WELCOMING REMARKS

On behalf of the organizing committee, I am pleased to extend a warm welcome to all
speakers and attendees from Perlis, Kedah, and Penang to the 1st Northern Malaysia
Pharmacy Research Conference in 2022. This conference is held in collaboration with
the State Pharmaceutical Services Divisions of Kedah and Penang as well as the
Malaysian Pharmacists Society (Kedah-Perlis).
It is vital to incorporate study findings into current pharmacy practice. It enhances patient
care, strengthens services offered by pharmacists, creates the evidence base for
designing and commissioning new services, and advances knowledge of health services.
I hope this conference serves as a platform to enable a productive sharing and exchange
of ideas related to our conference’s theme for this year ‘Advancing Future Healthcare with
Pharmacy Research’.
The conference is expected to facilitate active interactions and benefit all participants in
gaining experience and expanding their network. Through such an orientation, we can
proactively contribute to optimize future healthcare and pharmacy-related services with
research. May the conference be successful and enjoyable. Thank you.

CONFERENCE THEME

Advancing Future Healthcare through Pharmacy Research

As healthcare professionals, pharmacists represent one aspect of the complex and
interdependent healthcare system. Pharmacists should have the capacity to generate and
disseminate knowledge through research and publications, and these endeavors can
subsequently advance pharmacy services, improve patient outcomes, and enhance the
efficiency of health care delivery. The new roles for pharmacists evolve in parallel with
evidence-based practice, a new paradigm in health services delivery. Therefore, the
conference’s theme is ‘Advancing Future Healthcare through Pharmacy Research’.

OBJECTIVES

• To encourage research presentation among pharmacists
• To disseminate research outcomes among researchers
• To gather researcher pharmacists around the Northern region of Malaysia to

share new ideas and learn from each other
• To promote and offer recognition to new researchers

OP1 PERCEPTION OF HALAL PHARMACEUTICALS AMONG STATE HOSPITAL

Introduction: Halal pharmaceuticals industry has expanded globally, which is one of the
elements in Shariah compliance hospitals. With the emergence of COVID-19 vaccines, the
Halal status has become a debate causing some to reject it.

Objective: To explore the perception of halal pharmaceuticals among hospital outpatients.

Methods: In-depth interviews were conducted among adult Muslim outpatients collecting their
medications. Subjects were recruited in the waiting area of the outpatient pharmacy, Hospital
Tuanku Fauziah (HTF). Interview sessions were audio-recorded and transcribed verbatim.
The transcripts were analysed using thematic content analysis.

Results: Ten outpatients were interviewed. Overall, the patients were proactive in observing
the label to ensure they received the correct medications. All patients were not familiar and
not curious about the concept of ‘halal pharmaceuticals’ and ‘Shariah-compliant hospital’. The
patients also trusted the government will provide only the safest and Halal medicines to them.
Most patients did not prioritise halal status while choosing their medicines. They still would
consent to receive non-halal treatments for life-saving conditions.

Conclusion: There was a good perception of halal pharmaceuticals among outpatients in
HTF. There are many opportunities for patient education on this topic as the public is currently
not familiar with the terms and halal concept in medical settings.

OP3 HEALTH UTILITY AND HEALTH-RELATED QUALITY OF LIFE OF HIV PATIENTS IN A
MALAYSIAN STATE HOSPITAL

Introduction: Assessment of the health-related quality of life (HRQOL), health utility (HU) and
self-perceived health (SPH) is essential in understanding the wellbeing of people living with
HIV (PLHIV) during the COVID-19 pandemic. There is a lack of studies investigating the three
measures simultaneously.

Objective: This study aimed to determine the factors that influence HRQOL, HU and SPH.

Methods: This was a cross-sectional study using a self-administrated questionnaire among
PLHIV receiving treatment in the HIV clinic of a state hospital in Perlis, Malaysia. Subjects
were recruited in the waiting areas during their routine clinic visits. The Malay language
questionnaire consisted of WHOQOL-HIV BREF to measure HRQOL, EQ-5D-5L to measure
HU and EQ-VAS to measure SPH. Spearman’s correlations and multiple linear regressions
(MLRs) were used to assess the association between the domains of HRQOL, HU, SPH and
the socio-demographics.

Results: 88 respondents were recruited. In the MLR model, an increase in age and income
of MYR 3171-3970 (≈USD 7 7-948) increased HRQOL. Part-time employment decreased
HRQOL. Having tertiary education and income >MYR 8 0 (≈USD 8) decreased SPH.
Mode of transmission other than sex and intravenous drug use decreased all three health
indicators. There was a significant moderate correlation between HRQOL and HU (r=0.421),
HRQOL and SPH (r=0.265), and non-significant correlation between HU and SPH (r=0.201).

Conclusions: HIV patients in Perlis, Malaysia had generally a good quality of life despite the
COVID-19 pandemic. A higher income increased the HRQOL but decreased SPH. Increasing
age, without working part-time increased HRQOL only.

Introduction: Pharmacy Value Added Services (VAS) was introduced by the Ministry of
Health Malaysia to reduce the congestion at pharmacy dispensing counters by providing
alternative measures for patients to refill their prescription.

Objective: This study was conducted to investigate patients’ awareness and attitude towards
pharmacy VAS, and factors associated with the use of this service among patients in Perlis.

Methods: A cross-sectional study was conducted from January to March 2021 at the
pharmacy waiting area of all public health clinics in Perlis. All patients (≥ 8 years old) taking
chronic medications and with the need to follow-up at the health clinic in Perlis were recruited
in this study using quota sampling method. Data was collected using a validated self-
administered questionnaire. Descriptive analyses on the usage, awareness and attitudes of
respondents were performed, followed by multiple logistic regression.

Results: Of 582 samples, the mean awareness and attitude score were 62.7% and 79.3%
respectively. However, only 13% of them utilized the VAS. Factors including age (p=0.001),
gender (p=0.001), ethnicity (p=0.031), educational (p<0.001) and occupational status
(p<0.001), monthly income (p<0.001), duration of staying in Perlis (p=0.003), home to clinic
distance (p=0.002), awareness (p<0.001) and attitude score (p=0.001) were found to be
significantly associated with the use of VAS.

Conclusion: There was a moderate awareness and good attitude towards VAS. Further
studies are required to further identify the reasons of not using the service despite their
awareness and attitude.

Introduction: Pain is a common condition affecting all age groups, ethnicities, and both
sexes that is impacted by various biological, psychological, and social factors and is usually
a high treatment priority for patients seeking care. In Malaysia, the prevalence of chronic
recurrent pain was reported to be around 7%, with a higher rate in the older age group.
Pharmacists played a significant role in managing pain, however there were no published
studies done particularly among pharmacists in Malaysia.

Objective: To assess the level of knowledge and attitude towards pain management among
pharmacists in Hospital Tuanku Fauziah (HTF), Perlis.

Methods: This is a cross-sectional study using the Knowledge and Attitudes Survey
Regarding Pain (KASRP) questionnaire for assessing the knowledge and attitudes on pain
management. A total of 76 pharmacists were eligible for the survey from all pharmacy units
in HTF. The data were collected from November 2021 to February 2022.

Results: 69 pharmacists participated, of this, 23 participants (33.3%) scored above 60%
(having good knowledge and attitude). There was no significant difference between male and
female scores (p=0.153) and years of working experience (p=0.065). There were significant
differences between different age groups (p=0.035) and pharmacy units (p=0.010).

Conclusion: This study discovered that the current level of knowledge and attitudes about
pain evaluation and management among pharmacists in HTF was suboptimal. This calls for
a more effective training sessions to enhance knowledge on pain management among
pharmacists to improve patient care.

Introduction: Liquid oral medications are more favourable among paediatrics than solid oral
medications due to dose flexibility and ease of administration. While solid medications are
cheaper and easier to store and transport, they possess swallowability issues and are
primarily intended for adults. A better understanding of this issue can help prescribers select
suitable formulations for paediatric patients.

Objective: To explore the perception and acceptability towards the use of solid oral
medications among prescribers in the paediatric outpatient clinic of Hospital Tuanku Fauziah
(HTF).

Methods: A qualitative study using in-depth interviews was conducted among ten prescribers
working in the paediatric clinic. The interviews were audio-recorded and transcribed verbatim.
Thematic content analysis was performed.

Results: Ten doctors participated in the interviews. Prescribing consideration was based on
patients’ or parents’ preferences, swallowing ability, availability of medications, dosage-
related factors and cost of medications. Drug administration challenges included patient-
related, drug-related, safety and efficacy concerns. Most prescribers agreed to accept any
request formulation from parents/patients as long as the request is reasonable. Pharmacists
helped prescribers to select appropriate dosage forms for paediatric patients by providing
drug-related information.

Conclusion: Most prescribers had good perception and were open for suggestions about
changing paediatric medications from liquid to solid for those aged 7 to 17 years old.

PP4 PRESCRIBING PATTERNS OF ORAL ANTIBIOTICS DURING AFTER-OFFICE-HOURS

Introduction: Antimicrobial resistance (AMR) is correlated with the misuse and overuse of
antibiotics. Malaysia National Antimicrobial Guidelines (NAG) 2019 made specific
recommendations based on local AMR situation to optimise the use of antibiotics through
rational and evidence-based prescribing. However, this would only be effective if there is
good adherence among practitioners.

Objective: To review antibiotic prescribing patterns for the outpatients of Hospital Tuanku
Fauziah, Perlis, Malaysia during after-office hours.

Methods: A cross-sectional study was conducted from 1st October to 31st October 2020 at
the outpatient pharmacy of Hospital Tuanku Fauziah based on prescriptions received during
after office hour services. A standard data collection form was used describe the prescribing
patterns and to assess their concordance to NAG recommendations. The data was analysed
using SPSS 20.0.

Results: A higher prevalence of antibiotic prescribing discordance to NAG 2019 among
females (124, 51.7%) compared to male patients. In terms of prescribers, specialists (63.6%)
were found more in accordance with NAG 2019 compared to the medical officers (51.8%).
Discordance prescribing was more observed among paediatric patients (64.4%) were more
antibiotic prescribing discordance to NAG 2019 than the older group age populations and the
highest prevalence of antibiotic prescribing accordance to NAG 2019 were found among
elderly patients (69.2%). Gender, sources of prescriptions, prescriber category, age were
found to be significantly associated with concordance to NAG 2019.

Conclusions: During after-hours services in HTF, it was found that there were antibiotic
prescribing discordance to NAG 2019. This was especially in the management of UTI.

PROVIDING VACCINATION SERVICES

Introduction: Vaccination is an effective public health intervention in reducing morbidity and
mortality of infectious diseases. Community pharmacists (CPs) in Malaysia are not authorised
to administer vaccines as compared to other countries.

Objectives: This study aimed to assess CPs' readiness and willingness to provide
vaccination in Malaysia, identify potential barriers to and factors supporting the provision of
this service.

Methods: A cross-sectional study was conducted among Malaysian CPs from April to June
2021. A validated online questionnaire was distributed through social media, instant
messaging, email, and pharmacy societies.

Results: Of 492 CPs recruited, 439 (89.2%) expressed willingness to provide vaccination
services to the public, 403 (81.9%) agreed with the accessibility of community pharmacies to
the public, and 73.4% agreed that their role in vaccination could help to improve the overall
vaccination coverage rate. The lack of pharmacist training in vaccination and concerns on
maintaining patient safety were identified as barriers to CPs' implementation of vaccination
services, with 52.8% and 47.8% of them agreed to it, respectively. Training sessions and
operational guidelines on providing vaccination services are required to overcome the
barriers.

Conclusion: CPs in Malaysia were ready and willing to provide vaccination services to the
public. However, the implementation demands official approval from the policymakers,
specific training workshops and constant evaluation of CPs’ skills in providing vaccination.

PP13 PUBLIC PERCEPTION TOWARDS THE ROLE OF PHARMACIST AND THEIR
WILLINGNESS-TO-PAY FOR PHARMACY DISPENSING SERVICES

Introduction: In Malaysia, pharmacists mainly provide their services either at government
healthcare facilities, community pharmacies or at industrial settings. To date, pharmacists in
Malaysia do not charge customers for dispensing fees.

Objectives: The study aims to explore the public perception on the roles of pharmacist as
healthcare professionals and to evaluate the public willingness-to-pay for pharmacy
dispensing services.

Methods: This is a cross-sectional study conducted in Perlis, Malaysia using a self-
administered questionnaire distributed at selected public places. The questionnaire
assessed participants’ demographic characteristics, perception towards the roles of
pharmacists, nature and frequency of pharmacy visits and also willingness-to-pay for
pharmacy dispensing services. Data were then analyzed using SPSS version 20.0.

Results: 380 respondents completed the questionnaire. There were positive perceptions
towards the roles of pharmacists. Majority of the respondents were able to distinguish
pharmacists from other healthcare professionals (54.4%), appreciate the counselling
provided by pharmacists (68.8%) and hoped that pharmacists would play greater role in
healthcare system (66.5%). 44% of the respondents would pay for the pharmacy dispensing
services. Gender, age, race, educational status, and employment status were significantly
associated with willingness-to-pay.

Conclusion: The study provided valuable findings regarding the public perception of the
roles of pharmacist in Perlis, Malaysia. Most of the respondents were found to appreciate the
role of pharmacists and the services that they provided.

LABELLED PRESCRIPTION MEDICATION: A CROSS-SECTIONAL STUDY

Introduction: Labelled prescribed medication aimed to provide essential information on the
proper administration of the dispensed medication. However, patients often neglect the
information provided on the label and resulted in jeopardized patient’s safety.

Objective: This study aimed to investigate patient’s understanding and compliance towards
labelled prescription medication.

Methods: An observational cross-sectional study was conducted among inpatients with
chronic diseases in Hospital Tuanku Fauziah from August until November 2021. The data
was collected using a structured data collection form. Patients’ understanding was measured
using DFIT score whereas MyMAAT scores for compliance.

Results: Of the 72 patients participated, mean age was 59.5±16.0, 55.6% male, 81.9%
completed secondary education, 51.4% employed, 26.4% with >3 comorbidities. Mean DFIT
and My MAAT score were 74.7% and 43.6% respectively. Those with secondary education
level, ≤ comorbidities, < hospital admissions had lower understanding on their medications
(<100% DFIT score) as well as lower compliance (<54 MyMAAT score).

Conclusion: This study confirmed the occurrence of poor compliance to the labelled
prescription medication among chronic patients. Interventions need to be done among the
targeted group to better their treatment outcomes.
