GENERAL SAFETY POLICIES FOR ALL CAMPUS MEMBERS
LSC-Kingwood strives to: (1) protect everyone who enters FOR YOUR SAFETY AND
buildings; and (2) remain operational during the Spring THE SAFETY OF OTHERS
2021 semester. Safety measures to mitigate and minimize
exposure, risk, and interruptions have been established.
Safety protocols, contact tracing, and access restrictions
are necessary strategies that have been put into place.
Cooperation and adherence to established safety
measures are critical.
During Spring 2021, access to all LSC-Kingwood locations
will be limited. Employees identified to Return-to-Building,
students enrolled in face-to-face and hybrid classes, and
approved vendors are the primary individuals who should
be on campus. All other visits to the campus should remain
infrequent and avoided unless necessary.
LSC-Kingwood will not host or allow any outside events,
meetings, or gatherings in any of its spaces typically used by
the community unless preapproved by the president in
advance. Guest speakers should be limited to Webex or
other virtual events.
Non-Classroom Activities that are still permitted on
campus include:
• Admissions and Registration. [see more on pg. 17]
• Library services by appointment. [see more on pg. 19]
• Tutoring services by appointment. [see more on pg. 19]
PRIOR TO ENTERING CAMPUS

GAINING CAMPUS ACCESS
To gain access to the campus, every individual (student, employee, and visitor) must pass a temperature check
and complete the digital LSC Health Declaration Form at the location that they want to visit.
The form is accessible through a QR code posted near a designated checkpoint. On a smartphone, pull up the
camera function and aim it at the QR code. It should ask to reroute to the online health declaration form. Some
individuals may need to download a free QR reader onto their smartphones. We ask that everyone who intends
to visit the campus first check to ensure his/her phone has this capability before arriving
and download a QR scanning app, if necessary, ahead of time.
Note: Examples of how to complete the Student, Employee, and Visitor forms are on 1)
pages 8–9.
Access the form using this link or the QR code: www.LoneStar.edu/hd-en.htm

ACCESS STEPS 1
2
Anyone entering any LSC-Kingwood building must meet certain criteria and 3
follow established guidelines. Anyone who does not meet certain criteria and/ 4
or does not follow established guidelines will be denied entry. 5
6
• Step 1: Persons will approach the checkpoint closest to their
7
classroom or destination.
• Step 2: Before entering, persons must use the provided QR code to
complete their health declaration on their mobile phone or device.
Notes: Everyone must complete the Health Declaration Form each day when they
arrive on campus (not at home). If it’s completed too early, persons may be asked to
do it again.
LSC login information will be needed. Contact OTS (832-813-6600) in case there’s
an issue. If persons don’t have a smartphone, iPads are available inside the check-in
station for use.
• Step 3: Put on a mask and wear it correctly before proceeding.
Masks are available at the checkpoint entrance. Gloves are available
on request inside.
• Step 4: Once inside, persons will take their temperature at one of the
temperature kiosks.
Notes: Only one person is allowed per station to check in. If there is someone
ahead in line, please wait on the designated markers.
• Step 5: Persons will show the green checkmark on their mobile
device to the checkpoint personnel or use an iPad to complete their
health declaration form.
• Step 6: Once approved by checkpoint personnel, persons should take
a wristband, place it on their wrist, and proceed to their destination.
*Wristbands assure that appropriate screening occurred prior to entrance
into the building. Wristbands must be discarded upon exit when leaving
campus for the day.
After receiving a wristband for the day, an individual should bypass check-
in when returning from short excursions off campus (lunch break, a trip to
the car, etc.).
Anyone who does not follow the established guidelines will be denied entry.

CAMPUS SAFETY
MEASURES
OVERVIEW
Lone Star College has invested millions of dollars in securing personal protective equipment
for students and employees. While most people will have their own face coverings, masks are
available at Designated Entrances. Gloves are also available at Designated Entrances.
LSC-Kingwood has hand sanitizer available in all buildings, sneeze guards/plexiglass barriers in
key locations, and rearranged furniture to accommodate physical distancing.
Face-to-face class sizes have been adjusted to ensure physical distancing is possible
in classrooms.
Access to buildings will be restricted and controlled. Likewise, contact tracing measures are in
place in the event of a positive COVID-19 case on campus.
Buildings will be continuously cleaned while open. They will be deep cleaned each weekend.
BEING IN THE BUILDING
While in the building, all employees and students will be required to follow established protocols, minimize des-
tinations visited on campus, and self-regulate in a manner to contribute to the greater good and overall safety
of others on campus. Remaining in a building between classes, whether to work or study, is discouraged unless
otherwise unavoidable. If you’re waiting for a class to begin, follow protocols, do not congregate, and limit your
destinations. Do not rearrange the furniture. All furniture has been arranged for social distancing. Do not use
furniture covered in plastic or marked out with blue tape. Do not remove plastic from the furniture.
Note: If you are an employee who needs sanitization PPE in your workspace/classroom, please see Appendix G.
Face Coverings
Masks are required to be worn in an appropriate manner (covering the mouth and nose) while in the building.
LSC-Kingwood can provide a mask if needed. A face shield may be worn; however, a shield does not replace a
mask or other covering of the mouth and nose.
If you have a medical condition that prohibits you from wearing a mask:
Please contact the appropriate person below before traveling to the campus. Checkpoint personnel will not
allow you to proceed without a mask.
• Students: contact Disability Services at [email protected].
• Employees: contact Human Resources at [email protected].
Please note that many facial coverings are not considered equivalent to a face mask. Anyone wearing coverings
such as gaiters, t-shirts, bandanas, face shields without a mask, or any other covering that does not adhere to
the CDC recommendations above will be provided a single-use face mask to ensure compliance. Face masks
must be worn in such a manner that both the person’s mouth and nose are always sufficiently covered.
11
Elevators
• Elevators are reserved for individuals who cannot use stairs or walk long distances.
• Elevators will accommodate only one person at a time to ensure social distancing.
• Individuals waiting for the elevator should maintain six feet of distance from the doors while waiting for the
elevator to arrive and ensure any occupant exits before entering the elevator.
• To minimize surface touching, individuals using the elevator should use an object (such as a pen cap) or
their knuckle or elbow to push elevator buttons.
Stairways
• All individuals using the stairs are reminded to maintain social distance of at least six feet.
• Individuals using the stairs should ensure no others are on the stairs before they begin their ascent
or descent.
• If another person is already on the stairs, please provide that individual enough space to exit the stairs
before using them.
• If an individual encounters another on the stairs, please pass each other safely and quickly.
• Individuals using the handrail should immediately wash their hands and avoid touching their face.
• Gym facilities, showers, the theater, the stage, the art gallery, and all other communal spaces previously
opened to the community, employees, and students will remain closed for the foreseeable future unless
in use for a class. Pre-approval will be needed to use these spaces.
Disposing of PPE
Dispose of any single-use PPE items (masks, gloves, etc.) in special PPE waste bins with red bags. These are
located at checkpoint entrances/exits.
Restrooms
Even restrooms have been reconfigured to ensure proper social distancing. Please follow posted max occupancy
signage and avoid using areas marked off by blue tape.
ID Cards, Keys, and Key Card Access
Employee and student IDs are not being made at this time except for essential badges needed for workforce
sites. Leadership in programs that need workforce badges should contact Anthony Carreras, interim dean of
instructional support, at [email protected].
Employees who need to unlock doors
• Please do not call dispatch to open doors.
• All classroom doors should be unlocked using a card swipe. If your badge needs additional permissions,
report it to your division staff. If you are a new employee without a badge or an existing employee who
has lost yours, you can request an access card from your division staff.
• If you need a key to access your office, please follow the normal procedure of contacting your division
staff for a key request form.
Copiers for Employee Use
Copy machines are available on campus for employees to use. Below is a list of locations.
• APA: 118, 200, and 109A • CLA: 123 and 218
• CLB: 108 • LIB: 101 and 202A
• MUS: 109 • SCC: 104, 138, 144, 152K, 222, and 223A
• SIB: 100Z and 202Y • TIB: 108
• AC: 108, 114, and 220 • PTC: 120K
Copiers for Student Use
Copy machines are not available for students; however, they can scan documents by downloading and using
the free Microsoft OneDrive App. Use your Lone Star credentials to sign in. For instructions, click here.
12
Handwashing & Hand Sanitizing
Students, faculty, and staff should wash their hands often with soap and water for at least 20 seconds. Common
sense should be used about the appropriate times to wash hands.
Hand sanitizer is available throughout the entire campus.
Minimizing Destinations
Students and employees may need to access multiple classrooms, offices, and service areas on campus. Everyone
is welcome to go to the locations that must be accessed. However, everyone should consider alternatives to
physically accessing multiple locations. For instance, telephone calls, Webex meetings, and virtual office hours
are examples of ways to minimize destinations visited.
Protocols require destination tracing for each workplace/office visited. You will be required to “check in” so
administrators can identify individuals to isolate if doing so becomes necessary.
Minimizing destinations entered on campus will help reduce the number of people who may have to be isolated if
you are diagnosed with COVID-19.
Please do not enter any space that is indicated as closed or only open to approved personnel. Please heed
signage, barriers, and any other form of blockage that indicates closure.
Food and Drink
Although inconvenient, no food service of any kind will be available in any LSC-Kingwood building. Vending
machines have been emptied. The SCC Café is closed. Water fountains have been disabled.
Students and employees may bring their own drinks and hydrate throughout the day as necessary, inside
and outside.
Food Protocols for Students
There are currently no designated eating areas on campus for students. Students are asked to eat off campus. If it
is otherwise unavoidable, students should eat at outside seating areas, and observe social distancing. Tables and
benches should not be shared. Masks should only be removed while eating. Students should try to wipe down
spaces after eating, if possible.
13
Food Protocols for Employees
• Full-time employees should take breaks off campus, outside, or in their office alone.
• Supervisors should ensure that their part-time employees take breaks off campus, outside, or alone in a
designated breakroom.
• If using a breakroom:
Appropriate signage must be posted regarding protocols and maximum occupancy. If activating a new
breakroom space, submit a Creative Services request in ServiceNow to obtain signage.
Breakrooms should be limited to one person per room, and breaks should be staggered to avoid employees
eating in the same location, preferably with 15 minutes between employees to allow for sanitization.
Employees should wipe down/sanitize anything touched after use.
Common items (traditional coffee pots, ice machines with scoops, etc.) should not be used at this time.
Ice dispensers, microwaves, refrigerators, and k-cup coffee brewers are permitted.
All employees should continue to wear masks while on break except when eating.
LEAVING CAMPUS
All students and employees should leave promptly after the required on-campus activity is complete. Buildings and
parking lots lock shortly after scheduled activities. Lingering can result in needing to walk around locked buildings
to get to cars or vehicles locked in lots.
If temporarily leaving, individuals should keep wristbands on to avoid going through the check-in process again.
14
CLASS ON CAMPUS
We have purposefully minimized the number
of face-to-face courses and restricted the
maximum capacity of those courses to
keep the number of people in buildings
to a manageable and reduced number.
For face-to-face and in-class meetings
for hybrid classes, everyone must follow
established protocols and self-regulate
personal behaviors to keep everyone safe.
Hybrid courses have dates/times of in-
person meetings listed in your schedule in
myLoneStar. However, you may not meet
each one of those times. Check your syllabus,
D2L class, and communicate with your
instructors regarding actual face-to-face class
meeting times.
To the extent possible, classrooms have been
scheduled to allow for time in-between classes for cleaning. To accommodate this schedule, students and faculty
are asked to leave the classroom as soon as the class is over. Please exit in a manner that maintains physical
distancing. Please wait to enter a vacant classroom until it’s time for the class to start. Do not congregate in the
hallway as you wait to enter a classroom. Maintain a six-foot distance at all times.
Classrooms and labs will have disinfectant wipes present. Some faculty may request that you wipe down your
workspace prior to leaving the area.
Masks must be properly worn at all times, even during class. Refusal to wear a mask could result in disciplinary
action and/or removal from campus. Please cover your nose.
While we hope to maintain face-to-face classes
throughout the entire semester, please understand it may
be necessary to shift to online instruction for a period of
time or for the remainder of the semester under certain
circumstances. For example, if a faculty member must be
isolated due to exposure to COVID-19, instruction may
continue online during the isolation period.
15
FACULTY WITH ON-CAMPUS CLASSES
Before Class
• Make sure you have practiced taking attendance in iStar and that your attendance roster is working
as needed. Do this before your first meeting on campus so that you can resolve any issues with OTS,
if needed. See Appendix C for an iStar attendance job aid.
• Ensure that your classroom is marked off for social distancing.
• Ensure that you have enough PPE to wipe down the class after it is over. If your classroom needs more
PPE supplies delivered to it, contact your division staff. They will coordinate with Ricky Saiz to have the
PPE delivered.
Notes: See Appendix G if you need additional PPE. See page 12 for ID cards, keys, and key card access.
Class
• Check to make sure all students have the wristband for the day showing they went through check-in.
• Only the students who are on your attendance roster for that day and time should be permitted entry.
If, for some reason a student from another section arrives, do not allow them to join the class.
• Take attendance in iStar.
• Do not move furniture or allow students to violate six feet distance between one another or you.
• Try to minimize touchpoints such as collecting paper. Utilize D2L to collect and distribute information,
if possible.
• If necessary, always remind students to keep their masks over their mouths and noses. Do not allow them
to pull down their masks when speaking.
After Class
• Ensure stations (and supplies, if necessary) are wiped down.

CONTACT TRACING
Lone Star College adopted the CDC’s definition of Close Contact as the THECB recommends. The CDC defines Close
Contact as “any individual who was within 6 feet of an infected person for at least 15 minutes or more over a 24-hour
period starting from 2 days before illness onset (or, for asymptomatic patients, 2 days prior to positive specimen collection)
until the patient is isolated.” Please note that the CDC finds that wearing PPE, even respirators, is irrelevant to this definition.
Additionally, Lone Star College uses a Presumed Close Contact definition in the following settings to heighten the safety of
individuals should a confirmed positive (CP) enter any of the following spaces:
CLASSROOMS. Any face-to-face (“F2F”) classroom, laboratory, or student-involved session a CP visits regardless of
visit duration. Instructors should take attendance of all individuals that visit these sessions every time a class meets F2F.
See Appendix C for an iStar attendance job aid. Local administrators should err on the side of protecting students (and
therefore ban as much outside visits to a classroom as possible). Only the professor and students listed on a course
roster should be in a classroom (as defined above). If it is necessary for someone other than the professor or students to
enter (such as police or OTS support), the professor should document that person’s presence for their own records, and
supply that information if a confirmed positive is identified as attending that class meeting. Moreover, recall that the CDC
recommendation – which LSC hereby adopts –requires isolating individuals going as far back as two days from the visit.
WORKPLACES AND WORK AREAS. The “four walls” of the room where an employee sits (or stands) as their
place of workplace destination will be considered Presumed Close Contact with all the individuals that also have as
a final destination those specific “four walls.” Walking through a space is not the same as having that place as your
workplace destination for the purposes of this presumption. Moreover, visiting a Workspace or Workarea for a visit that
does not qualify under the CDC’s definition of Close Contact will not be presumed Close Contact in this instance.
CHECKPOINTS. Any individual working a checkpoint table, counter, etc., at the time the CP proceeded through the
checkpoint that had personal interaction with the individual. Local administrators should always ensure that no loitering
occur at the checkpoints for this reason and that all individuals working checkpoints be clearly tracked at the local level
with ready lists available. Personal interaction means human-to-human touching for the purposes of this RtB 1.6; if a
checkpoint proctor/receptionist did not touch, both wore masks, and otherwise remained six feet away from the entrant,
there is no need to isolate the individual working the checkpoint.
CHECKPOINT LINES. Any individual in the checkpoint with the CP that both (a) checked in within 5 minutes of the
CP, and (b) was either three people behind, or three people ahead, of the CP within those 5 minutes. In other words, the
College will presume no more than 6 people (in addition to the CP) require isolation after coming into Presumed Close
Contact with the CP.
See Appendix D Return to Building 1.6 Confirmed Positive Individual Isolation Protocols.
Each day before reporting to campus, you must complete a daily self-screening and submit the Health Declaration form
to confirm you do not exhibit any symptoms potentially related to COVID-19. If you do not feel well, do not come to
campus. A student should notify their professor(s). An employee should notify his/her supervisor.
21
CLOSE CONTACT + PRESUMED CLOSE CONTACT = ISOLATION LIST
CONFIRMED POSITIVE INDIVIDUAL ISOLATION PROTOCOLS
Close Contact, as defined by the Center for Disease Control (CDC), states “any individual who was within 6 feet
of an infected person for a cumulative total of 15 minutes or more over a 24-hour period starting from 2 days
before illness onset (or, for asymptomatic patients, 2 days prior to test specimen collection) until the time the
patient is isolated.” Please note that the CDC finds that wearing PPE, even respirators, is irrelevant to
this definition.
If an individual is COVID-19 positive and was present on campus, LSC will compile a Close Contact list and a
Presumed Close Contact list. Together, these become the Isolation List. The lists will be identified through various
sources, such as a conversation with the COVID-19 positive individual, collection of checkpoint screening lists
(which are time and date stamped), classroom attendance rolls, and tracking by staff members of visitors
to offices.
If circumstances dictate, additional people may be added to the Isolation List.
Individuals on the Isolation List will be notified as soon as possible.
Supervisors who receive a report from an employee must report immediately to [email protected]
and email Kaleigh VonDerVor ([email protected]).
READMISSION TO CAMPUS
Returning to campus will be gauged according to the Minimum Requirements for Ending Isolation or Quarantine
(see Appendix E). Please note that individuals should not decide their own clearance level but instead wait
to receive official clearance from OGAC. A building clearance certificate will be issued via email as an
attachment. Please see Appendix F for the Isolation/Quarantine List Notice FAQ’s.
COMPLIANCE AND ENFORCEMENT OF PROTOCOLS
All students, faculty, and staff depend on each other to comply with protocols, keep each other protected from
exposure, and to ensure the semester successfully proceeds. If anyone has concerns about the implementation of
the college’s protocols, please email Kaleigh VonDerVor, special assistant to the president, at
[email protected].
Students who refuse to comply with established protocols may be subjected to the Lone Star College
non-academic misconduct policies (VI.F.1.01 Student Discipline Policy for Non-Academic Misconduct). Faculty
may refer students to Garth Howard, interim vice president of student success, at [email protected].
Employees who refuse to comply with established protocols may face disciplinary action in accordance with
college policy.
Thank you for your cooperation and for adhering to the established protocols.

APPENDIX D: RETURN TO BUILDINGS 1.6 CONFIRMED POSITIVE INDIVIDUAL
ISOLATION PROTOCOLS
Lone Star College —effective August 22, 2020—adopted the CDC’s definition of Close Contact as the THECB
recommends. The CDC defines Close Contact as “any individual who was within 6 feet of an infected person for at
least 15 minutes or more over a 24-hour period starting from 2 days before illness onset (or, for asymptomatic patients,
2 days prior to positive specimen collection) until the patient is isolated.” Please note that the CDC finds that wearing
PPE, even respirators, is irrelevant to this definition.
Both the THECB and the CDC recommend the College institute contact tracing to determine the individuals with which
an infected individual had Close Contact to facilitate notice and quarantine protocols. The College hereby adopts the
CDC’s definition of Close Contact for the purposes of its Return to the Buildings (RtB) protocols. The College will
require quarantine or isolation protocols, informed by CDC guidelines, for at least all individuals that have Close
Contact with an individual that is confirmed positive for COVID-19 (“CP”).
Moreover, the College will also presume Close Contact (“Presumed Close Contact”) in the following settings in addition
to the definition established by the CDC to heighten the safety of individuals, and the College will also require at least
this second group of individuals to quarantine or isolate, as follows:
• Classrooms. Any face-to-face (“F2F”) classroom, laboratory, or student-involved session a CP visits regardless of
visit duration. Instructors should take attendance of all individuals that visit these sessions every time a class meets
F2F. Local administrators should err on the side of protecting students (and therefore ban as much outside visits to
a classroom as possible). Moreover, recall that the CDC recommendation – which LSC hereby adopts – requires
isolating individuals going as far back as two days from the visit.
• Workspaces and Work areas. The “four walls” of the room where an employee sits (or stands) as their
place of workplace destination will be considered Presumed Close Contact with all the individuals that also have
as a final destination those specific “four walls.” Walking through a space is not the same as having that place as
your workplace destination for the purposes of this presumption. Moreover, visiting a Workspace or Workarea for
a visit that does not qualify under the CDC’s definition of Close Contact will not be presumed Close Contact in this
instance.
• Checkpoints. Any individual working a checkpoint table, counter, etc., at the time the CP proceeded through
the checkpoint that had personal interaction with the individual. Local administrators should always ensure that no
loitering occur at the checkpoints for this reason and that all individuals working checkpoints be clearly tracked
at the local level with ready lists available. Personal interaction means human-to-human touching for the purposes
of this RtB 1.6; if a checkpoint proctor / receptionist did not touch, both wore masks, and otherwise remained six
feet away from the entrant, there is no need to isolate the individual working the checkpoint.
• Checkpoint Lines. Any individual in the checkpoint with the CP that both (a) checked in within 5 minutes of the
CP, and (b) was either three people behind, or three people ahead, of the CP within those 5 minutes. In other
words, the College will presume no more than 6 people (in addition to the CP) require isolation after coming into
Presumed Close Contact with the CP.
The Presumed Closed Contact List (the aggregate of items 1-4 above) will be combined with the Closed Contact List the
CP reports. Together these lists comprise the Isolation List that the RtB Owner, or a designee, must provide OGAC as
soon as practical. The Isolation List will follow CDC protocols for isolating individuals (e.g., Readmission Standards—a
separate document). The RtB Building Owner retains the discretion to isolate additional individuals at their discretion,
but in no event shall an RtB Building Owner isolate a smaller group than that above (e.g., an RtB Building Owner can
elect to isolate everyone in the building at the same time as the CP as circumstances require).
Building owners should also be very intentional about groups that are given the option to isolate and those that are
required to isolate. Lone Star College puts individuals (employees) on paid administrative leave that must isolate, but
people that elect to isolate must follow normal time off procedures. Please be intentional about how you communicate
the difference between employees that must isolate compared to those that may isolate (if at all). Individuals isolating—
who are not diagnosed with COVID-19—should self-isolate for 14 calendar days from their last potential exposure.

APPENDIX F: ISOLATION/QUARANTINE LIST NOTICE FAQS
Isolation/Quarantine List Notice – FAQs
Q 1: Why are you receiving this notice?
A 1: You have been placed on a Lone Star College Isolation List. This occurred because (1) someone in your
classroom, laboratory, or class-related session tested positive for COVID-19; (2) someone in your
workspace tested positive for COVID-19; (3) you worked at a checkpoint and had personal contact with
someone at that checkpoint who testedpositive for COVID-19; or (4) someone who checked in immediately
before or immediately after you tested positive for COVID-19.
Q 2: Does this mean you were exposed?
A 2: The college presumes exposure beyond the Centers for Disease Control and Prevention Close
Contact definition. The CDC defines exposure as an interaction with someone “within 6 feet for a total of
15 minutes or more.” In most cases, you are receiving this notice even if your interaction did not meet this
definition but were merely within the vicinity of the person who tested positive for COVID-19.
Nevertheless, the college encourages you to treat this notice as a CDC-defined Close Contact exposure,
for your safety and that of others.
Q 3: What should you do next?
A 3: The college encourages you to self-quarantine and monitor your symptoms. Watch for fever (100.4°
F), cough, shortness of breath, or other symptoms, found here: https://www.cdc.gov/
coronavirus/2019-ncov/symptoms-testing/symptoms.html. The CDC says you should seek
emergency medical care if you have trouble breathing, experience persistent pain or pressure in the
chest, new confusion, inability to wake or stay awake, or bluish lips or face (this is not an exhaustive list)
—if you do, call 911. Seek non-emergency medical care as you deem appropriate.
Q 4: Should you get tested?
A 4: The college does not require that you get tested. You can of course get tested if you or your healthcare
provider determine that testing is an option. Please note that a negative test will not shorten your
quarantine time with the college.
Q 5: Who will give you more information and whenwill they contact you?
A 5: Someone from the Office of Governance, Accountability, and Compliance will contact you soon. If
you have been confirmed COVID-19 positive, expect communication within 24 hours of receiving this
notice. If you were directly exposed, expect communication within 2 to 3 business days. All others
should expect communication within 3 to 5 business days.
Q 6: How long until you can return to campus?
A 6: Only the Office of Governance, Accountability, andCompliance can clear you to return. If you do not
experience COVID-19 symptoms, you can expect to be cleared to return 14 days after exposure. If you test
positive for COVID-19 or experience COVID-19 symptoms, you can expect to return once symptoms
have cleared in accordance with CDC guidelines and the college’s RtB Protocols. You will receive a
clearance certificate that confirms you are clearedto return to the buildings.
Q 7: Who do you contact if you have questions about your class assignments, grades, or any other class-
related questions?
A 7: Contact your instructor, your dean, or your department chair. The person that sent you this notice will
have the specific contact information for these individuals if you do not already have it.
Q 8: Who do you contact if you have questions about work assignments, work arrangements, or work
in general?
A 8: Contact your direct supervisor. Any pay questions can be directed to the Office of Human Resources.
