1. Certify that the information given in my application are true and correct, and I acknowledge that
any false and/or misleading information or documentary evidence may result in cancellation of my
enrolment in the programme(s)

2. Understand that the offer letter issued will become void, if I fail to submit all the relevant
documents and pay the required course fees by the due date.

3. Understand and agree that UPSI shall cancel my student visa and notify the relevant government
agencies/bodies if I remain inactive without notifying the Registrar in writing. (Applicable to
international student only)

4. Authorize and grant UPSI the right to use my personal information such as name, credentials,
image voice and/or the like through printed, audio, visual and electronic means, of the activities
that I participate during my educational programme(s) at UPSI in specially prepared materials
such as displays, brochures, websites and the like for training and promoting the continuation and
improvement of the UPSI programme(s) and/ or any such purpose incidental to it.

5. Understand and agree that if I revoke my consent to any of the terms that are specified in clause
8 herein, I must do so in writing to UPSI and the action taken prior to the revocation will not be
affected.

6. Understand and agree to comply with all policies, rules and regulations of UPSI including the
rules and regulations of the respective departments.

7. Hereby declare that I have read and understood all the terms herein and agree to abide by them.
