ALL NURSING PROGRAM APPLICATIONS ARE ONLINE

• ALL Nursing Program applicants will need to use the online application, and
will NOT be required to turn in hard copies in Admissions.

• Make sure you have the following documents scanned to PDF format before
you start:

• Signed Nursing Specific FERPA (download from Nursing Website),

• a Credit Advising Credit Transcript (directions are with Application Instructions),

• High School final Transcript/Copy of Diploma or GED-(if applicable), and

• Hepatitis B Quantitative Titer

• If you are a Transition Applicant, a copy of your current license with issue &
expiration date on it (not the “diploma” style license).

• You will then go to http://www.lonestar.edu/nursing-tomball.htm and click on
the BLUE “Apply” button.

• Follow the directions on the instruction sheet (click the top link in the
instructions to get to the application). Read the FAQ’s that pull down with
frequently asked questions.

READ CAREFULLY! HEPATITIS B TITER: HBSAG

(Lab result must be reported as a NUMBER)

• Is a blood test to see if you are immune to Hepatitis B after your complete
series of three (3) vaccinations. It cannot be drawn until AFTER a minimum of
30 days from your last shot in the whole series of 3 vaccinations.

• It must be current within the last THREE (3) years (if longer than 3 years ago,
have it RE-DRAWN BEFORE YOU GET ANY BOOSTER VACCINATION).

• You must have a titer indicating “Immunity” as a numerical value at the time
of application (known as a positive titer).

• **NOTE: If your titer values indicate you are not immune, then you must
submit evidence of your first series, the negative titer, and evidence of at least
your first booster in the next series of 3-4 boosters with your application.

Scan all three of those documents into a single PDF file to upload with your
application.

HEP B (CONTINUED)

If this is your FIRST SERIES, you must have the completed series of

vaccinations PRIOR and TITER to application deadline:

• We highly recommend the CDC-approved, Regular Series if you have enough

time:

• Vaccination (Booster) #1 (day 0)

• Vaccination (Booster ) #2 (day 30)

• Vaccination (Booster) #3 (day 180)

• Redraw the QUANTATATIVE TITER (day 215)

This series will take a minimum of 7 months to complete, so do this AS SOON AS
POSSIBLE BEFORE THE DEADLINE!!!! If the deadline is less than 7 months, then
you may use the accelerated series, which is a different vaccine from the Regular
(see next slide)….

• **Failure to keep on the schedule with shots or titers can disqualify you from
completing clinicals, and thus the program.

HEP B (CONTINUED) IF LESS THAN 7 MONTHS PRIOR TO APPLICATION DEADLINE

• We recommend the CDC-approved Accelerated Series with Twinrix vaccine
(for Hepatitis A and B) if you run short of time:

• Booster/vaccination #1 (day 0)

• Booster/vaccination #2 (day 7)

• Booster/vaccination #3 (day 30)

• DRAW the QUANTATATIVE TITER (day 65)—*this is where you are then eligible to
apply

• Booster/vaccination #4 (day 365)

• Redraw the QUANTATIVE TITER (day 395)

This series will take a minimum of 2 months to complete prior to having a titer,
so do this AS SOON AS POSSIBLE BEFORE THE DEADLINE!!!! You will be required
to have an extra injection and a second titer must be drawn as well ON TIME one
year later to continue.

LSC – TOMBALL NURSING PROGRAMS

• Associate Degree (ADN) Specialization
Integrated Curriculum (commonly called the “Generic” or “Basic”

Track) = to become a Registered Nurse (RN)**

• Transition Program = for a LVN/Paramedic to become a RN**

• VN Certification = to become a Licensed Vocational Nurse (LVN)**

• CNA (Certified Nursing Assistant) is available after successful completion of the

first semester in the ADN & VN programs. Students may apply to “challenge” the
exam with Texas Health and Human Services. Regular CNA courses are only offered
through Continuing Education

** Licensure is with the authority of the Texas Board of Nursing. Nursing programs cannot guarantee
licensure: graduation is just one step in the process toward becoming eligible to license as a RN or LVN.

PROFESSIONAL INFORMATION

• By 2020 it is a goal that 80% of all RNs in the USA will have
earned a BSN (bachelor of science degree in NURSING).

• Even though there is a Nursing Shortage, new graduates do
not typically walk into an “ideal” work schedule: MOST
new grads start with a “day time orientation” lasting
several weeks but then are regularly scheduled on nights.
Nurses work weekends and holidays. Schedule
requirements vary with different agencies.

• Most nurses work 12 hour shifts and 3 days a week is
considered “full time”, which makes you eligible for
benefits.

EMPLOYMENT TRENDS FOR RNS

• There continues to be a shortage of nurses in the USA. However, many urban
hospitals are seeking Magnet status that requires the majority of staff to hold a
BSN. There are still many jobs rurally or in other non-acute care settings, so
students may need to consider relocation or seeking employment in agencies
other than hospitals after graduation.

• LSC is also committed to assisting students for easy transition to the BSN by
formally articulating with universities. We are working on expanding the
number. We currently have formal agreements with the following universities:

 Assists in the determination of predictable healthcare needs

EMPLOYMENT TRENDS FOR VOCATIONAL NURSES

• Vocational Nursing is one of the fastest growing areas of need in the USA .
Most LVNs work in long-term and skilled-nursing centers.

• Other areas of growing need are clinics, schools and home health.
• 100% of LSC Tomball graduates are placed in nursing jobs.
• Many eventually transition to ADNs and become RNs while working.

HESI TEST DUE DATE CAUTION

• Make sure you TAKE HESI early! The deadline is one week prior to the closing
of the application period. All deadlines are at 4:00 PM.

• The assessment centers schedules and administers the admission
HESI, the nursing department has nothing to do with administering it.
Call the Assessment Center if you have questions.

NURSING ENTRANCE EXAM

• Applicants must test on the following sections of the HESI A2/Entrance Exam at the
same testing session:

a) Reading comprehension

b) Grammar

c) Vocabulary/General Knowledge

d) b. Math

e) c. Anatomy & Physiology

f) Critical Thinking

• Each Individual SECTION must have minimum passing score of 75% for ADN and
Transition & 70% for VN in order to apply. Critical Thinking must be 750 for ADN
and 700 for VN.

• The first 5 (five) section scores will be averaged and divided by 25 to get your
admission test score.

• If HESI A2 exam was taken somewhere other than within the LSC System, official
scores have to be sent directly to Nursing Department by Evolve/Elsevier. See
Nursing Department website for details.

NURSING ENTRANCE EXAMS (CONTINUED)

• Scores are good for one year and all sections must be complete in ONE testing.

• HESI may be taken every 60 days (start counting the Day AFTER you take it) or
greater and is now available year round (check with Assessment Center for dates).
You must complete it ONE (1) WEEK prior to the Admission deadline by 4:00 PM.

• The HESI exam will be administered in the LSC-Tomball Assessment Center on the
second floor of the LSC-Tomball Community Library in Room L201. Specific dates
and times the exam will be offered at each campus are listed in the registration
pages at: http://www.lonestar.edu/17426.htm. Testing is done by Assessment
Centers. Only the most RECENT score set will be used for selection, NOT the
highest. If taken less than 60 days apart, only the first score will be used.

• Five and ½ (5.5) hour time limit (unless you have documented ADA
accommodations). All six sections must be from the same testing session.

ADN BASIC &TRANSITION PROGRAMS:

BIOLOGY, MATH & PHARMACOLOGY REQUIREMENTS

ADN Basic and Transition Programs

• All BIOL courses must be less than 5 years old at the time of the application
and be minimally 4 credits (lecture with lab). If embedded BIOL courses expire,
they will need to be repeated in the appropriate semester.

• Students must be college ready in reading, writing, and math as evidenced by LSC
approved exam scores (within the past 5 years at the time of application to the program)
showing placement into ENGL 1301 and MATH 1314 or higher. If a student has
completed certain coursework, then college-level placement scores will be
waived: ENGL 1301 with a grade of C or better (to satisfy the college ready in reading
and writing) and one of the following courses to satisfy the college level in math:

• MATH 0309/MATH 0310 (if taken prior to Fall 2019) with a grade of C or better OR

• A college-level MATH course listed on the LSC core curriculum with a grade of C or better
(college-level MATH courses only)

• RNSG 1301 (Pharmacology) for the Transition Program must be less than 5
years old at the time of the application deadline and be 3 credit hours.

VOCATIONAL NURSING PROGRAMS

• Students must be COLLEGE READY in Reading, Writing, and Math as evidenced
by LSC approved exam scores (within the past 5 years at the time of
application to the program) showing placement into ENGL 1301 and MATH
1314 or higher.

• If a student has completed certain coursework, then college-level placement
scores will be waived: ENGL 1301 with a grade of C or better (to satisfy the
college ready in Reading and Writing) and one of the following courses to
satisfy the college level in math:

1. Take the credit class again (BIOL 2401) and make a minimum grade
of C. Your highest grade will be used for admission scoring
calculations.

2. Take BIOL 2404 and make a minimum grade of C to refresh both BIOL
2401 & 2402 (if both were taken more than 5 years ago) --Your BIOL
2401/2402 highest grade will be used for admission scoring

RNSG 1301 MUST be current within 5 years for the program requirement if a
basic track student. It must be 3 credit hours if transferred.

• 1. take the credit class again (BIOL 2420) and make a minimum grade of C.

ADDITIONAL REQUIREMENTS FOR CLINICAL
ROTATIONS

• Our clinical sites require annual drug screening after the initial one you may
also be tested again randomly as well as for cause.

• Criminal background checks must be done through the Texas Board of
Nursing: only they can clear you to go to health care agencies. Your
information will be submitted on a roster, and the Board will email you
directions on how to proceed with fingerprinting.

• If you have a hit on your criminal record, a history of substance abuse, or
mental health issues, you must see the Nursing Director at the campus of
your first choice, so you can be submitted early on a roster to the Board of
Nursing to see if you need to start the Declaratory Order (DO) Process. Once
fingerprints are obtained, the TXBON will notify you if you are required to file
a D.O. You are advised to collect official court/medical documents early and
have them ready to submit as it may take 120+ days to get an outcome letter.

• Test of English as a foreign language (TOEFL) with score of 560 or higher on a paper exam, 220 or
higher on a computer exam and 83 or higher on an Internet based exam.

• International English Language Testing System (IELTS ) Academic Test with an overall score of 6.5
with a minimum of 6.0 in all of the four modules for the Academic Test.

• Provide documentation of graduation from high school from an English speaking country such as
Canada (other than Quebec), Great Britain, Australia, New Zealand, South Africa, and Ireland.

• If graduated from English speaking high school in a country not listed above in #3, provide official
transcripts and a notarized letter from the headmaster/principal of the high school stating that
English was the primary language of all the courses taught. If transcript is translated, it is not
acceptable.

• Provide documentation of graduation from a college or university with a degree in the United
States, or other English speaking countries such as Canada (other than Quebec), Great Britain,
Australia, New Zealand, South Africa, and Ireland. If the transcript is translated, it is not acceptable.

• SCORES MUST BE RECEIVED DIRECTLY FROM TOEFL/IELTS AND by the posted deadlines at 4:00
PM! NO EXCEPTIONS.

Both the National Council for the State Board of Nursing and the Texas Board of Nursing recommends inclusion of English proficiency as
criteria for applicants to insure the applicant's ability to use and comprehend spoken and written English at a sufficient level for entry into
practice. Based on this, proficiency in English is required for LSCS nursing programs for both patient safety and student success.

AFTER SELECTION TO THE PROGRAM: ADMISSIONS PROCESS ITEMS
REQUIRED FOR FINAL ACCEPTANCE

• Letters of Acceptance or Declination are sent via Email to your My.LoneStar.edu
account. (This will NOT automatically forward to D2L, so either check frequently or link accounts.)

• Letter of Intent must be returned by deadline date (bring, fax or email scanned
copy with signature)

• CPR Certification: MUST be American Heart Association (AHA) BLS for Providers.
• Completed TB/PPD, Hepatitis B titer, Hepatitis C screening, Booster Immunizations

(if needed) & titers proving immunity for MMR & Varicella* with numerical values
only.

• Satisfactory Medical Examination, SSN Verification and BON Background Check or
Cleared Declaratory Order (DO) letter of less than one year of age.

*Measles, Mumps and Rubella AND Chickenpox (varicella)

• Submission of TOEFL or IELTS scores if required directly to the Nursing Office
once accepted. (High school graduates from Non-English countries or those taking

GED in language other than English-see college website for details)

Tracker and separate SSN verification account must be completed by
deadlines given in your acceptance email.

Missing Deadlines will result in you being
disqualified and being replaced.

HEALTH REQUIREMENTS AT LSC TOMBALL AFTER

YOU ARE ADMITTED

Clinical sites are rotated, so all students must meet the highest
requirement.

• Titers (blood samples tested for immunity) are required for:

• Rubeola (measles)
• Rubella
• Varicella
• Mumps
• Hepatitis C screening
• Hepatitis B must be quantitative (numerical values) . The Hepatitis B titer is the only one due when

you actually file your application, unless you are using accelerated Twinrix schedule, then a second
titer is also required when due.

• TB skin test/gold test or chest x-ray is an ANNUAL requirement (get it close to the deadline
given in your acceptance letter so it is good ALL year.

• Annual Flu season immunization (August-September of admission year). Nasal spray is not
acceptable.

HEALTH REQUIREMENTS-- CONTINUED

• If you ever had a positive TB skin Test, you will be required to submit proof of
that reaction along with an ANNUAL chest x-ray specifically to rule out
Tuberculosis. You may also submit a Quantiferon Gold test in lieu of the x-ray
a current . If you no longer have the report, you maybe required to have
another PPD prior to the chest x-ray.

• Hepatitis B titer must be indicative of immunity by NUMBERS. If non-immune,
you must have the series of BOOSTER vaccinations and will be required to stay
on schedule with them. Hepatitis C Screening is the only one that can read
“positive” or “negative”.

• Background Check positive and/or Declaratory Order was not filed early enough to
get clearance BON letter, or is too old at Texas Board of Nursing

• Lack of Complete Health requirements or Titers for hepatitis B, measles, mumps,
rubella and chickenpox. Hepatitis C screening. Note: Hepatitis B series takes 3-7
months to complete!

• PPD/TB skin test/Gold Test not complete (it takes 72 hours) or not current through
end of semester.

• Missing RNSG 1301 Pharmacology course deadline. It MUST be completed by the
end of Nursing I in the fall. (You can take pharmacology prior to starting the
program as long as it was within the past 5 years.)

• TOEFL/IELTS SCORE NOT AVAILABLE by deadline set in admission documents.
