NAME OF ARTIFACT: Individual Learning Monitoring Plans
PROFESSIONAL STANDARD STRAND:3.1 learners’ gender, needs,
Strengths, interests and experiences.
ANNOTATIONS:
1. Last November I conducted individual reading through
calls, I noticed that this student couldn’t read the words
correctly and his mother was coaching him. Since I
couldn’t visit him because of the pandemic . I decided to
give him supplementary reading materials.
2. I noticed that this student was able to read the words
correctly. Parents guidance was a big help in learners
improvement.
3. I am very happy that this student can read
paragraph/stories with comprehension.

During this new normal of education, I utilized ICT tools which
are very useful in my class. I made PPT slides, I also share many
lesson that can be found in youtube inorder for them to understand
the lesson well.

PROFESSIONAL STANDARD STRAND:7.4 Professional Reflection and
Learning to improve practice
When I read the artifacts in LDM Practicum Portfolio , I was
so confused. There are lots of question that came into my mind,
“ Where will I get these artifacts? How can I finish my
portfolio?, but through reading and reading the tool , I realized
that it’s not hard especially when you have documented all the
needed artifacts.
This collection of work reflects my abilities in my
performance. Through this LDM course inspite of the pandemic
that we are facing, I tried to educate my students and
communicate parents through media platforms.
It serves as my guide /tool in enhancing my capabilities in
achieving my goals for the good of my students.
ANNOTATION
My reflective summary expressed about my personal
experiences on how I go along with this LDM course and how I
learned a lot from it.

With this new normal, Team Grade five never stop our

Since face to face is not possible, I communicate to parents
and students using messenger and calls. I also created group
chats to both parents and students . I personally gave feedback
to parents who are not a member of our chatgroup.
