Respected Fellow Rotarians and Friends,
One more time my club trusted and assigned me to work as
the coordinator of the Annual Booklet publication committee. My sincere thanks to
IPP Arjun Thapa and newly appointed President Ramesh Prasad Koirala and the club
team for your believe and cooperation to come up with this publication.
It is aimed that this annual booklet of the club gives you opportunity to learn about
club members, basics of RoWtary, summary of the good works that the club complet-
ed last year and moreover the plan of action for the RY 2020-21.
The last quarter of the RY 2019-20 was unpredictively difficult to accomplish some
important service projects. Similarly, the lockdown and the recommended precau-
tions restricted us to visit frequently and work together at the printing press longer.
However, I was fortune to get support from my committee members and friends to
accomplish this task.
I am grateful to the Charter President Dr. Sunu Dulal and PP Binod Koirala for their
relevant write-up for this booklet. I am thankful to the Presidents of RC Dronfield, UK
RC Bristol Breakfast, UK and RC Neoteric, Hong Kong for their best wishes messag-
es. Similarly, I would like to express my sincere thanks to financial committee chair
for the Souvenir publication Rtn. Shiva Prasad Parajuli for his hard work to collect
advertisements for this booklet.
The RI President for 2020-21, Rtn. Holger Knaack has given the slogan Rotary Opens
Opportunities. I am a strong believer of this and I am very much sure the planned
activities of the club for the year 2020-21, opens lots of opportunities for hundreds
of underprivileged people through education, skills training, self-employment and
medical care.
I wish President Ramesh Prasad Koirala and his team all the best and I put my com-
mitment of active involvement to accomplish the plan for the year.

Dear fellow Rotarians:
Ask any Rotarian what led them to Rotary, and you’ll usually get different answers.
Why? Not only does Rotary mean different things to members in different parts of the
world, it also has a unique meaning for each Rotarian.
However, we are all brought together in three important ways. We believe in Rotary
values and all share the same Four Way Test. We believe in friendships and love
spending time together. And we take great satisfaction in helping others – we are
committed to Service.
The District Board has assumed important responsibilities and I thank them for this
hard work. They have made a commitment to help grow Rotary, adapt to the digital
future and make Rotary more appealing to younger generations.
We have important work in front of us and so many opportunities to open for others and
ourselves.
I hope you will continue your journey with us, as we continue to serve our communities
so that future generations can thrive as Rotary Opens Opportunities.

My Dear Fellow Rotarians, Greetings to you all.
Let me congratulate you for taking the responsibility of the
leadership of your club. Rotary club's leadership is something
which is most enjoyable. Along with fun it is very important that
we understand our responsibilities and work hard to achieve our
goals.
We are passing through a very unprecedented time. Something we have never
witnessed in our whole life. The suffering and the threat that many are facing are at
times unbearable. But as they say “when the going gets tough the tough get going” we
Rotarians are also tough people and we also need to get going to help and assist
people who are in difficulty during this time of pandemic. As this pandemic is going
to remain here for a long time I think each club should identify the people, the
community where our help is required the most and move ahead to assist them. In
doing so we also need to make sure that we keep the virus at bay from ourselves and
our family.
The Rotary year 2020-2021 is going to be a very different year for all of us. Lot of
Rotary programmes are going to be on virtual platform. During the challenging times
we have to do challenging things and the most challenging task will be for the clubs
to have their regular weekly meetings. I know these meetings cannot be in person but
we can always stay in touch with our club friends through virtual meetings. These
virtual meetings will certainly help members to be engaged with Rotary.
Doing service projects will be another challenge. But this also should not be very
difficult as we can plan our projects now and implement them once things become
normal which I know will happen very soon.
Rotary Club of Pokhara Fishtail is a club which has been doing great work in our
District and expectation from this club is very high. I urge each member of this club
to adapt to the new environment during this crisis and take advantage of the new
opportunities. Our RI President Rtn. Holgar Knaack has given us the theme “Rotary
Opens Opportunities” and we all know it certainly does. So let us all "Trailblazers"
get together to open this opportunity with the mindset of "Together We Can."

Message from Outgoing President
Dear President,
Fellow Rotarian Friends,
First and foremost, I would like to extend “Congratulation and
BEST WISHES for the successful tenure” to Mr. Ramesh Prasad
Koirala for assuming the post of President for the Rotary Year
2020/021 and his team.
I am delighted to express that I have completed my second term as President/RY
2019/020. I have served the same in the RY 2004/05 and would like to extend my
sincere thanks to all my club friends for believing to carry out such responsible
post for the second time. What I experienced during the time was that The Rotary
education and understanding is always new and never ending. In the new era of
Rotary Movement, the Rotarian holding responsible post should possess minimum
computer literacy and have a good writing skill (corresponding skill). Also he should
spare a good time for Rotary services.
The Rotary Year 2019/020 brought me a lot of opportunities and challenges. By
opportunities I mean to say that it’s been my second term as well as I was just
retired from my long term association with the commercial bank services and had
full time to spare with the Rotary. The District Governor for District 3292 Nepal for
the RY 2019/020 was from my home town Pokhara and District Conference to be
held in Pokhara. I had experienced Past Presidents and active Rotarians in my club
to support me to carry out plan of actions. To state about challenges, I must say
that I had the legacy of club’s successful history and I had to carry further with
success and to bring some new projects and programs. In between my tenure we
had to face pandemic, Corona Virus-COVID 19 which brought stand still to the whole
world in the form of loss of people, deterioration of health, no human movement,
closure of education institution, created unemployment problem, and deterioration
in economy. Our club is having ongoing big projects where some foreign Rotary clubs
commitment was there. But due to pandemic they showed their inability to continue
the support. However our club’s past president Mr. Binod Koirala had made some
initiation and exercises and got success to avail the fund from foreign rotary club
which will help to complete the running projects. It was very greatful that many of my

club members had contributed to The Rotary Foundation during the period for which
I would like to extend thanks to the big hearted members. I appreciate, thankful and
grateful to Mr. Binod for his untiring contribution to the club activities and society
in general. Considering the ongoing pandemic, our Rotary have had new avenues to
address. During the period, we had virtual meetings due to lock down in the country
and also the RI Convention held virtually which is totally new and for the first time
such event took place virtually.
I am grateful and would like to extend thanks to the Past Presidents, Board members,
Avenues Chairs and Directors and all the active members of my club who had been
always supportive. Without their guidance and support, it might not have been
possible to accomplish the good results. I am grateful to Club Secretary Mr. Deepak
Pokharel and Club Treasurer Ms. Laxmi Kunwar, who upon request continued the
same post for this RY 2019/020 too, Active Rtn. Ashok Chalise, Rtn. Shiva Parajuli,
Serg at Arms and PP Rtn. Nar Bahadur Gurung, , PP Ms. Laxmi Thapa Giri has also
contributed a lot in the area of Days for Girls, Fishtail Fund and other club activities.
I would also like to offer thanks to District Governor Mr. Kiranlal Shrestha, Assistant
Governor (AG) Ms Meenu Kunwar Shrestha, Guest speakers PDG Dilendra Raj
Shrestha, PDG Sanjay Giri, Dr. Tirthalal Upadhayay, Dr. Dhruba Bahadur Adhikari, Dr.
Bhoj Raj Gautam, Mr. Rabi Baral, Dist. Chair Public Image Committee, staffs of Ozone
Press, meeting venue hotel, Atithi Resort and others. I also extend my thanks to
my club’s associated concerns Rotaractor President Shonu Sunar, Interactor, Rotary
Community Corps and others.
I am committed and will be always available for the club and also share you all that I
will be available in the new role as Regional Chair, Rotary District 3292 Public Image
Committee, Region – IV for the RY 2020/021.

Message from President
Dear fellow Rotarians and friends,
I was admitted to this wonderful club in 2010 as an active
member. While entering the club, I had very limited knowledge
about Rotary. During this 11 year journey in Rotary, I have well
realized what a dynamic organization it is and feel proud to be
a Rotarian. I am very glad and thankful to those friends who
invited and encouraged me to join this club.
“Service above Self” is more than a motto of Rotary. It is a core value and had
changed lives of millions of people throughout the world. Rotary is a group of diverse
individuals coming together to impact communities, locally and globally, by effecting
positive change through service and philanthropy. Rotary also provides a framework
for personal and business relationships, networking and fellowship.
The Founder of Rotary, Paul Harris and a small group of businessmen in the Chicago,
USA started Rotary more than 115 years ago. Since then, Rotary has grown as an
international humanitarian organization with 1.2 million members in 36,200+ clubs
in nearly 200 countries. Rotary has led the way to eradicate Polio, feed the hungry
people, provide clean water, literacy and bring peace to the world.

We know, because of the Covid-19, community service in the field is getting tough. At

our community in large. I look forward to another exciting, service oriented and fun

WHAT IS ROTARY ?
“Rotary is an organization of business and professional persons united
worldwide, who provide humanitarian service, encourage high ethical
standards in all vocations and help build goodwill and peace in the world”.
In 1976, the Rotary International Board of Directors requested three men,
who were then serving on Rotary’s Public Relations Committee to create
a concise definition of Rotary. After numerous drafts, the Committee
presented above definition, which has been used ever since in various
Rotary publications.
Rotary is a Secular organization open to all persons regardless of race,
color, creed, religion, gender or political preference. There are 542 Districts,
35,109 Clubs and 1,235,536 members (as of May 31, 2015) located in 207
countries and geographical regions worldwide. The members of Rotary
Clubs are known as Rotarians.
ROTARY MOTTO:
Rotary’s principal motto, “Service Above Self,” is a powerful testament to
the enthusiasm with which Rotarians embrace the ideal of service.
FOUNDER OF ROTARY:
In the fall of 1900, Paul P. Harris, an attorney met fellow attorney Bob
Frank for dinner on the north side of Chicago. They walked around the
area, stopping at shops along the way. Everyone in the neighborhood
waved to Bob. Harris was impressed that Frank was friendly with many
of the shopkeepers.
In his recollection of the occasion Paul Harris tell us: “I couldn’t help but
feel it would be pleasant if men of this caliber could get together often.
Say one man from each business or professional, meeting socially.”
“The thought persisted that I was experiencing only what had happened
to hundreds, perhaps thousands, of others in the great city … I was sure
that there must be many other young men who had come from farms and
small villages to establish themselves in Chicago ... Why not bring them
together? If others were longing for fellowship as I was, something would
come of it.”

This initial vision by Paul to enhance fellowship among his friends laid
the foundation for Rotary.
THE BIRTH OF ROTARY:
On February 23, 1905, Paul Harris had dinner with his closest friend,
Chicago coal dealer Silvester Schiele. Afterwards, they walked over the
river to Room 711 of the Unity Building in downtown Chicago, where they
met their host, Gustavus Loehr, a Mining Engineer; and another friend,
Hiram R Shorey, a Merchant Tailor. Harris proposed that they form a club.
This was the first Rotary club meeting. They decided to call the new club
“Rotary” after the practice of rotating meeting locations. Silvester Schiele
was selected to be the first President of the Rotary Club of Chicago- the
number One Club. After only few meetings, one of the founder member
Gustav Loehr left the organization.
In February 1907, Harris was elected the third President of the Rotary
Club of Chicago, a position he held until the fall of 1908.
In August 1910, Rotarians held their first Convention in Chicago. The 16
clubs that existed at that time united to form the “National Association
of Rotary Clubs”. The new association unanimously elected Harris as its
first President for the year 2010-2011.
In 1912, the name changed to International Association of Rotary Clubs
to reflect the addition of Clubs in other countries. The name “Rotary
International” was adopted in 1922.
ROTARY GUIDING PRINCIPLES:
Throughout Rotary’s history, several basic principles have been developed
to guide Rotarians in achieving the ideal of service and high ethical
standards. They serve as a foundation for relationships with each other
and the action we take in the world.
OBJECT OF ROTARY:
First formulated in 1910 and adapted through the years as Rotary’s
mission expanded, the Object of Rotary provides a succinct definition of
the organization’s purpose as well as the Club member’s responsibilities.
The Object of Rotary is to encourage and foster the ideal of service as a

basis of worthy enterprise and, in particular, to encourage and foster:
• FIRST: The development of acquaintance as an opportunity for
service;
• SECOND: High ethical standards in business and professions; the
recognition of the worthiness of all useful occupations; and the
dignifying of each Rotarian's occupation as an opportunity to serve
society;
• THIRD: The application of the ideal of service in each Rotarian’s
personal, business, and community life;
• FOURTH: The advancement of international understanding, goodwill,
and peace through a world fellowship of business and professional
persons united in the ideal of service.
THE 4-WAY TEST:
One of the most widely printed and quoted statements of business ethics
in the world is the Rotary “4-Way Test.” It was created by Rotarian Herbert
J. Taylor in 1932, when he was asked to take charge as President of the
Chicago based Club Aluminum Company, which was facing bankruptcy.
Taylor looked for a way to save the struggling company mired in
depression-caused financial difficulties. He drew up a 24-word code of
ethics for all employees to follow in their business and professional lives.
The 4-Way Test became the guide for sales, production, advertising and
all relations with dealers and customers, and the survival of the company
was credited to this simple philosophy .Herbert Taylor became President
of Rotary International during 1954-55. The 4-Way Test was adopted by
Rotary in 1943 and has been translated into more than 100 languages
and published in thousands of ways. The message should be known and
followed by all Rotarians.

• Is it FAIR to all concerned?
• Will it build GOODWILL and BETTER FRIENDSHIPS?
• Will it be BENEFICIAL to all concerned?
MISSION OF ROTARY INTERNATIONAL:
The mission of Rotary International is to provide services to others,
promote integrity, and advance world understanding, goodwill and peace

through fellowship of business, professional and community leaders.
Including to:
• Support its member Club/s in fulfilling the Object of Rotary
• Globally promote the concept of service as a worthy endeavor
• Strengthen and expand Rotary to communities around the world
• Foster unity among member Clubs of the association
MISSION OF ROTARY FOUNDATION:
The mission of The Rotary Foundation is to enable Rotarians to advance
world understanding, goodwill and peace through the improvement of
health, the support to education, and the alleviation of poverty.
CORE VALUES:
Rotary adopted the core values in 2007 as part of the Rotary Strategic
Plan, understanding that these five values are fundamental traits of being
a Rotarian. Since their adoption, the core values have been reaffirmed by
the Board and are strongly supported by Rotarians throughout the world.
Rotary International’s core values are:

• Integrity • Leadership
Rotary’s core values represent the guiding principles of the organization’s
culture, including what guides members’ priorities and actions within
the organization. Values are an increasingly important component in
strategic planning because they drive the intent and direction of the
organization’s leadership.
Service:
We believe that our service activities and programs bring about greater
world understanding and peace. Service is a major element of our
mission. Through the plans and actions of individual Clubs, we create a
culture of service throughout our organization that provides unparalleled
satisfaction for those who serve.

We believe that individual efforts focus on individual needs, but combined
efforts serve humanity. The power of combined efforts knows no
limitation, multiplies resources, and broadens our lives and perspectives.
Fellowship leads to tolerance and transcends racial, national, and other
boundaries.
Diversity:
We believe Rotary unifies all people internationally behind the ideal of
service. We encourage diversity of vocations within our membership and
in our activities and service work. A Club that reflects its business and
professional community is a Club with a key to its future.
Integrity:
We are committed to and expect accountability from our leaders and
fellow members, both in the results of our efforts and in the processes we
use to accomplish our goals. We adhere to high ethical and professional
standards in our work and personal relationships. We are fair and
respectful in our interactions, and we conscientiously steward the
resources entrusted to us.
Leadership:
We are a global fellowship of individuals who are leaders in their fields of
endeavor. We believe in the importance of leadership development and in
leadership as a quality of our members. As Rotarians, we are leaders in
implementing our core values.
All of these core values are reflected in the Object of Rotary and The

AVENUES OF SERVICE:
Based on the Object of Rotary, the Five Avenues of Service are Rotary’s
philosophical cornerstone and the foundation on which the activities of
a Club are based:

(i) Club Service focuses on strengthening fellowship and ensuring the
effective functioning of the Club.
(ii) Vocational Service encourages Rotarians to serve others through
their vocations and to practice high ethical standards.
(iii) Community Service covers the projects and activities the Club
undertakes to improve life in its community.
(iv) International Service encompasses actions taken to expand
Rotary’s humanitarian reach around the globe and to promote world
understanding and peace.
(v) Youth Service recognizes the positive change implemented by

Rotary International Council of Legislation (CoL) on April 28, 2010 has
recognized the youths in Rotary by approving enactment to add “New
Generations” as the Fifth Avenue of Service. The CoL of 2013 had made
an amendment to change the name from “New Generation” to “Youth
Service”.

LEADING SCHOOLS TOWARDS E-LEARNING -
A ROTARY INITIATION

Covid-19 pandemic has hit the entire world so badly not only in
human health but multiple sectors. No doubt, school education is
one of the seriously damaged sectors. In Nepal, schools are closed
for more than 3 months with no clue to reopen yet. Nevertheless,
at least one thing that has been largely practiced during this
pandemic is connecting the World virtually. People have realized
how important the virtual communication is and to having such
equipment and accessories for the future.
However, in our local context, because of poor infrastructure and
accessibilities, it has been much challenging to poor students in
the government schools to be connected and get virtual learning.
They simply do not have any smart phone or computer at their
homes. Of course, it’s too hard to support them on individual basis,
but we have realized our initiation to establish E-learning centers
in the government schools is a great support.
In the post Covid-19 era, we can assume lots of changes coming
up in educational system and we must equip and prepare for the
changes on teaching/learning modalities. In such, our efforts
to set up e-learning centers will be more demanding and useful.
E-learning center in schools can open opportunities for children
from poor families connecting with the virtual World for best
learning.
Rotary Club of Pokhara Fishtail has been working with EduTech
Nepal and ICO to build modern computer laboratory network to
create hopes and opportunities for children in the government

schools. In last two years, the club has set up 6 computer labs
(E-learning centers) in the following schools and connected 3,900
students with modern e-learning technology.

The e-learning centers are also best example of partnership
between school and different supporting organizations. Since,
the school’s contribution is there, they feel more ownership on
the project. Basically, the school prepare a room for the lab with
flooring, curtains in windows, coloring and furniture. The club
matches the cost to EduTech Nepal for hardware/software. After
the lab installation teachers get training and orientation on its
uses.
We have planned to set up at lest 20 such e-learning centers by
2024 A.D. The club has been getting request for e-learning center
from many schools, as they have impressed with the already built
centers by the club in other schools. The club select schools
following the set selection criteria largely with an assurance of its
sustainability. Based on the available resources, we are trying to
reach as many schools as possible.
Though it’s a partnership project of RC Pokhara Fishtail with
Edutech Nepal, we welcome any other clubs in the country and
abroad and moreover other organizations to join hands and reach
as many students/schools as possible.

BASICS OF COMMUNICABLE DISEASE
AND LIVING WITH COVID-19

There are enormous types of extremely small organisms around us. They
are called microorganisms (MO). They are classified into virus, bacteria,
fungus and protozoa. They all do not cause sickness. MO which causes
sickness are called pathogen. The disease caused by pathogens are
called infectious disease. Infectious disease is transmissible and are
known as communicable disease. Virus are the smallest pathogen. It is
inactive when it is on nonliving things but become active when comes
in contact with living thing. It has very short reproductive cycle and can
multiply into billions within few hours and cause disease. If virus is a living
or non-living thing is still a controversy. Viruses are capable to change its
structure and properties over a period of time thus its infecting power
and seriousness of illness also keeps on changing over a time.
Virus infections are the commonest illness we encounter in our daily life.
Seasonal flu is the commonest viral disease causing respiratory infection
affecting about 34-100 crores people every year. Viral diarrhea (पखाला),
measles (दादुरा), chickenpox (ठउे ला), rubella (जर्मन दादरु ा), different types
of hepatitis, warts (छालाको मसु ा), oral herpes (ओठमा हुने खटिरा), dengue
fever are example of common viral disease seen in our country.
Incubation period is the time interval between exposure (contacting) to
a communicable disease and appearance of the first sign or symptom,
which is 2-14 days for Covid-19. Exposure means entry of pathogens into
healthy body called host. Host when get symptoms of the disease, they
are called symptomatic. Asymptomatic do not have symptoms but they
can transmit disease to other people. So they are risky for the community.
Window period is the time between getting infection and ability to test
positive by lab. During window period host do not test positive and do not
have symptoms but are capable to transmit the disease. Latent period is
the time interval between entry of pathogen into a host body and ability
to transmit disease. During latent period, they are incapable to transmit
disease. Infectious period is the time when an infected person is capable
to transmit disease to susceptible host.
Some communicable diseases are present in certain area all the time.
For example, tuberculosis, malaria, dengue, typhoid, meningitis, viral

hepatitis etc. These are called endemic disease. When endemic disease
increase to higher level than expected, it is known as epidemic. Nepal
gets epidemics of dengue, cholera, malaria, kala azar, acute gastro
enteritis, Japanese encephalitis epidemic every 3-4 years. Sometime an
infectious disease acquires high infectivity rate and spreads to several
countries at a given time, called pandemic. SARS, MERS, Ebola Swine
Flu has been pandemic in last 15-20 years, Covid-19 pandemic has been
worst pandemic after Spanish Flu pandemics of 1918-1919.
When a pathogen enters into host body, it will reach to its most favorable
place called target organs and start its life cycle. When reaching to
certain number, it starts to attack to the target organs. During same time
our body defense system, known as immune system becomes active and
produces certain substances called antibody and immunoglobulin (Ig)
which will try to kill or neutralize the pathogens. If we become sick or not
is determined by load of pathogens, its power catted virulence and our
power to kill the pathogens. After recovery from disease, antibody and Ig
persists in our body and we are immune to that disease. Infection with
certain virus gives us lifelong immunity e.g. small pox (बिफर), measles
(दादुरा), chicken pox (ठउे ला), mumps (हाडँ े), rubella (जर्मन दादरु ा) etc. HIV
infections attacks to our immune system and our body fails to produce
antibody and Ig even for simple pathogens and we may die of simple
infections. We have heard about treating the Covid-19 patients by giving
blood (plasma) of a person who have recently recovered from Covid-19
is based upon theory that blood of such donor contains antibody and Ig.
Amount of Ig present in plasma of can be accurately measured. Amount
of Ig decreases after certain time and may completely disappear in
certain time interval.
Any person who comes from an epidemic area may be exposed to the
disease. Quarantine is separation of such suspected person from healthy
people. How long should exposed people be kept in quarantine depends
upon incubation period of that disease. For Covid-19, it is 2 weeks. When
someone is kept in quarantine, they are examined for presence of disease
by clinical examination and laboratory tests. Anyone who is positive for
the disease need to be kept separately which is called Isolation. When
there is presence of pathogens in food, drinks or any items of our use, it
is known to be contaminated. These contaminated items are the source
of infection. Disinfection is the process of making contaminated items
pathogens free. Washing with soap and water, exposing on sunlight, use
of chemicals, boiling are common ways of disinfection.
Immunity is the power of body to protect from an infection. There is
immune system in our body which produces certain complex substances
known as antibody and immunoglobulin (Ig) which attacks and destroys

the pathogens. We need different antibody and Ig to fight different
disease. They remain in body for certain period of time and disappear.
Antibody and Ig are also produced after vaccination. Immunity produced
after infection or vaccination remain in body are called active immunity
and they remain much longer, sometime for whole life. Sometime we can
get antibody and Ig produced in other people’s or animal’s body from
their blood. This burrowed immunity is called passive immunity. Snake’s
anti-venom, tetanus serum and hepatitis B serum are example of passive
immunity.
When large number of people in a community have antibody for a
disease, it provides indirect protection to those who are not immune to
it. This is a herd immunity. In order to get herd immunity, most of the
people should get immunized or they should recover from the disease.
When a communicable disease remains in community for long period
herd immunity comes in action. Balanced diet, undisturbed.d 7-8 hours
of sleep and regular exercise (at least fast walking 30 minutes every
day for 5 days in a week) helps to keep immunity power to optimal level.
Stress and exhaustion reduces immunity. Meditation, Yoga, listening
music, singing, and thinking positive reduces the stress. If all people get
vaccinated, and no new cases occurs for 3-5 years, then the disease may
be eradicated. Small pox vaccine was invented on 1798 and eradicated
from world on 1979. With the financial support of Rotary Internal and
International Partners and individual Rotary Clubs, 2.5 billion children
were vaccinated for polio for several years and it has been eradicated
from world except Afghanistan, Nigeria and Pakistan. Now malaria,
measles, mumps, rubella are in process of eradication with vaccines and
case control.
The presence of antibody and Ig in our body can be detected by testing
our blood using a chemical coated dip-strips or cassette. This is a very
simple procedure and can be done at bedside in few minutes. It do not
need a microscopes or special instruments or special skills. It is just like
using a litmus paper to measure pH of a solution. It is now commonly used
to test urine sugar, pregnancy, malaria, HIV, tuberculosis, dengue, certain
cancer and so many other conditions. Its drawback is low in sensitivity,
which is the capacity to test positive case correctly. Sensitivity for
Covid-19 RDT is only 34-80. This test gives 20-66 percent false negative
result that means miss to diagnose positive cases. Sometime it tests
positive to a negative case known as false positive result. Thus we can
miss about 50% of cases in average using this RDT method. A false
positive report will give stress to the patient and family, who is actually
negative. Similarly, a false negative report will give false impression of
being disease free and will not be isolated and transmit the disease. In

Covid-19 infection, it takes 10-14 days to develop antibody hence this
test is useful only after 2 weeks of getting infection.
Reverse Transcription Polymerase Chain Reaction (RT-PCR) is a test
process in which small viral nuclear particle, which is very difficult to
detect due to its small size, is magnified and multiplied hundreds of
thousands times so that it is easily detected in the test. This testing
method is more complicated and needs standard laboratory, precise
complex machine, specific chemical reagents and trained human
resources and takes longer time for the result to come. Its sensitivity is
very high, sometime up to 80-90%. It can detect the virus at the very early
stage of infection.
For an infection to be transmitted, there is need of a sick person called
source, a media for transport of pathogen, and a person to get disease,
called host. Medium is usually air, water or food. Some disease needs a
vector for its transmission. Vector are the insects or animals that carry
pathogen from a sick person to healthy one. Female mosquito is a vector
to transmit malaria, dengue and filaria. Some disease of animals can also
be transmitted to human beings for example rabies and plagues, these
are zoonotic disease. Covid-19 has been transmitted to human from wild
bats, hence it is also a zoonotic disease.
Corona virus is not a new virus, it was first seen in humans in 1930
but it was not so infective and virulent. With some structural changes
(mutation) it caused pandemics of SARS in 2002 and MERS in 2012.
Now it has come as Covid-19. It is Novel Corona Virus full name being
“Severe Acute Respiratory Syndrome Coronavirus-2”, in short Covid-19.
It was first identified in Wuhan City, Hubei Province of China and reported
to WHO on 31 December 2019. It rapidly spread to many countries and
WHO declared it as global health emergency on 30 Jan and as a global
pandemic on 11 March. Millions of virus are expelled by an infected
person during sneezing, coughing or talking. These viruses immediately
float in the air or get deposited in any items lying around. So called social
distancing, use of a mask and frequent hand washing is based to break
these chain of transmission. Details of some of the pandemics are given

From the above data we see that every year about 3-6 lakhs of people
die of seasonal flu every year globally but we are not much concerned
but as of today, 10 July 2020 midday, only 1.24 crore people have been
confirmed to be positive, 72.41 lakhs have recovered and 5.57 lakhs have
died in last 6 months due to Covid-19 and we are panic. It is due to the
fact that it is a new disease and we do not know how its course will go.
Countries like USA, UK, Spain, Italy France could not manage this
pandemic well. The reasons for this may be that other pandemics of this
century were not so severe and considered Covid-19 also will go similarly.
They thought it is a problem of China and their countries are far away.
They did not get prepared in time. In other side, Korea, Singapore, Taiwan,
Vietnam they had encountered more cases and deaths due to Swine Flu
and they took serious steps in its control and management. Vietnam
has a population of about 10 crores and it has only 355 confirmed cases
with zero death because they quarantined all returnees in quarantine and
prevented from spreading inside the country.
Living with Covid-19; Some Facts and Myths
It is likely that Covid-19 is going to remain for some time. Hence we
have to adopt ourselves to live with it. As Covid-19 is caused by a
virus, antibiotics do not cure it. There are no vaccines till now. Now it is
clear that it is not so deadly disease and its infection can be controlled
and prevented by following some guidelines. For correct and recent
information and updates and guidelines for its prevention please log
on Nepal government’s website https://covid19.mohp.gov.np Common
symptoms include fever, dry cough, tiredness, breathing difficulties,
loss of smell and taste. About 80% do not show any symptoms. Out of
remaining 20% only some will need treatment in ICU and only 2-3% will
die of it. So far in Nepal only mild forms have been seen with death rate
of less than 0.3%. The virus primarily spread between people during close
contact. Most often via small droplets produced by coughing, sneezing,
and talking. People may also become infected by touching a contaminated
surface and then touching their face. It is most communicable during
the first three days after the onset of symptoms. The standard method
of diagnosis is by real-time reverse transcription polymerase chain
reaction (rRT-PCR) from a nasopharyngeal swab. Masks are the most

important protective measures when we go out of our house. Malaria,
dengue, typhoid and other illness are common cause of fever and if we
are living in these endemic area, we must immediately seek medical
service if we get fever.
Alcohol:
Drinking alcohol do not protect from Covid-19. The harmful use of alcohol
increases risk of health problems and it can be dangerous. Thermal
scanners are used to detect the temperature. Fever is common symptoms
of almost all infections. Hence use of thermal gun is just to know if one
has fever or not. Fever is not always present in Covid-19 infection.
Drugs for Covid-19: So far there are no proved medicine to cure it
and no vaccines to prevent it. Similarly, no Ayurbedic, Homeopathy or
Naturopathy are effective to cure it. Likewise, so many ingredients and
spices we use in our kitchen like turmeric (बेसार) garlic (लसुन), onion
(पय् ाज) cinnamon (दालचिनी), zinger (अदुवा), pepper (मरिच), all do not cure it.
These ingredients have some properties to increase our immunity power
and may help to increase our immunity but cannot completely prevent
from getting disease. Even people who have good immunity system may
also get the disease if the viral load and virulence is high. Role of insects
to spread Covid-19 Theoretically, house fly, mosquito and other insects
like cockroach can transmit Covid-19 but in practice there is no evidence
to suggest it. Exposing body or taking hot shower do not prevent us
from the infection but they can be refreshing and good for body. Steam
inhalation (तातो पानी बाफ लीने) will help to relieve symptoms of common
cold. It may help to keep the nasopharynx in better condition but will not
prevent from Covid-19. Use of hand dryer will not prevent transmission
of Covid-19.
Which age group is most risky to get Covid-19? Covid-19 can infect people
of all ages. Those who go out of home more frequently are at more risk.
One with pre-existing medical conditions such as asthma, diabetes, heart
disease and overweight are likely to get more severe form and death rate
among them is also higher. Old people have low immune power and they
are also at more risk.
If my antibody test is positive, will I get reinfection? Usually one gets
antibody in blood after getting infectious disease which remains in blood
for some time. This antibody usually protects from second infection.
Summary
We are going to live with Covid-19 for some time. Hence it is important
that we follow certain rules so that we can protect ourselves from it.

We should consider that everybody outside our homes and every items
around us are infected with the virus and apply the preventive measures.
Chances of getting exposed to disease increases in closed places like
shopping malls, halls, restaurants, poorly ventilated rooms and places of
social gathering. Getting chances of infection increases if one spends 10
minutes or more with infected person, sitting at distance of less than 2
meter without wearing a proper mask.
Please follow the following tips and get protected from Covid-19
• Stay at home as far as possible.
• Use masks whenever you go out of house.
• Keeping social l distance of at least one meter, preferably 2 meter.
• Wash your hands frequently and thoroughly. Do not touch your
nose, mouth and eyes with contaminated hands.
• Maintain a balanced diet, stay well hydrated (drink enough water),
exercise regularly and sleep well. Avoid tobacco and alcohol.
• Spent quality life by reducing stress and anxiety. Listen to music,
watch movies of your choice, singing songs and think positive
and remain happy.
