Congratulations!

Congratulations on challenging your final semester of dental school! This publication
is one of several created by the Office of Student and Diversity Affairs to prepare you for that

special day of recognition and celebration. Your Commencement ceremony marks the
beginning of your professional career in the world of oral health. We are working
to provide you with tools and resources for your new and exciting journey.

We have scheduled ADA Success Seminars, resumes writing workshops, a career fair and
other professional development sessions for your classes. We hope you choose to take
advantage of these great programs. This is a very exciting time, but if you do not plan
ahead, all of the programs and events can be quite overwhelming. We hope this guide

helps you to organize all of your upcoming deadlines, events and sessions. We are excited
to be on this journey with you.

SAVE THE DATES

Tuesday, January 23- DMD and DMDAS Classes of 2018 Spring Class Meeting.
Meeting will take place from 1pm to 2pm in Lecture Hall South. This is a mandatory
meeting as important commencement information will be presented.

Tuesday, January 23 and Wednesday, January 24- Graduation Composite Photo
Session.

Wednesday, January 24- UIC Career Services Resume Writing Workshop for
Dental Professionals. Workshop will take place from 1pm to 2pm in Lecture Hall
South. See page 5 for more information.

Thursday, January 25- American Dental Association Success Seminar on Finding
a Job. Presentation will take place from 1pm to 2pm in Lecture Hall South. The Illinois
State Dental Society will be sponsoring boxed lunches for the DMD and DMDAS
Classes of 2018.

Thursday, February 1- UIC Student Legal Service Introduction to Employment
Agreements for Associate Dentist. Presentation by Attorney Ted Kwon from 1pm to
2pm in Lecture Hall South.

Friday, February 2- UIC College of Dentistry 2018 Career Fair. See page 4 for more
information.

Tuesday, February 13- American Dental Association Success Seminar on
Understanding Employment Agreements. Presentation will take place from 1pm to
2pm in Lecture Hall South. Attendance is mandatory.

Wednesday, May 9- Exit Meeting. The mandatory exit meeting will take place at
9:30am in Lecture Hall South. At this meeting you will receive your Commencement
regalia. In addition, information regarding financial aid/loan consolidation and
repayment information will be provided by the Office of Financial Aid.

Saturday, May 12- UIC College of Dentistry 2018 Commencement Ceremony. The
ceremony will begin at 12:30pm. Doors will open at 11:30am for general public seating.
All graduates and family hooders must arrive by 9am.

AWARDS

The UIC College of Dentistry has a long standing
tradition of recognizing dedicated faculty members
who strive for excellence in the classroom, clinics or
labs with the presentation of a golden apple. In
May, your class has the opportunity, for the very last
time, to recognize a faculty member who has truly
made a lasting impression upon your class.

Your class president will be accepting nominations for your final golden apple award. Please be
sure to submit a nomination of a faculty member who helped to make your time in clinics and
classroom exceptional.

DECLARE YOUR

INTENT TO GRADUATE

Have you already submitted your intent to graduate? The deadline to declare your intention is this
Friday, January 26, 2018. This is one deadline you do not want to miss! The UIC College of Dentistry
Director of Office of Academic Affairs, Mr. Tim Sullivan, has created easy to follow instruction on how to
submit your intention:

To declare your intention to graduate you must complete the online Intent to Graduate. Students
should go to https://my.uic.edu and login. In the "University Links" portet, click on
"Student/Faculty/Staff Self Service Login - Main Menu." Then click on the "Graduation" tab and you will
see the link "Apply to Graduate" and then follow the instructions.

• Your current program and degree information should appear. If it is incorrect, or if you are currently
attempting to apply to receive a degree that is not listed (receiving a master's while in a professional
program, for example), submit a graduate degree that is listed. A text box will be provided in a later
screen where you can type in any corrections or messages.
• If the program and degree information appears to be from a previous program for which you are no
longer interested, follow the same procedures listed directly above.
• If you are working on two programs in a joint program, information for both should appear. If not,
provide the information in the text box that you are also seeking the missing degree/program.
• If you are working concurrently on two programs and are receiving only one for this term, provide that
information in the text box. (Note: this is only for concurrent programs. Students in officially approved
joint degree programs must receive both degrees in the same term.)

Make sure that you complete the text box, if degree information needs to be changed.
Once you submit the Intent to Graduate a confirmation will appear on the screen, which you may print.
You need to wait for that confirmation to appear before leaving the browser, as that is the only notice
you receive of successful submission. If you submit an Intent to Graduate successfully and you later try
to submit another one for the same term, the system will not allow you to do so.

The Intent to Graduate site also allows you to input a diploma mailing address. Diplomas are typically
mailed one or more months after the end of the term of graduation.

College Clearance

Checklist

Be on the lookout for emails from UIC College of Dentistry Office of Academic Affairs Assistant
Director, Mr. Carlos Finney, regarding your college clearance checklist. If you have not received
clearance in the following areas your diploma will be held until you have completed the check out
process. In future emails Mr. Finney will provide more details on what is required to check out of
each area for a successful clearance.

Please note that you will need to check out of the following areas
-Clinic Dispensing
-Central Lab
-Lockers
-Managing Partners
-NBDE II Passing Score (D4s only)
-COD Exit Survey

Continuously checking emails for pertinent graduation information is the best way to ensure that
your graduation process is an easy and smooth transition!

HONORARY

FAMILY HOODER

Interested in having an honorary family hooder, but unsure what the qualifications are to request
one? An honorary family hooder must be an immediate family member with a doctoral degree
(domestic or international) in a healthcare discipline. If you plan on having an honorary family
hooder please collect the following information about your hooder:
• College/University of their DDS/DMD/MD/PhD degree
• Field of Study
• School Colors
• Height and weight
Once you have confirmed your honorary family hooder please complete the Family Honorary
Hooder Regalia Form: https://goo.gl/forms/g5fbfzaembgLeRJ03. All honorary family hooder
requests must be submitted by Monday, February 12, 2018.
