Samrakshan

Sharing an update on Samrakshan outcome
192 Samrakshan Childbirth outcome forms received from 4 states: UP, MP, Kerala and TN
4 neonatal deaths: All 4 were stage 3 FGR, 3 of 4 had EFW centiles < 3 (early FGR), 1 had EFW 10 - 50 centiles (late
deterioration). All 4 were recommended steroids for lung maturation, CTG monitoring and urgent childbirth as early as
possible, All live born with GA at delivery 29, 30, 34 and 35.
Stillborn 1: Stage 3 FGR, EFW < 3, maternal PE (nulliparous, not known high risk of PE), examined at 31 GA weeks,
adequate liquor, recommended immediate delivery, delay of 24 - 36 hours from advice to childbirth, baby still born while
transferring mother to OT.
Note:

▪ No Maternal Mortality
▪ No neonatal mortality in SGA (n = 9, EFW 3 - 10 with normal Doppler study), GA at delivery for this group ranged from

32 to 39 weeks; one childbirth each at 32 and 33 weeks was due to deteriorating maternal condition (PE), if we exclude
those two cases, GA at childbirth mean 36. 43 weeks and mean Birth weight was 2471.43 grams. Low number of outcome
forms in this group, hence just sharing and this is not a definitive statement.
Samrakshan neonatal mortality rate (for now) is 4 neonatal deaths in 191 live births for whom outcomes forms are
available = 20.94 per 1000 live births
Samrakshan Perinatal mortality rate is 5 of 192 births = 26.04 per 1000 total births.
To compare, NFHS 4 reported neonatal mortality rate 30 per 1000 live births and perinatal mortality rate 36 per 1000
births. More recent reports pegged neonatal mortality rate at 25.4 per 1000 live births and will wait for the full NFHS 5
report for the latest.
The EFW 3 - 10 with normal Doppler is presented above as it differentiates SGA from true FGR, and SGA with normal
Doppler can be carried to term. Preliminary stages but we are seeing this is possible.
Getting more data in as trimester specific forms and especially child birth outcomes forms will help us provide more
robust practice changing information. Team Samrakshan thanks everyone who are submitting forms and hopes more
members will join the family.

We are thankful for encouraging the breast outreach program to happen in Dehradun, Uttarakhand. It was conducted
under your banner Indian College of Radiology and was organized by IRIA Uttarakhand Chapter. The event happened on

fortunate to have Dr. Bijal Jankharia. IRIA Outreach Program Director as the key speaker. She delivered the lectures on
BIRADS lexicon and BIRADS Assessment Categories. She discussed interesting cases and had good interaction with the
house of 69 participants. Almost half of the attendees were post graduate students who were very much benefited. The
local speaker was Dr. Manju Saini, who talked about the benign breast diseases. We were fortunate to get accreditation
by Uttarakhand medical council of one credit hour for this program. Looking forward for such more programs in
future. Thanks and Regards Dr. Manju Saini Secretary, IRIA, Uttarakhand chapter

Samrakshan Programme at Bhubaneswar, Odisha
Samrakshan two day CME was conducted by Odisha Radiology & Imaging
Association on February 22nd, 23rd 2020, at DM Convention centre,
Bhubaneswar. The CME and workshop on Fetal Radiology and Samrakshan
were attended by 80 delegates. The state leadership of IRIA did an
exemplary effort to organise the event. Thanks to  President Dr. M. V.
Kameshwar Rao Sir and Secretary Dr. Premraj Satpathy.
My sincere gratitude to Rao Sir, Premraj and Central Council members,
Dr. Basantha Manjari Swain Ma'am, Dr. Kamal Lochan Mohapatra,
Dr. Seshadev Panigrahi, Dr. Sheetal Kaur, who took their time off from their
busy schedule to receive us and to spend with us an evening of camaraderie,
friendship and fruitful discussions on how to take Samrakshan forward in
Odisha.
I must acknowledge that it was a great evening get together we had.The event was jointly inaugurated by Dr. Jayasree
Mohanthy, Dean and Professor Radiology, SCB Medical college, Cuttack along with me and Rao Sir.Thanks to the great
academics shared by Dr. Neelam Jain, Dr. Bavaharan, Dr. Praveen Nirmalan, Dr. Srinivas Shentar, Dr. Sanjeet Mishra and
Dr. Sheetal Kaur, the program was vibrant with discussions and enthusiasm to take forward the mission of Samrakshan
throughout Odisha. The success of the event was quite evident as the active discussions continued even during the tea
breaks. Medical college faculties and individual practitioners found Samrakshan a platform that can take Odisha Radiology
forward in the years to come.
Thanks once again to Dr. Rao Sir, Dr. Premraj Satpathy & Odisha Radiology.

Samrakshan Programme at Jabalpur, M.P.
Jabalpur conducted the Samrakshan CME program on February 23rd along with the 35th Annual state conference of MP
IRIA. This was the second CME on Samrakshan in MP after the Samrakshan inaugural CME at Indore in September.
The Samrakshan academics were presented by Dr. Lalit Sharma, Dr. Sudhir Ghokale and the workshops were done with
elegance by Dr. Raghav Aggarwal.
The panel discussion with Obstetricians, Neonatologists and Radiologists generated a lot of interest amongst the
delegates as the discussions were vibrant and had a strong connect to the ground realities each specialist encountered in
their day to day practice. The relevance of Samraksham screening for Preeclampsia and fetal growth restriction was amply
evident when the Gynec HOD of Jabalpur MH stated that they had 5 - 6 eclampsia cases every week which is quite an
alarming number that reflected the gravity of the situation.
The National President Dr. Deepak Patkar, IPP Dr. Hemant Patel, President - Elect Dr.C.Amarnath, Treasurer Dr. Pankaj
Sharma, M.P. IRIA State President Dr. Vijayakumar Verma, Dr. Arvind Jain, Dr. R. K. Sodani, Dr. O. P. Tiwari  Dr. Rakesh
Mishra, Dr. Durgesh Rati, Dr. Santhosh Gandhi and Dr. Pankaj Yadav graced the occasion.
Dr. Pushparaj Bhatele had promised to organise the Samrakshan program at the time of Indore Samrakshan CME, though
he admitted his inability to comprehend fetal imaging as he doesn't practice it. My sincere gratitude to Dr. Pushparaj
Bhatele, for, he kept his word, true to a man of honour and commitment. His gesture proves that each and everyone,
though you are not into fetal radiology and imaging can contribute towards the noble cause of Samrakshan to reduce
perinatal mortality in India.
Fraternal regards,
Dr. Rijo M. Choorakuttil
