[Open] PTW Roles and Responsibilities Strictly Confidential Page 9 # Signatory Roles & Responsibilities 1 Permit Applicant a) Subcontractor Supervisor / Vendor / 3rd Party / MMHE Supervisor b) Safe execution of work activities for which the PTW is issued, by performing the following: Prepare the PTW and complete the necessary supporting documents where required, i.e.JHA, safety checklist including: • Specific work description and location • Known hazards and hazardous activities • Specify the controls and precautions for safe execution of the work and worksite preparation • Required tools, material and equipment used for the job including PPE • Ensure valid certificate available, prepared and approved Discuss the work and highlight any special precautions with the PV & WL Attend PTW Coordination Meeting, discuss the activity associates with PTW and requirements of the job hazard analysis (JHA) and PTW with the WL & PV Ensure that conditions are acceptable prior to authorisation of the PTW. Visit and inspection the worksite with the PIR upon authorisation of the PTW by the PI prior commence work Acknowledge the details of the PTW/certificate including any potential hazards, and all the required precautions and conditions specified Communicate all the requirements to the WL and workers for the job Ensure the controls are prepared and maintained throughout the work activity. Obtain relevant signatories on prior commencing, after completion or suspension of work and revalidate the PTW Discuss all ongoing or suspended PTWs with the incoming PTW PIR during shift handover Ensure on completion or suspension of the work, the site left in a safe and secured condition. Ensure the workers comply with the requirements set in the PTW/certificate e.g. physical boundaries, precautions, type of work and validity time Receive hand back and close-out the PTW on job completion


[Open] PTW Roles and Responsibilities Strictly Confidential Page 10 # Signatory Roles & Responsibilities 2 Work Leader a) Note: Work Leader can be Subcontractor Work Leader / Vendor Work Leader / 3rd Party Work Leader / MMHE Work Leader b) Responsible for planning and supervising safe execution of work as follows: Confirm site conditions is safe for work execution as required by PTW form and certificates and agree by signing them Explain to workers on JHA and PTW requirements at start of each shift or commencement of work Ensure the involved workers received adequate and clear instruction on the PTW requirements i.e. potential hazards, controls and emergency response. Display the active permit with JHA at the worksite. Supervise work and always remain at work site during work execution Ensure all potential hazards associates with the job and control in place as per PTW requirements and there is no incompatible work in the vicinity of activity. Ensure scope of work in each PTW form is limited to a single job Conduct joint site inspection at work site with PIR to verify all precaution and mitigation in placed prior to work execution and PTW closure Ensure no deviation from condition as defined in PTW form. WL may take on additional responsibilities as PA when deemed necessary; and Return PTW form to PIR for suspension or upon completion of work.


[Open] PTW Roles and Responsibilities Strictly Confidential Page 11 # Signatory Roles & Responsibilities 3 Permit Verifier (PV) a) Permit Verifier is MMHE Discipline Supervisor / Engineer b) A person who is appointed by employer, responsible to revalidate & endorse the PTW c) PV is responsible for the safe execution of work activities for which the PTW is issued, by performing the following: • Known hazards and hazardous activities • Specify the controls and precautions for safe execution of the work and worksite preparation • Required tools, material and equipment used for the job including PPE • Ensure valid JHA & certificate available, prepared and approved d) Ensure all precautions are specified, controls in place, and work is performed in a safe and efficient manner. e) Responsible to update list of authorised PTW signatories regularly. f) Responsible to instruct PIR that work site shall be inspected and safe before work begins and on work completion. g) Ensure handover for outstanding work after each shift/crew change is effective. h) Before endorsing work permit, PV shall verify and ensure the following: All hazards associated with work are identified and risks have been reduced to as low as reasonably practicable Authorized work does not endanger other ongoing operations in the facility Work preparation such as energy isolation/ LOTO have been completed and applicable certificates have been approved JHA had been reviewed with PA & WL before endorsing it; Other permits/work activities that may interact have been cross-referenced Supporting certificates ensure they are made known to concerned parties; and Required equipment and PPE are adequate for completion of work safely.


[Open] PTW Roles and Responsibilities Strictly Confidential Page 12 # Signatory Roles & Responsibilities 4 Permit Issuer Representative (PIR) a) Permit Issuer Representative (PIR) can be Lead MMHE Engineer / MMHE Engineer /Technical Executive/MMHE Lead Discipline / MMHE Construction Superintendent) b) Responsible for: Conduct site verification to ensure all conditions and safety precautions set out on the PTW form and related documentation are implemented on site before allowing the work to proceed Ensure hazards associated with job are identified and risks have been reduced to as low as reasonably practicable Ensure authorized work does not endanger other ongoing operations in the facility; physically mark, barricade and place tag on items/equipment to be worked onsite Cross reference other permits/work activities that may interact Verify implementation of all control stated in all supporting certificates and ensure that certificates as well as relevant documents are attached and made known to all concerned parties Ensure required equipment and PPE are adequate for safe completion of the work Ensure work site is inspected before work begins, suspended and on completion Ensure effective handover for outstanding work after each shift/crew change.


[Open] PTW Roles and Responsibilities Strictly Confidential Page 13 # Signatory Roles & Responsibilities 5 Permit Issuer (PI) Permit Issuer is the most senior person for the operation at offshore i.e MMHE HUC Lead Authorize and approve the PTW Ensure there is no incompatible work in the vicinity of activity. Specify the controls and precautions for safe execution of the work Ensure that the work does not conflict with other ongoing permits (jobs) and are cross-referenced to other permits for jobs that may interact with each other Assign PIR to prepare the work area/equipment with all the necessary precautions Ensure required supporting certificates and documents are attached and made known to all concerned parties carrying out the work Communicate with everyone affected by the work before the work begins, when the work is suspended and when the work is completed Ensure joint site visit and verification are conducted by the PIR together with the WL prior to approval / revalidate of the PTW Ensure that conditions are acceptable prior to authorisation of the PTW. Receive hand back and close-out the PTW on job completion For application of PTW working in confined space, Permit Issuer (PI) shall be trained as an entry supervisor and authorised gas tester and authorised by employer to issue the permit to work.


[Open] PTW Roles and Responsibilities Page 14 # Signatory Roles & Responsibilities 6 Permit Coordinator Personnel (Appointed PMT/HSE/Supervisor) who administer and maintain the PTW Register as highlighted by the applicant and audit the implementation on site. Updating and maintaining the daily PTW Register. The PTW Register shall be properly documented. Review supporting documentation on the permit including JHA, safety checklists, etc. Monitor/ audit / review the PTW system. Advise Permit Issuer on any incompatible work activities and potential hazardous operations. 7 Acknowledgement Personnel - PETRONAS Project CSR Acknowledge all PTW applied for the work 8 Individuals Working Under the PTW/ Certificates All individuals, including both owner employees and contractors working under the PTW/certificates shall ensure that they: Have received PTW training/instruction and have a good understanding of the PTW system Do not start any work requiring a PTW/certificates until it has been properly authorised, issued and available at site. Receive a briefing from the work leader (WL) on the particular job and they understand the hazards and the precautions taken or to be taken Comply with the instructions/conditions specified in the PTW/certificate Leave the site and any equipment they are using in a safe condition when the y stop work Stop work and consult with their WL if in any doubt, or if circumstances change. Attend PTW Awareness training during HSE Induction

Shall ensure all personnel under his/her supervision operating PTW received appropriate training Shall implement PTW system effectively Issue Letter of Appointment (LOA) to all list of signatories 10 Project HSE Manager, Executive & Supervisor Can be appointed as PTW Coordinator Monitor, regularly update and review PTW implementation on site. Conduct PTW Verification and First Line Assurance as stipulated in 5.15 Update and maintain the list of PTW personnel/ signatories 11 Subcontractor HSE Personnel Check and ensure the necessary of supporting documents, site preparation by Applicant are in place as per PTW requirement. Conduct site surveillance during work activity and stop work if any non-compliance occurs at site. Work closely with Permit Verifier and Permit Issuer if any issue / non-compliance at site. Conduct PTW Verification and First Line Assurance HSE Personnel is not the Applicant of the PTW.


PTW SYSTEM


[Open] PTW System Page 17 Permit to Work shall be applied and approved prior commencement of work at site The PTW Form is categorized to Hot Work & Cold Work. The PTW register to be updated on daily basis by the PTW Coordinator. The PTW Form/certificates will be one (1) original copy and two (2) carbon copy. The original copy shall be displayed at worksite. The second (2nd) copy shall be kept at control room & the third (3rd) copy shall be kept by Permit Applicant. PTW form shall contain the following sections: Requisition/Job description where the PA shall provide clear information about the work to be executed Identification of potential hazards or hazardous activities Identification of the required worksite preparation or precautions. Any special precautions will also be listed here. Identification of the required personal protective equipment. Listing of the supporting certificates/documents. Confirmation of the joint site visit by PIR & PA/WL before work commencement. Endorsement by PV, approval of the PTW by the PI and acceptance of the conditions by the PA. Revalidation of the PTW by the WL, PV and PIR Close-out of the PTW by the WL, PIR and PI on completion of the job.

[Open] PTW Certificates Types & Application Process Page 22 # Certificate Description 1 Working at Height Certificate (WAHC) Detail requirements for working at height are defined in Safe Working at Height Procedure; PSP-KS30 2 Lifting Certificate (LC) Lifting more than 8 tons of load. Lifting load of weight more than 80% of rated capacity of the lifting equipment. Transporting personnel using lifting equipment except those which are specifically designed for personnel lifting or transfer e.g. sky-lift, personnel transfer basket used in offshore operations. Lift of special loads such as imbalanced load (unknown center of gravity due to odd shape). Coordinated lift whereby the weight exceeds the maximum lifting capacity of one of the lifting appliances e.g. using of multiple cranes. Lifting operations for testing purposes to ensure integrity or functionality of the lifting equipment after installation, structural repair or modification of the equipment including functional or proof load tests. Other lifting operation which are deemed as critical or high risk as defined in Risk Assessment e.g. blind lift, lifting through hatches, congested area, night lifting, lifting at sea during rough sea condition, poor visibility due to fog or sand storms. For normal lifts not listed above, the use of JHA or checklist is required in the absence of a PTW


[Open] PTW Certificates Types & Application Process Page 23 # Certificate Description 3 Confined Space Entry Certificate (CSEC) Confined Space Entry (CSE) Certificate shall be filled and attached to PTW for all work in confined space. Entry Supervisor shall be the person to declare all precaution required for work in confined space has been implemented. Certified, competent and authorized AGT & ES, these requirements shall be complied with. Detail requirements of work in confined space is defined in Confined Space Entry Procedure; PSP-KS-28. 4 Gas Test Form (GTF) Gas Test Form shall be filled and attached to respective PTW form which requires gas testing. Authorized Gas Tester (AGT) is responsible to conduct gas testing as well as fill in the Gas Test Form. He/she shall be certified, competent and authorized to conduct the gas testing. Gas testing shall be conducted as part of site preparation and joint site visit with WL & PIR to qualify that workplace is safe for work to commence. Gas testing shall be carried out on regular basis as per interval stated in PTW form (Section 3) for the work. All gas test results shall be recorded in Gas Test Form.


[Open] PTW Certificates Types & Application Process Page 24 # Certificate Description 5 Radiation Certificate (RC) Radiation Certificate shall be filled for handling and isolating of equipment containing ionising radiation source. Certificate shall be attached to PTW as part of its application and approval. RPO/RPS shall be the person to declare that all precautions required for work involving ionising radiation source have been implemented. 6 Temporary Electrical Installation Certificate (TEIC) This certificate is used for control of temporary electrical installation such as generator set, Earth Leakage Circuit Breaker (ELCB) and power cable and socket. 7 Electrical Energization Certificate (EEC) When equipment is ready to be permanently energized, a dedicated EEC shall be prepared and certified by CEP. EEC will be authorized by AEP before any energisation take place. CEP and AEP to ensure the equipment had been applied and approved for De-Isolation. Detail requirements of the Electrical Energisation Certificate are defined in the Isolation and Lock Out Tag Out (LOTO) Procedure; PSP-KS-204


[Open] PTW Certificates Types & Application Process Page 25 # Certificate Description 8 Electrical LOTO Certificate (ELC) Electrical LOTO Certificate shall be filled for work on electrical equipment. Authorized Electrical Person (AEP) shall confirm that he/she has electrically isolated the equipment to be worked on and has taken suitable precautions to prevent accidental reconnection. Detail requirements of the Electrical LOTO Certificate are defined in the Isolation and Lock Out Tag Out (LOTO) Procedure; PSP-KS-204 9 Mechanical Isolation Checklist (MIC) & Blind Register Mechanical Isolation Checklist shall be filled for work on process lines and equipment to confirm energy is positively isolated. Detail requirements of the Mechanical Isolation Checklist are defined in the Isolation and Lock Out Tag Out (LOTO) Procedure; PSP-KS-204

[Open] PTW Coordination Meeting Page 31 Daily coordination To review but not limited to following: • Activities, requiring permit to work are planned and coordinated in order to avoid risks caused by simultaneous operation. • Simultaneous work in close proximity. In the event of incompatible PTW requisition, the Permit Issuer to decide the job priority • Incompatible works are properly managed by providing controls and agreed by the members • Communication between all parties involved in the activities being carried out prior commencement and after completion of the activities. • Special competencies required for the job. • Resources required.


[Open] PTW Removal Page 32 Where require PTW to be removed; Cancellation – Revoke the permit and stop the job. New permit is required if job is to continue. Suspension – Remove the permit and stop the job. If circumstances allow the job to continue with PIR’s approval, return the permit to the PA/Work Leader. Permit Cancellation Permit Suspension Change in job scope/work description Emergency situation where work must stop/evacuation takes place Incident affecting the permit When any of the supporting certificates has expired or been cancelled Stop work due to any violation of the PTW requirement Change in permit conditions/hazard control measures


[Open] PTW Completion & Closure Page 33 a) When the job is complete, the WL shall ensure that the site is left in a safe condition before signing off the PTW and returning it to the PI & PIR. The PIR & PI shall close out the PTW only when satisfied that the job is properly completed, and housekeeping has been done. b) The respective competent persons shall proceed to normalize any isolations that have taken place and closeout the associated certificates. c) Prior to notify the job completion, the Permit Applicant should ensure that the worksite has been left in a satisfactory condition and housekeeping exercise has been done (if required). d) The closed permit shall be surrender to PTW Coordinator for record keeping and update in PTW Register.


[Open] Lock Out Tag Out (LOTO) Page 34 LOTO system shall be applied to isolate the energy source to ensure that energy will not be restored while people working on the equipment. The LOTO system applies to all commissioning related disciplined i.e. electrical, mechanical and piping. Request for LOTO shall be submitted at least 24 hours before starting of any activities. Any ad-hoc activity is subjected to Commissioning and Construction / Hook Up Permit Issuer Representative approval. Only authorized personnel allow to isolate/de isolate LOTO system. The key to the safety padlocks shall be secured in a safety lockout box which shall be place in dedicated area. Site visit shall be conducted to visually identified flanges, valves and electrical equipment to be utilized for LOTO. Electrical LOTO system is mean to manage and control electrical safety on critical activities prior energization and de-energization. LOTO system is applied to MV/LV Switchboard, switchgear, MCC and Distribution Boards. The circuit shall be proven energised using an approved calibrated multimeter. Affected equipment from the LIVE system shall be physically located, tagged and locked to isolate it from all energy sources.


[Open] Lock Out Tag Out (LOTO) Page 35 Mechanical and Piping LOTO system is mean to prevent accident related to release of gases or liquid from the line, vessel or tank which may include chemical and pressure hazard. Apart from LOTO, usage of bull plugs caps and blinds or isolated flange are consider as part of isolation method. Valve which is in LIVE commissioning mode shall be isolated using LOTO. Pressure inside the hydraulic / pneumatic lines shall be drained / bled prior to implementing LOTO.

[Open] Assurance & Inspection Page 40 First Line Assurance & Inspection a) PTW verification shall be conducted using PTW Functional Checklist (FC) by weekly basis. b) PTW Verification Audit shall be conducted by weekly basis and meet the target ratio of 1:3. c) Daily inspection shall be carried out by the PMT to ensure compliance with and the effectiveness of the PTW System. d) The inspection is to verify that: • Permits are properly displayed at the worksite. • Permits have been properly completed. • Required safety equipment is in place. • Safety precautions listed on the permit are adequate and are in place.
