Linn Community Nursing Home inc, AED ACQUISITION

Kansas Health Foundation Fund, $639, Awarded 10/21/2019

Project Summary: Install an AED (Automated External Defibrillator) in our nursing home to be available for use with
residents, staff, day care children or visitors.

Use of Funds: We purchased an AED and a Cabinet.

Impact & Results: We have an AED now available for our residents, families, employees, day care (adult and child)
and visitors to Linn Community Nursing Home. We have purchase a unit compatible with our ambulance service
systems so we can enhance lifesaving care if needed.

KSDS, Inc., Puppy Kits

Washington County Opportunity Fund,
$1,034, Awarded 04/24/2019

Project Summary: KSDS (formerly Kansas Specialty Dog
Service) is requesting funding for puppy kits for our puppy
raisers. Kits include a KSDS plastic bag, puppy manual,
leash, collar, id card, wash cloths, poop bags, and a bro-
chure and a donated puppy blanket.

Use of Funds: Grant funds were used to purchase a quanti-
ty of 25 of The Puppy Primer books, 100 cotton KSDS logo
imprinted bags, and 30 collapsible travel silicone bowls.

Impact & Results: The items purchased for the puppy kits enhanced the volunteer puppy raising program at KSDS
because it will assist the puppy raisers as they begin their journey in raising a little puppy. The Puppy Primer book
offers suggestions and facts about puppy raising, the new cotton bags with the KSDS logo on them helps promote
KSDS, as it has the logo, website, and phone number on it, and the collapsible bowls are very handy to have when
traveling with a puppy.

We have ordered enough of the bags to provide to 100 puppy raisers.

City of Hanover - Hanover Park PRIDE,

Project Summary: Our project includes sidewalk for the Vets Park and making it
handicap accessible for all to enjoy.

Use of Funds: The grant funds were specifically spent to add a handicap accessible
sidewalk at Vet’s Park playground in Hanover.

Impact & Results: Adding the component of a handicap accessible sidewalk to
the north side of the playground is what has changed as a result of our project. This
allows any individual in a wheelchair to gain access to the playground area without
the burden of going through the grass or over the edging piece. They now have the ease of watching children and/
or grandchildren play on two sides of the playground area where the children are visible. This added addition to
our Veteran’s Park area now has a sidewalk leading into the picnic area, so many families are able to host events
such as birthday parties, anniversaries, sporting event parties, etc. in this location. If we did not have the grant money
available to us for this project, it may not have been completed when it was and the time period for gaining a
handicap accessible sidewalk may have taken more years to complete. Our group was able to complete the entire
project that was were destined to finish. We had to work over several days as the weather did not cooperate with
us. It was either too hot or rainy, but it was completed and the finished project has added such a nice touch to our
area. We had around 12 volunteers that came out and framed the sidewalk and worked to pour concrete. This
completed project will serve many of the growing families we have in our small town of Hanover. We are very
grateful to have this grant money available to us to better our community.

Washington County Senior Citizens Services, Friendly Corners - Freezer

Kansas Health Foundation Fund, $789, Awarded 10/21/2019

Project Summary: The Washington County Senior Citizens Services is requesting $789.00 to purchase a new freezer
for the kitchen at Friendly Corners. This would be a replacement for an older freezer which is in danger of quitting.

Use of Funds: The grant money was used to purchase a Crosley Freezer for use in Senior Citizens meal preparation.

Impact & Results: The meal site in Washington County was in dire need of a new freezer. We find that there just
wasn’t enough funds to purchase larger items that we were in need of and use on a daily basis. We were needing to
replace a freezer which was running inefficiently and with much noise. We were worried that it may quit all together
and ruin the contents. We can not afford the waste that this would amount too, but we also couldn’t afford to
purchase a new freezer.

The present freezer was donated by a kind citizen in past years, so it has served many years and is getting old and
no longer dependable. We provide a great service to the senior population of Washington County. For only $4.00
a senior at the age of 60 or above can receive a hot nutritious meal. We are also funded by programs through the
state and federal government. The Washington County Commission is a great help to us as well with funds to help
pay the expenses. Especially during the winter months the older population has problems with getting out and
socializing. Depression can set in as well as just the inactivity of the body to keep a healthy mind and body. With this
program we are helping with both to keep everyone a little more healthy by serving nutritious meals. As a result
of the grant funds it puts us to rest in not worrying about the old freezer quitting. This helps all of our senior
citizens in Washington County.
