INTRODUCTION
ABOUT US POLIKLINIK NAZMIR was established on 15 November 2001, located at No 25 & 27, Jalan M1, Taman Merdeka, Batu Berendam, 75350 Melaka. Currently, one branch operated at No 45, Jalan TU 49A, Kompleks Komersial Boulevard, Taman Tasik Utama, Ayer Keroh, Melaka which started operation on 29 March 2012. The NAZMIR POLYCLINIC GROUP is majority-owned by NAZMIR RESOURCES SDN BHD, which is responsible for managing the administration, finance, and opening of clinic branches. In POLIKLINIK NAZMIR, we provide comprehensive primary care medical services to all our clients, from the general public to the corporate and statutory bodies in Malaysia. We are also providing consultancy to many governmental departments, professional organizations, and industrial corporations.

OUR COMMITMENT TO THE CLIENTS POLIKLINIK NAZMIR is a private clinic registered under the Ministry of Health, Malaysia. Services provided at our clinic are subject to the Laws of Malaysia, under Act 50, Medical Acts 1971 & Act 586, and Private Healthcare and Services Act 1998. We are committed to providing the best healthcare services by the guidelines and regulations set by the Ministry of Health, Malaysia including the processes involved with our patients’ personal information and data. The purposes of the information obtained are as follows: • For patients’ treatment in government or private hospitals or other clinics • For administrative and treatment purposes related to the patient's insurance company. • For emergency use where the patient is unable to give permission. • For the handling, storage, and updating of our clinical information system. • To keep records of patients as required by Malaysian law. We will not disclose the patient’s personal information to individuals or organizations not directly related to our clinic unless with the patient’s permission. We have the right to access patients’ information and health records. However, patients have the right to amend or correct any changes to the personal information and data kept by us.
