1. Thomas Jefferson Elementary School 2022 TCAP Schedule
○ Developed operating procedures for TCAP testing through a master schedule and
small group schedule.

2. Thomas Jefferson Elementary Giving Tree
○ In the absence of a school counselor, I organized and led the Giving Tree and
utilized community resources to help provide for our students during the holidays.
○ Giving Tree form to request assistance.
○ Giving Tree letter as an invitation to give back.
○ Example of tags used as ornaments on the Giving Tree.

3. Do the Math Inventory & Weekly Materials
○ Example of a Do the Math inventory and weekly materials list for a fractions kit.
○ This was used to keep inventory and to guide teachers and instructional assistants
on which materials to check out for each module.

4. Tennessee Arts Commission 2022 Student Ticket Subsidy (STS) Grant
○ Awarded the STS grant for the amount of $1,800.00 to provide our students with a
performance by the Barter Players of Barter Theatre.

5. Thomas Jefferson Elementary School PTO Amazon Smile Fundraising
○ Working alongside members of the PTO, developed a flyer to inform families on
how to connect to our PTO through Amazon Smile so that a percentage of their
purchases will be donated directly to our school.

6. Thomas Jefferson Elementary T-Shirts
○ Utilized community resources through our PTO and local vendors to provide
Jefferson Jaguar t-shirts for every student at field day.
○ Utilized community resources through our PTO and local vendors to provide
commemoratory t-shirts for every 5th grade student at 5th grade promotion.

The Jefferson Elementary Giving Tree
An Invitation to Give Back

Dear Jefferson Families,

If your family is looking for an opportunity to share with others during the holiday season, we hope you will consider
our Jefferson Elementary Giving Tree.

On Thursday, November 18th, Friday, November 19th, and Monday, November 22nd, the Giving Tree will be set up in the
office. Each ornament on the tree represents a Jefferson student, and has that student’s sizes/wishes in 4 gift
categories: shirt, pants, shoes, and toy. If you are unable to come by and choose an ornament, you may sign up to be
assigned a student electronically via a Google Form at bit.ly/JeffersonTree2.

To help ensure children receive equity in gifts we ask that families participating in the giving limit purchases to the 4
items on the ornament. To respect the needs and wishes of all families and the privacy of students, we ask that families
participate in purchasing for students only through this program (i.e. please do not ask teachers or staff if a
particular child can be gifted).

If you have any questions, please contact me via email [email protected]. Please note, that once our new school
counselor is here (hopefully very soon) they will be the primary contact person for the Jefferson Giving Tree.

All gifts should be returned to the office, unwrapped no later than Friday, December 3rd. To ensure privacy and
safekeeping, please do not send items to school with your student.

The Jefferson Elementary Giving Tree

Dear Jefferson Families,

This year the Jefferson Elementary Family is organizing a Giving Tree to assist Jefferson Elementary Families who may
be in need of holiday help. Our Giving Tree will be limited to our current Jefferson Elementary students. If there is a
need and your family is not receiving help from any other agency/organization, please complete the requested
information below for your Jefferson Elementary student(s). You may also fill out this form electronically through a
Google Form at bit.ly/JeffersonTree.

It is our goal that your child will receive one item from each category (shirt, pant, shoes, toy). When the items are
collected, you will be contacted by the school to come by and pick up the items. Please return this form to the office

Dear Sara Leimkuhler:

The Tennessee Arts Commission is pleased to inform you that 2022 Student Ticket Subsidy (STS) grant application #2108-
13761 for Thomas Jefferson Elementary School has been awarded funds in the amount of $1,800.00.

IMMEDIATE ACTION NEEDED: If your school has never received a Student Ticket Subsidy grant (or if you do not know),
please send this October 2018 version of the W9 form completed with your school payment address to
[email protected]. This form takes several days to process and could delay payment of the grant if not submitted
immediately. NOTE: Do not submit a version of this form that is older than the 2018 version. See upper left-hand corner of
the form for the correct version.

Also, please log into your online grants account and click on the Organizations tab on the left-hand side. Double-check that
the information in your Organization profile is complete and accurate including: Contact Details, School Information,
Organization Information, Fiscal Year End Date, and Accessibility Coordinator Information. Make the necessary revisions by
clicking Edit and Save. For more information about this required Organization Information, see the STS Grant Guidelines.

Vital information, including instructions on how to be prepared to receive this grant award, is now in your application/grant
record. To access this information:

Log into the online grants system at https://tnarts.fluxx.io.

Locate the application/grant #2108-13761 in the Submitted Applications link within the grey navigation pane on the left side
of your profile or the Active Grants link within the grey navigation pane on the left side of your profile. It will be in only one of
these two sections of your online grants account depending on the status of your application so make sure to check both if
you cannot locate it at first.

Open the record and scroll down to the Documents section.

Locate and open STS Grant Contract.docx. Please note that it may take 7-10 days from the date of this email for the STS
Grant Contract to appear in the Documents section. The STS Grant Contract is for your reference only and does not require
a signature.

Making changes to your project or activity: If a funded event is cancelled due to weather or other unforeseen circumstances,
the organizing teacher or school principal must notify the Tennessee Arts Commission within ten (10) working days from the
date of cancellation to change the date, reschedule, or cancel the event. Every effort should be made to reschedule the
project during the current school year. You may not use the grant award to carry out the project during the following school
year.

To request funds and close out your grant, schools must submit the STS Evaluation online using the instructions on the STS
EVALUATION & REQUEST FOR FUNDS INSTRUCTIONS webpage no later than 30 days after the field trip or artist visit or
by June 1, 2022, whichever comes first. Failure to do so may result in the school not receiving the STS grant funds. To locate
the STS Evaluation, log into the online grants system at https://tnarts.fluxx.io and select Reports Due under Grantee
Reports. If you do not see an Evaluation form in the Reports Due section, it is likely that the TN Arts Commission does not
have a W9 form on file for your school or the W9 form is still processing.
