1.0 Forewords
This document represents the Emergency Response Plan (ERP) for PROJEK NAIK
TARAF HOAPITAL TAWAU, TAWAU which has been awarded to SRI ESBINA (M)
SDN. BHD.
This Emergency Response Plan is developed to produce a guideline for our Site Office and
for Site emergency handling processes with safe and guided actions in dealing with various
types and levels of emergencies.
This ERP is prepared as a guide for emergencies and may be revised over time to
accommodate any changes in its processes or as required by revisions in other requirements
such as by law; etc.

2.0 Introduction
2.1 Background
Sri Esbina (M) Sdn. Bhd. is committed to place Safety and Health first in all our
activities in order to create and promote a safe working environment for all its
employees. It is company’s policy to ensure all site activities are conducted with
safety and health issues as FIRST priority.
To further these efforts, the management will endeavor to:
2.1.1 Provide and maintain a safe place to work and a safe system of work
2.1.2 Ensure that all staff are informed, trained and supervised on how to
perform their jobs safely and without risks to health
2.1.3 Investigate accidents and near miss incidents; and to take corrective
measures to ensure that the accidents or incidents will not recur

2.1.4 Comply with all legal requirements on safety and health as stipulated in
Occupational Safety and Health Act 1994, Factories and Machinery Act
1969, its attendant Regulations and approved codes and practices; where
appropriate, applicable and practical
2.1.5 Review this ERP as and when appropriate
2.2 Objectives
The operational purpose of this ERP is to localize an emergency within our project
site / site office operations and if possible, to eliminate it and minimize the effects
of the accident/incident on our staff/workers and/or property within the project site /
site office.
2.2.1 To provide guidance on responses to be taken in the event of an
emergency for the protection of staff/workers/personnel, property and to
the public
2.2.2 To ensure that all emergency equipment are properly maintained, ready
for use and all designated/appropriate personnel are trained in the proper
use of the said equipment and emergency procedures
2.2.3 The ERP recognizes the many different types of emergency conditions
that can arises and the fact that certain decisions will be hard to make at
the time of emergency; therefore, the ERP will try to provide for
procedures to be adhered/implemented and to outline responsibilities for
and actions to be taken by the designated staff/personnel in an
emergency
2.2.4 To provide a clear identification of the site / site office, its locations,
location of hazardous materials stored and quantities on site
2.2.5 To ensure an effective channels/mode of communication exist between
staff/personnel for the co-ordination of tasks required for management
of an emergency to prevent any hindrances
3.0 Organization & Duties
3.1 Emergency Response Team (ERT)
An Emergency Response Team (ERT) had been established to take priority actions
in the event of an emergency. This ERT ideally, will be trained in fire fighting and
first aid.

3.2 Duties & Responsibilities of the ERT
3.2.1 Emergency Commander

between Emergency Coordinator and sub-cons.

appraisals/postmortem of ERT performances where applicable

3.2.2.1 To assist and advise Emergency Commander and implement
actual emergency response procedures
3.2.2.2 Decides on the level of alarm as appropriate (local site or
external)
3.2.2.3 Advises or authorizes the ERT Logistic / Admin to make
necessary callouts and notification
3.2.2.4 Investigates/examines the emergency to determine causes or
sources
3.2.4 Logistic / Admin
3.2.4.1 To assist Emergency Commander with ERT administrative work
3.2.4.2 To assist Emergency Commander with appropriate reporting as
deemed necessary and/or as advised by ERT Chairman
3.2.4.3 To provide documentation support/minutes of meeting for all
ERT activities
3.2.4.4 Immediately inform Bomba / Ambulance when instructed

3.2.5.1 To assist Emergency Commander / Emergency Coordinator for
monitoring, implementing and conducting activities at applicable
site upon emergencies

Coordinator and applicable Sub-Contractors’ representatives or
their workers for applicable actions, supports and activities
during any emergencies
3.2.5.3 To assist Emergency Commander / Emergency Coordinator for
investigations or examinations of site emergencies inclusive of
assessment

3.2.6 Fire Fighters
3.2.6.1 Response to emergency calls and assess the current situation and
report to the Emergency Commander / Coordinator
3.2.6.2 If fire is manageable immediately put it out with fire
extinguishers
3.2.6.3 To assist Floor Leaders during emergency evacuation
3.2.7 First Aiders
3.2.7.1 Respond promptly to an incident/accident when requested,
within the limits of your HSE approved first aid training while
safeguarding your own health and safety and that of others.
3.2.7.2 Know how to secure additional first aid assistance as and when
necessary and if necessary, arrange, without delay, professional
medical assistance if the nature and degree of the injury is out
with the limits of your training
3.2.7.3 Be aware of the location of the nearest first aid box, its contents
and the ensuring that the contents are replenished when
necessary
3.2.8 Assembly Area Chief
3.2.8.1 To do a head count of all workers and report to the Emergency
Commander of those missing.
3.2.8.2 Assist Floor Leaders during emergency evacuation
4.0 Notification Procedures
4.1 Notification to Site Personnel
All personnel will be notified of an emergency/incident via:
4.1.1 Voice alerts (shouting, screaming) and hand signals
4.1.2 Hand Phones
4.2 Notification to Site Office
The first person to discover any emergency shall immediately inform Site Office if
appropriate and safe by telephone/hand phones/voice alerts or to activate alarm

system. The information shall be as detailed as possible; which ideally should
contain the following information:
4.2.1 Name/designation of person giving the information
4.2.2 Exact location of the emergency/incident
4.2.3 Date, time and nature of emergency/incident
4.2.4 Scope of emergency/incident and any visually noted injuries/damages
4.2.5 If external assistance is required
4.2.6 Any other useful information
4.3 Notification to Head Office
The ERT Secretary will inform RSJV/SEB of any emergency or incident upon
instructions or advice from the ERT Chairman via telephone or hand phone; to be
followed or supported with following information:
4.3.1 Name & designation of the person giving the information and who
authorized the notification
4.3.2 Exact location of the emergency/incident
4.3.3 Date, time and nature of the emergency/incident
4.3.4 Extent of emergency/incident and number of injuries/casualties
4.3.5 Actions already taken or that will be taken to control the
emergency/incident
4.3.6 External assistance required
4.3.7 Any other information that may be of assistance
4.4 Notification to External Agencies
4.4.1 Notification to applicable external emergency activities or governmental
agencies such as Fire Department, Police, Emergency Medical services
(ambulance and/or hospitals) shall be done by the ERT Secretary as
instructed by the Emergency Commander
4.4.2 During the emergency, Emergency Commander shall ensure that all
activities/actions are concentrated towards the control/direction of

emergency/incident handling and situation control. Any statutory
reporting as required under governmental legislation shall be complied
with after the situation was brought under control or the situation had
returned to normal
4.5 Media & Public Response
A major emergency or incident may attract attention from the media and public. In
such circumstances:
4.5.1 The ERT Chairman shall handle the situation at site after advice or
discussions with either Sri Esbina (M) Sdn. Bhd. Top Management or
JKR site representatives.

5.4.2 Emergency Response Procedure
Upon knowledge of emergency, Site ERP will be responsible for the
following processes:
5.4.2.1 To carry out or lead or instruct (whichever method is more
applicable and appropriate in terms of safe working practice/
environment) sub- contractors’ representatives/workers for
immediate rescue work or basic emergency handling
Examples:
a. If trained in basic first aid, to render basic first aid or move
any exposed personnel into “recovery” position as conditions
permit
b. Handling small fires with available fire-extinguishers or fire-
hose
c. Carry out unconscious personnel from emergency area to
safe area
5.4.2.2 To immediately liaise with ERT Commander and applicable
site’s Sub-contractor’ Representative for
instructions/advice/actions

5.9 Termination of Emergency
5.9.5 Emergency is terminated when the situation is brought back to normal
5.9.6 operations
All relevant parties (ERT, site personnel, Site Office staff, Emergency

An Emergency Events Report will be prepared based on the sequences
of events as recorded by ERT Secretary
5.10 Debriefing Processes
5.10.5 Level 1 Emergency
The ERT and Safety & Health Committee will conduct an investigation
to assess the cause(s) of the emergency and prepare a report. Procedures
or action plans may then be revised/created during a review of the
existing system to prevent a recurrence of such emergency in the future
The ERT Secretary will fill up the Emergency Response Evaluation /
Debriefing Report when carrying the postmortem on the effectiveness of
the Emergency Response Team and the Emergency Response Plan. ERT
Chairman will then conduct a review for any improvements and/or
revisions to the ERP if applicable
5.10.6 Level 2 Emergency
Site Office, Site Personnel and involved Sub-Contractors will be
informed when the situation or premises or conditions are declared
“safe”.
The ERT and Safety & Health Committee will investigate to assess the
cause(s) of the emergency and prepare a report. Procedures or action
plans may then be revised/created during a review of the existing system
to prevent a recurrence of such emergency in the future
A full report on the emergency will be prepared and submitted to Sri
Esbina (M) Sdn. Bhd. Head office and Client.
The ERT Secretary will record the minutes of applicable the Emergency
Response Evaluation / Debriefing meeting when carrying the post
mortem on the effectiveness of the Emergency Response Team and the
Emergency Response Plan. ERT Chairman will then conduct a review
for any improvements and/or revisions to the ERP if applicable

5.10.7 Level 3 Emergency
Site Office, Site Personnel and involved Sub-Contractors and general
neighbors surrounding the site/premises of the emergency will be
informed when the situation or premises or conditions are declared
“safe”.
The ERT and Safety & Health Committee and Top Management will
conduct an investigation/inquiry to assess the cause(s) of emergency and
prepare a report. A Disaster Recovery Plan may be devised and
submitted to applicable Government agencies for approval and actions if
deemed necessary
6.0 Training
6.1 Training
Site personnel will be given internal and external training pertaining to safety and
emergency response handling to ensure that all site personnel are well prepared and
trained to respond to an emergency.
Where practicable and applicable, a training program has been planned for site
personnel. Some of applicable training will be conducted by the appropriate
government departments and others will be conducted by qualified/experienced Sri
Esbina (M) Sdn. Bhd. staff as internal training courses
All training shall be recorded under Site Training files with attendance taken.

6.2 Mock Drills
Emergency Response drills ideally should be conducted once a year. This is to
ensure that all site personnel are well trained and prepared should an emergency
occur. After a drill is carried out, a post mortem meeting shall be conducted to
evaluate the degree of competence, level of response and to review any applicable
improvement for future implementation
7.0 Emergency Response Plan for Theft
