Dear License Reviewer:

Please amend the above referenced material license to reflect the following modification:

1. Effective immediately amend to name Michael Klug, R.Ph. as Radiation Safety Officer. Mr.
Klug shall replace Mark Masterson, R.Ph. in this capacity. Mr. Masterson will continue to serve
as an Authorized Nuclear Pharmacist at this facility.

Mr. Klug was previously listed as the Radiation Safety Officer for this license. A written
acknowledgement of his responsibilities is included as Attachment A.

All other items relating to our radioactive material license and established Radiation Safety Program
remain unchanged at this time. Please contact April Chance, Manager, Radiological Affairs, at (314)
