2) PRELIMINARY EVALUATION
A. History Taking
Patient’s information: Nurul Asyiqin binti Azrul Hisam
Purpose of visit:
- Contact lens fitting
- Correct refractive error and improve cosmetic appearance
External observation: patient’s posture, head and eyes have no abnormalities
Ocular History:
- Wears spectacles for 12 years (current spectacles for 2 years)
▪ Chief of complain: Dry eyes when driving (didn’t blink as much as needed)
▪ Frequency: every time driving
▪ Problem on both RE and LE
▪ Problem starts: 6 months ago
▪ Level of severity: 2/5
▪ No pain in any eye movement
▪ Relief by closing eyes for few seconds
▪ No other underlying symptoms
- First time visit for contact lens fitting
- Never use contact lens
- Eye trauma/surgery/disease: -
▪ Family ocular history: 4 out of 6 wears spectacles
▪ Ocular disease: -
Medical History:
- No systemic disease
- Medication/allergic/supplement: -
▪ Family medical history: - pt’s mother has diabetes mellitus (currently with
insulin injection)
Contact lens history:
- Never wear contact lens
Examiner’s evaluation: - pt need to do a few eyes examination especially TBUT
concurrent to her dry eyes and to check if she compatible to wear cl. Other than that, need
to consult patient about proper way to handle cl since its her first time.
