Dependent Verification

Who is an eligible dependent?

• Your legally recognized spouse
• Your natural child, stepchild, or legally adopted child
• A child required to be covered pursuant to a Qualified Medical Child Support

Order (QMCSO)
• A child with proof of legal guardianship who resides with you
• A foster child
• Dependent verification is required during Open Enrollment

A child is considered an eligible dependent until the end of the calendar year in which the child
attains age of 26. A child who is continuously incapable of self-support due to a disability
documented by SSI or SSA may remain eligible for coverage after attaining age 26.

• SurgeryPlus+ evaluates each surgeon and their facility performance before they

can participate in the SurgeryPlus+ Surgeons of Excellence network

• Each provider is licensed, board certified, accredited, has completed fellowship, and

• Procedures scheduled through a SurgeryPlus+ Care Advocate will receive full

concierge service and all out-of-pocket costs will be waived, including

consultation, surgical procedure, and post-procedure appoints for up to 90 days

Supplemental Gap Insurance

Gap insurance is a supplement to your medical insurance that can help offset medical expenses
you may incur when hospitalized or before you meet your deductible and out-of-pocket
maximum limit in the new plan year.

In order to enroll in Gap insurance at NSU, you must be simultaneously covered under a
medical insurance plan at NSU. Gap insurance plans have the same tiers as the NSU medical
insurance plans - employee only, employee plus child(ren), employee plus spouse, and family.

subject to minimum participation requirement for NSU
Premier & Preferred Gap $4,000/$8,000 Gap employees. In the event NSU does not meet the minimum

Dental HMO, the Preventive Plus PPO covers basic & lenses is available from EyeMed*. The Base plan
preventive services, and the High Coverage PPO provides new frames every 24 months, the Enhanced
covers basic, major, and preventive services. plan provides new frames every 12 months.

Retirement Savings

Employees who are eligible, working 19.2 hours or more per week and have
attained age 21, can contribute to the 401(k) Retirement Savings Plan at Nova
Southeastern University on the first of the month following their date of hire

Employees who meet the criteria for non-matched contributions and have
completed one year of service at Nova Southeastern University are eligible for
matched contributions in the 401(k) Retirement Savings Plan at Nova
Southeastern University. Eligible employees can enroll on the first of the month
following their one-year employment anniversary.
