RE : ANNUAL MAINTENANCE SERVICE CONTRACT RENEWAL.

As you are aware, the Waste Management Service Contract between Radit Jumbo
Logistics and FSK Corporate Sdn Bhd has been in effect since 05-01-2022. Our Waste
Management Service Contract last year was successful, and now our contract will expire
next month on the date of 04-01-2023. We sincerely hope that you must have had a
delightful experience of our maintenance services during the contract period.

2. We would also like to inform you that in order to continue our services without
interruptions, we request you to kindly renew the contract and provide us with your valuable
work order. The agreement shall continue automatically and be renewed for a similar and
remain same as previous term and is subject to the following as follows: -

□ TYPES AND SIZE OF BIN : OPEN RORO BIN // 12’ (L) x 6’ (W) x 4’ (H)

□ NO OF BIN REQUEST : 1 unit

□ TERM OF COLLECTION : On Call

□ DURATION OF CONTRACT : One (1) (Month / Year)

□ CONTRACT PERIOD : 05-01-2023 to 04-01-2024

□ TYPE OF CHARGES : RM 300.00
a. Transportation per trip (RM) : FOC
b. RORO Bin Rental per month (RM) : FOC
c. Dumping Fee per tonnage (RM)

3. Furthermore, if all as above are agreeable, our Annual Maintenance Service Contract
Renewal for the current year as below details for your kind perusal.

4. If all details and particulars as stated above are agreed, a copy of the latest WMS
agreement renewal will be prepared and sent to you. Please fill in the space below with chop
and signature for our next action.

year than before. Again, I will be thankful for your cooperation and approval of the contract
renewal.
