This sheet provides an analysis of the projected future housing requirements for
people falling under the TCP remit where an assessment has been undertaken.

Pathway

A need for a staged approach has been identified for 6 individuals.

1 requires a step down to low secure followed by locked rehab then discharge. 2
require step down to locked rehab followed by open rehab then discharge. 2 require
step down to low secure prior to discharge. 1 requires step down to locked rehab
prior to discharge.

2 people have been identified to discharge into community support and 1 person is
already in supported accommodation.

Community Accommodation Model

For all 9 individuals the eventual community accommodation has been identified as
shared accommodation within a high support environment.

Location

1 person is currently placed in community support outside of NWL and 1 other has
been identified as requiring accommodation out of area.

A preference for accommodation within NWL is identified for 5 people, 1 individual
has identified preference for Brent and the other for Ealing.

Comments

These figures do not include projections for people who have not had a
housing needs assessment. Because of the small numbers in each borough
projections re future accommodation requirements against these individuals
have only been made at TCP wide level.

Information has not been included that relates to future housing needs of people
identified at risk of admission.

For the overall modelling across the TCP it has been assumed that 3 people per
borough identified at risk will require alternative accommodation, with the remainder
being supported within their existing accommodation/support arrangements with
enhanced support.

While Brent is atypical in having 6 people currently supported within medium secure
services this future support requirement for shared accommodation is at variance
with other CCGs/boroughs and may benefit from review.

In order to ensure that projections of future accommodation needs are as accurate
as possible it is recommended that the NWL TCP Housing needs assessment is
completed for CCG and Spec Comm funded patients who have not been assessed.
It may also be of benefit to clarify the MOJ restrictions for the individual identified as
requiring accommodation outside of NWL
