At Zadok Technologies, we believe our benefits
should make a difference…to you, to your job and
to the life you lead outside of work. Benefits are
more valuable than ever before and are a
significant part of your total compensation
package. We are continually looking for ways to
ensure our programs are meaningful for you.
For the second year in a row, Zadok is absorbing
100% of the increase for medical insurance. The
Blue Cross Blue Shield rates are increasing by
48% effective 2/1/2021 and management at
Zadok has made the decision to keep your
deductions and benefits on your health plan the
same as they currently are with NO increase in
premium! If you do not wish to make any
changes to your current elections for medical
and/or other benefits, your current elections will
rollover to the new plan year. If you do wish to
make changes, please follow the instructions on
Page 5 of this booklet, which is a guide on how
to log in to the Benefits Connect website to
Enroll or Make changes to your current benefits.
Benefit Basics
Zadok Technologies offers comprehensive benefits to promote health and financial security for you and
your family. This booklet provides you with a summary of your benefits. Please review it carefully so you
can choose the coverage that is right for you and your family.
You are eligible for benefits if you work an average at least 30 hours per week. Your benefits are effective
on the first day of the month on or after your date of employment. You may also enroll your eligible
dependents for certain coverages. Eligible dependents are defined as-
• Your legal spouse
• Your legal children up to age 26
Once your benefit elections become effective, these benefits remain in effect until the next “Open
Enrollment” period.
QUALIFYING LIFE EVENTS
You may add or remove a dependent OR add or cancel coverage if you experience a Qualifying Life
Event.
A Qualifying Life Event occurs if you experience an event such as:
• Marriage
• Divorce
• Birth of a child
• Adoption of or placement for adoption of a child
• Change in employment status of employee or spouse
• Qualification by the Plan Administrator of a child support order for medical coverage
• Entitlement to Medicare or Medicaid
You must notify Human Resources within 30 days of a qualifying life event. Depending on the type of
event, you may need to provide proof of the event, such as a marriage license, birth certificate or proof of
prior coverage.
During a qualifying life event, you CANNOT change your current medical plan option.

Zadok Technologies contributes to the cost of your health insurance and pays 100% of the cost for a Basic
Life insurance & AD&D of $10,000.
Voluntary Dental, Vision, STD, LTD, and Life is also available to full time employees. Rates for STD, LTD
are determined by your age and income.

How to enroll in the Zadok benefits using
the Benefits Connect online Website
Welcome!
Your Open Enrollment journey starts here. Welcome to an exciting and easy experience
to elect your employee benefits.
Online Access
Step 1: To log into the Benefits Connect website. Open your web browser and type
benefitsconnect.net into the top tool bar.
Step 2: At your home screen, you will see a Company ID, Username and Password
Welcome Box.
Step 3: The company ID is “Zadok”, your username is the first six characters of your last
name (if applicable), followed by the first letter of your first name, which is then
followed by the last four digits of your Social Security Number.
Step 4: Your initial password is your Social Security Number (SSN). Please note, your
SSN should contain no spaces or dashes. Once you are signed in, you may need to
change your password.
Step 5: Click Sign In to enroll.
NOTE:
Once you are logged in, verify your personal information, add dependents, make your
elections and add your beneficiary. Be sure to click “Finish” when you are complete with
your enrollment.

Employees have three options to choose from within
Blue Cross.
Option 1 is a High Deductible HSA qualified plan
which allows you to open a tax favored personal
Health Savings Account. See your Medical Plan
Options on the following page for more details.
Option 1 has a $6,000 deductible with a $6,000
maximum out of pocket for individual.
Options 2 and 3 are “copay” type plans that offer
copays at the doctor’s office and for prescription
medication. Option 2 has a $5,000 deductible with a
$5,600 max out of pocket for individual. Option 3 has a
$1,500 deductible and $4,500 max out of pocket for
individual.

Life Insurance Benefits
Life and Accidental Death & Dismemberment (AD&D) Insurance Coverage
Administered by Dearborn
Life Insurance provides financial security for the people who depend on you. The life insurance
benefit will be paid to your designated beneficiary in the event of your death while employed
with Zadok. We provide full-time employees with Term Life Insurance and AD&D up to $10,000
at No Cost to the employee.

Basic Group Term Life Benefits reduce by 35% of the original amount at age 65 and further reduce to 50% of the
original amount at age 70. Benefits terminate at retirement.
Voluntary Life & AD&D – Employee Paid Benefit
You have the option to purchase additional life insurance coverage for yourself, your spouse
and/or children. This benefit is 100% employee-paid. You must enroll in the benefit in order to
cover your dependents. Premiums will be deducted on a post-tax basis. Benefits are subject to
reductions at age 65.

Short & Long Term Disability
Voluntary Short-Term Disability (STD) Insurance Coverage
Voluntary Short-Term Disability provides income if you become disabled due to any off-the-job
injury or illness. Zadok offers group STD insurance on a voluntary basis to their full-time
employees.

• This benefit will last for a duration of 11
weeks
• Elimination Period is 14 days for both off-the-
job injuries and illnesses.
Voluntary Long-Term Disability (LTD) Insurance Coverage
Voluntary Long-Term Disability insurance picks up if you are disabled when the STD coverage
ends and if Long Term Disability is elected. Zadok offers LTD insurance on a voluntary basis to
their full-time employees.

401(K) Plan

Benefits You Receive:
To help you prepare for the future, The Quality Companies, LLC sponsors a Safe Harbor
401(k) Plan as part of its benefit package.
With this plan, you can save up to the IRS maximum allowed per calendar year on a pre-
tax basis. (For calendar year 2021, the maximum contribution is $19,500. For
participants age 50 and over, the additional “catch-up” contribution is $6,500, for a total
annual maximum of $26,000).
By saving on a pre-tax basis, you reduce the taxes you pay today and delay paying taxes
on the money you save, as well as your account earnings, until you withdraw your funds
from the plan.
Who Is Eligible and When:
All full-time employees are eligible to participate in the 401(k) Plan. You may begin
participating in the plan upon your date of hire.
Employer Match:
In additional to your contributions, The Quality Companies, LLC helps you save by
matching a percentage of the money you contribute. The company makes an annual
match on behalf of the employees. Because the plan is a Safe Harbor plan, this match is
100% vested. There is no vesting schedule.
To be eligible for the Employer Match, an employee must make contributions to the
plan during the plan year.
The current Employer Match is as follows:
100% of first 3%, then 50% of next 2%
If you contribute 5%, your match will be 4%. You may contribute up to 100% of your salary.
There is no match on any contributions over 5%.
The plan offers a very diverse set of investment options within several categories (stable,
conservative, moderate, aggressive and stock/bonds) through Voya Financial.
