5 STEPS TO FOLLOW IF YOU ARE INJURED ON THE JOB

On the day of the occurrence, BEFORE your shift ends: STEP 1

Report your occurrence (injury or illness) whether you feel injured at the time or not, IMMEDIATELY to STEP 3
your supervisor and the EH Nurse or AD/Nursing Supervisor. STEP 4
STEP 5
BEFORE CLOCKING OUT and LEAVING THE BUILDING complete the Employee Injury Packet.

If it is regarding a needle stick or exposure, complete process immediately. PLEASE include the Patient’s
name/account and room number in your report.
If you are a contracted service, complete the Injury Packet, notify EH or the AD/nursing supervisor, and
then notify your agency for guidance.

Medical treatment, (if required), is authorized ONLY by the Employee Health at Ext 3971 or

3712, or the AD/Nursing Supervisor after hours, on weekends or Holidays.

If seen by an MD, return completed paperwork and Physician’s report to Employee Health after
treatment. (After hours, return to AD/nursing supervisor).

Contact the Employee Health after your accident at 832-912-3971 or 832-912-3712 and following all
medical treatment (ER, office appointments or any therapy i.e. Physical Therapy, etc.) If prescription is
required, obtain authorization prior to having filled.
