MESSAGE FROM IMMEDIATE PAST PRESSIDENT
My Dear Respected Fellow Rotarians!
I flew the jet plane with 30 members on board along with me.
I took off on the first of July in 2020. And ever since then I have
been flying on and on. Started our very first flight of Rota Year 2020/21, with a great fearful jerk in the turbulence of Covid 19 which was out of my control. But as per the RI theme: “Rotary Opens Opportunities” we never had to crash land. We kept flying. Yes, sometimes we flew over mountains of little confidence, sometimes over soft clouds of happiness and many times had to fly over oceans of sorrows too. Keeping in mind that life must go on in any kind of situation, we fastened our seatbelts of Hope and Faith and continued our flight.
I found myself a very lucky Captain as I had such supportive crew members on board. Some of you were the helpful airhostesses, some were medical personnel while others also were counsellors, always ready to help whoever, whenever, wherever was in need. Thank you all. We did have some arguments and disagreements during this flight but that was a great lesson for me to experience and learned a lot. I as the captain of the plane took everything positively and tried flying my plane out of the clouds of disturbance. And despite some turbulences, I believe we had a smooth flight and smooth landing!
Huge thank you to my respected DG Rajiv sir and AG Rama mam who trained me in such a way that I could in whatever way control my plane. I hope that I have achieved the height of our club. I definitely know that I gave my 110% to safe land our flight of 2020/21. It has been my honor to serve as your President, RC Baneshwor.
Jai Nepal! Jai Rotary
IPP. Rtn. Kalpana Devi Piya Shrestha

EDITORIAL
Each Rotary year the Rotary International President presents the Theme for the Rotary year.
All the great themes, long or short, they all sum up the vision of Rotary. Last year RI President Holger Knaack2020-21 chose the theme “Roraty Opens Opportunity”, and we opened opportunities during COVID 19 pandemic to Serve and to Stay Safe. We protected ourselves and our loved ones and some of us lost our loved ones. Yet we rise above all to “SERVE TO CHANGE LIVES”, which is the theme of Rota Year 2021-22.
Rtn. Shekhar Mehta, RI President 2021-22 announced the theme, “Serve to Change Lives” on 1 February during the Rotary International Assembly virtually. Shekhar Mehta says serving others changes lives, including our own, which in definite we all follow through our life time. He spoke about how participating in service projects through Rotary changed him as a person and made him empathize more with the needs of others. Soon after joining his club, he helped carry out projects that benefited rural communities in India.
Taking inspiration from RI President Sekhar Mehata, in Nepal we all can change communities for the betterment of all. For which we all at RC Baneshwor shall promise to “Serve to Change Lives”, during this difficult time of world pandemic.
Thank You Rtn. Kalpana Devi Piya Shrestha, RC Baneshwor President 2020-21 for your time and effort to club, and your leadership to carry forward club legacy COVID 19 world pandemic.
Welcome, Rtn. Mahendra Raj Bhusal, RC Baneshwor President 2021-22, and a very successful tenure.

CHALLENGES OF HOSPITAL MANAGEMENT, PLANNING AND ROLE OF THE ROTARIANS
Mahendra Raj Bhusal Director, Health Care Int’l Consult (P.) Ltd.
As defined by WHO health is ” A complete state of physical, mental and social wellbeing, not merely the absence or disorder” (WHO, 1948). It means that complete state of wellbeing is health and hospitals one of the major tool for promotion of health. WHO in 2010 has addressed the role and importance of hospital management for a quality health (WHO, 2010).
There is no internationally accepted clear definition for hospital.
There are several types of hospitals based on the facilities, equipments and services, i.e. district hospitals, provincial hospitals, speciality hospitals and referral hospitals, teaching University hospitals and other types of health care facilities. There is no international standard defining what should be the minimum services that each hospital should
provide during a health crisis. Each country has to develop a national policy and technical guidelines to deal with a health crisis. Before defining the essential services, the ministry of health has to define the essential health services that the health sector will provide to the community in order to identify what more specific services will be delivered by hospitals.
Hospitals are not managed by professional: In Nepalese context, hospital management skill and concept of hospital management is still beyond the priority of the government and the government still does not have any policy guidelines over the hospital management sector. Lack of professional skill in the person responsible to manage the hospitals has created a problem in hospital management. The government run or supervised hospitals still do not have positions for the hospitals. Government policy has not mentioned anything about the hospital professional managers. However, the private hospitals have started hiring them which is still out of the government policy.
Quality is in shadow: Once people arrive, there must be a high level of quality and competent care, qualified and available physicians and allied health professionals that are readily available and accessible. Quality care can be measured and compared to national and international norms, average life spans and causes of illness and death. The quality and suitability of the hospital environment is a key contributor to the patient experience. Although some of the improvements have been made from the private and government in terms of infracture and buildings over the few years, a significant proportion of the estate remains unfit for purpose. Affording investment in capital schemes will be increasingly challenging in the Nepalese context.
Lack of Effective planning: Effective planning is needed more than ever to ensure that any capital funding available delivers tangible benefits to patient care. Early and consistent clinical engagement is vital to enable true alignment of clinical services and facilities. It is essential that the planning of hospital facilities responds to changing models of care and is
35

undertaken through a genuine ‘whole system’ approach.
Healthcare facilities take too long to plan, design & build : To build one hospital in an area it takes long time to make a plan and construct the hospital. It is one of the complex task to complete and involve diverse human resources, technology and system. From medical gas system, Operation theatre, Critical care to the support service like waste management environment analysis.
Healthcare facilities are too expensive to build and run: The cost of care is getting expensive day by day for the patient care. It cost huge amount of money to build it and buy the medical and non-medical equipment. Private sector is facing of the problem of lack of investment to buy and build the Hospital in the Morden time. Healthcare takes an increasing portion of the economy and the investment. At the moment in Nepal invested the 5% of GDP in health sectors which is huge amount to afford country like Nepal.
The demand for healthcare is rising: The 21st century brings many new and emerging issues like Covid 19 & Similar pandemic and communicable disease & issues are putting challenges in one side to handle the situation. More communicable disease which are not expecting today is going to be increase and resources to handle the situation and we do not have the resources to handle those situation in terms of Building, technology and qualified human resources.
Inequitable distribution of health facilities and resources: If you see the city area there is everywhere health facility and leading for the unhealthy competition. In far remote are people need to so much of distance for the basic health services, where there will not get qualified manpower, medical equipment and other resources and people lose their life. It is called Healthcare delayed is healthcare denied so we have to solve this issues a soon as we can from government and private sector.
Role of Rotarians in Hospital Management & Planning: In conclusion, the poor socio- economic conditions, hard geographical conditions and lack of awareness of the people and traditional beliefs and superstitions, lack of will in the political parties and their leaders are the main burning issues for the improvement of health sector in Nepal. These all are problems associated with the hospitals and ultimately with their management. As Rotarian we can contribute on system planning and effective management system with the support of the Rotary international. Disease prevention and control is one of the focused area of the Rotary service. Awareness and support financially in association with Rotary international is one of the role of the Rotarians. As the cost of investment is huge in health care and medical equipment is high and we can get funding and support for the quality equipment in hospital for the patient care. Rotarians also play the role in supporting and advocating the equitable distribution of health service in Nepal. In the health facility Rotary club can help through the global grant where need huge investment with good quality equipment like medical gas system, Cath lab setup, Kidney dialysis setup and other medical equipment support with full of resources.

SORCERY
- SP KOIRALA
Scene One:
The juggler’s manifold shows begins
When he walks with his rough steps
And the bare eyes decoyed
When the vision gets entangled in the amorous juggler Towards and unknown horizon of hope
Along with his uncontextual illusory descriptions The eyes fall into the net of sorcery
When startled once again
The amused onlookers fall all at once
On the surface of the earth!
Scene Two:
Right at that moment
Lines of a canal is sketched in the farmhouse And a canal is dug on a clean paper
They clap
Sing in praise
Sweets distributed
And false water is flown in the canal. Before the water begins to flow
There in the city
Real dreams are woven
And in no time grow myriad of mansions Like the caterpillars in soybean field!
Scene Three:
Inside the black spectacles I
Indulging in a delight of sorcery
The onlookers of scene one are counting the clapping And there in the polluted hearts
Embracing the quilt of shyness
The jugglers of scene two
Are turning into the sorcerers!
