CITATION CHECKLIST



This document contains the list of mandatory and optional Rotaract District Citation requirements given to
Rotaract Clubs at the beginning of the year 2021-2022. The District Citation will be awarded to clubs that
fulfill their duties as a Good Standing Rotaract Club and complete a balanced programme of activities
according to the given criteria.

POINTS TO NOTE

● All activities must have been undertaken and completed between 01st of July 2021 and 31st of May
2022, and be in accordance with the Rotary Code of Policies and Rotaract District Administrative Policy
which clearly outline the role and the scope of the clubs.

Supporting documents

● Kindly avoid detailed descriptions in the columns of the checklist.
For the requirements that have been marked with a *, the Supporting Document Reference No./Notes
column can be used to give the required details.

● All supporting documents should be attached to this document itself, as annexures. State the relevant
reference letter/number of the Citation Requirement as the name of the attached document for ease of
tracking and matching it with the correct evidence. Please ensure that the letter/number is mentioned
accurately in the Checklist column provided.

● The approval of the Guide Club Coordinator should be obtained for the duly completed Checklist form
via email prior to final submission.

● The soft copy of the duly completed Checklist Form in Word format along with the above email
approval should be sent via e-mail to Rotaract District Secretary Rtr. IPP Thisura Ramanayake at
[email protected] with the respective Guide Club Coordinator on copy on or before
11.59 p.m. on Tuesday, 07th of June 2022.

● The completed form should also be annexed to your Rotaract Club Annual Report due to be submitted
on or before 12th June 2022.

● Evaluation and verification of the club’s accomplishments would be done via the District Official
appointed by the District Rotaract Representative and/or the supporting documents submitted along
with this Citation checklist.
