MATH PRE-ASSESSMENT BASELINE

STUDENT: Alonah Smith

GRADE: 7th Grade

DATE: November 23,2019

ASSESSOR: Mrs. P Denise Smith (Tutor)

ABOUT THIS ASSESSMENT

Alonah took the diagnostic Math Test November 2019. This diagnostic tests knowledge of grades 6-7

math concepts, ability to determine when and how to apply those concepts, as well as how to accurately

calculate solutions. She also answered separate math questions which tested her knowledge of 4th and

5th grade math concepts. Her answers give insight to the Math success rate of her current grade level.

WHAT DOES THIS RESULTS MEAN?

• How Alonah Smith compares to other 7th graders at the end of the year

• How well Alonah Smith understands previous grade level Math concepts

• How well Alonah Smith applies previous grade level Math to understand current grade level

Math

• How well Jameeyah Nelson can solve Math problems at his current grade level

ALONAH SMITH PERFORMANCE ON THE 7th GRADE MATH DIAGNOSTIC

Performance Level

Students who perform at Level 1

show EXTREMELY LITTLE

UNDERSTANDING of the

expectations of their grade. They are

likely to need lots of support to meet

the Math expectations for their grade

level

Alonah is proficient in basic concepts (adding, subtracting, and multiplying)

whole numbers. applications and calculations for grades 1-5. Alonah struggles

with grades 6 and 7 math concepts due to lack of understanding of principles

beyond basic whole numbers as well as lack of understanding how to

incorporate building basic skills into the next level. This is impacting her ability

to perform at a proficient level beyond grade 4 or 5. Alonah understood and

correctly answered 1.5 out 22 problems correctly. She remembered seeing

some of the information while not remembered seeing others at all.

Answered correctly --Problems 8 (partial did not simplify) and 49

The Areas of Opportunity which are impacting her most are

o 4th – 6th Grade Numbers and Operations Fractions (4.NF)

o 4th Grade Measurement and Data (4.MD) Perimeter and Area

o 5th Grade Operations and Algebraic Thinking (5.OA) Order of Operation

o 5th- 6th Grade Add, Subtract, Multiply and Divide Decimal (5.NBT)

o 6th Grade Ratios and Proportions (6.RP)

o 6th Grade Expressions and Equations (6.EE)

o 6th Grade Statistics (6.SP)

NEXT STEPS

• Needs to practice using calculator for all math operations including

division (excluding fractions)

• Needs to learn concept, application, and calculation for a fractions with

unlike denominators

• Needs to concept, application and calculation for decimals

• Need to learn Order of Operation

• Needs to learn Grade 6 Ratios, Expressions, One-Step Equations, Area, and

Perimeter ( how to set up and calculate)

• Practice two Grade 6 concepts 15 minutes daily
