้้ง�บุุคลากรและ คนไข้้ทำำงานกัับบุุคลากรทุุกฝ่่ ายได้้โดยปราศจากเงื่อนไข ่� คิดิเสมอว่่าบุุคลากรทุุกคนเสมืือนครอบครััวเดีียวกัันและ ผู้้ป่ ่ วยที่มา่�รัับการตรวจรัักษาก็ถืื็อเสมืือนญาติิ” ผมขอขอบคุุณทางราชวิิทยาลััยศััลยแพทย์แ์ห่่ง ประเทศไทยที่่� ได้้ให้้เกีียรติิคััดเลืือกผมเป็็ นสมาชิิกดีี เด่่น และได้้เชิิญให้้เขีียนบทความแสดงความรู้้สึ ึกของ ชีีวิิตศััลยแพทย์์ เนื้้� อหาทั้้� งหมดในบทความนี้้� มาจาก การดำำเนิินชีีวิิตของตนเอง ตั้้� งแต่่การตััดสิินใจที่่� จะเป็็ น ศััลยแพทย์์และเป็็ นความคิิดเห็็นเห็็นส่่วนตััวที่ห่�วัังว่่าจะ เป็็ นประโยชน์สำ์ ำหรัับศััลยแพทย์ทุ์ุกท่่าน

But then again, too few to mention. แต่่มัันก็็น้้อยเสีียจนไม่่จำำเป็็นต้้องเอ่่ยถึึง I did what I had to do ผมทำำ ในสิ่่�งที่่ผ�มต้้องทำำ And saw it through without exemption. ผมมองมัันทะลุุทุุกอย่่างแบบไม่่มีีข้้อแม้้ใดๆ I planned each charted course ทุุกเส้้นทางผมวางแผนไว้้เป็็นอย่่างดีี Each careful step along the by way, ทุุกย่่างก้้าวผมไตร่่ตรองอย่่างลึึกซึ้้ง� But more, much more than this, และยิ่่�งไปกว่่านั้้น� I did it my way. ผมทำมัำ ันทั้้ง�หมด...ในแบบของผม...
