If any changes need to be made to your appointment, please notify us at least 24 hours in advance.
A Fee of $25 will be charged to credit card on file for Missed appointments (NO SHOWS) and or
Late Cancellations.

If it is your first visit, please arrive at least 10 to 15 minutes before your appointment start time in order to
complete all necessary paperwork without cutting into your session time.
For all returning guests, please arrive at least 5 to 10 minutes before your appointment time to check in
and get situated.
Every appointment has an allotted time period reserved especially for that guest, and we want you to get
the most out of your appointment time.

In order to provide you with quality results we ask that you arrive promptly to your appointment to
allow sufficient time for you service.
Arriving more than 10 minutes late may result in having re-schedule your appointment and the late
cancellation policy will apply.
Late arrival for refills may be served for the remainder of their allotted service time, as time and other
bookings permit.

Should be turned onto silent mode and put away in order to minimize distractions and potential hazard
Please be courteous and respectful of your technician’s time and work by keeping your phone on silent
until you have left the session room.
We reserve the right to refuse service if you use your cell phone.

We ask that you remove makeup around your eye before your appointment.

*Children under the age of 12 must be accompanied by an adult who can supervise them in the lobby

while you receive your service.
We will not supervise your children and cannot be held responsible for any children—if you bring a child

with you who is under the age of 12 and there is no one to supervise them, your
appointment will be canceled and the late cancellation policy will apply.

GENERAL HEALTH AND SAFETY

Prior to application you should notify and discuss with your certified Eyelash Extensionist, if you have recently
or frequently experienced any of the following:

 Unusual sensitivity or skin reaction to Cyanoacrylate-based adhesives
 Moderate to severe allergies in combination with abnormal eye discharge.
 Any eye disease or medical condition, such as Conjunctivitis (“pink eye”)
 A compromised immune system due to cancer treatment, hepatitis, or advanced AIDS.
 Any type of cancer
 Alopecia
 Skin disease
 Any metabolic or endocrine disorder
 Blunt trauma in or around the eye area.
 Intoxication or impaired motor skills due to medications, alcohol or any other drug

I do not have any condition as noted above, or any other condition, which I am aware, that would affect the
general use or application for eyelash extensions

AFTER CARE

With proper care and fills your eyelashes can last indefinitely.

 Do not get your eyelashes wet for the first two hours.
 Do not use a spa, sauna or pool for the first 24 hours to allow extensions to properly bond.
 Avoid pulling or rubbing your eyes.
 Do not use eyelash curlers with your extensions.
 ONLY use oil-free makeup remover.
 Brush eyelashes daily to keep tangled free.
 Proper hygiene is very important. Please keep lashes clean by washing daily.
 Avoid rubbing lashes, blot dry with a towel and let air dry
 Avoid oil based or waterproof mascaras.
 Only apply the mascara to the tips of your lashes if desired.
 Use care when applying facial and eye creams to avoid the lash line. Too many oil-based products can

break down the adhesive.
 touch-up appointments are recommended every two to three weeks to maintain your new eyelash

extension investment

Lash Extension Consent Form

I , agree to have eyelash extensions applied to my natural eyelashes and or/removed
and retouched by P3 Lash Studio certified lash extension professional.

I understand that in rare occasions there are risks associated with having lash extensions applied to or
removed from my natural lash. I further understand that in rare cases in part of the procedure eye irritation and
discomfort could occur. I agree that if I experience any of these conditions with my lashes that I will contact P3
Lash Studio and if requested have my lash extensions removed.

I understand and agree to the after-care instructions provided by P3 Lash Studio. I realize and understand
that failure to maintain proper after care may result in lash extensions falling out and/or decrease the longevity
of my service.

I understand and consent to having my eyes closed and covered through the duration of the application

Classic Full Set $130

Classic lash extensions are applied to each one of your natural lashes.
A variety of lengths are used based on your natural lash length giving you a natural full look.

Classic Refill $85

New Lash Extensions are applied to new natural growing lashes.
Removal of lash extensions that have been previously applied and outgrown the natural lash are replaced with
new lash extensions.

Safely removing all lash extensions from natural lashes.

Q: What are eyelash extensions?
Eyelash extensions are synthetic lashes that are applied one by one to your natural eyelashes.

Q: How long do they last?
With proper care and maintenance they can last between 2-3 weeks, exact timing varies per person.
To maintain your look, come in for a re-fill session every 2 weeks.

Refills are more economical, take a shorter period of time, and keep your lashes looking natural and full
