q HCSF is serving as the Project Evaluator on the Florida Department of Health in
Miami-Dade’s Partnership for Improving Community Health (PICH) project with the
Center for Disease Control and Prevention

q Building on previous community health investments, PICH aims to address a range
of chronic disease risk factors and provide additional opportunities for chronic
disease prevention and management

q This project aims to increase the number of smoke-free public multi-unit housing
complexes; increase availability of healthy food and beverage options; increase
physical activity opportunities in early childcare settings, worksites, and parks,
and by improving community designs to make streets safer for pedestrians and
encourage physical activity; and provide more residents with access to preventive
services and self-management programs for chronic diseases.

The Health Council of South Florida in partnership with Florida
State Senator Rene Garcia, Miami-Dade County Mayor Carlos
Gimenez and key community leaders, have created a multi-sector
public-private partnership called the Miami-Dade County
Residents' Health Initiative.

This initiative is focused on transforming our healthcare delivery
and payment system. This healthcare transformation will ultimately
improve the health, quality care, and reduce overall healthcare
costs through a sustainable, innovative multi-payer payment model
and delivery system in Miami-Dade County.

Working together, we aim to create a successful innovation model
for healthcare delivery in preparation for the upcoming proposals
by State Innovation Models which are funded by the Center for
Medicare and Medicaid Innovation (CMMI).

AIDS Insurance Continuation Program (AICP)

q For over 25 years, HCSF served as AICP’s centralized administrators, in partnership with
15 community-based organization across the state of Florida.

q AICP enables its beneficiaries to maintain vital health insurance benefits ensuring them
access to needed care and treatment and avoids the transfer of HIV/AIDS care cost to
the public sector by decreasing the demands of low income persons with HIV/AIDS off
Medicaid and other public sector programs.

q As the landscape of healthcare changes, AICP must be retrospective in an effort to
maintain its viability and sustainability. A proven track record of successfully preserving
low-income Floridians private health insurance and cost savings to taxpayers is only as
effective as the flexibility to move with the changing landscape.

q As such, effective March 31, 2015, the HCSF no longer administers AICP. All AICP
services will be provided through the AIDS Drug Assistance Program (ADAP) central
offices in Tallahassee and the local county health department in the state.

The Florida Community Health Worker Coalition (FL CHW Coalition) is a passionate group of
volunteers dedicated to the promotion of the community health worker profession in Florida.

q The Health Council of South Florida is the fiscal agent
