KEY ACHIEVEMENTS

MSQH Accreditation Programme

MSQH is a recognised organisation and has Hospitals surveyed for the year 2020 are

W W W . M S Q H . C O M . M YWith the current COVID-19 pandemic, all on-site surveys were put on hold from March 2020 to September

2020. As a result of this, hospitals due for survey during these periods have been given an extension of
the Accreditation Status between 3 to 12 months. This also implies that the implementation of the 5th
Edition Hospital Accreditation Standards need to go beyond July 2021.

The COVID-19 pandemic has prompted the Society to rapidly review its standard operating procedure
for conducting survey and training for the years ahead. The Society embarks into conducting a virtual
and/or hybrid survey in year 2021 as part of its business continuity solution and to ensure the
sustainability of its client’s accreditation award in any given times especially during the COVID-19
pandemic condition. In the event where an on-site verification is required or where there is shortcoming
in terms of technology, resources and/or constraints from the facility and services, a hybrid survey will be
an alternative choice. The virtual survey will be using online technologies namely video conferencing,
file-sharing and virtual visits. This practice will be the new norm in the MSQH survey management.

During the year, membership dropped from 1,728 to 1,408 (19%) members compared to year 2019. There
was a number of members who did not renew their membership despite renewal notice circulated in 2019
and 2020. The reduction of membership is partly due to the pandemic COVID-19 and the movement control
order which has disrupted business operations. It was also due to members who left the organisation.

Goal 2 – Develop and strengthen collaboration and partnership with
stakeholders and accreditation organisations

The Committee continued to oversee the implementation of a range of new initiatives conducted by the CEO
and MSQH Executive Managers. These initiatives flowed on from the MSQH Strategic Plan and aim to
consolidate MSQH’s position as the leading healthcare accreditation provider in Malaysia, with engagement and
collaboration with national and international partners. Key to MSQH’s ongoing development is the building of a
range of strategic alliances and networks with key bodies, jurisdictions, public and private sector stakeholders
nationally and internationally.



KEY ACHIEVEMENTS

Strategic Alliances and Partnership

MSQH places strong emphasis on being a pivotal part of the healthcare accreditation industry through its partners
on the areas of patient safety and continuous quality improvement. MSQH continues to provide a partnership
approach with recognised institution and organisation with the aim to establish mutual understanding in the MSQH
and respective accreditation system and portrayed an attestation of conformity in the accreditation programmes.

❖ Collective partners between MSQH, APHM, MMA and MOH to promote quality in healthcare provided by
healthcare facilities and services in Malaysia

❖ Partner with Standards Malaysia to adopt the MS ISO 15189:2014 Medical Laboratory as a prerequisite
in the MSQH Hospital Accreditation Programme 5th Edition Standards;

❖ Expert member in the World Health Organisation Healthcare Patient for Patient Safety;
❖ Pilot study on patients for patient safety with Patient for Patient Safety Malaysia (PFPSM) in collaboration

with the Ministry of Health Malaysia and the World Health Organisation (WHO) Collaborative Centres;

❖ Collaborate with MHTC on the development of Centre of Service Excellence Guidelines, medical travel
standards and promotion of MSQH accreditation programmes;

❖ Reviewing abstracts for the ISQua International Conference since 2005 and BMJ International Forum on
Quality and Safety in Healthcare since 2017;

❖ Membership and ISQua surveyor in the ISQua External Evaluation Accreditation Council;
❖ Board Member of the Asian Society for Quality in Health Care;
❖ WHO Technical Consultant in Healthcare and Patient Safety;
❖ BMJ and ISQua Organising Committee Members;
❖ Partner with GHA on medical travel accreditation programme;

The statistically significant studies findings among all the studies mentioned above are:

1. In 1st Edition MSQH Hospital Accreditation Standards, Criteria in Domain 1 (Organisation and
Management) and Domain 3 (Policies and Procedures) could probably easier to comply and the rating
is more predictable as compared to Domain 2 (Human Resource Development and Management),

Domain 4 (Facilities and Equipment), Domain 5 (Safety and Performance Improvement Activities) and
Domain 6 (Special Requirements). Statistical analysis (Kruskal Wallis test) shown significant difference
of Compliance Score between different domains.

2. Hospitals with number of beds between 100-299 perform better than hospitals with lower or higher bed
numbers. This finding coherent with the finding explained by E. Getzen in his book ‘Health Economics’.

3. In 1st Edition Hospital Accreditation Standards, the Service Standards that is highly and positively
correlated are: (a) Radiology and Emergency Service Standards; (b) Surgical and Emergency Service
Standards. The standards that is highly but negatively correlated are: Environmental Services and
Critical SCN Service Standards

4. There were great agreement between both Surveyor and Councillor in relation to giving appropriate score
to hospitals. However there were some disagreement seen away from score 16. Councillor tend to
modulate the score when the score is extreme (lower the councillor rating when surveyor rating is too
high and higher the councillor rating when surveyor rating is too low). Both Surveyor and Councillor tend
to help hospitals passing the borderline rating of 19.

5. There is a significant difference of compliance to MSQH standards between new-entry hospitals in the
1st Edition and the new-entry hospitals in the 2nd Edition. New-entry hospitals in the 2nd Edition comply
to MSQH standards better (scores 1.75% higher) than new-entry hospitals in the 1st Edition. There were
several explanation to this including (1) MSQH provides better education and support in 2nd Edition of
MSQH Hospital Accreditation Programme; (2) the standards were more comprehensible in 2nd Edition
Hospital Accreditation Standards ; and (3) Hospitals that undergone accreditation later (in 2nd Edition)
had more time to adapt and conform to the standards before initiating the accreditation process.

84% of Public Hospitals (n = 121) had underwent accreditation survey process for at least once in the last 20
years. However, only 55.4% (n = 67) public hospital were accredited in the year 2020. 53 public hospitals had
ended their accreditation commitment with MSQH. Central Government had done their best to encourage
accreditation. MoH allocated RM 5.72 mil in 2010 - 2013, RM10.99 mi in 2014 - 2018, and RM5.95 mil in
2018 -2021 for accreditation and training with MSQH. However, there are under-utilization of the fund when
the allocated budget was not being used up by Public Hospitals for MSQH Hospital Accreditation Programme
or MSQH Training Programme.

Key Achievements

5.1 Financial Performance

A detailed financial position with audited accounts for the year ended 31st December 2020 and budgets
for 2021 and 2022 are reported under Treasurer’s Report.

5.2 Human Resource

5.2.1 MSQH Employees
As at 31st December 2020, the Society has nineteen (19) full time staffs, two (2) part-time
Project Managers and a Non-Executive Technical Advisor employed with the Society. During
the year, Professor Dato’ Dr Hj Abdul Rahim Bin Abdullah was appointed as MSQH Chief
Executive Officer effective 1st July 2020. Associate Professor Dr M.A.Kadar Marikar was

MSQH staffs have been working from home and on a rotation basis since March 2020.
Workplace health and safety of all employees are the Society’s main priority at all times. The
Society has successfully received the government’s PRIHATIN Wage Subsidy Programme in
2020 and this is reported under Treasurer’s Report for the year ending 31st December 2020.

engineers, nurses, medical administrators and allied health professionals while contributing to the MSQH

International Recognition

MSQH is an organisation that has achieved the internationally acclaimed accreditation award from the
International Society for Quality in Health Care (ISQua) for the following :-

1) Organisation and Surveyor’s Training Programme – December 2020 to December 2024
2) Standards – Hospital Accreditation Standards 5th Edition June 2017 to June 2021. ISQua has granted

a 12-month extension for the award until June 2022 in view of the COVID-19 pandemic.

REPORT ON THE FINANCIAL STATEMENTS

We have audited the financial statements of Persatuan Kualiti Kesihatan Malaysia (Malaysia Society For
Quality In Health) (MSQH), which comprise the balance sheet as at 31 December 2020, and the income
and expenditure account and the statement of cash flows for the financial year then ended, and a summary
of significant accounting policies and other explanatory notes, as set out on pages 5 to 20.

EXECUTIVE COMMITTEE’S RESPONSIBILITY FOR THE FINANCIAL STATEMENTS

The Executive Committee of the Society are responsible for the preparation of financial statements that
give true and fair view in accordance with Financial Reporting and the rules and regulations of the
Society and Societies Act, 1966 in Malaysia and for such internal control as the executive committee
determine are necessary to enable the preparation of financial statements that are free from material
misstatements whether due to fraud or error.

AUDITORS’ RESPONSIBILITY

Our responsibility is to express an opinion on these financial statements based on our audit. We
conducted our audit in accordance with approved standards on auditing in Malaysia. Those standards
require that we comply with ethical requirements and plan and perform the audit to obtain reasonable
assurance about whether the financial statements are free from material misstatement.

the financial statements. The procedures selected depend on our judgment, including the assessment of
risks of material misstatement of the financial statements, whether due to fraud or error. In making those
risk assessments, we consider internal control relevant to the Society’s preparation and fair presentation
of the financial statements in order to design audit procedures that are appropriate in the circumstances,
but not for the purpose of expressing an opinion on the effectiveness of the Society’s internal control. An
audit also includes evaluating the appropriateness of accounting policies used and the reasonableness of
accounting estimates made by the Executive Committee, as well as evaluating the overall presentation of
the financial statements.

We believe that the audit evidence we have obtained is sufficient and appropriate to provide a basis for
our audit opinion.

1. PRINCIPAL ACTIVITIES

The Society is an internationally recognised Malaysian organisation for the promotion and
improvement of quality and safety in health.

The objectives of the Society in achieving its goals are as follows:

i. Advocate and Facilitate Continuous Improvement of Quality and Safety in the
Provision of Health Services, including standards development, education, performance
assessment and accreditation.

ii. Develop and strengthen collaboration and partnership with stakeholders and
accreditation organisations.

iii. Undertake and promote research into quality and safety in healthcare.
iv. Promote best practices and dissemination of information in relation to quality and

safety in healthcare.

2. SIGNIFICANT ACCOUNTING POLICIES

(a) Basis of preparation

The financial statements of the Society have been prepared in accordance with Malaysian
Private Entities Reporting Standards (“MPERS”) and the requirements of the Societies Act,
1966 in Malaysia. The financial statements of the Society have also been prepared on a
historical basis and are presented in Ringgit Malaysia (RM).

(b) Equipment and depreciation

Equipment are stated at cost less accumulated depreciation and any accumulated impairment
losses if any.

Depreciation of equipment is provided for on a straight- line basis to write off the cost of each
asset to its residual value over the estimated useful life, at the following annual rates :

Furniture and fittings 10 %

Amortisation Information Systems 20 %

An item of equipment is derecognised upon disposal and the difference between the net
disposal proceeds and the net carrying amount is recognised in the income and expenditure
account.

(c) Impairment of assets

The carrying values of assets are reviewed for impairment when there is an indication that the
assets might be impaired. Impaired loss is recognised whenever the recoverable amount is less
than the carrying values of the asset. The recoverable amount is the higher of an asset’s net
realisable value and its value in use (by discounted future cash flows).

An impairment loss is charged to the income statement immediately. Any subsequent Increase

in the recoverable amount of an asset is treated as reversal of the previous impairment loss and

W W W . M S Q H . C O M . M Yis recognised to the extent of the carrying amount of the asset that would have determined (net
amortization and depreciation) had no impairment loss been recognised. The reversal is

recognised in the income statement immediately.

(d) Income tax

Current tax is the expected amount of income taxes payable in respect of the taxable profit for
the current year and is measured using the tax rates at scale rates. For the Society, tax expense
is relating to finance income earned on fixed deposits placement and subscriptions from non-
members, other income are not taxable.

(e) Inventories

Inventories are measured at the lower of cost or net realisable value based on the first-in
first-out principle. Net realisable value is the estimated selling price in the ordinary course of
business, less selling expenses.

(f) Provisions

Provisions are recognised when the Society has present obligations as a result of a past event,
and it is probable that out flow of resources embodying economic benefits will be required to
settle the obligations and a reliable estimate can be made of the amounts of the obligations.

(g) Revenue recognition

Revenue received for accreditation such as survey certification, seminar and workshop are
recognised as income when received.
Fees received before commencement of the above will be recognised as deferred income and is
credited to the income account in the period the above training and/or survey takes place.

Subscription fees received are fees received from new and existing members. Membership fees
are renewed annually.

(h) Employee benefits

(i) Short - term employee benefits

W W W . M S Q H . C O M . M YSalaries and bonuses are recognised as an expense in the financial year in which the
associated services are rendered by employees.

(ii) Defined contribution plan
The society make contributions to the Employees’ Provident Fund (EPF) and social
security scheme (Socso). Such contributions are recognised as expense in the income and
expenditure account as incurred.

(i) Financial instruments

Financial instruments are recognised in the statement of financial position when the Society has
become party to the contractual provisions of the instrument.

Financial instruments are classified as liabilities or equity in accordance with the substance of
the contractual arrangement. Interest, gain and losses relating to financial instruments classified
as liability are reported as expense or income.

(i) Cash and cash equivalents

For the purpose of the cash flow statement, cash and cash equivalents include cash on
hand and at banks and deposits at call, which have an insignificant risk of changes in
value.

(ii) Receivables

Receivables are carried at anticipated realisable value. Bad debts are written off in the
year in which they are identified. An estimate is made for doubtful debts based on a
review of all outstanding amounts at the year end.

future for services rendered.
