Chemotherapy Safety

Chemotherapy safety is a great concern not only for patients but for the
nurses that
administer these lifesaving medications. Starting a Chemotherapy
Safety Initiative
Project was the focus of my ACE III project. This is
an ongoing task force
collaborating with Dr. Mauer's office, Infusion
center, Pharmacy, Laboratory, and our
EVS departments.

GOALS

Our goals were:

Increase the safety of administration of chemotherapy.
Increase CNA education re: handling body fluids of chemotherapy patients.
Identify patients who are currently receiving or have received chemotherapy
in
the past 48 hours by using a blue cart on the patient's door.

We first determined that all 4 North nurses would complete the ONS Chemotherapy
Biotherapy Certificate
Course to ensure that only Chemotherapy Certificate
nurses administer chemotherapy. We also
determined that we should follow
the ONS recommendations of using proper personal protective
equipment
or PPE to include chemotherapy approved gloves and gowns. Our task force
determined that
we were not using proper PPE by not wearing chemotherapy
approved gowns while administrating
chemotherapy.
OUTCOMES
All 4 North nurses are now currently working towards completing the ONS
Chemotherapy Certificate
Course. Hopefully all will be completed by the
end of 2016. We now include chemotherapy approved
gowns in our PPE for
chemotherapy administration and all 4 North nurses have had education
on proper
use of the PPE. We now have blue carts that are placed on a
patient's door to identify them as
chemotherapy patients. Ongoing
education continues for staff on proper handling of body fluids.

FUTURE GOALS

We are currently working with Pharmacy, Lab, and EVS on how to manage
chemotherapy spills. We are
also working with Dr. Maurer's office,
Pharmacy and our IT department on implementing chemotherapy
orders via
COPE. We also work with our Education department to provide ongoing education.

As the Stafford Hospital (SH) Lead Endoscopy RN, my manager approached
me to

preparation for Joint Commission. I recognized the need for the implementation
of a
uniform SH Endoscopy CBOT and Annual Competency compliance by Joint
Commission standards. My ACE project focused on preparing SH Endoscopy
CBOTs for Joint Commission and initiating Annual Competencies. I am also
the
designated SH Endoscopy Policy Reviewer by management since 2013.
As a result of the project study, 3 Endoscopy RNs and 6 Techs were identified
with
incomplete/inconsistent CBOTs. No Annual Competencies had been completed
for
3+ years. I have taken ownership of the SH Endoscopy CBOTs. It is
a priority to
have 100% compliance.

I sent out emails to each RN and Tech to meet with me to complete their
CBOTs.
We are required to be 100% compliant when Joint Commission visits
SH. I created
an Endoscopy Annual Competency check off sheet. I emailed
Endoscopy Reps for
training and In-Services.
A literature search was performed using CINAHL and consistent data found
initial
competency should be attained and annual competence should be
maintained.

I plan to continue to review SH Endoscopy CBOTs annually. I will ensure
that RNs
and Techs complete Endoscopy Annual Competencies. I will foster
a good working
relationship with the Perioperative Nurse Educator to continue
Joint Commission
compliance.


Project Updates:
SH Endoscopy CBOTs were completed October 31, 2015.
Endoscopy Annual Competency Form is used in real time, checking off skills
as it is performed during a procedure.

MSET team. Being able to do two follow ups at 4 and 8
hours post call at Stafford
Hospital was proving to be difficult (MWH
MSET RNs out of staffing and complete
two follow ups post MSET call).
After researching more about patient deterioration
following MSET in cases
when the patient wasn’t transferred to an ICU, I discovered
that
the more appropriate window to follow up is within within 6 hours.

In collaboration with Debra Marinara and David Squeglia, we adapted the
SH
MSET/Rapid Response Policy to fit our hospital needs. After policy
change, I wanted
to be able to see if this new policy was being complied
with specifically the 6 hour
window and no longer than 8 hours.

After educating the ICU nurses at our monthly staff meeting, I developed
a
performance improvement tool to collect data in March-August of 2013.
It was noted
that there was poor compliance with appropriate documented
follow up. It was noted
that our MSET RNs had a wide range in their ICU
experience levels some being just
at 6 months in the ICU. Therefore I
was going to need to relook at education of ICU
the staff. I adapted an
existing MSET CBL for new hires to focus on Stafford and
included a post-test
to monitor understanding. In addition I developed a CBOT for
MSET RNs
at Stafford.

Over the next 6 months, there was a sustained improvement in follow up
and
documentation of the follow up visit. I also developed an evaluation
form for the ICU
RNs to be able to voice problems they encountered during
their MSET calls, i.e., no
one present in patient room, no current vital
signs. Likewise, I also developed an
evaluation form for the floor nurses
to communicate their issues with MSET visits.
This information was discussed
at the SH MSET/Resuscitation meetings and helped
guide our plans for better
communications between the ICU staff and the inpatient
floor RNS.

The next steps in this project will be to look at patient outcomes post
MSET and
resuscitation at my facility. We have already demonstrated a
reduction in patient
code blues per patient days from 0.37% in 2013 to
0.16% in 2015. We will look at
overall patient outcomes at time of discharge
and whether patients were readmitted
within 30 days. MSET calls within
6 hours of ER admission are currently referred to
the emergency department
manager for further review.

baccalaureate degree from 50 to 80 percent by 2020. These leaders
should partner
with education accrediting bodies, private and public funders,
and employers to
ensure funding, monitor progress, and increase the diversity
of students to create a
workforce prepared to meet the demands of diverse
populations across the lifespan.
Based on this recommendation, a Strategic
Workforce committee was convened at
Mary Washington Healthcare (MWHC).
From this committee, conversations
convened between University of Mary
Washington (UMW), Germanna Community
College (GCC) and MWHC to develop
a RN to BSN program at UMW. MWHC
Foundation donated $25,000 to UMW to
hire a nursing consultant to assist in
creating the program proposal.

In April 2013, the UMW Rector and Board of Visitors approved the proposal
for the BSN completion
program. After submitting and being granted approval
by the State Council of Higher Education the BSN
Completion Program was
initiated fall of 2014. MWHC Foundation committed tuition reimbursement
for the
registered nurses attending RN to BSN completion studies. As well
as supporting flexible scheduling for
associates to attend classes, MWHC
supports Dr. Lisa Lucas (Director, Clinical Informatics) and Dr. Mary
Jane Bowles (Clinical Nurse Specialist, Critical Care Services) to serve
as adjunct faculty for UMW.


May 7, 2016, UMW conferred the first graduating class of RN to BSN graduates.

PICC Line Reduction Team: A Multidisciplinary Approach

Services
The purpose of the project was to evaluate MWHC’s approach to the
use of
peripherally inserted central catheters (PICC) lines and develop
a process to reduce
their use. Mary Washington Hospital placed 3.5 times
more PICC lines (2014 HMC

benchmark data) than hospitals in our benchmark group

Background information:PICC lines are invasive, placed in the central compartment and are associated
with higher risks than peripheral vascular access devices. CLABSI (central
line associated blood stream
infections) are reportable quality metrics
which can impact reimbursements & increase length of stay.
Treatments
are often delayed during off-hours when patients are waiting for line
placement. All Stafford
patients had to be transported to MWH during off-hours/weekends
by ambulance for line placement as
they did not have the coverage for
this service during off hours. Transporting patients round trip from
Stafford
Hospital to MWH for vascular access costs $730.20 for Advanced Life Support
transport and
$280.77 for Basic Life Support transport.


Four main areas drove the project – Patient Safety, Access, Quality,
and Cost. From a patient safety
perspective there was a desire to reduce
the number of central line associated blood stream infections
(CLABSIs)
and deep vein thrombosis (DVT) events in the upper extremities of patients
receiving PICC
lines. Placement of the lines was limited to the time Interventional
Radiology was open as well as
scheduling opportunities for inpatients.
There was a concern that the placement of PICCs and central lines
was
excessive and at times inappropriate based on the patient’s clinical
need for the line. Finally, the costs
associated with the existing process
for placement of PICC lines – device costs, patient length of stay,
and
actual delays in patient care associated with the delay in line placement
– needed to be reduced.


Based on this, a multidisciplinary team including representation from
Anesthesia (CRNAs), Interventional
Radiology, Hospital Administration,
Vendors, Nurse educators, Medical Staff, Finance & Coding, Nursing
Informatics, Endoscopy, IV Therapy, and Central Supply convened to develop
a project plan. The
Performance Improvement Project was designed to determine
if there was an indication for Midline
catheters at Mary Washington Healthcare
as well as to determine whether CRNAs could support
Interventional Radiology
by placing the midlines during off-hours. The advantages to a successful
Midline
Catheter project included:

Patient Care: Timely line placement and elimination of ambulance transports
from Stafford
Hospital on nights and weekends

Patient Outcome: Utilization of a less-invasive alternative to central and
PICC lines,
meaning less adverse complications and potentially shorter
hospitalizations
and readmissions

Patient Satisfaction: Can be performed in the patient’s room. Eliminates the
need to
replace a peripheral IV catheter every 3 days – less needle sticks.
Patient can go home with the midline

Reduced Costs: Midline catheter cost is $210.28 less than a PICC line.
CRNAs are already
in-house during off-hours to provide back-up support to

Interventional Radiology

Components of the planning phase of the project included:

Explored the benefits of introducing a midline catheter into the health system
Explored options by networking with “best practice” hospitals
for practice
algorithms and devices which was vetted with IR and ICU colleagues
Evaluated midline benefits and options to include dwell times, utilization
&
expense metrics
A vascular access algorithm needed to be developed for use when consulting
all patients for PICC line access
A device needed to be chosen and piloted using the algorithm for decision-
making
and extensive nursing education would be required
Ongoing Assessment of value and pitfalls, making adjustments to insertion
techniques, supplies, care and maintenance.

The team established measureable goals for the project: selection, trial
and successful implementation of a
new process for midline catheter placements
by February 2015 and utilization of in-house CRNA to place
midline catheters
off-hours and weekends when IR is closed by August 2015. Metrics identified
to measure
the success of the project included:

Plan analysis for cost-effectiveness and efficiency;
Increased utilization of midline orders/volumes;
Financial savings identified through the use of midline catheters;
Reduction in number of patient transports from Stafford to MWH on weekends
for line placements; and
Increased ability to initiate treatment modalities faster.

Midline catheterization is an important intermediary option for peripheral
intravenous

access. The tip of midline catheter terminates below the axilla
compared to a PICC
line’s tip which terminates above the right atrium.
This use of this catheter covers the
infusion window gap. Peripheral IVs
remain in place for 1-3 days. Midline catheters
may remain in place for
up to 28 days. Peripherally inserted central lines may
remain in place
for anywhere for time frames greater than 28 days. While less
invasive
than PICC, midline catheterization remains an advanced practice skill
that
requires placement under ultrasound guidance.
Termination of Catheter Tip

The project team consisted of 4 main phases. Phase I consisted of multidisciplinary
team meetings and development of a work plan that included the creation
of
vascular access practice algorithm (MWHC PICC / Midline Screening Tool

attached below); the trial new midline product in Interventional
Radiology; creation of
procedural note and billing forms; development
of nursing education, revision of
policies and ordersets; and formally
defining the CRNA support hours at both Mary
Washington Hospital and Stafford Hospital.

Phase II consisted of the development of the equipment kits for floor
midline placement and the evaluation
of equipment options. Factors included
in the evaluation of the equipment included: ease of use (time for
placement/insertion),
error rates, product interchangeability and cost. Additional actions in
this phase
included staff training on computerized order entry, CRNA education
related to the use of ultrasound and

devices, formal expansion of the
CRNA scope of practice, identify and correcting billing opportunities,
and
expansion of in house CRNA coverage at Stafford Hospital. The administrative
components of
operationalizing this new model included the revision of
existing policies and ordersets, creation and
definition of billing and
CPT codes for Interventional Radiology and the CRNAs, and the creation
of an
algorithm for the PICC line reduction team. Finally logistical elements
were addressed prior to the project
go live. These included the completion
of an assessment of the current equipment used in Interventional
Radiology,
obtaining mobile ultrasound equipment for line placements outside of IR,
development of
standardized midline carts for line placement at the bedside,
collaboration with Supply Chain for
establishment of equipment PAR levels
on nursing units, in IR and on the midline carts; investigation of
procedural
device alternatives with the device vendors, outreach communication of
process redesign to
include community physicians, hospital medical staff,
case management and nursing, orderset build into
the electronic system.


The CRNA group was impacted the most with training. Several methodologies
were used in the training
approach classroom, one to one teaching, observation,
and skill validation. Both group and 1:1 training
approaches to education
were used. Content included:

Midline catheter device training using reference power points, development
of
a YouTube video-based demonstration, procedural placement checklist
Observated midline catheter placement in IR (NPs, Radiologists and
Technologists)
Ultrasound techniques and hands-on training

All 50 CRNAs attended an 8-hour ultrasound course
All 1:1 CRNA training facilitated by Mrs. Hagmaier for system-wide
consistency
in practice
CPOE Order Entry training

Project Evaluation:

The two phased project timeline of Interventional Radiology implementation
of new
product go live February 2015 and CRNA model go live August were
successfully
achieved.

Initial Volume Data (as of 8/10/15):

Data for line placements from February 2015 through August 10, 2015 revealed
1143 (69%) PICC line placements and 521 (31%) Midline placements. Based
on the
defined placement algorithm the rate of conversion from initial
orders for PICC lines
to midlines was at 20.9%.

Initial Financial Data:
Cost Comparison
A cost savings of $210.28 per midline placement was demonstrated yielding
a total
cost savings February 2015 – August 10, 2015 of $109,555.88.

The incidence of CLABSI was noted to have been positively impacted by
this
project.

Overall, an increase in midline catheter placements has been noted with
CRNA
successfully placing over 30 lines during off hours in the 6 months
period after go
live. Both physician and nurse satisfaction has been noted
with the ability to perform
the midline placement at the bedside. Financial
savings were seen. Additionally, a
reduction in patient transfers from
Stafford Hospital to Mary Washington Hospital
solely for vascular access
occurred. Unexpected benefits of the project included the
additional option
of midline catheters placement at the bedside for unstable patients
that
would otherwise receive a more invasive line (central line) due to their
inability
to travel to IR and the availability of CRNAs to support IR
overflow thereby reducing
patient wait time for vascular access allowing
for faster initiation of ordered therapies
and collaboration and enhanced
teamwork between IR and the CRNA group.
