Letter from Jim Brown, CEO

Thank you for taking time to complete the Vital Voices engagement survey conducted in October. HCA North Cypress’ overall engagement index
improved by one point to 84. This marks the fifth consecutive survey in which our engagement has improved and represents an all-time high
our facility. Engagement amongst Nursing remained the same survey-to-survey at 79. Participation equaled that of the May survey with 67% of
us providing feedback. In total, we received 706 comments of which 85% were of a positive nature.

Our engagement index of 84 was the fourth best in our division and our participation was the second highest. Our division also had an overall
engagement index of 84. HCA Gulf Coast Division’s engagement index was the highest amongst all divisions in our company.

For the specific areas surveyed, nine of eleven categories improved. Two categories remained the same from May. The categories for HCA
North Cypress scored the highest were Care, Retention, and Support:

Care: My manager cares about me as a person.
Retention: I plan to be working at this organization one year from now
Support: My manager helps to remove barriers that could prevent me from doing my best work.

Four departments had engagement index scores of 100. Those areas were Performance Improvement, Sports Medicine (hospital campus), Histology, and Retail Pharmacy.

The follow.ing areas had engagement index results greater than 90.

Of all areas receiving survey results, 23 departments improved, nine remained the same from May, and eighteen declined. The largest improvement for a single department
was MRI.

In an upcoming department meeting, your director or manger will be sharing your individual department’s results as well as their action plan for improvement. While I am very
pleased with these results, I do want us to continuously work to make HCA North Cypress a more enjoyable and fulfilling place to work. Thank you again for your feedback.

Farewell

Dear North Cypress Family,

It is with very mixed emotions I announce that Wes Wesson, Vice President of Operations, has left us to go back
home and join the HCA Medical City Denton team as their VP of Ops. His last day at North Cypress was October
28th.

Wes was our senior leader supporting Lab, EVS, Pharmacy, Security, and Valet, and he oversaw our Robotic and
Bariatric service lines. Wes fit in quickly and was willing to take on projects with no questions asked. In 7 months,
he created lasting relationships and was highly engaged with his departments. He was agile with problem solving,
used his Performance Improvement (PI) background to his benefit, and truly exemplified servant leadership. Those
that know him know Wes connects easily with anyone – even patients and families in the hallway. He brought a
great balance of enthusiasm and maturity to his role that I think is a unique combination.

We recognize that being back home in the Dallas area (and with HCA) is where he needs to be for his wife and
daughters. Additionally, the North Texas division is where he started and where he always wanted to get back to, so
I am pleased to see this come to fruition. Please know that the entire senior leader team will be here to serve you
and continue to support you and your departments as you care for patients. I will serve as the interim lead with the
departure of Wes, and we will be begin to determine the best organizational structure moving forward.

North Cypress employees have a history of generously providing Christmas gifts to children in the
community through Cypress Assistance Ministries (C.A.M). This year we will continue that
tradition, but in addition, we will help our own employees who may have difficulty providing
gifts for their children at Christmas.

Angel ornaments have been placed on the Christmas tree in the Main Hospital Lobby. These
ornaments will have the age and gender of the child and some ideas of what they are wishing
for. There will be a table with a list of children needing gifts near the tree. Please write your
name and ¾ ID next to the child you would like to provide a gift for and take that child’s
ornament off the tree. Please hold on to the ornament you chose and attach it to your gift when
dropped off.

If you need assistance with Christmas gifts for your children, please complete an application from
HR or from your Director and submit to Elise in Human Resources. HCA North Cypress employee
applications must be submitted to HR by Friday, November 12th and gifts must be picked up by
the same employee on Friday, December 17th. Employees seeking assistance will be kept

Donorsconfidential.

Red angel gifts (HCA Employees) submitted to HR by December 7th.

It is the goal of this program to allow our employees to assist you with Christmas gifts. It is not intended to provide families with
all of their Christmas needs.

Guidelines for Completing your Application
o Eligible employee must make less than $39,000 annually or submit a written request explaining special circumstances
o Children must be legal dependents of the employee
o Children must be 18 years of age or younger
o Special Needs dependents of any age are eligible for this program
o Between $30.00 and $50.00 will be spent per child
o Applications and gifts will be turned into Human Resources
o This program will maintain absolute confidentiality

admitted late afternoon yesterday. She said she came
forEmcpaloryiene gof tlhike eMofnathmily! here 5-6 years ago, and her experience was terrible and
she has not been back since. However, after a friend
Kudos Award recently told her she had a great experience here, she
specifically asked EMS to bring her here and she raved to
me about the ED and the MS3 nurses saying she’s never
seen such focused and attentive staff. She said she can
absolutely tell there’s a difference and it must be due to

potential to cause harm to a patient but never reaches the
patient due to the actions of a staff member. These type of
events are encouraged to be reported by placing an event in
Meditech. Each month, these are reviewed and one is chosen
to receive the “Great Catch Award”. All staff members are
responsible for patient safety standards but this award
recognizes a staff member that demonstrates an exceptional
commitment to patient safety.
