Congratulations and commendations are also extended to the researchers who have dedicated their efforts in conducting quality research studies in Selangor. The achievements throughout the year 2022 indicate that a total of 22 studies were presented, and 8 studies were successfully published either at the national or international level. The findings of these studies have the potential to enhance the professionalism of pharmacy members and provide high-quality services to patients. Over the years, the allocation of authorship for article publication has been a concern, in tandem with the corresponding increase in the number of articles being published every year. The concern regarding authorship mainly surrounds the issue of omissions and inclusions of authors, whereby one thinks that he/she has been omitted from a list of authors despite a significant contribution, or one claims that individuals listed as authors are unwarranted (1). Although about 65% of journals have specific authorship policies (2), this article is written with reference to the recommendations by the International Committee of Medical Journal Editors (ICMJE) to provide guidance on the role of authors and contributors regarding scientific article publication. The ICMJE concludes that an author is someone who has "made substantive intellectual contributions to a published study" (3). ICJME Recommendations AUTHORSHIP DISPUTE Who's an author and who's NOT? Despite the definition and recommendations on authorship for publication, the terms "significant" or "substantial" contribution remain unclear, not only to novices but also to experienced researchers. The aim of this article is to briefly discuss the concerns and conflicts surrounding authorship allocation and provide guidance to researchers on authorship allocation. 1 2

Data Curation Management activities to annotate (produce metadata), scrub data and maintain research data (including software code, where it is necessary for interpreting the data itself) for initial use and later reuse. Writing - Original Draft Preparation, creation and/or presentation of the published work, specifically writing the initial draft (including substantive translation) Writing - Review & Editing Preparation, creation and/or presentation of the published work by those from the original research group, specifically critical review, commentary or revision – including pre-or post-publication stages. Visualisation Preparation, creation and/or presentation of the published work, specifically visualisation/ data presentation. Supervision Oversight and leadership responsibility for the research activity planning and execution, including mentorship external to the core team. Project administration Management and coordination responsibility for the research activity planning and execution. Funding acquisition Acquisition of the financial support for the project leading to this publication. CRediT Author Statement To clear the air on “significant” or “substantial” contribution, the structured Contributor Roles Taxonomy (CRediT) was introduced in 2014 and is being used in over 120 journals (4). The CRediT idea is a collaborative work between Harvard University and the Wellcome Trust, the ICMJE, researchers and publishers. CRediT was introduced with the intention of recognising individual author contributions, reducing authorship disputes and facilitating collaboration. CRediT offers authors the opportunity to share an accurate and detailed description of their diverse contributions to the published work (Table 1). Table 1. CRediT statements should be provided during the submission process and will appear above the acknowledgement section of the published paper (Figure 1). It is important to note that, the CRediT in no way changes the journal’s criteria to qualify for authorship. Figure 1. Example of authorship statement IRREGULARITIES Authorship irregularities occur when authorship credit is given when it is not due and not appropriate (5) (Table 2). Term Definition Guest authorship Inclusion of an individual who does not meet authorship criteria. Pressured authorship A person who uses their position of authority to apply pressure upon staff more junior to them to include them as an author, even though they do not qualify. Ghost authorship The failure to name an individual as an author when they have contributed substantially to the research or writing of the article. Table 2. Types and definitions of authorship irregularities TIMING It is important that research and supervisory teams agree on a publication strategy and policy on authorship from the start of the project (1). However, changes in the authorship decision are bound to happen along the journey of completing a research project. Any changes in authorship allocation should be communicated with the author involved, as well as the entire research team. To say the least, authorship matters should be clarified and finalised before article submission for publication, as some journals requires all authors to give final approval of the manuscript before subjecting the article to peer review process. Adding and/or deleting authors at revision stage or after acceptance are generally not permitted, but in some cases, it may be warranted. Upon providing the reasons for the changes in authorship, approval of the change during these stages is at the discretion of the journal’s Editor-in-Chief. 4
RESPONSIBILITY It is the responsibility of the principal investigator to decide and assign authorship to individuals who have made significant and substantial contribution to the research manuscript. In reality, this decision is usually made upon discussion with the research team members. However, upon manuscript submission for publication consideration, the corresponding author is responsible for ensuring that the descriptions are accurate and agreed by all authors. The use of artificial intelligence in the ‘birth’ of research articles is a trend that is gaining much attention nowadays. In keeping with the fourth ICMJE recommendations, chatbots should not be listed as authors because they cannot be responsible for the accuracy, integrity, and originality of the work, and these responsibilities are required for authorship (3). ARTIFICIAL INTELLIGENCE-ASSISTED TECHNOLOGY In conclusion, determining who qualifies for authorship is a crucial yet somewhat intricate process. It is essential to initiate the discussion on authorship early in the research and writing stages. The guidance provided by the ICMJE recommendations, CRediT author statements, the journal's authorship criteria, and the thoughtful consideration and judgment of the principal investigator are utilised to guide the allocation of authorship and prevent authorship disputes. RESEARCH PERSONALITY Dr. Sahimi is a dedicated clinical pharmacist with a remarkable career spanning several decades. After completing her bachelor's degree at the Universiti Sains Malaysia, she underwent training at Hospital Kota Bharu and later served at the Central Medical Store Sarawak. In 2006, she obtained her master's degree in clinical pharmacy from the Universiti Kebangsaan Malaysia, followed by a PhD from the International Islamic University in 2016. DR SAHIMI MOHAMED Education Background Career Growth Since 2007, Dr. Sahimi has been providing exceptional patient care as a clinical pharmacist at Hospital Tengku Ampuan Afzan, Kuantan. Her unwavering commitment and professional growth led her to assume the prestigious role of Chairperson for the Cardiology Committee Pharmacy Services Division at the Ministry of Health (MOH) in 2012. In this position, she spearheads the strategic planning, implementation, and management of clinical pharmacy services for cardiology patients, ensuring the delivery of high-quality pharmaceutical care. In 2021, Dr. Sahimi was promoted to the position of Anticoagulant Subject Matter Expert (SME) in Pharmacotherapy Cardiology and transferred to Hospital Sultan Idris Shah, Serdang. This promotion reflects her expertise and proficiency in anticoagulation therapy, which she developed through her participation in a sixweek training program at the Anticoagulant Clinic North America (ACNA) in Texas in 2008. Furthermore, she underwent advanced training in anticoagulant therapy for cancer patients at the prestigious Cleveland Clinic in Ohio, USA, in 2023. Professional Contribution Dr. Sahimi's passion for research was ignited during her master's degree program, where her supervisor, Prof. Dr. Rosnani Hashim, played a pivotal role in nurturing her interest. As an SME in anticoagulation, she focuses her research on critical areas such as the prevention and treatment of venous thromboembolism, stroke prevention in atrial fibrillation, and the development of comprehensive anticoagulation management strategies. Throughout her career, Dr. Sahimi has made significant contributions to the scientific community through her publications. With over 15 papers authored and co-authored by her, her research provides valuable insights into the complexities of anticoagulation therapy, including advancements in treatment options, clinical outcomes, and management approaches. By identifying innovative approaches and best practices, she strives to enhance patient safety and well-being. Her research findings serve as valuable resources for healthcare professionals, enabling them to make informed decisions and provide evidence-based care to their patients. 8

My journey in clinical research began in 2015; when my ward consultant requested me to submit a research paper to a local nephrology conference. I was a junior pharmacist then, thus did not know what to expect and how to conduct a good study. However, through his patient guidance and supervision, I managed to complete and present my first study at the conference. Since then, I have gradually developed a love for clinical research. Clinical research is extremely important in the field of medicine and health sciences because it provides us with evidence supporting usage of drugs in managing different diseases. With clinical research, we would be able to discover new treatment for difficult-to-treat diseases and save patient’s life. I am grateful for the research opportunities given by the nephrologists in Hospital Selayang and not to forget my ex- and current research teammates who have struggled through thick and thin with me throughout my research journey. 13 Introduction Fungal peritonitis (FP) is an uncommon complication of peritoneal dialysis (PD) and is associated with high mortality and technique failure rate. The optimal treatment for FP is still not well established and intraperitoneal (IP) amphotericin B has been reported to cause chemical peritonitis. This study aims to investigate the role of IP amphotericin B in treating FP and the factors affecting the outcomes of FP. Methodology Outcomes of all FP diagnosed between January 1, 2011 and March 30, 2021 were reviewed retrospectively. Before January 2015, FP cases were treated with intravenous (IV) amphotericin B and oral flucytosine; from January 2015 onwards, IP amphotericin B was given in additional to the standard antifungal regimen. Results During the study period, 48 FP cases were diagnosed. Yeast was isolated from 77% of the cases with Candida parapsilosis (29%) the most frequently detected yeast. The susceptibility of yeast isolates to amphotericin B and flucytosine were 96% and 100%, respectively. On average, Tenckhoff catheter was removed 14 days from the diagnosis of FP. The overall PD resumption rate was 44%, similar among patients who did/did not receive IP amphotericin B (44% vs 43%, P=0.94). The technique failure rate after 12 months was similar among those who did/did not receive IP amphotericin B (81% vs 79%, P=0.83). Overall mortality was 19%, similar between those who did/did not receive IP amphotericin B (21% vs 14%, P=0.61). Mortality was also similar between those who were infected by yeast and mold (19% vs 18%, P=0.96) and those who had Tenckhoff catheter removed early (≤1 week) and late (>1 week) from the diagnosis of FP (18% vs 19%, P=0.89). Mortality was higher among those with serum albumin ≤25 g/L (35% vs 8%, P=0.02). None suffered from chemical peritonitis with IP amphotericin B. Conclusion Even though IP amphotericin did not cause chemical peritonitis, it did not seem to improve the outcomes of FP. Hypoalbuminemia was associated with higher mortality in patients with FP. I was first involved in clinical research when I was a provisionally registered pharmacist. I had the opportunity to be involved in more cardiology researches and clinical audits when I join cardiology department as a clinical pharmacist. Cardiology care is my main research interest and passion. Being able to contribute and being part of the investigator-initiated research team under senior pharmacists and senior cardiology consultants broaden my horizon and motivates me to find better ways to treat various cardiovascular diseases. I was also fortunate to join industry-initiated research team to learn the system involved and get exposure on global research projects. In recent years, I have also gained interest in cardio-oncology and cancer survivorship research to help improve cardiovascular health in cancer survivors in the future. I would recommend my peers and juniors to take up research and audits. These activities are important measures to have local data and it may change clinical practice and improve patients’ outcomes. 15 Background Atrial fibrillation (AF) and Diabetes mellitus (DM) often coexist. As DM is considered while calculating the thromboembolic risk for AF patients, little is known about the prescription pattern of antithrombotic therapies in patients with AF and DM and their clinical outcomes. Purpose In this subsidiary study, we examined the prescription patterns of antithrombotic therapies and clinical outcomes of AF patients with type 2 DM. Methods We analyzed data from a single-center cohort of patients with a primary diagnosis of AF in a tertiary cardiac referral hospital in Malaysia from 1st January 2018 to 31st December 2020. Patients' clinical data and information related to antithrombotic therapy were traced through electronic Hospital Information system. A data collection form was used for data collection. The primary endpoint of the study was a composite cardiovascular (CV) event which consists of all-cause mortality, acute coronary syndrome (ACS), ischemic stroke and transient ischemic attack (TIA). The safety endpoint of the study was a bleeding event, defined as hemoglobin drop more than 2 g/dl, blood transfusion and bleeding at critical area. Results Of the 1006 AF patients (59.2% male; mean age 64.2 (12.1) years), 400 (39.8%) had a history of DM. Of these, 45.8% (n=183) were using warfarin; 46.5% (n=159) used direct oral anticoagulant (DOAC), 54.0% (n=216) used a single antiplatelet, 11.5% (n=46) used a double antiplatelet, 8.0% (n=32) used triple therapy which consists of two antiplatelet agents with one anticoagulant. The use of single antiplatelet agent (54.0% vs 46.4%, p=0.018) and double antiplatelet agents (11.5% vs 7.1%, p=0.016) was significantly associated with AF with DM patients, whereas there was no association between anticoagulant use and AF patients with or without DM (85.5% vs 82.5%, p=0.209). There was no association in composite CV events between AF patients with or without DM (12.0% vs 10.4%, p=0.427). The proportion of subjects who reported having bleeding events were also did not differ by the presence of AF patients with or without DM. (4.5% vs 2.8%, p=0.151). Conclusion Diabetes was associated with increased use of antiplatelet agents; however, DM was not associated with increased risk of composite CV events and bleeding events in patients with concurrent AF. The lack of an association between diabetes and CV risk contrasts with previous research, which could be due to improved diabetes treatment in this cohort of patients with relatively low fasting sugar readings. Further study on the degree of blood glucose as measured by glycosylated hemoglobin (HbA1c) is needed to confirm the finding. It acts as a support department that plays an important role, especially the procurement and supply of drugs and disposable medical equipment to units and patients. Services begins with basic units that provided pharmaceutical services such as medical stores, inpatient pharmacies and outpatient pharmacies only. OUR HISTORY HSIS, Serdang is a government-funded multi-specialty hospital located in the district of Sepang in the state of Selangor, Malaysia. This hospital is located near Putrajaya, the Malaysian federal government administrative centre. HSIS, Serdang is a reference for clinics and hospitals in the vicinity from Serdang, Kajang, Putrajaya, Dengkil and Puchong. Serdang Hospital uses hospital IT environment called Total Hospital Information System (THIS). 17
OUR STORY Starting with the basic function of the pharmacy profession, which is the procurement and supply of drugs, the Department of Pharmacy continued to grow to meet current needs and demands. This development included Inpatient Pharmacy services and Outpatient Pharmacy services. Since HSIS, Serdang is a reference hospital for the disciplines of Cardiology and Nephrology, the focus of the development of the Pharmacy Department is also in line with the development of HSIS, Serdang. Now, the Inpatient Pharmacy service has an additional 2 pharmacy satellites in addition to the Ward Supply Pharmacy to supply drugs using the Unit of Use (UOU) and Unit Dose (UDS) systems. Ward Pharmacy Services now covering for 6 disciplines and 25 wards. For Outpatient Pharmacy services, the Pharmacy Department is now expanding with 2 pharmacy satellites, namely the Daily Treatment Pharmacy Satellite and the Emergency Pharmacy Satellite. To date, the Outpatient Pharmacy service provides 5 outpatient Medication Therapy Adherence Clinic (MTAC) services and 2 value added services. In December 2022, Pharmacy One Stop Centre (POSC) has began to operate with the opening of Heart Centre of HSIS, Serdang. The Department of Pharmacy has successfully received the recognition from the Amalan Persekitaran Berkualiti 5S by Perbadanan Produktiviti Malaysia since 12 May 2011 and has managed to maintain this recognition for 7 consecutive years.
