Lost Transportation Passes
MTACC Employees who have lost transportation passes or building access badges should contact the
Human Resources department for the forms and the procedure to replace them. Employees will be required to
complete a notarized statement with an application for badge replacement and submit those documents
to the agency who issued the pass (e.g. Long Island Rail Road, Metro North Railroad, and New York City
T ransit for a MetroCard.)
Lost Building Access Cards
MTACC Human Resources will assist in providing the documents and directions to replace or change
the floor access on your building access card. Building access cards are provided by building security or
an MTA agency, depending on the employee access required. Frequent requests to replace lost cards will
incur restrictions on card replacement.
Building Security Services are provided by Securitas.
The building services office is located at:
2 Broadway, elevator to the Mezzanine level
(646) 252-1027
