Kottayam 63 KTM.0810 Mar Sleeva Medicity Hospital Biomedical waste management is excellent, with careful attention to segregation and lab pretreatment. No faults were found in the waste segregation area. The committee meets every six months, and they have a spacious, clean storage room. Bags are weighed, registered, and scanned using the IMAGE Customer portal app. The hospital also functions as a biomedical waste transfer station in Kottayam district, with proper waste-handling arrangements and vehicle washing facility. Cochin Brahmapuram Project: We are given possession of three acres of land by the Government invoking the Disaster Management Act but despite our continued follow-ups, the Revenue Department/LSGD is yet to issue us with a Lease Agreement of this allotted land. Already we have raised this issue in the Single Window Clearance Board Meetings also. Trivandrum Palode Project: The property land tax Rs.2,208/- is paid by us for the year 2023-24 [6 Acres & 80 Cents]. Renewal of the perimeter-boundary-works and the physical measurement of our property has to be done as this property lies adjacent to the forest land. Adoor Kinfra Project: KINFRA allotted three acres of land inside the Adoor KINFRA Park for starting the next CBWTF by us. Licence Agreement for five years already entered into and the Consent to Establish is issued by the Kerala State Pollution Control Board. The perimeter fencing as well as temporary electricity connection is already done. As per the Standard ToR received from the SEIAA; we have conducted an EIA Study of the area through the accredited agency Pragathi Labs from Hyderabad, and they submitted the EIA study report and uploaded the study report to the State Environment Impact Assessment Authority through the Parivesh online portal 24th June 2023. The SEAC (State Environmental Appraisal Committee) reviewed the matter during a meeting held on September 7, 2023. The SEAC has assigned Dr. R. Ajayakumar Varma and Smt. Beena Govindan from SEAC for the evaluation of the EIA documents, conducting field inspections, and preparing a report. The site visit and observation report has been submitted by them in the 153rd SEAC meeting chaired by Dr. R. Ajayakumar Varma. The Committee discussed the salient aspects of the project and findings in the details and raised the following points: S.N. Points raised by the SEAC Clarification and Status 1 As per the letter dated 18.10.2022 of the MoEF & CC, it is stated that the grant of EC to BWWTF projects by SEIAA must be based on gap analysis studies undertaken by the concerned SPCB duly highlighting the difference in the BMW generated vis-s-vis the treatment capacity available. Accordingly, collect the details from the SPCB and submit the same. KSPCB entrusted the NIIST to conduct the Gap Analysis Study. However, Dr. Sharafudheen K.P. Secretary met Dr. Ajayakumar Varma, the Chairman of the SEAC and get clarifications in this regard. Therefore, the proponent should be directed to submit the above details along with a detailed plan for land modification including the engineering feasibility of the site. There are possibilities of accidental spillage of contaminants to the drainage system which will impact the downstream population severely. Therefore, details of hydrological and hydrogeological studies and the precautionary measures required for preventing such accidental contamination of water bodies should be provided as envisaged in ToR 32 The possible wastes anticipated from the process are plastic & metallic from shredder & autoclave), ash (from incinerator), vehicular loading having the possibility of spillages if any. However, the storage and processing areas are provided with impervious floored rooms. The movement raw and treated waste inside the unit will be transported by conveyor belts. Floor washing and vehicular washings will be channeled and routed to ETP. All transporting vehicles are closed container type. In all the above areas, there is least possibility of spillage and carried away from the surface water runoff. ZLD will be adopted within site. Mitigation measures : Utmost care will be taken to avoid the spillage of wastes or wastewater. During post project monitoring GW and SW quality will be carried away inside plant (GW) and its surrounding (GW&SW) to take necessary control measures. However, during plant construction phase proper leveling, with well-designed walls (more depth, width and higher) will constructed escape water towards south direction and the runoff will be collected in water harvesting pits. Deep rooted plants will be planted to bind particles. 17 Impacts of the project activities on the water environment of the impact zone and their mitigation measures should be provided as envisaged in ToR 34 To minimize the water usage, dry scrubber system is adopted. To minimize the water pollutants well designed ETP provided which will take care of treatment of effluent generated in the plant. Zero liquid discharge is proposed. No wastewater will be discharged outside the premises (ZLD). Impervious layer will be provided to the waste and ash storage areas to avoid the contamination of ground water and soil due to percolation of leachates. The effluent generated will be routed to ETP for treatment. More Impact on the water environment is given in section 4.5. 18 Drainage plan details within the proposed project site and its immediate surroundings should be provided as envisaged in ToR 35 Drainage map within the project site and its immediate surrounding area shown (first order and second order streams) in Figures 2.3 & 3.3.1. 19 A certificate from the local body or agency supplying water is not provided as envisaged in ToR 37 We are in the process to get the certificate 20 A certificate of adequate power supply is not provided as envisaged in ToR 38 Certificate obtained from the Kinfra Manager and it will be updated 21 The provision of rainwater harvesting (RWH) is provided in the EIA report. Based on the discussions, the SPCB provided a letter mentioning that there is no need of public hearing for the site. 23 The EIA report identified and analyzed only randomly selected impacts. The impacts of various activities of the project should be identified and analyzed and mitigation measures proposed for all the identified impacts We have reported possible negative as well as positive impacts of important environmental components and social components as far as possible. Attempts are taken to minimize negative impacts and maximize the positive impacts by adopting suitable control measures. We have covered all the management guidelines for handling the biomedical waste as far as possible. If any missed, we will follow the expert guidelines from SEAC committee. Chapter 4. 24 In the EIA report, it is stated that the water requirement for the project is 115 KLD, 50 KLD from the KWA and 65 KLD from treated water. In the application, the water requirement is mentioned as 50 KLD and the source as tanker supply. There is a contradiction in the statement and it requires to be clarified The anticipated water requirement for the project is 115 KLD. However, the fresh water requirement for the project is only 50 KLD as recycling of water is proposed . Remaining water of 65 KLD will be taken from the treated effluent. Zero liquid Discharge is proposed at the site. All treated effluent will be reused within the unit. Detailed in Section 2.6.3, Table 2.10 & Fig. 2.11 25 Water balance statement incorporating the sources and usages for various purposes Provided in Figure 2.11, is updated with the source. 26 It is stated in the application that the capacity of STP is 150 KLD and ETP is 25 KLD. In the EIA report, it is stated that ETP capacity is 200 KLD. The project is expected to have sewage generation as well as process discharge such as wash water. Therefore, the characteristics of liquid waste will be different and accordingly the treatment process also may differ. Therefore, it is required to provide clarification regarding the liquid waste characteristics and quantity of discharge from different sources, treatment process, plant capacity, predicted quality of the treated water from the different treatment processes and their utilization potential so as to achieve the zero-discharge target. The proposed capacity of ETP is 150-200 KLD in the EIA report. (Section 2.6.3.1.) A combined treatment is proposed. 27 Details of tertiary treatment proposed for wastewater (effluent and sewage) treatment in the unit and proposal for utilization / reuse of treated water as part of conservation of natural resource The filtration system (tertiary treatment ) is detailed in the ETP section. (Section 2.6.3.1) 28 The EIA report stated that the domestic waste will be handed over to the municipal authorities, but the Gram Panchayat in which the site is located does not have adequate waste management facilities. Therefore, a plan for the management of domestic waste should be provided The domestic waste including food waste , paper waste, plastic & waste. Anticipated quantity per day from per person is taken as 0.3 kg. The food waste will be treated in the compost pit/Biobin within the site. Paper, glass and plastic waste if any will be recycled with authorized recyclers. Admin Manager and Operations Manager attended the meeting. • No IMAGE plant visit in this quarter, but I have visited the proposed Adoor plant site and very much satisfied about the site. • IMAGE related meeting -visited the Keraleeyam IMAGE pavilion and the IMAGE pavilion at Kovalam where the National IMA Conference was conducted. • Classes conducted during this quarter to promote IMAGE one, in a Retired Doctors meeting. • IMAGE CTB copies two given to an Executive engineer and a Highschool teacher at the IMAGE pavilion Keraleeyam. • IDAM NA MA MA video shown to one IMA GB at Chirayinkil • The IMAGE bags and containers are of Good Quality Now • Improper segregation- any cases of improper segregation and needle prick injuries have been reported from Trivandrum, in spite of repeated warnings and orientation classes taken by coordinators incidents are being repeatedly taking place. So strict action to be initiated against the concerned HCEs. Some major HCES are also there. • More than 100 HCEs were Temporarily stopped during this Quarter. • any other IMAGE related matter--The Affiliation Procedures to be speeded up for the HCES run by IMA members. 2. Dr. C. R. Sen, Kollam: With pleasure I am presenting the report of activities of IMAGE, Kollam district for the last quarter of the year, 2023. • There were no collection related issues in the district. Similarly, no long-standing payment issues were also reported. • A total number of 74 new affiliations in Kollam district in this quarter. Seven modern medicine doctors could be made IMA members during this period while 15 doctors were reluctant to take the membership even after repeated requests. • IMAGE office was contacted mainly for affiliation related proceedings. • Chairman and Secretary were contacted to inform the inability to attend the Managing committee meeting on 15-10-2023. • Attended the online pre managing committee meeting on 9-10-2023. Coordinator in four institutions where retraining was given. • Service to 2 HCEs were permanently stopped as per request of the HCEs. There is no permanent stoppage due to nonpayment of dues. • Initiative taken to distribute the Paristhithi Mitra Award to Dr. K Damodaran memorial hospital (State level third prize) and Kalarirasayana ayuvedic hospital (District level First prize) in the general body meeting of IMA Kollam branch on 22-12-2023. • Visited Image stalls in the State conference venue at Thiruvalla and National conference venue at Kovalam. • An increase in the bed strength was noticed on routine visit by the south zone coordinator in two HCEs and as per the report, the bed strength were updated. In another HCE, the bed strength was reduced from 7 to 6, as per the request of the HCE and subsequent verification. • In short, I am very happy to report that the functioning of IMAGE in Kollam district is very smooth and regular. I am taking this opportunity to thank all the leaders of Image and IMA for the help and cooperation. 3. Dr. Manimaran K., Pathanamthitta 1) New affiliations for this quarter are 23. 2) Nearly 40 Govt HCE has the pending dues of Rs 2,16,639 /- from the PTA District. Rs 18,545 were collected from the 12 Govt HCE. We are following the remaining Govt HCE for the early settlement of their dues. Dr Ramaraju and Adv. Mr. Sunil had helped me a lot in this process. 3) 2 new members were joined in IMA Pandalam branch through IMAGE. 4) HCE visit - none. 5) IMAGE office contact: 5 times. Total Rs. 78491 Three Doctors were made IMA member while recommending new affiliations, during the quarter. During the quarter the District Representative made 2 visits to General Hospital Alappuzha- GT ALP.0005. There were some ambiguities regarding the invoice raised in October 2021 and it is being cleared. Other HCE visited was Cherthala Taluk Head Quarters Hospital where again the superintendent expressed her inability to trace the relevant documents as it was transferred to the record section. During the quarter IMAGE office was contacted many times in relation to clarity of pending dues as well as receipts from those HCE’s. Improper segregation was reported from 4 HCE’s during the quarter. On 15th October ALP.0026 VSM Hospital Mavelikkara, where IV bottles were found in yellow bags. On 16th October ALP.1098 Rishiram Medical Centre, where needles were found in red bag and IV bottles in yellow bag. On 17th October ALP.0034 Sahrudaya Hospital, where food and general waste were found in yellow bag. Poorly maintained storage space was also noticed. On 20th October GT ALP. 0151 Veterinary Dispensary, where general waste was found in red bag. Appropriate actions were taken against these HCE’s and gave warning those institutions do not repeat such mistakes in future. 15 HCE’s were stopped for nonpayment of operational charges in the district in the quarter. On 13th December Paristhithi Mithra Award District level certificate was handed over to Kinder Hospital in the General Body meeting of IMA Cherthala branch held at Kinder Hospital auditorium. 5. Regarding the quality of bags, no complaint was reported. Improper segregation was not reported. New affiliations given were mainly non allopathy. 9. As usual the First Quarter of the IMA Year 2023-24 has gone through very smoothly at Wayanad Dist. Regarding activities: - There were no Collection related issues at Wayanad during this period. There were no issues at any of the HCEs, where my presence is required. IMAGE office has contacted me twice to confirm about the affiliation of Dr Rahina’s Clinic, Panamaram and Dr Rajalekshmi’s Doctors Scan at Kalpetta. Nobody from Wayanad has visited the IMAGE Plant and not conducted any meetings or classes in relation with IMAGE during this quarter. Quality of bags and containers supplied by PEPS are in good condition as I confirmed it from my own Clinic. No improper segregation has been reported at our area. We had Nineteen New Affiliations: - OCT 23 - Lab 2, Veterinary 1, Palliative Home Care 1. NOV 23 - Dental 2, MM Clinic 1, Ayurveda 1, Scan Centre 1. DEC 23 - MM Clinic 5, Lab 2, Dental 1, Ayurveda 1, Tattoo Studio 1. We are forced to stop our service to Thirty-Five HCEs during this quarter - Eighteen at October, Thirteen at November and Four at December. By regular follow up we could restart our service to Twenty-six HCEs - Seven at October, Thirteen at November and Six at December. That’s all about activities. It’s very positive to know that some of our old HCEs are coming back for our services. There was critical time for us when the workers given notice to start strike. But anyhow for our good sake the strike has been compromised. Again, this quarter we have received for our IMAGE Docufiction Idam Na Ma Ma the “Best Second Documentary Award” on MSVF 2023 At IMA NATCON TARANG: We could present Biomedical Waste Management-IMAGE Experience by our Secretary Dr Sharafudheen at Management Conclave. A big congrats to our deserving Secretary Dr Sharafudheen for receiving the prestigious IMA National President’s Appreciation Award for his outstanding contribution towards IMAGE. Govt Addl Secretary has signed MOU for the revision of rates for one year ending 27th Dec 2024. District Wise First Prize of IMAGE Paristhithi Mithra Award will be distributed to Family Health Centre, Noolpuzha, Wayanad on 18th January Thursday at 3PM. All these successes have come due to our combined effort especially Our Chairman Dr Abraham Verghese, Secretary Dr Sharafudheen and the whole IMAGE Family. In between we had a very sad news, the sudden demise of the nephew of our Chairman Dr Abraham Verghese (brother’s son). Our Heartfelt Condolences to Abraham Verghese Sir and the bereaved family. That’s all from Wayanad. 13.Dr. Bennet Chacko, Kannur • New affiliations during Oct-Dec 2023 - 65 • All new affiliations send to me were personally interviewed and documents verified. • Improper Segregation - 7 HCEs • Proper explanation has been sought, training has been conducted and collection restarted after completion of training. • Major pending dues were in KNR.0622 -5 Lakh and KNR.0004 -1.98 Lakh. • The pending dues were informed to the concerned authorities and steps taken to follow up the payment of the dues. • Contacted the IMAGE office for clarifications regarding affiliations and the response of the office staff was good. • Contacted the IMAGE secretary once regarding the issue of an IMAGE staff getting injured on duty at Taliparamba. • Attended MCM meeting at Adoor, Hotel Wyte Portico on 15/10/2024. • The distribution of Paristhithi Mithra Awards (3rd prize dental clinic) is being planned for January 2024 after discussing with Iritty IMA leaders. 14.Dr. Kishore Kumar, Kasaragod 1) Two collections related issues were sorted out by my personal approach in HCE. 2) Regarding collection of dues., Image staff was contacted and asked to inform if there are any difficult issues. 3) One doctor was recruited as IMA member. 4) None of HCE were needed to be visited for any cause. Dr. Velayudhan K., Northeast Zone: Attended IMAGE MCM meeting at White Portico Adoor along with Dr. Rasitha, District Representative, Palakkad on 15th October 2023 and I visited Adoor Kinfra Park site (for IMAGE proposed plant) with Dr. Suresh Kumar M. and Dr. Rasitha. K. Most of the meeting attended leaders also visited the site. It is an interesting fact that the visitors to IMAGE plant is increasing, and their remarks and notes are encouraging and will boost public image about IMAGE plant and activities. On 02-12-2023 Students and faculty from PMSA Vocational Higher Secondary school Chappanangadi and on same day another team of Students from Malampuzha, Ashramam Higher Secondary school, a model residential school under Tribal Development Department Palakkad. Our staff Miss. Anjitha, Sri. Silvester, Sri. Jinish, Sri Sudheer and the GJM staff Sri. Naveen received these teams enriched them with all information about IMAGE, IMAGE plant, details of Biomedical waste Treatment Facility and functions, ETP, STP etc. They have done wonderful communication and teaching which is very well reflected in Students remarks and notes. Pragathi Environment Engineer Ms. Resmy M.R was present to provide an explanation to the Committee. Dr. Sureshkumar M, Convenor, New Project Committee, Mr. Peter Samuel Manager (Operations) IMAGE, Mr. Sunil P.A Manager Admin & Mr. Pramod, Technical Consultant were present at the site. The first online meeting of the New Projects Committee along with other partners involved in the implementation of the Adoor Project & the officials of IMAGE was conducted on 07th Dec 2023. The persons doing the water management study Mr. Thampi & team visited the site along with Dr. Sureshkumar, Mr. Peter Samuel & Mr. Pramod on 11.12.2023. M/s. Brace Consultants managing the Civil works visited the site on the same day. After the visit we had a meeting at Hotel Park Residency where these consultants were staying, and they promised to speed up the paperwork to get speedy sanction from SEAC. Mr. Shameem conducted a Contour study and submitted the report. The second review online meeting of the New Projects Committee was conducted on 11.01.2024.
