Child Policy

Bunny lash has a no children policy. For the safety and comfort of your children, yourself, and our guests,
we appreciate your support and understanding of our guidelines regarding having your children in the
salon and spa. Please make all necessary child care arrangements prior to your visit.

Late/tardy policy

We know your time is precious and we do our best to run on time, planning to arrive 15 minutes prior to
the start of your service will allow us to confirm all information we have on file for you is current, and
support your service beginning at the scheduled time. Life happens. If you are running late, please call
ahead. We can notify you if we will still be able to accommodate you or, if we need to, reschedule to
another time. We will always do our best to see you but we have to consider the impact on our other
Guests, as well. Appointments are in high demand and your timely cancellation will allow you, and other
clients, access to timely services. Please be aware of our cancellation policies regarding No-Shows, Late-
Shows and Same Day Cancellations.

Same Day or Late Cancellation

We do everything we can to help remind our Guests of their upcoming appointments. We
provide 24 hour confirmation calls in addition to text message or email reminders for Guests
who have agreed to this service. As a courtesy to other Guests and our Staff, kindly provide 24-
48 hours advance notice should you need to cancel, reschedule or otherwise make changes to
your appointment. Any cancellation/reschedule made less than 24hours in advance will result in
a charge equal to 50% of the reserved services.

No-Show:

A “No-Show” is a client who does not show up to their scheduled appointment without canceling
a minimum of 24-hours in advance. Failure to be present at your scheduled appointment time
without notice, will be recorded in each client’s chart as a “No-Show”. A second occurrence of
“No-Show” without reasonable cause may result in client discharge from the facility.

Service Deposits

A valid credit card is required to hold your appointment, but you will not be charged until the
day of your appointment. This better ensures your end result and ensures your appointments
are booked appropriately. this serves as a deposit towards the completion of your service
appointments require a deposit at the time of booking. A flat deposit of $50 for full sets and $25
deposit for new client fills.

that you are unhappy with your service, let us know within 72
hours of the date of service and we will ensure that you are re-
booked with your technician. In most instances, the fix is a simple
solution that may take less than an hour to resolve. We strive to

CONSENT FOR EYELASH PROCEDURE:

I have agreed to have Suite Bunny Glam™ eyelash extensions applied to and/or removed from my eyelashes. Before my qualified
professional can perform this procedure, I understand I must complete this agreement and provide my informed consent by signing
and dating where indicated below. For valuable consideration, in order to have my Suite Bunny Glam™ eyelash extensions applied
and/or removed from my eyelashes:

1. Waiver of Liability. I understand there are risks associated with having artificial eyelashes applied to and/or removed from my
existing eyelashes, and that notwithstanding the utmost of care in the application or removal of these products, there still exist risks
associated with the procedure and product itself, which include, without limitation, eye irritation, eye pain, discomfort, and, in rare
cases, blindness when improperly handled. As part of this procedure, I understand that a certain amount of eyelash adhesive
material will be utilized to attach the artificial Glam Lashes™ to my existing eyelashes. Even though the Professional may apply or
remove my Glam Lashes™ properly, I understand adhesive material may become dislodged during or after the procedure, which
may irritate my eyes or require further follow-up care, at my own expense to prevent damage to my eyes. I also understand there is
more than one technique for applying Glam lashes™ to my eyelashes, and I will not attribute any liability to Professional or Suite
Bunny Glam, as a result of this procedure or the use and care of these lashes. I also agree to defend, indemnify and hold harmless
Professional and Suite Bunny Glam from any and all claims, actions, expenses, damages and liabilities, including reasonable
attorneys’ fees which might be asserted against them as a result of my having this procedure performed, or my purchase of these
Lavish Lashes™ products. As used in this agreement, the terms “Professional” and “Suite Bunny Glam” include all of their respective
officers, directors, agents, employees, successors and assigns.

2. Permission to Use Pictures. I hereby grant to Professional and Suite Bunny Glam the full right to take, publish and reproduce
photographs of me, my face, my eyes and/or eyelashes, both before and after this procedure, for any advertising, education, or other
purposes whatsoever, including the right to retouch these photographs as deemed necessary by Professional or Suite Bunny Glam. I
further expressly assign any copyright in these photographs to Suite Bunny Glam. I also grant my consent for Professional and Suite
Bunny Glam to use my image and likeness as contained in these photographs for any advertising or other purposes, along with any
comments I may provide.

3. Care and Maintenance. I agree to follow the care and maintenance instructions provided by Suite Bunny Glam and/or
Professional for the use and care of my Glam Lashes™, and that if any follow up care is required due to my own mistake or
negligence, or failure to follow these instructions, this will be at my own expense and risk. I understand that if I do any of the
following, it may result in damage to my Glam Lashes™ or may cause my lashes to fall off prematurely. Knowing this I agree to
follow these tips for best results: I will avoid oil based eye products as these will loosen the bond of my Glam Lashes™. I will avoid
getting my lashes wet within the first 24 hours after my application. For the first two days after application I understand it is best to
avoid swimming, saunas or steam rooms. If I experience any itching or irritation, I agree to contact my Suite Bunny Glam™
Professional immediately to have the lash extensions removed. I agree to avoid using waterproof mascara and to not use an eyelash
curler, perm, or tint my Glam Lashes™. I agree to not pick, pull or rub my Glam Lashes™. I understand that I should not attempt to
remove my lash extensions on my own or with any product, but that the procedure requires that my lash extensions be
professionally removed.

4. No Known Medical Conditions / Informed Consent. I have read and completed the Suite Bunny Glam Client Intake Form in its
entirety and in truth. I acknowledge that I have been advised of the potential harmful or negative side effects (such as the premature
shedding of my eyelash) that the lash extension procedure or removal may cause to those who have specific medical or skin
conditions. I understand that the adhesives and adhesive remover are a skin, eye and mucus membrane irritant and that in rare
cases persons may be allergic or have hypersensitivity to synthetics, cyanoacrylate or formaldehyde, which in small amount may be
present in the adhesive. I understand that the procedure requires that I lay still for up to 2 hours or longer with my eyes shut, and
that if I wear contacts, I must remove my contact lenses for the duration of the lash extension application or removal. I further state
that I have no known medical condition that might be aggravated by the procedure or any medical condition that would prevent me
from complying with or heeding to the professional’s or Suite Bunny Glam instructions or these warnings. If any action is brought to
enforce the terms of this Agreement, the prevailing party shall be entitled to its costs and reasonable attorneys’ fees. Any claims
arising out of this agreement will be resolved through binding arbitration using the rules of the American Arbitration Association.
This agreement will remain in effect for this procedure, and all future procedures conducted by Professional or any other
professional conducting business at the salon/spa establishment listed above. I agree that this Agreement is binding upon me, and
my heirs, legal representatives and assigns. I represent that I am over 18 years of age and that I have the right to enter this
agreement, or if I am under 18 years of age, I have had my parent or legal guardian consent to this agreement, and his or her

After Care

Thank you for visiting Suite Bunny Glam. We hope you are loving your new Glam lash extensions! Please
review the information below to keep your lashes looking beautiful.

Eyelash Extension Aftercare Information: The eyelash cycle is approximately four to twelve weeks. Do not be
alarmed if some of your lashes fall out quickly (they most likely were at the end of their cycle). We tend to
shed 2-4 natural lashes daily. The majority of extensions will stay through a complete cycle if you follow
these guidelines:

 Your natural eyelashes should not get wet for a minimum of two hours; 24 hours would be ideal
to allow the adhesive properly to bond to a dry eyelash (soggy eyelashes do not make for a good
adhesion). A dry washcloth can be rolled up and placed over your brows when you shower to
protect your extensions from becoming totally saturated.

 Do not use regular or waterproof mascara. Most brands of mascara contain chemical properties
and oils that will break down the adhesive. Removing mascara from your lashes can cause the
extensions to fall off prematurely as well. If you want to further enhance your lash extensions, we
have a specially formulated eyelash extension mascara that is ideal for coating your
extensions. Ask your lash stylist for more information.

 Take special care around the eye area when cleansing your face. Gently cleanse the eye area and
do not scrub or rug your eyes. Avoid any type of oil-based product around the eyes. Again, the
oils will break down the adhesive and cause the extensions to fall off. Resuming with normal
activities is ok (i.e. - face washing). When possible, it is best to wait up to 12 hours.

 Never use a clamp-style eyelash curler on your extensions. This can permanently damage not
only the extensions, but your natural eyelashes as well. Eyelash combs are great for grooming
your lashes every morning

 Suite Bunny Glam adhesives are cured by moisture, however, by avoiding extreme variation of
heat, humidity, and cold weather (i.e. – hot showers, saunas, zero degree weather), your
extensions can last longer. Normal activities like swimming, can resume after the appointment

 Be sure to schedule a Suite Bunny Glam™ Re-Glam appointment every 2 to 3 weeks for timely
touch-ups. This will keep your lashes looking full and natural, indefinitely.

 Do not attempt to remove the extensions yourself. Have them removed by your professional Suite
Bunny Glam artist. Brush your lashes daily. Do not sleep directly on your lashes. Come to your
Glam appointment with clean lashes.
