Purpose

The purpose of this procedure is to define the control process for qualifying staff members to
independently lead / facilitate Reliability Centred Maintenance, System Assurance (HAZOP or risk
assessment) or Human Factors studies.

Scope

Applicable to all staff who are working on RCM, HAZOP, Risk Assessment or Human Factors
studies in the Operations Division.

Definitions

D1 Assessment Authorities
Respective Assessment Authorities shall determine the administration arrangement for their own
process of the initial registration / renewal of qualification / re-assessment based upon their
organisational hierarchy structure e.g. HF Specialists request HFMC’s re-assessment via line
managers and HF Working Groups.

For RCM Facilitators, AM-SIWG oversees the Qualification Criteria and takes the role of SME of
this procedure; AM-C&CWG will lead the implementation of qualification and training system
including the provision of training, endorsement of training materials and assessment criteria, the
initial registration, the renewal qualification, performance of job practices, updating the
qualification database, etc.; OA Team will organise the annual RCM Facilitation Skill Workshop
and attend the first workshop of each RCM project to provide assistance on RCM facilitation.

For SA Practitioners and HF Specialists, SM-SA&C and HFMC are responsible for the provision of
training and the matters relating to qualification respectively.

D2 Job Practices
Job practices refer to the undertaking of relevant RCM, SA or HF studies, which include facilitating,
reviewing, commenting and/or vetting applicable study reports.

D3 Log Sheet
A log, used by applicants, records his/her job practices for initial registration application and
renewal of qualification (Exhibit E3).

D4 Qualification Criteria
Criteria defined for the initial registration and the subsequent renewal of qualification (Exhibit E1).

D5 Re-visit of RCM Study Report
The validity of an RCM study report shall be confirmed if changes to assets have been revealed per
the criteria defined in the Divisional Procedure P/OD/AMS/029 - “Control of Reliability Centred
Maintenance Study”.

D6 System Assurance Practitioner
System Assurance Practitioner, within the context of this procedure, refers to the HAZOP
Facilitator and/or the Risk Assessment Specialist.

Details of Action Steps

Note: This section provides detailed actions / practices for implementing the above flowchart.
Here, “Requisite” refers to actions that must be complied with for the purpose of statutory
and safety compliance, or the assurance of the achievement of desired results. “Good
Practice” refers to practices for action parties’ consideration or reference only; action
parties have the flexibility to modify them in order to meet the needs of the prevailing
situation.

 Staff members assigned to lead / facilitate RCM, SA and HF studies
<2> Requisite must be qualified RCM Facilitators, qualified SA Practitioners or

<10> Requisite  For the RCM analysis and its re-visit, they shall be facilitated by a
qualified RCM Facilitator or a staff completed RCM and guided by a
Requisite registered RCM Facilitator in facilitation activities (see Details of
Action Step <5> of P/OD/AMS/029). Exhibit E5 shows the detailed
arrangements of the RCM Qualification Process.

 For the SA studies, the SA Practitioners lead the studies as follows:
- HAZOP related to modification of Safety Critical System shall be
facilitated by a qualified HAZOP Facilitator.
- Risk assessment shall be done by a qualified Risk Assessment
Specialist.

 For the HF studies and reports, they shall be conducted / reviewed by a
qualified HF Specialist.

The Qualification Criteria (D3) shall be reviewed during the 3-yearly
review of this procedure.
For details of the training, workshop attendance and job practice
requirements, refer to Exhibit E1.
 The registry of valid qualifications with their expiry dates shall be

accessible to stakeholders in the OK Mall e.g. SA Practitioners and HF
Specialists on SA&HF Creator website and RCM Facilitators on AM
Corner website.
 The database of qualifications including results of application
assessment, log sheets, etc. shall be maintained and retrieved by the
responsible Assessment Authorities.
The Log Sheets shall be sampled regularly, to assess the implementation
status and the achievement of job practices.
