International Patient Safety Goals (IPSGs) REDUCE THE RISK OF PATIENT HARM RESULTING FROM FALLS Preventing falls by: 1. Screen all patients for fall risk. 2. Reassess fall 2.1 Daily 2.2 Post operation or procedure; 6 hours post -delivery 2.3 Upon transfer from one unit to another unit 2.4 Changes in patient’s condition 2.5 After fall 2.6 Upon returning from home leave 3. Apply orange tag on wrist for high-risk patients and place orange falling star signage on the patient’s room door. 4. Apply fall prevention tools e.g., bed cladding and Wireless Bed Alertamat. IMPROVE THE SAFETY OF HIGH-ALERT MEDICATIONS High-Alert Medications are medications involved in high percentage of errors, carry higher risk for adverse outcomes, which includes Look Alike Sound Alike (LASA) medications. High-Alert Medications include: 1. Insulin 2. Narcotics and opiates 3. Chemotherapeutics 4. Heparin & others anticoagulant 5. Injectable muscle relaxant 6. Injectable anaesthetics & sedative preparations Ensure the following: 1. Concentrated electrolytes should not be kept as ward stocks except for specific departments. 2. Safe storage, correct labelling, and safe administration of High-Alert Medications. ENSURE CORRECT PATIENT, CORRECT PROCEDURE/ SURGERY AND CORRECT SITE 1. Verify correct patient, procedure/ surgery, and site. 2. Ensure availability of relevant documents, test result(s), X-Ray reports(s) and required equipment for the case. 3. Mark the operate site with an Arrow ( ). 4. Time Out is conducted before surgical incision or the start of the procedure. REDUCE THE RISK OF HEALTH CARE-ASSOCIATED INFECTIONS Practice 5 moments of hand hygiene: 1. Before touching a patient. 2. Before clean/aseptic procedures. 3. After body fluid exposure/risk. IDENTIFY PATIENT CORRECTLY Use 2 unique identifiers to identify patient: 1. Before administering medication, blood, or blood products. 2. Before taking specimen or blood sample for laboratory test. 3. Before provide treatment and procedure. 4. During registration. 5. Before serve food. 6. When handover care to another medical unit. 7. Before handover newborn. Outpatient & Inpatient 1. Full Name 2. Date of Birth DO NOT USE PATIENT’S ROOM NUMBER AS AN IDENTIFIER! IMPROVE EFFECTIVE COMMUNICATION 1. Improve verbal and telephone communication among healthcare personnel. 2. Reporting critical results of diagnostic tests. 3. Implements a process for handover communication with ISBAR.
