NAMMUDE AROGYAM IMA NAMMUDE AROGYAM Immediately after taking Charge in Nov 2023 we, the chief editor and secretary, discussed all matters related to Magazine in detail and re- organised the Editorial Board and advisory board. Before planning the December issue, we conducted an emergency staff meeting in last week of November 2023 and evaluated the present situation; official, legal, financial and magazine issues were discussed in depth and planned for layout and cover page design change. We could release magazine with a colour painting with theme “പുതിയ ആകാശം, പുതിയ ഭൂമി” denoting the new Changes we were going to exhibit. We formed a state Wide membership/Subscription Committee of Nammude Arogyam and in December 2023 conducted our first meeting. It was a grand success and leaders from all Districts enthusiastically participated and put forward many valuable suggestions and ideas. We have prepared a QR Code to make Subscription Procedure easy and it was printed in a sticker and was distributed to all branch officials during presidents and secretaries meet. The next issue marked by the cover of Madanan, the famous artist. A new column ‘വായനാർ ഇടെപടുു’ was started and the highlight was the Cartoon page in last page. The most important thing was the new printing paper we used for the magazine. The new Print paper totally changed the visual appearance of our magazine and made it at par with the leading front line magazines in Malayalam. Layout and the size of the letters were also changed for good. Leading writers like Dr. Ekbal Started writing In that issue. That issue fetched appreciation from all quarters. We approached different banks for subscription in their branches all over Kerala and their response was very positive. At the same time approached the Library Council authorities for the same in all their branches. We could convince The State Secretary of Library council Mr. TV Madhu and he informed us about his willingness to send Circular in this regard to all branches. Heartfelt thanks to Dr. K Suresh Babu, Dharmasala, Kannur who toiled hard to achieve the positive nod. Dr P. Sasidharan, Chairman Emergency life support was a great supporter of our activities. He and his team used to highlight our magazine in all their classes and demonstrations state-wide. During the Managing Committee meeting it was decided that IMAGE would sponsor a very good number of copies to support Nammude Arogyam. It was a great gesture from IMAGE Team. Thanks a lot to Dr. Abraham Varghese and Dr. Sharafudheen. Along with such activities we were active in motivating Individual Doctors to subscribe. One of our members Dr. Haneesh from Kodungallur sponsored 82 Copies as token of appreciation after seeing our magazine. Dr. Santhosh, Thalassery sponsored 11 copies of our Magazine. Like that we could increase our subscription leap and bounds. More than 600 new subscriptions we could gather within 2 months from Individual members. IMA KERALA DOCTOR’S CLUB IMA KERALA DOCTORS’ CLUB We are pleased to present an update on the current status of the IMA Kerala State Doctors’ Club. Over the past three months, significant progress has been made in various aspects of our activities. Steady Growth and Stability: The Doctors’ Club has demonstrated steady growth and stability in its activities. We are pleased to report that we now have a stable team of three staff members who are dedicated to ensuring the smooth functioning of the club. Diverse Programs and Events: We have been actively involved in organizing and hosting various programs and events related to the medical fraternity. This includes association-related programs, Doctors' parties, and events conducted by doctors outside the headquarters. In the last three months alone, we have successfully executed 14 major programs and approximately 26 minor events. Quality Food and Service: One of our key strengths lies in the provision of quality food and service, which has been widely appreciated among doctors. We have consistently received positive feedback regarding the standard of our offerings. Marketing and Outreach: Efforts have been made to enhance our visibility among the medical community through strategic marketing initiatives. We have distributed flyers to different groups to increase awareness and promote our club among various doctor's networks. Financial Performance: While our primary focus is not solely on profit generation, it is worth noting that we have been able to maintain a steady profit margin. However, there have been some challenges regarding delayed payments, which need to be addressed promptly. Governance and Infrastructure: Regular in-house meetings of officials and director board meetings are conducted to ensure effective governance. We have recently expanded our director board by adding several senior members to enrich our leadership team. Plans are underway to enhance the infrastructure of the club by constructing a proposed air-conditioned hall on the top floor, which will contribute to creating a more welcoming family atmosphere. Operational Enhancements: Efforts have been made to streamline our operations, including the disposal of old scrap materials to maintain cleanliness and hygiene standards. During the recently conducted National Conference Delegate, the club operated seamlessly for 3-4 consecutive days, catering to the needs of attendees. New menu cards have been printed, featuring additional dishes, and separate menus have been introduced for room service, catering to the diverse preferences of our members. While we have made significant strides in various aspects of our operations, there is still room for improvement. We are currently in the planning stages for the construction of a dedicated Doctor Club building, equipped with comprehensive facilities. This building will be erected on a 22-cent plot of land owned by the Doctors’ Club. Sd/- Sd/- DR. BENNY P.V. DR. SYAM D. COMMITTEE FOR PREVENTION OF LIFESTYLE DISEASE COMMITTEE FOR PREVENTION OF LIFESTYLE DISEASE Action plan:- Since the medical profession is dedicated to protecting the health of others, it is particularly interesting to assess how well they may have protected their own health. Do Doctors as a group indeed have a better mortality record than others of the same ages in India? Doctors in Kerala are dying younger when compared to the general public according to a study conducted by the Indian Medical Association. While the life expectancy of an Indian is 67.9 years and that of a Malayali is 74.9 years, the mean ‘age of death’ for a Malayali Doctor is 61.75 years. A majority of Doctors in Kerala die due to cardio-vascular diseases and cancer. Hence it is imperative that the medical practitioner should be more vigilant on his health status. Periodically he should assess the parameters and see to its that he is maintaining good health. To create an awareness, probably which is less among the modern medicine practitioners, this committee proposes to have awareness classes for medical professionals and to the general public at large. We will have awareness to our colleagues and to the public simultaneously, a two prong activity. For IMA members……….. 1. CME program for doctors focusing Lifestyle Disease Prevention to be conducted by each local branch. This could be during GB meeting or as part of IMA AMS/IMA CGP activity, 2. Regular Health tips/Red flags during GB. 3. Cardiovascular diseases (CVDs) are the leading cause of death worldwide and an estimated 17.9 million people died from CVDs in 2019.1 CVDs have also become the leading cause of deaths in India. Among adults over 20 years of age, the estimated prevalence of coronary heart disease (CHD) in India is around 3-4% in rural areas and 8-10% in urban areas. Studies have shown that CVD affects Indians nearly a decade earlier and nearly half of CVD deaths occur before the age of 70 years. In a comparative analysis of GBD 2016 study, it was found that traditional risk factors are the major determinants of CVD in both India and United States of America. Proportions of various risk factors like high blood pressure, elevated blood cholesterol, tobacco use, obesity and unhealthy diet which contribute to the CVD burden are the same for both countries. Unfortunately the incidence of CAD has shown a tendency to come down in the INDIAN MEDICAL ASSOCIATION 278th State Working Committee 18th February 2024 KPM Tripenta Hotel, Kozhikode Celebrating Fraternal Fellowship 386
developed world including USA, it is remaining the same or increasing in the developing countries like India. This is because the risk factors for the development of cardiovascular diseases are better controlled in the high income countries than in low and middle income countries like India. For example, the control of hypertension is around 50% in USA while it is 15% in India. Other risk factors are also poorly controlled in India. Development of cardiovascular diseases earlier in India compared to high income countries is most likely because of the earlier onset of risk factors like hypertension, diabetes and hypercholesterolemia. In a previous study, it has been shown that young adults in Kerala have a prevalence of hypertension of about 12.5% while the proportion of young adults with hypertension in USA is only 7%. The earlier onset of cardiovascular risk factors is most likely due to reduced physical activity, dietary indiscretion, obesity, and the stress of daily living. In brief, Indians have an earlier onset of cardiovascular risk factors coupled with poor control of these risk factors. Most studies from Kerala shown that the prevalence of coronary artery disease is high in Kerala compared to rest of the country and probably similar to many developed countries. Since doctors are knowledgeable and aware of cardiovascular risk factors, one would expect them to have a lower prevalence of risk factors like hypertension, diabetes, obesity and hypercholesterolemia, and when present receive more effective treatment and adopt healthy life style. Hence they were also expected to have a lower prevalence of CAD compared to the general population in Kerala. We don’t have reliable data on the prevalence of CAD among doctors in Kerala. As mentioned earlier, a casual evaluation of causes of death among doctors as per the records of social security schemes of IMA Kerala suggests that the majority of deaths among doctors occurred due to Cardiovascular disease and that too prematurely. It appears that in Kerala, doctors generally have a higher prevalence of risk factors that, when present are not properly treated and controlled. Considerable epidemiologic evidence indicates that populations and individuals with optimally controlled cardiovascular disease (CVD) risk factors and health behaviors experience very low rates of CVD events.8 In 2010, the American Heart Association (AHA) introduced the concept of ideal cardiovascular health metrics (ICVHMs), comprising of healthy lifestyle behaviors and ideal metabolic measures, to promote optimal cardiovascular health in the population.9 Several population-based studies have shown the beneficial effect of ideal cardiovascular health on cardiovascular disease (CVD) events.10 In 2022 AHA modified this and added sleep duration as well and called the renewed check list “Life’s Essential 8”. *Know your Number* In this background, we plan to undertake a project called Know Your Numbers. This involves a survey among IMA members with a questionnaire to assess the prevalence of cardiovascular risk factors and indices of ideal cardiovascular health. Participation in this survey is voluntary and the information provided will be kept with utmost care to maintain confidentiality. If any publications arise from this survey, information related to cardiovascular risk factors will be reported only with the consent of the participating doctors. The members are asked/requested to approach a competent laboratory to get the values for fasting blood sugar, lipid profile, SGOT and SGPT and serum creatinine. They will be asked to report their weight, height, waist circumference and blood pressure. There will be questions related to physical activity, dietary habits and sleep patterns. An expert committee will go through the report of each member and will provide feedback after the completion of the survey to those who desire so. For the General Public………. Most of the local branches are conducting classes for the public in this regard. An effective coordination can be done by the committee. a) Awareness classes in schools and colleges, Kudumbasree units, Residential associations, Technoparks employees and NGOs can be arranged with the help from Local bodies in the jurisdiction of local branches. Both physical and online classes can be conducted. A direction from State Government to the Local bodies, Kudumbasree units and to the educational institutions will be helpful. b) Health tips regarding Lifestyle Disease Prevention during awareness classes/ commemoration of specific days. The activities will kick start after launching in the State Working Committee on 18th February 2024. Sd/- Sd/- Sd/- DR. P. GOPIKUMAR DR. SYAM N. DR. R.K. COMMITTEE FOR PREVENTION OF HOSPITAL VIOLENCE COMMITTEE FOR PREVENTION OF HOSPITAL VIOLENCE The committee comprises of Zonal & District Coordinators besides the Convener, Vice Chairman & Chairman. The objectives are: Creating Awareness among all doctors about various aspects of Hospital Violence Empowering all branch officials with guidelines stipulated by IMA, especially for prevention of Hospital Violence Ensuring effective, planned action and follow up in all incidents. Preparing a regularly updated statistical fact sheet, to be used as a tool in speeches, articles, channel debates press conferences, fighting court cases and for any quick reference. A zoom meeting was conducted on 21.12.2023. The State President and Secretary along with two thirds of the office bearers participated in the meeting, wherein constructive discussion about the modus operandi of the committee transpired. It was decided to go ahead with the moto of ‘zero tolerance to violence’, to curtail the horrendous phenomenon of hospital violence, which otherwise can spread like wildfire. The District Coordinators will keep in touch with branch officials, offering help in incidences of Hospital Violence. The importance of an’ immediate, properly worded police complaint, from the victim’ was stressed upon. Everyone was directed to be familiar with Green Book pages 181 to 186. And finally, the meeting emphasized the role of practicing doctors in preventing hospital violence, by self-evaluation and correction, to avoid any dishonesty & unethical practises which can erode the public's respect towards medical profession. A whatsapp group of the committee is functioning properly with sharing of documents and news about incidences, leading to an effective interaction. In the last two months, the committee officials attended five District Committee meetings to convey its message briefly. More such attempts are being planned. On the anvil are plans to provide some material assistance to health care institutions in association with other agencies of IMA, and the committee envisages positive interference in cyber-crimes against doctors, with the help of PP Scheme. Coming to the statistics, three years back, it was informed to the Legislative Assembly that about 10 to 12 cases of HV take place every month in our State. This year, in the two months from 1st Dec till 31st January, there had been fourteen cases of Hospital violence ( of different intensity) which we know of : (District wise, Thrissur -4, Kozhikode -2 : Malappuram -2, Kollam -2, One each from Palakad, Kottayam, Pathanamthitta & Thiruvananthapuram. Four of these were minor issues, settled on site by involved parties or the Health services officials. Details are yet to arrive in two cases, and in all the other eight cases, the Hospital Protection Act (Kerala Healthcare Service Persons and Healthcare Service Institutions - Prevention of Violence and Damage to Property 2012) and its 2023 amendment were invoked. We are yet to reach any assumption from these values, but are, of course keeping a vigil on all the cases.
