It gives me great pleasure to pen a few lines for the 5th edition of HTAA Pathology Service
Handbook. There has been rapid advancement in the medical field and the Pathology
Department has met this challenge head on. Accurate results with fast turn around time are
needed to help clinicians to treat and monitor patients but these need quality samples which
this handbook aims to address.
I hope our doctors and paramedical staff will fully utilize this handbook towards this.
I would like to commend the staff of HTAA Pathology Department for their commitment to
ensure that our hospital provides quality care to our patients and congratulate them on the
publishing of this updated handbook.

FOREWORD INTRODUCTION
Assalamualaikum and greetings,
I would like to congratulate the team editing the HTAA Pathology Service Handbook, 2018. It
takes some time since the last manual produced in 2011 and therefore, an updated manual is
very much in need to notify all our lab users with the current service provided.
There are rapid change in procedures and technology in each laboratory or unit ever since,
contributing much to the differences as compare to the last edition.
This latest handbook serves mainly as a guide for maintaining sample integrity and appropriate
sample management. Being the referral lab for Pahang, I’m very much hoping that this handbook
will be a useful guide not only for our local clients in HTAA, but also to the other District Hospitals
or Health Clinics in Pahang. .
We also welcome further communication and comments for us to improve the service.

It has been 7 years since the 4th edition of HTAA Pathology Service Handbook (2011) was
distributed. Since then there have been many changes in terms of types and methodology of
the tests offered to our clients. Outdated tests have been replaced by new ones and there has
been an expansion in the range of tests as well as the setting up of a Molecular Laboratory for
PCR testing.
The information in this handbook is to help laboratory users on the correct procedures of
samples and results management. The accuracy of results as we all know are dependant on
the quality of samples collected. Hence, we have taken great pain to advice on the correct
procedure of sample taking and transportation. Therefore we hope that this handbook will be
utilised well by every user.
I would like to take this opportunity to thank our Hospital Director for his strong support as well
as our clinicians for their feedback and welcome comments following further improvement of
this edition.
The success of this handbook will not be achieved without the hard work of my staff and I would

INTRODUCTION
HTAA, KUANTAN Makmur is the main referral center for diagnostic services in the state of Pahang. It has

been accredited by Skim Akreditasi Makmal Malaysia for MS ISO 15189 : 2007 since
August 2011 and for MS ISO 15189 : 2014 since 2015 in the field of medical testing i.e .
histopathology, cytology, chemical pathology, haematology, microbiology and virology
which comply with the international standard .
LOCATION
The Pathology Department and main laboratory is located at the ground floor of the
hospital main block. The ACC Clinical Laboratory is located at the second level of the
Ambulatory Care Center building.
ORGANIZATION STRUCTURE
The department is divided into 7 units. See attach organisation chart (Page vii).
QUALITY ACTIVITIES
Quality Policy
It is our policy to provide reliable and timely results to our customers through
continuous improvement of the quality system.
a. The objective of our Quality Management System is to provide medical laboratory
testing services in accordance with the applicable standards and good laboratory
practices to satisfy the requirements and expectation of our customers.

b. We shall ensure that all testing and related activities are carried out in accordance
with documented procedures.
c. All the laboratory personnel shall familiarize themselves and comply with
the policies and procedures in the Laboratory Quality Manual and associated
documents at all times.
d. This laboratory shall strive to maintain accreditation and certification according to GENERAL OPERATING POLICIES
MS ISO 15189 : 2014.
Quality Improvement Activities
Pathology Department participates in many quality improvement activities such as :
• Internal quality control monitoring
• Participation in external quality assurance programme for nearly all tests.
• Monitoring turn-around-time.
Apart from the above quality improvement activities, improvement opportunities are
also identified through customer complaints, customer satisfaction surveys, incidents
reporting, management review meetings and internal audits and where appropriate,
corrective actions are implemented and reviewed to ensure that action has been taken
and non-conformities do not recur.
SAFETY MEASURES
Strict safety measures are practised according to the Laboratory Safety Manual (version
2014).
RESEARCH
The Pathology Department is involved in clinical research and development within the
department as well as other departments in HTAA and local universities.
SERVICES AND FUNCTIONS
The department of Pathology provides diagnostics and consultancy services in the
following disciplines of pathology :

These services are carried out by respective laboratories. Ambulatory Care Centre
(ACC) Clinical Laboratory provides basic routine chemical pathology and haematology
tests to serve the specialists clinics there.
Apart from offering diagnostic services, the department also acts as a training centre
for various levels and categories of staff including Medical Officer, Scientific Officer,
Medical Laboratory Technologist, student from Institute of Higher Learning and
Pathologist for gazettement in various disciplines in pathology.
SERVICE HOURS
The Pathology Department provides 24 hours service for certain diagnostic testing in
the Integrated Laboratory and Transfusion Laboratory.
Bacteriology Lab provides services from 8:00 am to 9:00 pm.
The other laboratories (Cytology, Histopathology, Serology, Specialized Haematology,
Specialized Chemical Pathology, Mycology, Molecular and ACC Clinical Laboratory)
provide diagnostic testing during office hours from 8.00 am to 5.00 pm.
Test schedules are included under the list of tests.
A medical officer and pathologists are on call for consultation or assistance after office
hours.
RECEPTION COUNTER
This area is involved in receiving, checking and sorting of specimens before the
specimens are sent to the specific laboratories for analysis.

Pathology Counter
Office hour : Receive all specimens for biochemical, haematology,
serology, parasitology and microbiology. Specimens for
histopathology, cytology and molecular test must be sent
directly to the respective laboratory.
After office hour : Receive all specimens except blood transfusion samples. GENERAL OPERATING POLICIES
Blood Transfusion Counter
Specimens relevant to transfusion medicine testing must be sent directly to
the Transfusion Laboratory at any time (24 hours).
PRE-ANALYTICAL REQUIREMENTS
INTRODUCTION
Pre-analytical is any procedure that takes place before an analysis. Pre-analytical
errors are estimated to constitute 50-70 % of errors. It is important to understand and
apply correct pre-analytical procedures because it may affect patient’s result.
Note: Correct patient identity is mandatory.
REQUEST FORM
A standard laboratory request form i.e PER-PAT 301 is used for all categories of tests
except otherwise stated in Page 117-162.
The following information must be provided in every request form :
i. Patient’s detail : Full name, full identity card (IC) number,
registration number (R/N), sex and
age.
ii. Source : Ward, clinic and name of hospital.
iii. Patient’s clinical summary : Relevant clinical summary including provisional
diagnosis and treatment. Abbreviations are dis-
couraged.
iv. Test details : Request must specify the test required.
v. Specimen : Date and time of specimen collection. Type of
specimen and anatomic site (if relevant).
vi. Requester detail : Doctor’s name, signature and official
stamp.

Mark (/) at the appropriate box for the tests required. Tests which are not listed in
the request form should be stated under the column OTHERS. The type of specimen
should be stated under the appropriate column for non-blood specimen.
HTAA, KUANTAN All request forms must be signed either by a House Officer, Medical Officer or Specialist
depending on the category of tests requested.
Urgent requests must be justified by clinical summary, diagnosis and reason for the
urgency. The word “URGENT” must be written clearly or stamped preferably in red at
the top on the right hand corner of the request form.
PER-PAT-301 are supplied by the Stationery Section of the Administration Division.
Venipuncture and Blood Specimen Collection
Use large bore needle (less than 22 gauge) for venipuncture.
All specimens must be sent in the correct containers as the use of inappropriate
containers will cause a wrong / misleading result.
Follow correct order of draw to eliminate the risk of cross contamination of additives
and to reduce the risk of clotted specimens.
The specimens should be collected by the ward or clinic staff, properly labelled and
despatched to the laboratory in appropriate containers as specified.
The specimen containers must be labelled with full name of patient, patient’s IC
number, date of collection, ward identity and test requested.
The specimen containers should be placed in biohazard plastic bags with the respective
request forms inside the second pouch of the bag.
Please use different biohazard bag for each patient. In condition when multiple type of
specimen obtained from a patient (e.g. urine and blood), please use separate biohazard
bag for each type of sample.
Multiple timed samples (e.g. glucose tolerance test) from the same patient can be sent
in the same bag. In this case, it is essential that each sample is clearly labeled with the
time of collection.
STAT/URGENT specimen must be stamped/written clearly in the request form before
sending to the laboratory.

Phlebotomy Technique
Postural Effects
• Change in posture from supine to erect or sitting causes a shift in fluid from the
interstitial space of about 12%.
• An increase of 5% to 15% is seen for most cellular and macromolecular analytes GENERAL OPERATING POLICIES
when specimens are collected sitting as compared to supine.
• Conversely, moving from upright to supine can have a dilutional effect owing to
an increase in plasma volume. The effect of postural change is accentuated in
patients with a tendency to edema.
Tourniquet Application
• Application of the tourniquet for more than 1 minute can result in
haemoconcentration, causing an increase in the concentration of large molecules
e.g. serum proteins that are unable to pass through the capillary wall.
• Prolonged tourniquet application also promotes anaerobic glycolysis resulting
in an increase in plasma lactate, a reduction in blood pH and increase in blood
potassium.
Specimen Collection and Procedures
Mixing by Inversion
• Most tubes contain an additive or clot activator that needs to be mixed with the
blood sample.
• Tubes with anticoagulants such as K EDTA and sodium citrate need to be mixed to

ensure that the specimen does not clot.
• Blood has to be mixed immediately after blood draw by holding the tube upright
o
and gently invert 180 and back.

Order of draw
Order of draw can be defined as sequence of blood inoculation into specimen container

All collection tubes need to be filled to the correct volume to ensure the proper

amount of blood for the amount of additives in the tube.
Blood has to be transfer immediately from the syringe into tubes to prevent
•
clotting.
UNDERFILLED : Insufficient for running the test.
: Higher concentration of additives blood ratio.
OVERFILLED : Potential for clotting.
: Lower concentration of additives to blood ratio.
Blood collection from IVs and catheters
• Blood should not be collected proximal to an IV site but preferably from the other
arm.
• If samples must be obtained from a catheter, heparin contamination and dilution
must be avoided. The line should be flushed with 5.0 ml of saline and the first 5.0
ml of blood or six dead space volume of the catheter discarded.
• If a syringe is used, a small volume (< 10.0 ml) syringe is recommended so that
clotting in the syringe during phlebotomy can be avoided.
AVOIDING HAEMOLYSIS
Haemolysis should be avoided because it invalidates certain laboratory determinations.
The presence of haemolysis of moderate or greater degree adversely affects the blood
specimen in several ways.
It releases erythrocytes contents such as Potassium, Lactate dehydrogenase and Acid
phosphatase into the serum. Thereby, elavating their levels.
It colours the blood plasma and serum which will interfere with colorimetric assays
especially photometric measurements using shorter wavelengths of the visible
spectrum.
As some red blood cells have been damaged, the packed cell volume (PCV) and red
blood cell (RBC) count will be falsely depressed. Although the haemoglobin value will
be accurate, the calculated red blood cell indices will be affected by the depressed PCV
and RBC.

Common Causes of Haemolysis
Collection of Blood
• During phlebotomy, try to avoid probing, traumatic venipuncture, using a needle
that is too small (less than 22 gauge) and drawing from a haematoma. GENERAL OPERATING POLICIES
• Make sure venipuncture site is dry. Residual alcohol may cause lysis of red blood
cells.
• A slow return of blood flow into tubes could also cause haemolysis. The pressure
created by the vacuum tube or syringe collapses the lumen of the vein against the
needle, therefore crushing numerous red cells.
• Fluttering of the lumen against the needle which can cause haemolysis can be
stopped by reducing the negative pressure exerted by repositioning the needle
with a slight rotation or deeper insertion.
• Avoid drawing the plunger back too forcefully if using a needle and syringe. Applying
excessive negative pressure can create haemolysis. Our natural tendencies to
use more force to enhance blood flow, however patience and alternating gentle
pressure with a short release will yield best result.
Transfer of Blood
• Expelling blood from a syringe through the needle into a tube with force can cause
haemolysis of the blood.
• Do not apply pressure to the plunger; allow the tubes to fill by the negative
pressure of the vacuum tube.
• Do not shake blood in the container to mix with anticoagulants as frothing results
in haemolysis. Gently mix by repetitive inversion 6 to 10 times. Agitation of whole
blood not containing an anticoagulant is certain to produce haemolysis.
Extreme Temperature
• Extreme temperature; hot or cold, can cause haemolysis. Avoid placing ice or
frozen gel packs directly on tubes of blood as it can result in haemolysis.

MEDICOLEGAL CASES
Specific guidelines:
HTAA, KUANTAN i. Specimens should be sealed and sent directly to the respective laboratory.
ii. Specimens should be sent to the laboratory by designated personnel.
iii. Chain of custody should be maintained at all times and record book should
accompany the samples.
TYPE OF CONTAINERS
The specimen should be sent to the laboratory in appropriate containers.
Refer to Table 1 : List of Tests Available at Pathology Department (page : 16).
TRANSPORTATION OF SPECIMEN
The specimens should be transported to the laboratory as specified according to the
type of test in appropriate time frame and according to transport requirement for the
test.
All specimens should be sent to the laboratory together with a despatch book or
specimen lists and receive acknowledgement from the laboratory staff.
The time of specimens received at the counter should be clocked in by the sender and
the specimens will subsequently be attended by the laboratory staff.
REJECTION
Requests which do not fulfill the laboratory requirement will be rejected. Below are
the common/primary rejection criteria:
• Different name, IC or RN written on the request form and specimen
• Specimen not labeled
• No specimen / no form received
• Unsatisfactory specimen preparation
• Wrong tube / container
• Leaking specimen
• Empty container / form
• Broken slide
• Specimen not sealed (medicolegal cases)

• Wrong request form
• Clotted specimen
• Lysed specimen
• No indication
• Incorrect blood : anticoagulant ratio
• Insufficient volume of specimen GENERAL OPERATING POLICIES
• Grossly blood stained specimen
• Test not done / not offered / obsolete
• Specimen received after office hour ( test done during office hour)
• No FBC result and blood smear received
• Specimen for PT/APTT not suitable for testing (> 4 hours)
• No diagnosis or clinical summary of patient
• Test requested less than 3 months from the previous request (for certain tests)
• Test temporary withhold / stop (already informed)
The ward or clinic staff will be notified if the specimen is rejected.
RESULTS/REPORTS
Routine test results from Chemical Pathology, Haematology and Microbiology will be
placed in the designated pigeon hole. These results can also be viewed and printed
from ward view station available in designated wards/clinics.
Result for drug abuse testing should be collected by respective wards/clinics/agencies
from the toxicology laboratory. HIV positive result will be despatched by hand (internal
request) or send by post (external request).
Histopathology, cytology and bone marrow reports should be collected by respective
clinics/wards/departments from the histopathology, cytology or haematology unit
respectively.
For ACC Clinical Laboratory tests, results can be obtained directly.
All critical results will be notified to the requester immediately. (INSERT LIST)
Enquiry of test results shall be made using the form for tracing laboratory results HTAA/
PAT/FORM3.

OUTSOURCING OF TESTS

The Pathology Department will be outsourcing certain tests which are not offered by
For outsourcing of tests, the following procedures must be followed :
Use an appropriate request form.
i.
ii. Request form must be completed with required information.
iii. The specimens together with the completely filled request form need to be
sent to the reception counter/respective unit (if necessary).
iv. Some of the tests (e.g. Primary Immunodeficiency screening) may required
appointment. The requesting doctor shall arrange for appointment with respective
refferal laboratory and inform Pathology department (respective laboratory).
v. Transportation of specimen to the referral laboratory and handling of the results/
reports will be managed by the Pathology department.
vi. Transportation of the specimen to the referral laboratory is scheduled once per
week. If sample have to be send to referral laboratory urgently on the day which
is not in the schedule, requester must arrange the transportation to the required
location.
vii. Pathology department has the right to reject a request if the above requirements
are not fulfilled.
