Mental health impacts our thoughts, actions, and interactions as it empowers us to navigate challenges, make informed decisions and build meaningful relationships. I would like to express my heartfelt appreciation to the Malaysian Family Medicine Specialists Association (FMSA) for organizing an impactful conference and aptly selecting the theme "Mind, Body, Soul. A Voyage of Wellness". This is a commendable effort to promote Mental Health knowledge and awareness amongst healthcare personnel in Primary Health Care settings. Mental health is a state of mental well-being that enables people to cope with the stressors of life, realize their abilities, learn well and work well, and contribute to the community. The toll of poor mental health extends beyond individuals and has a significant impact on the country as a whole. Poor mental health in a workplace setting can lead to decreased job performance, low job satisfaction, and even loss of employment. However, the stigma and misinformation is a barrier and can feel like an overwhelming obstacle for someone who is struggling with a mental health condition. Therefore, it is vital that all of us to address this be positive mental health advocates. As the Health Director of Negeri Sembilan State in Malaysia, I am honoured to be able to support and facilitate this meaningful event. In times of crises such as the current pandemic, Family Medicine physicians are responsible for providing care to patients in their homes, reducing the burden on hospitals. A biopsychosocial-spiritual model is a holistic approach that acknowledges the interaction between physical, psychological, social, and spiritual aspects to patient care and patient well-being. This holistic model focuses on the intrapersonal interactions of the physical body and the mind-body connection as well as the patient’s extra-personal relationship with the physical environment, family, friends, and community. As primary care providers, we need to equip ourselves with knowledge and be ready to explore treatment options for patients from various angles. Apart from that, FMSC is also a great platform for researchers to share their research findings either via oral or poster presentations. This is also a great time for catchingup with friends and colleagues. The role of a family can be a double-edged sword: it can either be a vital support system for a patient or it can be the cause for a patient’s despair and worsening health. Thus, a holistic health practitioner must address family issues so that the patient can achieve the best health outcome possible. This workshop will introduce participants on how to use a genogram when working with families. Participants will learn to use multi-generational genograms to position a patient's difficulty in the context of his/ her family relationships across generations. Once a multi-generational genogram is formulated, the health practitioner can then guide their patients to explore resources and strengths within the family to help patients cope with their challenges. Participants also will be taught on family sculpting; which is a useful tool to explore current family dynamics. This can create opportunities to work on positive interpersonal relationship changes within the family. Participants will also get to learn about ‘circular questioning’; a method introduced by a renowned Psychiatrist & Family therapist, Dr Arnon Bentovim. SOUL: A VOYAGE OF WELLNESS 39 NAVIGATING THE FUTURE OF FAMILY PRACTICE Deputy Director (Primer) Family Health Development Division Ministry of Health Malaysia Dr. Mastura Binti Ismail Primary care is not exempt from the strain facing so many sectors of the Malaysian economy, and health care in particular. Nevertheless, as the current situation continues to evolve, the impact of COVID-19 will be felt far beyond its population-based effects on morbidity and mortality, with sequelae including: i) Severely strained management of chronic disease ii) Increase in and worsening of mental health and associated effects iii) Exacerbation in difference of an already current and uneven delivery of medical and health care services for vulnerable populations. Despite the challenges being suffered during this pandemic, the practice of primary care stands to play a significant role in the management of these issues, employing previously underutilized technology such as telehealth, and overcoming the financial and physical limitations imposed by the pandemic on a routinely clinic-based patient care. During the crisis focus was placed on scaling-up of primary care capacities through well-designed innovation equipped with digital technology which helped deliver a successful health system response. SOUL: A VOYAGE OF WELLNESS 40 GLOBAL ENGAGEMENT IN IMPROVING RESPIRATORY HEALTH Globally, four lung diseases; lower respiratory tract infections, chronic obstructive pulmonary disease (COPD), tuberculosis and lung cancer are among the top twenty causes of death. Chronic respiratory diseases (CRD), which include asthma, COPD, bronchiectasis, and post-tuberculosis lung disease impose a significant burden on global health and the number of Disability Adjusted Life Years (DALYs). Most of these burdens occur in the low- and middle-income countries (LMICs). In addition, tobacco smoking, the leading cause of CRD, and air pollution are amongst the top ten risk factors for global disease burden. Primary care is the backbone to a healthcare system and is most apt to deliver cost effective care. It is the keystone for universal health coverage. However, the delivery of care for lung health in primary care face many challenges, spanning the maturity and strength of the primary care system in a country, investment (or lack of) in primary care infrastructure such as the health information systems, health services delivery and workforce investment in recruitment and reimbursement; awareness and knowledge about diagnosis and management of respiratory diseases, time constraints and heavy workloads especially in public health centres, access to and availability of vaccines, medicines and devices, respiratory prescribing rights of family doctors, priority (or lack of) given to lung diseases, management needs to tailor according to patients’ health literacy, health belief and culture. In LMICs, pulmonary rehabilitation services are lacking, and palliative care for severe COPD is scarce. Research for lung health in primary care has been neglected. A concerted effort is needed to address these gaps to promote a service that is funded, implemented, evaluated, and endorsed by governments and all stakeholders including advocacy organisations for the betterment of lung health. Organisations and research units/networks such as the International Primary Care Respiratory Group (IPCRG), Malaysian Primary Care Respiratory Group (MyPCRG), NIHR Global Health Research Unit on Respiratory Health (RESPIRE) are some of the global and national respiratory interested groups that promote lung health in primary care. The most common diseases include hypertension, diabetes, heart disease, stroke and cancer. These conditions are generally long-term and result from a combination of genetic, physiological, environmental and behavioral factors. As a result of urbanization and globalization, the typical Malaysian way of life has changed dramatically in recent years. This change in lifestyle coincided with a rapid increase in the number of Malaysians suffering from chronic diseases. A therapeutic lifestyle change (TLC) was first developed for the reduction of LDL cholesterol. Subsequently, the TLC approach was found to improve risk factors associated with metabolic syndrome and diabetes, including blood pressure, HDL, serum triglyceride and blood glucose. The TLC mainly utilizes specific dietary recommendations, increase physical activity and weight management. The session aims to enhance our understanding in the use of TLC in management of chronic diseases.
