Pain Points

Geomatics/Seismic Acquisition representative need to be assign as CSR for
offshore/onshore survey/seismic operation on top of primary roles as QA/QC

GPD PD&T/SKG/SKO/PMA/SBA require some training certification and
competency on HSE for CSR assigned for project

No available guideline or reference in term of HSE and technical for CSR
appointment from Geomatics/Seismic Acquisition for offshore/onshore project

Objective

To standardize requirement from project team and asset team (SKG/SKO/PMA/SBA)
on list of training that CSR must attend in order to qualify him/her as CSR for offshore
and onshore project.

To set validity of certificate for CSR training conducted by Upstream HSE with standard
certificate issuance. Recommended validity is 2 years.

To establish and roll out CSR Competency Checklist Form for CSR nomination and
approval.

Term of Reference

I. This CSR competency training will be applicable for all internal staff and external staff
from manpower third party QC services that involve in offshore and onshore site
operation.

II. Contract holder(s) of project focal(s) of their representative shall be responsible to
communicate with project owner/asset team on CSR appointment letter issuance and
approval.

III. Internal and external staff shall attend mandatory HSE training as per CSR
competency checklist requirement and to get approval from Head of Department
(HOD)/Head of Section (HOS) prior to mobilization.

IV. One competency checklist form is valid for one mobilization only for offshore project.
Any future mobilization will require another form to be filled in prior to mobilization.

V. For onshore project, singed form is valid for multiple mobilization on the same project
subject to HOD/HOS approval.

VI. Approved Competency Checklist Form to be kept by Contract Holder and HSE
Department for reference.

VII. CSR appointment letter to be issued by HOD/HOS.
