My objective as an Eyelash Technician is to make each
individual feel comfortable throughout their whole application
process. As they leave I want my service to give them a new
found confidence and great outlook to their day.

APPOINTMENTS

Appointments can be made online, or by calling. We do not accept appointments by e-mail or social media. A credit
card is required to reserve your appointment.

CANCELLATION / RESCHEDULING POLICY

We strive to accommodate our clients to the best of our ability. All of our appointment confirmations are sent via
text or email. Please honor your appointment, if you must cancel or reschedule please do so at least 6business
hours* before your appointment time to avoid a cancellation fee. If cancelling/rescheduling for 2 or more people,
please do so at least 24 hours before the scheduled appointment time to avoid a cancellation fee.

A credit card is required to hold appointments at Eye Love Lash Studio. If you choose not to provide your credit
card information, we will not hold your appointment. A $50 cancellation fee will be charged to the credit card on
file for each appointment that is cancelled less than 6 business hours before the appointment time.

• No-shows will be billed for the full (100%) amount for the appointment.

• Clients who have no-showed for appointments will be asked to prepay/pay a deposit towards their visit
when booking future appointments.

• All appointment cancellations must be made & confirmed via telephone. Appointment cancellations
through text or email are not accepted.

LATE ARRIVALS

We ask that our clients to arrive promptly for their appointments to ensure sufficient time for quality results.

• While there is no fee for late arrivals, the length of the appointment may be reduced due to time restraints.

• Guests arriving over 15 minutes late may be asked to reschedule, as accommodating late clients may
inconvenience the lash artists’ next appointments. While we will try our best to accommodate you,
understand that your appointment may be rescheduled at our discretion. In this event, the
cancellation/rescheduling policy will come into effect.

REFILLS

We do not perform refill services on work that was not done at Eye Love Lash Studio– NO EXCEPTIONS. If you
currently have lash extensions on, you must get them removed before receiving your first new set with us. This is to
avoid the risk of mixing different adhesives and materials from work done in other salons.

PAYMENT/GRATUITY

We accept Cash and all major credit cards. Gratuities are accepted and appreciate.

How to care for your Eyelash Extensions

Eyelash Extension Aftercare Information: The eyelash cycle is approximately four to twelve
weeks. Do not be alarmed if some of your lashes fall out quickly (they most likely were at the
end of their cycle). The majority of extensions will stay through a complete cycle if you follow
these guidelines:

• Your natural eyelashes should not get wet for a minimum of two hours; 48 hours would
be ideal to allow the adhesive properly to bond to a dry eyelash (soggy eyelashes do not
make for a good adhesion). A dry washcloth can be rolled up and placed over your
brows when you shower to protect your extensions from becoming totally saturated.

• Do not use regular or waterproof mascara. Most brands of mascara contain chemical
properties and oils that will break down the adhesive. Removing mascara from your
lashes can cause the extensions to fall off prematurely as well.

• Take special care around the eye area when cleansing your face. Gently cleanse the eye
area and do not scrub or rub your eyes. Avoid any type of oil-based product around the
eyes. Again, the oils will break down the adhesive and cause the extensions to fall off.

• Never use a clamp-style eyelash curler on your extensions. This can permanently
damage not only the extensions, but your natural eyelashes as well. Eyelash combs are
great for grooming your lashes every morning.

• Our adhesives are cured by moisture, however, by avoiding extreme variation of heat,
humidity, and cold weather (i.e. – hot showers, saunas, zero-degree weather), your
extensions can last longer. Normal activities like swimming, can resume after the
appointment

• Be sure to schedule an “Eye Love lashes” Re-Beautify appointment every 2 to 3 weeks
for timely touch-ups. This will keep your lashes looking full and natural, indefinitely.

• Do not attempt to remove the extensions yourself. Have them removed by your
professional eyelash extensionist.

CONSENT FOR EYELASH PROCEDURE:

I have agreed to have Eye Love Lashes applied to and/or removed from my eyelashes. Before
my qualified professional can perform this procedure, I understand I must complete this
agreement and provide my informed consent by signing and dating where indicated below.
For valuable consideration, in order to have my Eye Love Lash extensions applied and/or
removed from my eyelashes:

1. Waiver of Liability. I understand there are risks associated with having artificial eyelashes
applied to and/or removed from my existing eyelashes, and that notwithstanding the utmost of
care in the application or removal of these products, there still exist risks associated with the
procedure and product itself, which include, without limitation, eye irritation, eye pain,
discomfort, and, in rare cases, blindness when improperly handled. As partof this procedure, I
understand that a certain amount of eyelash adhesive material will be used to attach the
artificial Eye Love Lashes to my existing eyelashes. Even though the Professional may apply or
remove my Eye Love Lashes properly, I understand adhesive material may become dislodged
during or after the procedure, which may irritate my eyes or require further follow-up care, at
my own expense to prevent damage to my eyes. I also understand there is more than one
technique for applying lashes to my eyelashes, and I will not attribute any liability to
Professional or Eye Love Lashes, as a result of this procedure or the use and care of these
lashes. I also agree to defend, indemnify and hold harmless Professional and Eye Love Lashes
from any and all claims, actions, expenses, damages and liabilities, including reasonable
attorneys’ fees which might be asserted against them as a result of my having this procedure
performed, or my purchase of these Eye Love Lash products. As used in this agreement, the
terms “Professional” and “Eye Love Lashes” include all of their respective officers, directors,
agents, employees, successors and assigns.

2. Care and Maintenance. I agree to follow the care and maintenance instructions provided by
Eye Love Lashes, and/or Professional for the use and care of my Lashes, and that if any follow
up care is required due to my own mistake or negligence, or failure to follow these instructions,
this will be at my own expense and risk. I understand that if I do any of the following, it may
result in damage to my lashes or may cause my lashes to fall off prematurely. Knowing this I
agree to follow these tips for best results:

I will avoid oil based eye products as these will loosen the bond of my Eye Love Lashes.
I will avoid getting my lashes wet within the first 24 hours after my application.
For the first two days after application I understand it is best to avoid swimming, saunas or
steam rooms.
If I experience any itching or irritation, I agree to contact my Eye Love LashProfessional
immediatelyto have the lash extensions removed.
I agree to avoid using waterproof mascara and to not use an eyelash curler, perm, or tint my
lashes.

I agree to not pick, pull or rub my Lashes.
I understand that I should not attempt to remove my lash extensions on my own or with any
product, but that the procedure requires that my lash extensions be professionally removed.

3. No Known Medical Conditions / Informed Consent. I have read and completed the Eye Love
Lashes Client Intake Form in its entirety and in truth. I acknowledge that I have been advised of
the potential harmful or negative side effects (such as the premature shedding of my eyelash)
that the lash extension procedure or removal may cause to those who have specific medical or
skin conditions. I understand that the adhesives and adhesive remover are a skin, eye and
mucus membrane irritant and that in rare cases persons may be allergic or have
hypersensitivity to synthetics, cyanoacrylate or formaldehyde which in small amount may be
present in the adhesive. I understand that the procedure requires that I lay still for upto 2 hours
or longer with my eyes shut, and that if I wear contacts, I must remove my contact lenses for
the duration of the lash extension application or removal. I further state that I have no known
medical condition that might be aggravated by the procedure or any medical condition that
wouldprevent me from complying with or heeding to the professional’s or Lavish Lashes
instructions or these warnings.
If any action is brought to enforce the terms of this Agreement, the prevailing party shall be
entitled to its costs and reasonable attorneys’ fees. Any claims arising out of this agreement will
be resolved through binding arbitration using the rules of the American Arbitration Association.
This agreement will remain in effect for this procedure, and all future procedures conducted by
Professional or any other professional conducting business at the salon/spa establishment
listed above. I agree that this Agreement is binding upon me, and my heirs, legal
representatives and assigns. I represent that I am over 18 years of age and that I have the right
to enter this agreement, or if I am under 18 years of age, I have had my parent or legal guardian
consent to this agreement, and his or her relationship to me is as follows:
