Hello, I’m Rachel Dombrowsky, and as President and CEO of Oyster Bay Senior Campus, I’d like to thank you
for your interest in our residence.

People sometimes refer to the retirement period as the “Golden Years”, but unfortunately, those years are
not always so golden. When we are young we all have many dreams. Americans imagine their future home
with a white picket fence and a dog. We also imagine living in our house till the end and taking our last
breath at home, surrounded by the people we love.

The main reasons that people consider coming to a residence offering assisted living are because of a
chronic disease, they are overwhelmed with medication management and doctor’s visits, a lack of proper
nutrition or simply, but importantly, due to loneliness.

We all know too well that life changes, and when that happens, our life journey changes too.
Assisted Living is, for many, the next stage of the journey.

When establishing Oyster Bay Senior Campus, my dream was to provide the highest level of assisted
living, to preserve a person’s safety, care and dignity as they age. The kind words of appreciation from our
residents and their families are a regular affirmation that we are doing things right.

Our constant aim is to make every day better than the one before!
Here, we provide socialization, appropriate nutrition, doctors and nurses, and of course, freedom from
chores.

Your feedback is important to us. Our administrative staff will check in with you and your loved one on
a consistent basis to make sure that your family is happy and comfortable with the care and services
provided.

We are ready to guide you through this seemingly daunting process and make sure that your loved one

Our History

Our Campus story began in 1994 with Oyster Bay
Manor. Nestled in the heart of historic Oyster
Bay on the scenic Gold Coast of Long Island,
Oyster Bay Manor soon developed an excellent
reputation, known for its warmth, comfort
and care as an assisted-living residence. Six
years later, in 2000 we founded Harbor House,
the first assisted living residence in New
York devoted to the care of individuals with
Alzheimer’s disease and memory disorders.
Brookville Home Care, our licensed, onsite
private duty agency was established in 2014
and is committed to providing the finest Home
Health and Personal Aide care on Long Island.

The mission of Oyster Bay Senior Campus is to provide a customized and
comprehensive range of residential, home based and short-term solutions
to ensure the safety and security of our residents. We provide exceptional
care given by our expertly trained staff. At Oyster Bay Senior Campus we
are consistently challenging ourselves, raising the bar to make life better for
those who have chosen our home as their own. We maintain an unending
commitment to always act in the best interest of our residents while
maintaining their dignity and enhancing their quality of life.

Assisted Living

Assisted Living with the care you need

In our home-like environment, our staff provides just the right amount and
type of assistance so your loved one can remain independent, yet safe and
secure. Together, the on-site nurse, med techs and licensed aides make
sure our residents receive the very best care. Our culinary and nutrition
teams work together to prepare and provide delicious, balanced meals
and snacks. We take care to adhere to the special dietary needs of our
residents as well as offering alternate options. We provide a wide range of
therapeutic recreational activities daily, weekly entertainment as well as
off-site activities around the North Shore, Oyster Bay Manor is the perfect
place to call home.

At Oyster Bay Manor, we are committed to helping
your loved one live the best life possible. We not only
promote joy and delight, but help maintain your loved
one’s independence and vitality.

Recreation

The goal of our recreation staff is to get everyone involved and
keep them happily engaged with different therapeutic, recreational
activities throughout the day. Whether your loved one can
participate independently or requires some assistance, our staff is
always on hand to provide support and encouragement.

Medical Services

Our superior nursing staff includes an RN, a medical technician
and certified aides. We also provide on-site, board certified
specialty consultants. These include a gerontologist, a podiatrist,
ophthalmologist, psychologist and psychiatrist in addition to a
physical therapist, occupational therapists and social workers.
We are also pleased to offer medication management and
pharmaceutical services, as well as coordination of medical
appointments.

About Us

We are devoted to caring for individuals with Alzheimer’s
disease and memory impairments here at our specially
designed three-story residence.

Harbor House was the first residential facility in New York dedicated
to patients with Alzheimer’s disease and memory impairment. Our
residence is comprised of three completely separate and secure
floors. This unique approach allows your loved one to thrive
in an environment that is suited to his or her specific social and
cognitive abilities. Each floor has its own nursing team, med tech
and caregivers to ensure that every resident gets the care, attention
and stimulation they need. Having everything on one level allows
our residents to freely move around and feel comfortable with their
peers. Our therapeutic recreation team utilizes imagination and
knowledge to create appropriate programs for each floor to provide
activities that encourage social involvement, improve cognition,
movement and motor skills as well as giving each person a sense of
achievement.

Mission Statement

Here at Harbor House our mission is to provide
comprehensive, compassionate and dignified care
to our residents. Our caring staff is personally
invested in every facet of our resident’s well-being
from their outward appearance to their emotional,
cognitive and physical health. We maintain an
unending commitment to always act in the best
interest of our residents and their families.

Our Care

We cannot cure memory disorders. What we can do is
make sure that every day our residents spend with us is
filled with joy, dignity, care and respect, in an environment
that is safe and secure.

Our Nursing Team

Providing the highest standard of nursing is an integral part of the outstanding care we
bring to our residential setting. We are proud that Harbor House has more nurses on staff
than any other assisted-living facility in Long Island. In addition to our amazing RNs and
LPNs on every floor, we also have personal care aides on staff, maintaining a strict 5:1 ratio
per floor, as well as EMTs and med techs on staff, 24 hours a day.

Therapeutic Recreation

The goal of the therapeutic recreation team is to optimize the time residents spend in
communal settings amongst friends. With approximately 57 percent of Harbor House’s
square footage devoted to common areas, the generous size of the building allows for
spacious areas where our residents are safe to walk or wander around as they wish. We
will work with you to learn about your loved one’s interests and hobbies, and preferences
catering to their needs and helping them to feel at home. We offer various therapeutic
activities such as, music, pet and equine therapy. Our activities have an emphasis on
reminiscence, whether playing bingo, gardening, pursuing adaptive sports or some other
passion, we have something for everyone. Our creative recreation team organizes regular
socials, poetry sessions, religious services, trips and so much more.

Levels of Care

Your loved one is unique, his or her care should be as well. Our Levels of Care are based
on a four-stage assessment model that is performed by our nurse and care manager prior
to admission.
Our professional team evaluate each resident to ensure they receive the right amount and
type of care, based on their medical, cognitive and social needs. While independence is
always encouraged, it’s crucial for each resident to receive the care and support he or she
needs to flourish.

About Us

Caring for a loved one, while rewarding is often challenging. You don’t
have to do it alone. If your loved one needs care at home or extra
support in a facility, Brookville Homecare, our onsite homecare agency
may be the right choice for you. Our supportive staff will assist you in
obtaining the best homecare solution for your individual needs.

Brookville Homecare is NYS licensed, bonded and insured. Our
professionally trained HHA’s/PCA’s assist with activities of daily living
such as bathing, dressing, toileting, transferring and meals as well
as providing social stimulation. We offer a wide range of homecare
options, specializing in live-in and 12-24 hour care, under the skilled
supervision of our nursing staff.

Our registered nurse will perform a thorough, complimentary
assessment and create a customized care plan for your loved one. Our
care team will then match him/her with a highly qualified, expertly
trained caregiver. Equipped with the skill, knowledge and compassion
to form a successful caregiver/care recipient relationship enabling
your loved one to thrive in their home environment while giving you
peace of mind.

Monthly Fees

Semi-Private $4,230.00 Community Fee
Semi-Private Standard $4,795.00
Semi-Private L Layout $5,300.00 Residents pay a one-time community fee
Semi-Private Corner $5,545.00 of one month’s rent to cover the costs of

Short Stays/Respites $450.00 per day refurbishing the studio and maintaining
