Details:
Hafiz was asked to change the fluorescent light at the lobby (2) and since the person in
charge was not there he had to fill in (3). He was not well that day but yet came to work.
As he was changing the light, he was finding it difficult to replace the fluorescent and was
struggling (4) to stand on the ladder. Suddenly his leg lost balance (5) and he fell from
the ladder and landed on the floor. He screamed (6) in pain while holding his left leg. An
admin staff, Yogen who was passing by saw the incident (7) ran to help him. Later, the

C. CAUSES:
