Visits are ideal for aging or those with unique living arrangements
In-home examination/consultation
If laboratory testing is needed, we partner with mobile lab companies. Flat rate does not
cover labs or any additional diagnostic testing.
Free Medication Delivery
Through our Mobile Partners- we partner with other practices to have satellite exam rooms
throughout Chicagoland.

MULTI-MORBIDITY HIV/HEPATITIS AGING

PROGRAM FOR SKILLED NURSING FACILITIES - CONTINUUM




We follow our patients- even in skilled nursing facilities. If patient transitions to a long-term care
facility or a supportive living arrangement, Baal Perazim care will continue. We partner with long
term care facilities and supportive living arrangements to ensure continuity of care.


Residents with HIV/AIDS and Hepatitis often presents with Multi-Morbidity and if not managed, it
will lead to negative outcomes for patients in these unique settings. Baal Perazim’s Multi-Morbidity
HIV/Hepatitis Aging Program brings HIV/Hepatitis services to the facility.


What is multi-morbidity? Multi-morbidity is conceptualized as several serious health conditions
that cannot be cured to any great extent, occurring in an older person and engendering functional
and/or cognitive debility

The program is centered on closing the disparity of health gap in disadvantaged communities.
The Live Long- Live Strong Aging Wellness Program is a unique disease prevention and chronic
disease management program for seniors residing in the community and in senior housing. The
program brings wellness services to the seniors. The goals of the program are to promote wellness
in the aging, prevent hospitalizations, and manage chronic diseases.


Fee for Program
Services are covered by Medicaid and Medicare. If Baal Perazim Wellness, Inc is not in network
with your health insurance, you may qualify for the Charity Program until the process of getting in
network is completed.

Medication Management Systems
Medications are packaged and shipped to seniors’ home. So, no more waiting in lines. Senior have
the option of participating in the medication management program. The Living Long-Living
Strong Aging Program for Senior partners with local pharmacies to assist seniors with managing
their medications.

What is Baal Perazim’s Charity Care Program?
Baal Perazim Charity Care Program is the first of its kind. The overarching goal of the Charity
Care Program is to address the health disparities among men by offering safety-net
wellness care, regardless of their ability to pay. Services are offered on a self-pay sliding fee
scale.
