Hello ITS Internship Supervisors:

Thank you so much for serving as internship supervisors this semester! This is
simply a reminder that the student internship experience should come to an end
TODAY, April 14, 2017.  Please complete the attached form, and return directly to
me by Wednesday, April 19, 2017.  If you have more than one intern, I will need
a separate form for each student. Although the document is attached as a Word file,
you are also welcome to create a .PDF or simply print the document, complete by
hand, scan and email it back to me, if easier.  Please be completely honest when
evaluating your students. The students should also be personally requesting your
initials and/or signature on the logs that will be featured in their internship reports.
Feel free to contact me if you have any questions.  Thanks again for everything!

Sincerely,
Pam
