Internal Medicine Residency Program ANNUAL
REPORT

MISSION STATEMENT

Our mission is to develop our residents into physician-
leaders who have a strong foundation in patient care and
medical knowledge but are also skilled in teaching, scholarly

activity, leadership, and quality improvement.

PROGRAM AIMS

1. Equip graduates with the knowledge and clinical skills for excellence in the practice of
medicine in their careers.

2. Expose residents to a variety of training sites and embrace the educational opportunities
that arise from the diverse pathology and patient populations.

3. Promote individualization of the program to ensure graduate achieve the learning
necessary for intended roles/practice

4. Ensure residents will graduate as educators of students, colleagues, patients, and
themselves

5. Enhance professional development by providing residents with opportunities for scholarly
activity, leadership, medical education and/or quality improvement & patient safety.

6. Conduct residency training in an environment that is collegial, supportive, open to growth
and development, and awareness of resident well-being

• Developed a quality improvement project aiming to decrease same-day cancellations for inpatient GI
procedures.
• Assisted in quality improvement initiatives at TGH: Code Blue Committee, VTE Process Improvement
• Started a project with residents rotating on TGH wards to improve awareness on key metrics: Length of
Stay and Discharge order in by 11am. Preliminary findings show >90% improvement in metrics by the
end of their 4-week rotation. Plan is to continue until June 2022 and share the results on a national
conference.
• Helped teach and guide residents during their 2-week quality improvement rotation at TGH.
• Participate in IHI Practicum as a project sponsor to the following project: “Revascularization LOS
Reduction Collaborative”.

PALLIATIVE CARE DIDACTICS

The project idea originated from personal experience on the wards and units during my
intern year. There were numerous circumstances when a patient and/or family member had
questions about prognosis and goals of care. I frequently saw situations when the other
team members (often the more senior team members) are extremely busy, so they may send
the intern to have this conversation. However, just like any other area of medicine, if interns
and residents have not had adequate preparation for this kind of conversation, then they
are at risk for adverse outcomes such as miscommunication, providing inaccurate
information, incorrect prognostication, upsetting family and patients, etc. After seeing this
need, I wanted to develop a lecture series that provides adequate training on how to lead
these conversations as well as education about some key concepts in palliative care.
I worked with internal medicine and palliative care faculty to create a lecture series that we
have taught during the residents’ dedicated Friday afternoon clinic didactics. We created an
easily understood guide to having a basic goals of care conversation, with handouts
provided for easy reference after the lecture. We practice role playing different scenarios
during the lectures so residents can have hands-on experience with faculty in the field. We
also cover some commonly misunderstood topics in palliative care including the differences
between palliative care and hospice, and how to identify key decision makers when it is not
the patient (legal guardian, healthcare proxy, and surrogate). I was also able to work with Dr.
Oller to assign a palliative rotation at TGH for every internal medicine intern, with the hope
that the skills they gain during that rotation will further help them on wards, critical care
units, and in clinic.
In future years, I hope that this project can be further expanded. Some future goals would be
to include standardized patient practice and provide structured feedback to residents on
their strengths and weakness during goals of care conversations. Residents would also
benefit from a laminated card/pocket guide with some of the key lecture topics for easy
reference on the wards (right now it is just a paper guide). I also envision this expanding into
the medical school curriculum as well as it would benefit fourth year medical students as they
prepare to become interns.

CONTINUITY CLINIC DIDACTICS

“One of our key areas of improvement during this academic year has been the implementation of a dedicated outpatient didactic curriculum across all clinic sites.
Over the past several years, there was recurrent feedback on our internal surveys that harped on the different experiences/educational opportunities that residents
receive at the three distinct clinic sites. Residents were frequently asking for a designed outpatient curriculum that would both lay down a foundation of outpatient
primary care knowledge and also ensure that all residents received a similar educational experience regardless of their clinic site.

As a result, with the help of last year’s chief residents, our clinic site directors, and Dr. Oller, we were able to formulate an outpatient didactic curriculum that
launched in September 2021 across all clinic sites. Now, every week on Friday mornings, all clinic sites watch an approximately 20-30 minute pre-recorded lecture
from one of our clinic attendings on a specific outpatient topic. Given these videos are pre-recorded, one PGY2-3 resident at each site is pre-assigned as a
moderator to facilitate the session and field questions. All these topics are based on and coupled with a recently updated Yale Curriculum article that has been vetted
by Dr. Guerra. Thus, residents have exposure to these pre-recorded videos and the associated Yale Curriculum article on the Friday morning of the lecture, but also
have unlimited access on Box to re-watch and re-read. To date, topics that have been covered so far have included outpatient management of CKD, Hypertension,
COPD/Asthma, Obesity/Weight Loss, Smoking Cessation, and DVT with future planned topics including seizures, diabetes management, heel/ankle pain, and
depression.”

POCUS Equipment Support: This year in addition to the 5 Vscan portable ultrasound devices, the POINT OF CARE ULTRASOUND
program has also purchased 7 Butterfly portable ultrasound machines. These devices can be
plugged into a phone or tablet device and used to perform a bedside evaluation of patients in order 17
to improve diagnostic interpretation. These devices are available to all internal medicine residents.
POCUS Educational Resources: This year the program purchased the SonoSim package. This includes
access to the course library containing videos covering ultrasound for anatomy/physiology, core
clinical as well as advanced clinical topics and procedures. Residents can borrow demo ultrasound
probes compatible with the sonosim virtual model to assist in visual dissection and interpretation.
Noon Conference Didactics: At TGH every month, residents have a dedicated ultrasound noon
conference session. During this session, students receive a ~40 minute structured lecture, followed
by ~20 minute hands on session to apply techniques and solidify concepts. Lectures are formatted in
a system based approach and cover relevant physics, proper ultrasound probe selection, anatomy,
techniques on image acquisition and diagnostic interpretation.
POCUS Elective: As a continuation from last year, students have the opportunity to enroll in the
POCUS Elective hosted by Dr. Kapadia and Dr. Valenzuela. This two week course reviews relevant
physics, proper ultrasound probe selection, anatomy, techniques on image acquisition and diagnostic
interpretation. Didactics are hosted in both lecture format and at the bedside. Residents perform
bedside ultrasound on patients in order to improve their ability to obtain ultrasound images and
interpret diagnostic findings. As residents build their image profile, they are given direct feedback
from an attending on image quality and technique. Residents also spend 2 days with the cardiology
team on an ECHO elective learning and reviewing ECHO imaging.
12 Days of POCUS: During the holiday season, we rolled out the “12 Days of POCUS” mini lecture
series. Sessions included topic specific lectures (pulmonary edema, pneumothorax, PE, etc) aimed to
improve resident POCUS knowledge. The series concluded with a comprehensive clinical case using
ultrasound for the physical exam findings and finally a competition based quiz to reinforce concepts
learned in prior sessions.
POCUS Quality Improvement project : A longitudinal quality improvement project is being performed to
assess the efficacy and long-term retention of POCUS knowledge taught throughout the year.
Residents were given a baseline diagnostic interpretation assessment and after completion of the
year will perform a post test. This QI project has been extended to PGY2s for participation and
continuation for future research opportunities.
POCUS Champions This year we had several POCUS champions who volunteered to teach educational
POCUS sessions including noon conference didactics and the “12 Days of POCUS” mini lecture series
topics. These champions are Jenna Bilodeau, Jonathan Marks, Nicholas Mencer, Alan Roberts, Jordan
Smith, Wesley Wright

POCUS Future Goals

• Increase frequency of POCUS lectures: Residents on the
POCUS Elective can teach a topic based lecture to
residents during noon conference at the end of their
rotation in addition to the monthly didactics sessions
taught by the simulation chief.

• CCU/MICU Workshops: Just as we do ECG/CXR workshops,
have the CCU/MICU team provide a case based
presentation for a patient work up (PE, PNA, tamponade,
new HFrEF etc)

• POCUS Longitudinal certification: Pending COVID and
faculty availability, provide a longitudinal evening
curriculum aimed to cover POCUS for the internist. Those
with active participation and appropriate mastery of
concepts through testing would be granted a certificate of
completion.

• POCUS Friday Didactics/Modules: Increase continuity
between residents/topics by having Sonosim modules as a
part of ambulatory curriculum/Friday didactics

• Expand POCUS to outpatient use: Eventually incorporate
opportunities for outpatient POCUS use (joint injections,
thyroid nodule biopsy, etc).

SIMULATION-BASED EDUCATION

With the new Advanced Interprofessional Fellowship in Clinical Simulation,
internal medicine residents have had expanded access to simulation
opportunities. Code Blue training courses have been changed from a standard
mock code scenario to incorporate leadership skills and practice-based
learning for use of the defibrillator. Residents have also had hands on
opportunities to learn about the equipment needed during cardiac arrest
during noon conferences. We have also expanded and revamped our
procedure training to increase residents’ comfort with procedures such as
central line, paracentesis and thoracentesis.
My fellowship this year is the Advanced Interprofessional Fellowship in Clinical
Simulation at the James A. Haley Veterans’ Hospital. Through this fellowship, I
am learning the principles of simulation-based education for physicians,
nurses, and patients. This includes the classic example of mock codes and use
of technology to improve fidelity of the training as well as development of web
based educational that can be used for just in time learning. This year I am
working to implement continuous wave form capnography during cardiac
arrest at our hospital by training residents, nurses, and respiratory therapy
on the set up and interpretation of our monitoring system. The goal with this
project is to improve the care of our veteran population and establish that
simulation based education can lead to improved use of continuous wave for
capnography during cardiac arrest.

COVID pandemic at the VA

The VA hospital responded to the two COVID surges that occurred in the past 6 months. In the first surge that occurred from July to August, there was a dramatic increase in patients admitted to intensive
care unit; new-incoming interns and residents were flexible and volunteered to help the patient burden, and all medical providers worked hard together to provide the best care to patients. It was similar
experience during the second surge.

New Rotation (FLEX Position)

This year we created a new rotation called “VAH FLEX rotation.” We recognized the need for additional support for intensive care unit and medicine teams at the VA especially during COVID pandemic. The
roles of this position include but are not limited to helping with admissions in cardiac care unit (CCU), acting as day assist in medical intensive care unit (MICU) on Friday and helping with additional need
arising from inpatient service. Feedback from FLEX residents and inpatient residents regarding this rotation has been very positive; it promoted efficiency among the residents and often prevented residents
from violating ACGME duty hour rule.

Orientation Guide update and coordination

Dr. Paul, Dr. Cummings, Dr. Ong and I along with USF/VA Leadership group have reviewed and updated orientation guides for medicine service, MICU and CCU.

Morning report

This year, we restarted morning report to promote vigorous education among internal medicine residents at the VA. VA leadership and I established safe environment for residents to receive education in
person safely during the COVID pandemic. The in-person morning reports have been very interactive.
