iLASH’D IT BUSINESS POLICY

24 HOUR POLICY FOR CANCELLATIONS OR APPOINTMENT CHANGES:

Please notify iLash’d It at least 24 hours in advance for cancellation of scheduled appointment. This
allows you to reschedule and provides enough time to offer that available time to another client. NO
CALL NO SHOW will result to an automatic cancellation and deposit becomes non-refundable.

DEPOSITS:

iLash’d It requires a $25-dollar deposit on all full sets and refills. If you reschedule at least 24
hours in advance, deposit remains in place for your next appointment. DEPOSITS ARE
NON-REFUNDABLE.

EARLY ARRIVAL:

Early arrival is strongly suggested. Please arrive early due to each guest having a reserved
time scheduled. For all first-time guests please arrive 10 -15 minutes early (before your
scheduled time) to fill out Client Consent forms for eyelash extensions. This will allow you
your full session. For all returning guests, please arrive at least 5 -10 minutes before your
appointment time to check in, get situated, and be ready to go for your full reserved session
time. A courtesy appointment reminder in the form of a call, text and/or email is provided 24
hours for appointment confirmation.

LATE ARRIVAL:

Arriving late will cut into your reserved session time which may also result to a cancellation if
you arrive more than 10-15 minutes after your scheduled time. If you think you may be late
for your appointment, please contact iLash’d It to let us know. A courtesy call, text and/or
email is provided within 5 minutes into your scheduled and reserved start time. Arriving late

for your appointment your services will be altered to fit the remaining time. If this occurs, you
will be solely responsible for the full payment amount for the services booked.

NO SHOWS:

If you do not show up for your scheduled appointment, without contacting iLash’d It to
properly cancel your booked session your deposit is non-refundable. To secure any future
bookings it will be mandatory to have a credit card on file. Any clients or potential clients who
no show on more than one occasion will be required to pay in full before any appointments
are booked.

NO REFUND POLICY:

iLash’d It has a no refund policy on services completed. If any issues arise within 5 days of
services provided, a complimentary correction will be applied with the exception of
management approval.

Please place cell phones on silent mode to minimize distractions and interruptions during
your session. It is very crucial that your eyes are to remain closed during the entire procedure
to avoid unnecessary time added to your service to adhere to your scheduled session.
CHILDREN:
Child(ren) under the age of twelve are not allowed at appointments scheduled. Here at
iLash’d It we love children, however due to the circumstances of the services being provided
you will be unable to supervise your child(ren). Our staff will not be responsible for any
child(ren).
PLEASE ARRIVE WITHOUT MAKE-UP:
Please arrive without any eye make-up, mascara, eyelash strips etc. to your appointment as
this will cut into your services booked. There will also be an extra fee applied depending on
the amount of make removed if applicable. If this occurs, you will be solely responsible for the
full payment amount for the services booked.

GENERAL HEALTH AND SAFTEY
RECOMMENDATIONS/AFTER CARE

Eyelash extensions should be applied by an experienced certified
and/or licensees Eyelash Technician. Which should take place in a
sanitary setting that has been properly disinfected. With the
application of eyelash extension there are many safety and health
hazards to be placed into consideration. Not everyone’s eyes and skin
are able to handle certain ingredients in the adhesive which can
cause an allergic reaction. An allergic reaction to formaldehyde is very
common, one may experience itching, burning, redness and swelling
of the eyes. If you have a known sensitive of your eyes it is best to
advice your eyelash technician prior to your application. Therefore, it
is very important for your eyes to remain closed during the entire
procedure and follow all after care instructions provided by your
technician. In some case a patch test will be provided to test your
sensitivity to adhesive. Eyelash extensions are to only be applied to
each individual natural lash and should not encounter physical contact
with your skin. Properly maintained eyelash extensions increases the
longevity of your extensions.

After care of eyelash extensions plays a major role in the overall out come of
your experience. After care of eyelash extensions include:

 Thoroughly cleanse eyelash extensions daily with (baby shampoo &
diluted water)

 Avoid applying water proof mascara to eyelash extensions
 Avoid rubbing, tugging, pulling, picking and/or twisting on eyelash

extensions which may cause damage to your natural eyelashes.
 Brush eyelash extensions with mascara wand brush at least 2x a day
 Maintain regular fills every 2-3 weeks to keep eyelash extensions full
 Avoid oil-based products around the eye area
 Contact certified tech if you experience any irritation or discomfort

 No perming, tinting or curling eyelash extensions
 Allow certified tech to properly remove eyelash extensions

EYELASH EXTENSIONS CLIENT CONSENT FORM

____ I agree to have eyelash extensions applied and/or removed and retouched to my natural
eyelashes. By signing this agreement, I consent to the placement and/or removal of the eyelash
extensions by the licensed/certified eyelash extension professional.

____ I understand that in rare occasions there are risks associated with having artificial eyelashes
and eyelash extensions applied to or removed from my natural eyelashes. I further understand that in
rare cases as part of the procedure eye irritation and discomfort could occur. I agree that if I
experience any of these conditions with my lashes, that I will contact the certified/licensed eyelash
extension professional that performed this procedure and that it may be beneficial to have the
eyelashes removed.

____ I understand and agree to the after-care instructions provided by the certified/licensed extension
professional for the use and care of my eyelash extensions. I realize and accept the consequences of
failure to adhere to these instructions may cause the eyelash extensions to fall out and/or decrease
the time the lashes will last.

____ I understand and consent to having my eyes closed and covered for the duration of
approximately 1-2-hour procedure. Times may vary depending on the type and number eyelashes
applied.

____ I give permission to ILASH’D IT to take before and after pictures for use of social media
branding and/or advertising.

____ I am informing the certified/licensed eyelash extension professional of the following conditions
by marking with a check:

 Current use of contact lenses which I may be asked to remove during the procedure
 Current use of anything such as oil-containing sunscreen or moisturizers around the eyes
 Current use of eye drops of any kind, prescription or over-the-counter
 Current allergies or sensitivities
 History of recurrent eye or tear duct infections
 History of dry eyes or Sjogren’s Syndrome
 Recent history of Chemotherapy
 Other medical conditions which would prohibit or compromise placement and retention of

eyelash extensions

____ I agree to the following eyelash extension follow-up and maintenance instructions:
 No waterproof mascara
 No oil-based products around the eye area
 No tinting or perming of eyelash extensions
 No pulling or rubbing of the eyelash extensions
 No eyelashes strips should be applied to eyelash extensions

This agreement will remain in effect for this procedure and all future follow-ups conducted by the
certified/licensed eyelash extension professional. I read and understand that this consent agreement
is legal and binding. I have read fully understand all information in this agreement. I am over 18 years
of age and consent to the agreement and to the eye extension application procedure.
